    Sudden Unexpected Natural Death in the Youth; an Iranian Single Center Investigation

    Background: Sudden unexpected natural death (SUND) has not been studied in Iran. Herein we investigated its main causes in our country.Methods: Records of 80 cases registered to a single referral center were investigated to determine the distribution of sex, age, and etiology of death.Results: Fifty eight (72.5%), 6 (7.5%), 6 (7.5%) and 4 (5%) of our cases have died due to various types of heart diseases, cerebral events, pulmonary emboli and gastrointestinal bleeding (GIB), respectively. Moreover, men are victims of SUND more that women (83.7% vs.16.3%, respectively).Conclusion: Policies should be planned by the governments to prevent youth mortality in societies. These attempts should especially target ischemic heart disease

    Establishing and maintaining patent upper airway is the first and most vital step in the basic life support and maintenance of artificial airway is the most fundamental aspect of such support. The term tracheotomy is derived from the Greek word TOME (to cut) and implies the performance of a non permanent type of surgery. The term tracheotomy was actually coined by Lorenz Heister in 1718. The term tracheostomy is derived from the word STOMOUN (to furnish with an opening) and implies a more permanent opening into the trachea. The international organization for standards has named the act of cutting a hole a tracheotomy. The actual hole and the tube are both called tracheostomy. Patients with upper airway obstructions commonly have acute primary uncompensated respiratory acidosis. _ Tracheostomy under L.A. rapidly improved the acid base and ventilatory status by relieving the obstruction as evidenced by statistically significant improvement in ABG value. _ ABG is very useful diagnostic tool in upper airway obstruction

    Potential application of pomegranate seed oil oleogels based on monoglycerides, beeswax and propolis wax as partial substitutes of palm oil in functional chocolate spread

    In this research, the effectiveness of pomegranate seed oil oleogel as a partial replacement of fat phase in chocolate spread was studied. Monoglyceride (MG), beeswax (BW) and propolis wax (PW) have been used as structuring agents at 5 g/100 g concentration to gel pomegranate seed oil. The oleogels were then combined with palm oil at 1:1 ratio. Different techniques, including polarized light microscopy, synchrotron XRD, mechanical analyses, and oil binding capacity were used to study the physical and mechanical properties of the palm oil-oleogel systems and chocolate spreads. Results highlighted that MG, BW and PW chemical nature led to the formation of different crystalline network in palm oil-oleogel systems. Chocolate spreads containing palm oil-oleogels showed an increase in the mechanical parameters in the order of PW < BW < MG. This trend might be attributed to the chemical composition of oleogelators and physical bonds formed in the samples. During storage, crystal transformation in MG and structural reorganization in waxes (PW and BW) samples showed a gradual decrease and increase in hardness, respectively. These findings could provide useful information in the application of pomegranate seed oil oleogel for novel confectionery products engineering

    Do hands-free cell phone conversation while driving increase the risk of cognitive distraction among drivers? A cross-over quasi-experimental study

    Background: Background: Conversation over the cell phone while driving is a known risk factor for road traffic crash. Using handsfree to talk on the cell phone may remove visual and manual distraction yet not the cognitive distraction. Objectives: The purpose of this study was to better understand the mechanisms of cognitive distraction due to hands-free cell phone conversation (HFPC) while driving. Methods: Twelve male and 12 female University students in Tehran, Iran, were selected via the consecutive convenient sampling method, and randomly assigned to one of the following administrations of cross-over quasi-experimental study design, during year 2016: Administration 1, participants performed the tests while involved in HFPC, took rest for 60 minute, and then performed the tests another time without HFPC; Administration 2, participants performed the tests without HFPC, took rest, and then performed the tests another time with HFPC. Each participant moved to the other administration after 7 to 10 days. The Vienna test system (VTS) was used to measure simple and choice reaction time, selective attention, visual orientation, and visual memory. Linear regression analysis was used to study the change in test scores due to HFPC. Results: The mean age of participants was 27.1±5.3 years. A history of road traffic crash (regardless of severity) was reported among 9 (37.5) participants in the previous year. Hands-free cell phone conversation while driving was directly associated with mean time correct rejection score (P < 0.01) (selective attention), omitted response (P < 0.01) and median reaction time (P < 0.01) (choice reaction time), and mean reaction time (P < 0.01) (Simple reaction time). Moreover, HPFC was inversely associated with sum hits (P = 0.05) (selective attention), incorrect (P < 0.02) and correct response (P < 0.01) (choice reaction time), score based on viewing and working time (P < 0. 01) (visual orientation), and visual memory performance (P < 0.01). Conclusions: Using hands-free devices to converse during driving, impaired reaction time, selective attention, visual orientation and visual memory, which are essential for safe driving. Thus, the use of these devices does not preclude cognitive distraction and should be restricted. © 2018, Iranian Red Crescent Medical Journal

