    Effect of external PEEP in patients under controlled mechanical ventilation with an auto-PEEP of 5 cmH2O or higher.

    In some patients with auto-positive end-expiratory pressure (auto-PEEP), application of PEEP lower than auto-PEEP maintains a constant total PEEP, therefore reducing the inspiratory threshold load without detrimental cardiovascular or respiratory effects. We refer to these patients as complete PEEP-absorbers. Conversely, adverse effects of PEEP application could occur in patients with auto-PEEP when the total PEEP rises as a consequence. From a pathophysiological perspective, all subjects with flow limitation are expected to be complete PEEP-absorbers, whereas PEEP should increase total PEEP in all other patients. This study aimed to empirically assess the extent to which flow limitation alone explains a complete PEEP-absorber behavior (i.e., absence of further hyperinflation with PEEP), and to identify other factors associated with it.One hundred patients with auto-PEEP of at least 5 cmH2O at zero end-expiratory pressure (ZEEP) during controlled mechanical ventilation were enrolled. Total PEEP (i.e., end-expiratory plateau pressure) was measured both at ZEEP and after applied PEEP equal to 80 % of auto-PEEP measured at ZEEP. All measurements were repeated three times, and the average value was used for analysis.Forty-seven percent of the patients suffered from chronic pulmonary disease and 52 % from acute pulmonary disease; 61 % showed flow limitation at ZEEP, assessed by manual compression of the abdomen. The mean total PEEP was 7 ± 2 cmH2O at ZEEP and 9 ± 2 cmH2O after the application of PEEP (p < 0.001). Thirty-three percent of the patients were complete PEEP-absorbers. Multiple logistic regression was used to predict the behavior of complete PEEP-absorber. The best model included a respiratory rate lower than 20 breaths/min and the presence of flow limitation. The predictive ability of the model was excellent, with an overoptimism-corrected area under the receiver operating characteristics curve of 0.89 (95 % CI 0.80-0.97).Expiratory flow limitation was associated with both high and complete PEEP-absorber behavior, but setting a relatively high respiratory rate on the ventilator can prevent from observing complete PEEP-absorption. Therefore, the effect of PEEP application in patients with auto-PEEP can be accurately predicted at the bedside by measuring the respiratory rate and observing the flow-volume loop during manual compression of the abdomen

    Conflict of interest regulation in European parliaments: Studying the evolution of complex regulatory regimes

The final version is available from Wiley via the DOI in this record.The growing complexity of parliamentary ethics regulation adopted over the last decades makes the systematic examination of its nature and the rationales underpinning regulatory choices an important endeavor. In this paper we introduce conceptualizations and measurements of conflict of interest (COI) regulation directed toward assuring the impartial and unbiased decisionmaking of national parliamentarians. We distinguish the strictness of rules, the nature of enforcement, sanctions, and transparency requirements as core elements defining COI regimes. Applying our framework to 27 European democracies, we select two cases for in-depth analysis in which legislators chose very different solutions in response to growing pressures to regulate themselves, to inductively explore the drivers underpinning the choice of COI mechanisms: the United Kingdom, which adopted a highly transparency-oriented regime, and Belgium, which adopted a highly sanction-oriented COI regime. Echoing neo-institutionalist perspectives, the longitudinal analyses indicate how the two democracies’ different institutional environments shape distinct answers to similar functional pressures.European Research CouncilCollege of Social Sciences and International Studies, University of Exete

    A nonlinear model for marble sulphation including surface rugosity: Theoretical and numerical results

    We consider an evolution system describing the phenomenon of marble sulphation of a monument, accounting of the surface rugosity. We first prove a local in time well posedness result. Then, stronger assumptions on the data allow us to establish the existence of a global in time solution. Finally, we perform some numerical simulations that illustrate the main feature of the proposed model

    Flow Index: a novel, non-invasive, continuous, quantitative method to evaluate patient inspiratory effort during pressure support ventilation

