La malattia renale cronica e il trattamento dello scompenso cardiaco congestizio: il ruolo del cardionefrologo
Il rene è coinvolto, in prima battuta, nel mantenimento dell'omeostasi dei fluidi e dell'equilibrio idro-elettrolitico ed acido-base, nonché nell'eliminazione delle scorie tossiche. La funzione renale sembra essere strettamente collegata a quella cardiovascolare e, negli ultimi anni, il termine di 'sindrome Cardio-Renale' è stato utilizzato per spiegare le correlazioni cliniche tra patologia renale e cardiaca. Lo scompenso cardiaco congestizio (SCC) rappresenta la prima causa di ospedalizzazione nei Paesi occidentali e i pazienti affetti presentano spesso, in associazione, un quadro di malattia renale cronica (MRC) di grado variabile. Sono stati considerati diversi modelli fisiologici e fisiopatologici per spiegare come una condizione di malattia renale cronica possa influenzare lo stato di insufficienza cardiaca. Sicuramente la via metabolica del guanosin monofosfato ciclico (cGMP) e quello dell'ossido nitrico (NO) giocano un ruolo fondamentale di concerto con l'azione dei peptidi natriuretici atrial natriuretic peptide (ANP) e brain natriuretic peptide (BNP) sotto il controllo del sistema delle fosfodiesterasi sieriche. I pazienti con sovraccarico di circolo necessitano di terapie farmacologiche specifiche. La terapia diuretica, spesso in associazione (vedi tiazidici + diuretici d'ansa), rappresenta ancora quella di prima scelta ma, nei pazienti, refrattari alla terapia diuretica a dosi massimali, l'impiego di metodiche extracorporee, quali l'ultrafiltrazione (nelle sue diverse modalità d'impiego), può essere di grande aiuto per il paziente scompensato dal punto di vista cardiologico. The role of salt abuse on risk for hypercalciuria
<p>Abstract</p> <p>Background</p> <p>Elevated sodium excretion in urine resulting from excessive sodium intake can lead to hypercalciuria and contribute to the formation of urinary stones. The aim of this study was to evaluate salt intake in patients with urinary lithiasis and idiopathic hypercalciuria (IH).</p> <p>Methods</p> <p>Between August 2007 and June 2008, 105 lithiasic patients were distributed into 2 groups: Group 1 (n = 55): patients with IH (urinary calcium excretion > 250 mg in women and 300 mg in men with normal serum calcium); Group 2 (n = 50): normocalciuric patients (NC). Inclusion criteria were: age over 18 years, normal renal function (creatinine clearance ≥ 60 ml/min), absent proteinuria and negative urinary culture. Pregnant women, patients with intestinal pathologies, chronic diarrhea or using corticoids were excluded. The protocol of metabolic investigation was based on non-consecutive collection of two 24-hour samples for dosages of: calcium, sodium, uric acid, citrate, oxalate, magnesium and urinary volume. Food intake was evaluated by the three-day dietary record quantitative method, and the Body Mass Index (BMI) was calculated and classified according to the World Health Organization (WHO). Sodium intake was evaluated based on 24-hour urinary sodium excretion.</p> <p>Results</p> <p>The distribution in both groups as regards mean age (42.11 ± 10.61 vs. 46.14 ± 11.52), weight (77.14 ± 16.03 vs. 75.99 ± 15.80), height (1.64 ± 0.10 vs. 1.64 <b>± plusorminus </b>0.08) and BMI (28.78 ± 5.81 vs. 28.07 ± 5.27) was homogeneous. Urinary excretion of calcium (433.33 ± 141.92 vs. 188.93 ± 53.09), sodium (280.08 ± 100.94 vs. 200.44.93 ± 65.81), uric acid (880.63 ± 281.50 vs. 646.74 ± 182.76) and magnesium (88.78 ± 37.53 vs. 64.34 ± 31.84) was significantly higher in the IH group (p < 0.05). There was no statistical difference in calcium intake between the groups, and there was significantly higher salt intake in patients with IH than in NC.</p> <p>Conclusions</p> <p>This study showed that salt intake was higher in patients with IH as compared to NC.</p
Therapeutic strategies to slow chronic kidney disease progression