    Stability analysis of surface ion traps

    Motivated by recent developments in ion trap design and fabrication, we investigate the stability of ion motion in asymmetrical, planar versions of the classic Paul trap. The equations of motion of an ion in such a trap are generally coupled due to a nonzero relative angle θ\theta between the principal axes of RF and DC fields, invalidating the assumptions behind the standard stability analysis for symmetric Paul traps. We obtain stability diagrams for the coupled system for various values of θ\theta, generalizing the standard qq-aa stability diagrams. We use multi-scale perturbation theory to obtain approximate formulas for the boundaries of the primary stability region and obtain some of the stability boundaries independently by using the method of infinite determinants. We cross-check the consistency of the results of these methods. Our results show that while the primary stability region is quite robust to changes in θ\theta, a secondary stability region is highly variable, joining the primary stability region at the special case of θ=45\theta=45^{\circ}, which results in a significantly enlarged stability region for this particular angle. We conclude that while the stability diagrams for classical, symmetric Paul traps are not entirely accurate for asymmetric surface traps (or for other types of traps with a relative angle between the RF and DC axes), they are safe in the sense that operating conditions deemed stable according to standard stability plots are in fact stable for asymmetric traps, as well. By ignoring the coupling in the equations, one only underestimates the size of the primary stability region

    Effects of foliar application of melatonin on gas exchange and certain biochemical characteristics broccoli cv. Palam Samridhi

    Considering the rich nutritional status and possibility of broccoli in improving the profitable yield, and wide role of Mel in regulating the plant physiological process, an investigation was carried out at the division of Basic Sciences and Humanities during 2017 to investigate the effect of foliar application of Mel on leaf photosynthetic and biochemical attributes broccoli. Thirty days old and uniform seedlings of broccoli cv. Palam Samridhi were transplanted in the field at a spacing of 45 × 45cm. Different concentrations of Mel, viz. 0, 20, 40, 60 and 80 ppm were sprayed on the plant foliage at 15 days after transplanting (DAT) replicating each treatment four times. Leaf gas exchange and biochemical attributes were tested following the standard procedures. The Results showed the lowest stipulated rate of photosynthesis (10.87 µmole.m-2.sec-1), stomatal conductance (301.44 mole H2O.m-2ses-1) and leaf transpiration (1. 14 mole H2O.m-2ses-1) in untreated plants.  Different doses of Mel significantly increased the values of these attributes and the highest values of photosynthesis (18.63 µmole.m-2.sec-1), stomatal conductance (324.37 mmole.m-2.ses-1) and leaf transpiration (3.23 mmole.m-2.ses-1) with Mel 60 ppm were recorded. The alterations in different biochemical attributes were also evident due to foliar application of Mel and maximum leaf sugar (77.0 and 85.9µg/g), protein (56.9 and 77.3 µg/g), total phenols (260.1 and 339.9 mg/100g), antioxidants (142.8 and 159.9 mg GAE /100g DW) and MSI (94.89 and 97.43 percent) values with Mel 60ppm at 30 and 60DAT, respectively.     There Is Selective Increase in Pro-thrombotic Circulating Extracellular Vesicles in Acute Ischemic Stroke and Transient Ischemic Attack: A Study of Patients From the Middle East and Southeast Asia.

    Stroke attacks were found to be present at a younger age in patients from Southeast Asia (SE) and the Middle East (ME) resident in the state of Qatar. Extracellular vesicles (EVs), which are small membrane vesicles with pro-thrombotic properties, may contribute to the high risk of stroke in this population. Thus, total and cell-specific medium size EVs were counted by flow cytometry in platelet-free plasma from healthy volunteers and patients with transient ischemic attacks (TIA) and acute ischemic stroke (AIS) from SE and ME. Acutely, within 48 h of attacks, there was an increase in total endothelial EVs in TIA (6.73 ± 1.77; = 0.0156; = 21) and AIS (11.23 ± 1.95; = 0.0007; = 66) patients compared to controls (2.04 ± 0.78; = 24). Similar increases were also evident in EVs originating from platelets, erythrocytes, granulocytes, and leukocytes. Compared to controls, there was also an increase in EVs derived from activated endothelial cells, platelets, granulocytes, leukocytes, and pro-coagulant EVs (Annexin V) at 5 and 30-days following the acute events, while a decrease was observed in erythrocyte-derived EVs. This is the first study characterizing EVs in TIA and AIS patients from ME and SE showing an increase in EVs associated with endothelial and platelet cell activation, which may contribute to the elevated risk of stroke at a younger age in this population.Qatar University high collaborative grant (QUCG-CPH-2018\2019-2