    Background: The evaluation of patient effort is pivotal during pressure support ventilation, but a non-invasive, continuous, quantitative method to assess patient inspiratory effort is still lacking. We hypothesized that the concavity of the inspiratory flow-time waveform could be useful to estimate patient’s inspiratory effort. The purpose of this study was to assess whether the shape of the inspiratory flow, as quantified by a numeric indicator, could be associated with inspiratory effort during pressure support ventilation. Methods: Twenty-four patients in pressure support ventilation were enrolled. A mathematical relationship describing the decay pattern of the inspiratory flow profile was developed. The parameter hypothesized to estimate effort was named Flow Index. Esophageal pressure, airway pressure, airflow, and volume waveforms were recorded at three support levels (maximum, minimum and baseline). The association between Flow Index and reference measures of patient effort (pressure time product and pressure generated by respiratory muscles) was evaluated using linear mixed effects models adjusted for tidal volume, respiratory rate and respiratory rate/tidal volume. Results: Flow Index was different at the three pressure support levels and all group comparisons were statistically significant. In all tested models, Flow Index was independently associated with patient effort (p < 0.001). Flow Index prediction of inspiratory effort agreed with esophageal pressure-based methods. Conclusions: Flow Index is associated with patient inspiratory effort during pressure support ventilation, and may provide potentially useful information for setting inspiratory support and monitoring patient-ventilator interactions

    West Indian Elm ( Guazuma ulmifolia Lamk.) is one of potentialplant producing tannin which is useful for controlling obesity. Tannin canbe produced through in vitro and this compound could be increased bycalli culture. The medium composition for calli induction was necessary toproduce the optimal calli. The aim of this research was to obtain themedium composition for calli induction through in vitro. Young leaves ofWest Indian Elm from glass house were used as explants. Murashige andSkoog (MS) medium enriched with B vitamin group was used as basicmedium. The experiments were arranged in completely randomized designin factorial pattern with ten replications. For calli induction, variousconcentration of 2,4-D (0.1; 0.3; and 0.5 mg/l) and its combination withBenzyl Adenin of 0.1 and 0.3 mg/l were used as treatments. Parametersobserved were calli diameter, structure, colour, fresh weight andperformance during culture. Analysis of tannin was conducted by usingdried samples both (in vitro and leaves from glass house) and thenextracted. The result showed that there was interaction between 2,4-D 0.3mg/l and Benzyl Adenin 0.1 mg/l on calli diameter (28.7 mm), the biggestcalli diameter after sub culture (31.9 mm), and fresh calli weight (5.02 g)eight weeks after treatments. Tannin content obtained from all of the invitro treatments (3.72 – 4.27%) was higher than tannin from leaves(2.24%)

    Typhoon at CommsNet13: Experimental experience on AUV navigation and localization

    This paper presents two acoustic-based techniques for Autonomous Underwater Vehicle (AUV) navigation within an underwater network of fixed sensors. The proposed algorithms exploit the positioning measurements provided by an Ultra-Short Base Line (USBL) transducer on-board the vehicle to aid the navigation task. In the considered framework the acoustic measurements are embedded in the communication network scheme, causing time-varying delays in ranging with the fixed nodes. The results presented are obtained with post-processing elaborations of the raw experimental data collected during the CommsNet13 campaign, organized and scientifically led by the NATO Science and Technology Organization Centre for Maritime Research and Experimentation (CMRE). The experiment involved several research institutions and included among its objectives the evaluation of on-board acoustic USBL systems for navigation and localization of AUVs. The ISME groups of the Universities of Florence and Pisa jointly participated to the experiment with one Typhoon class vehicle. This is a 300 m depth rated AUV with acoustic communication capabilities originally developed by the two groups for archaeological search in the framework of the THESAURUS project. The CommsNet13 Typhoon, equipped with an acoustic modem/USBL head, navigated within the fixed nodes acoustic network deployed by CMRE. This allows the comparison between inertial navigation, acoustic self-localization and ground truth represented by GPS signals (when the vehicle was at the surface)

    miRNA-guided reprogramming of glucose and glutamine metabolism and its impact on cell adhesion/migration during solid tumor progression

    MicroRNAs (miRNAs) are small, non-coding RNAs about 22 nucleotides in length that regulate the expression of target genes post-transcriptionally, and are highly involved in cancer progression. They are able to impact a variety of cell processes such as proliferation, apoptosis and differentiation and can consequently control tumor initiation, tumor progression and metastasis formation. miRNAs can regulate, at the same time, metabolic gene expression which, in turn, influences relevant traits of malignancy such as cell adhesion, migration and invasion. Since the interaction between metabolism and adhesion or cell movement has not, to date, been well understood, in this review, we will specifically focus on miRNA alterations that can interfere with some metabolic processes leading to the modulation of cancer cell movement. In addition, we will analyze the signaling pathways connecting metabolism and adhesion/migration, alterations that often affect cancer cell dissemination and metastasis formation