Childhood chronic kidney disease commonly progresses toward end-stage renal failure, largely independent of the underlying disorder, once a critical impairment of renal function has occurred. Hypertension and proteinuria are the most important independent risk factors for renal disease progression. Therefore, current therapeutic strategies to prevent progression aim at controlling blood pressure and reducing urinary protein excretion. Renin-angiotensin-system (RAS) antagonists preserve kidney function not only by lowering blood pressure but also by their antiproteinuric, antifibrotic, and anti-inflammatory properties. Intensified blood pressure control, probably aiming for a target blood pressure below the 75th percentile, may exert additional renoprotective effects. Other factors contributing in a multifactorial manner to renal disease progression include dyslipidemia, anemia, and disorders of mineral metabolism. Measures to preserve renal function should therefore also comprise the maintenance of hemoglobin, serum lipid, and calcium-phosphorus ion product levels in the normal range
Fatores de risco para doenças crônicas não-transmissíveis: inquérito domiciliar no Município de São Paulo, SP (Brasil). Metodologia e resultados preliminares
MALATTIA RENALE CRONICA (CKD), FOSFOREMIA E CALCIFICAZIONI CARDIACHE: DATI PRELIMINARI IN UNO STUDIO DI PREVALENZA MULTICENTRICO
INTRODUZIONE. I livelli di fosforemia considerati finora “normali”
necessitano di una rivalutazione anche alla luce dell’azione di fattori
di crescita ad azione fosfaturica, come l’FGF23, i cui livelli cominciano
ad aumentare ben prima di quelli della fosforemia e/o del PTH
sierico. Di seguito verrà descritto l’assetto osteo – metabolico di una
popolazione di pazienti affetti da Malattia Renale Cronica (CKD) in
trattamento conservativo.
PAZIENTI E METODI. Sono stati arruolati 100 pazienti (58M/42F) con
età media 52 ± 6 anni; 10 presentavano diabete mellito in trattamento
insulinico, 88 erano ipertesi in trattamento con calcio – antagonisti,
beta – bloccanti ed alfa – litici. Tre erano affetti da entrambe
le co – morbidità, cinque presentavano una storia di cardiopatia
ischemica cronica e uno di questi era diabetico. Tutti presentavano
un GFR medio di 30 ± 4 ml/min (eGFR EPI). I pazienti sono stati sottoposti
a dosaggi di calcemia, fosforemia, FGF23 , PTH, PCR, Fosfaturia
ed ecocardiogramma.
RISULTATI. I risultati hanno evidenziato, con livelli di fosforemia considerati
“nei ranges” di normalità (3.5 – 5 mg/dl), un’elevata percentuale
di pazienti con presenza di calcificazioni cardiache. Circa il
50% dei pazienti presentava un Wilkins Score di 2, la restante metà
uno Score di 1, mentre l’interessamento valvolare aortico era presente
nel 40% dei pazienti. I livelli di PCR erano significativamente
aumentati nel 48% dei pazienti con correlazione statisticamente significativa
con i livelli di FGF23 e lo score di Wilkins. Non si sono
osservate correlazioni statisticamente significative con i livelli di fo-sfaturia e di iPTH
DISCUSSIONE. I dati in nostri possesso fotografano il quadro osteo
– metabolico di una popolazione di pazienti affetti da CKD in stadio
pre – terminale ponendo l’accento, in primo luogo, sui livelli di fosforemia
per i quali, evidentemente, vanno rivisti al ribasso i limiti considerati
come “normali”. Potrebbe, altresì, diventare fondamentale
intervenire precocemente sui livelli di FGF23 (con i chelanti del fosforo
attualmente disponibili) per rallentare la formazione delle calcificazioni
cardiovascolar
Combined therapy with benazepril and amlodipine in the treatment of hypertension inadequately controlled by an ACE inhibitor alone
In a multicenter, randomized, double-blind, placebo-controlled study, we evaluated the efficacy and tolerability of the combination of benazepril, 10 mg, and amlodipine, 2.5 or 5 mg once daily, compared with benazepril, 10 mg, monotherapy in patients with hypertension inadequately controlled with angiotensin-converting enzyme (ACE)-inhibitor monotherapy. After a 2-week placebo and 4-week single-blind benazepril, 10 mg once daily, run-in period, 448 patients, 213 men and 235 women, aged 24-73 years (mean, 55 years), with mean diastolic blood pressure (DBP) > or =95 and < or =120 mm Hg at the end of the benazepril run-in period, were randomized to receive one of the following treatments once daily for 8 weeks: (a) benazepril, 10 mg, plus placebo (BZ10); (b) benazepril, 10 mg, plus amlodipine, 2.5 mg (BZ10/AML2.5); or (c) benazepril, 10 mg, plus amlodipine, 5 mg (BZ10/AML5). Before the patients were admitted to the trial, at the end of the placebo run-in and the benazepril run-in period and at the end of weeks 4 and 8 of the treatment period, sitting and standing blood pressure (BP), heart rate (HR), and body weight were measured 22-26 h after the intake of the trial medication. Both BZ10/AML2.5 and BZ10/AML5 combinations showed better antihypertensive activity than did BZ10 monotherapy at the terminal visit as demonstrated by (a) the 24-h postdosing sitting and standing systolic BP (SBP) and DBP values, which were statistically lower with combination therapy than with BZ10; (b) the success rate, which was statistically higher with both the combinations (69.2% in the BZ10/AML2.5 and 65.8% in the BZ10/AML5 group) compared with the BZ10 group (40.5%). The tolerability was good in the three treatment groups. No significant abnormal laboratory data were detected. There was no difference in efficacy and safety/tolerability between the BZ10/AML2.5 and BZ10/AML5 group
Avaliação das condições de trabalho dos servidores braçais de instituição pública Working conditions of municipal employees: risk factors and measurement of morbidity, 1987
Descrevem-se as condições de trabalho dos servidores braçais da Prefeitura Municipal de Botucatu (PMB), Brasil, procurando identificar as condições mais freqüentes e mais graves de riscos ocupacionais. Foi analisada a morbidade registrada no ambulatório dos servidores municipais, no período de 17 de julho a 4 de dezembro de 1987; foram analisados os acidentes de trabalho sofridos e registrados pelos servidores municipais nos anos de 1984, 1985,1986 e 1987. A descrição dos setores braçais revelou que as tarefas executadas na PMB são, majoritariamente, executadas manualmente, sem tecnologia, evidenciando condições potenciais para gerar "acidentes de trabalho" e "posições antiergonômicas" como as condições de risco mais freqüentes. Os coeficientes de gravidade desses mesmos acidentes elevaram-se desde 1984, sendo o de 1987, 1,85 vezes maior que o de 1984. A análise da causa externa e natureza da lesão decorrentes dos ATs validam a descrição do processo de trabalho e das condições de risco ocupacionais observadas. Os agravos mais freqüentes registrados no ambulatório foram: hipertensão arterial, outras circunstâncias familiares não especificadas ("problemas pessoais"), lombalgia, gripe, alcoolismo crônico e gastroenterocolite aguda, sendo que todas essas doenças (exceto "problemas pessoais") foram mais freqüentes nos servidores braçais do que nos administrativos. Propõe-se a implantação de um programa de saúde ocupacional voltado aos servidores municipais da PMB.<br>The working conditions of municipal employees of Botucatu (State of S. Paulo, Brazil) are described with a view to identifying the most frequent and serious occupational risks. Office workers (clerks) were not encluded in this study. The morbidities registered in the municipal outpatient service (from July 17 through December 4, 1987) and in the occupational accident register for the period 1984-1987 were evaluated. The activities of the majority of the employees were found to be carried out manually with no technological assistance, likely to generate "occupational accidents" (OA) and related to inadequate (antiergonomic) positions. The coefficient of severity of the OA has increased so rapidly that in 1987 it was 1.85 times higher than in 1984. The analysis of the external sources and the nature of the lesions caused by the OA were closely related to the description of the activities and the risk factors observed. The most frequent ailments registered in the outpatient service were: arterial hypertension, lumbago,the common cold, chronic alcoholism, acute gastroenterecolitis and "personal difficulties".
