Weakly coupled states on branching graphs

We consider a Schr\"odinger particle on a graph consisting of $\,N\,$ links joined at a single point. Each link supports a real locally integrable potential $\,V_j\,$; the self--adjointness is ensured by the $\,\delta\,$ type boundary condition at the vertex. If all the links are semiinfinite and ideally coupled, the potential decays as $\,x^{-1-\epsilon}$ along each of them, is non--repulsive in the mean and weak enough, the corresponding Schr\"odinger operator has a single negative eigenvalue; we find its asymptotic behavior. Funktionelle Bauchwandrekonstruktion bei großen Narbenhernien

Einleitung Für die Versorgung großer Narbenhernien der Bauchwand sind immer wieder neue Operationstechniken entwickelt worden. Die häufig verwendeten Alloplastiken können zwar einen statischen, jedoch keinen funktionell-muskulären Ersatz leisten und sind darüber hinaus mit einer hohen Komplikationsrate belastet. Material und Methoden In einer retrospektiven, monozentrischen Studie haben wir die Ergebnisse einer neuen Operationstechnik untersucht, die das Prinzip der intraoperativen Expansion für die funktionelle Reparatur der Bauchwand mit innervierter Rectus-abdominis-Muskulatur nutzt. Die Adaptation der originären Bauchwandmuskulatur in der Mittellinie wird dabei durch intermittierenden Zug mit 3mm breitem Vicrylband erreicht.Das Patientenkollektiv umfaßt 39 Patienten im Alter von 33 - 88 Jahren, die von 1993 bis 2002 in unserer Klinik versorgt wurde . Die Größe der Narbenhernien maß bis zu 26 x 20 cm. Alle Patienten waren bereits mehrfach voroperiert. Die Beurteilung der OP-Ergebnisse erfolgte sowohl mit der funktionellen Myosonografie als auch mit konventionellen Funktionsanalysen (Janda und Lacôte) . Ergebnisse Von den 39 untersuchten Patienten verstarben 2 Patienten, 4 Patienten (10,2 %) erlitten ein Rezidiv. Die rezidivfreie Zeit reichte von 5 - 95 Monaten. Die Myosonografie zeigte eine erhaltene Funktion der Rektusmuskulatur postoperativ mit entsprechender Dickenzunahme um 1.9 mm im Mittel und Veränderung der Muskelbreite um 4.4 mm bei willkürlicher Kontraktion. In der Funktionsanalyse erreichten die operierten Patienten auf der Skala nach Janda und Lacôte mindestens den Wert von 3, im Mittel 4 Punkte. Schlussfolgerung Die mechanischen Eigenschaften des 3mm-Vicrylbandes erlauben die intraoperative Gewebedehnung der verbliebenen Rektusmuskulatur. Im Gegensatz zu den verwendeten Alloplastiken sind durch den Einsatz resorbierbaren Nahtmaterials weniger Komplikationen zu erwarten. Die niedrige Rezidivrate und die deutliche Verbesserung der Lebensqualität der mehrfach voroperierten Patienten wird von uns auf die funktionelle Rekonstruktion der Rektusmuskulatur zurückgeführt, die durch alloplastische Verfahren bisher nicht erreicht wurde

Hebedefektmorbidität beim DIEP-flap

Berlin, 22.-25.04.2008 Einleitung: Ein wesentliches Ziel der modernen Perforatorlappen vom Unterbauch (DIEP-flap) für die Brustrekonstruktion nach Mammaamputation ist die Schonung der Rektusmuskulatur. Der Funktionserhalt der Muskulatur ist abhängig von der Präparationstechnik. In unserer Studie wird die Interaktion zwischen der Muskel- und Nervendurchtrennung und der postoperativen Muskelfunktion untersucht. Material und Methoden: Unser Patientenkollektiv umfasst 42 Patienten. Im Zeitraum von 6/04 bis 06/07 wurden 44 DIEP-Lappen an unserer Klink nach dem gleichen operativen Standard von unterschiedlichen Operateuren zur Brustrekonstruktion transferiert. Die Standards beinhalten die beidseitige Präparation der Perforatorgefäße des Unterbauches, der SIEA-Gefäße, die Auswahl der 2–4 kräftigsten Perforatoren einer Seite und die schonende Präparation der Rektusmuskulatur und der motorischen Nervenäste.In einer prospektiven monozentrischen Studie haben wir die Rektusmuskulatur präoperativ und 6 Monate postoperativ untersucht. Für die Funktionsanalyse wurde sowohl die Myosonografie der Rektusmuskulatur als auch eine klinischen Untersuchung angewandt. Intraoperativ wurde die Anzahl und Lokalisation der Perforatoren, die Länge der gespreizten Muskulatur, die Länge der durchtrennten Muskulatur und die Anzahl und Lokalisation der durchtrennten intramuskulären Nerven in einer Skizze eingetragen. Die Relation zwischen der intraoperativen Muskel- und Nervenschädigung und der postoperativen Funktion wurde analysiert. Ergebnisse: Bei der Hebung des DIEP – flaps wurden im Durchschnitt 10,8 cm Muskulatur gespreizt, 8,2 cm Muskulatur getunnelt und 2,5 cm Muskulatur durchtrennt. In 41% (18 Pat) wurde 1 motorischer Nervenast durchtrennt, in 27,3% (12 Pat) waren es 2 und in 13,6% (6 Pat) 3 Nervenäste. Bei der klinischen Untersuchung 6 Monate postoperativ hatten 8 Patientinnen noch funktionelle Störungen beim Heben schwerer Gegenstände. Myosonografisch fand sich bei 3 Patientinnen eine Funktionsminderung: 1 vollständiger Funktionsverlust der Muskulatur mit Relaxatio, 2 relevante Minderungen der Kontraktilität Bei keiner Patientin fand sich eine Bauchwandhernie. Bei allen Patientinnen mit einer Beeinträchtigung der Muskulatur waren mind. 2 motorische Nervenäste durchtrennt worden. Schlussfolgerung: Die klinische und myosonografische Funktionsanalyse der Bauchwand ermöglicht die Erstellung von Standards zur verbesserten Operationstechnik. Unsere Ergebnisse zeigen, dass die Durchtrennung von 2 oder mehr motorischen Nervenästen vermieden werden muß. Die Länge der durchtrennten und gespreizten Muskulatur ist dagegen von geringerer Bedeutung

Small-Energy Analysis for the Selfadjoint Matrix Schroedinger Operator on the Half Line

The matrix Schroedinger equation with a selfadjoint matrix potential is considered on the half line with the most general selfadjoint boundary condition at the origin. When the matrix potential is integrable and has a first moment, it is shown that the corresponding scattering matrix is continuous at zero energy. An explicit formula is provided for the scattering matrix at zero energy. The small-energy asymptotics are established also for the corresponding Jost matrix, its inverse, and various other quantities relevant to the corresponding direct and inverse scattering problems.Comment: This published version has been edited to improve the presentation of the result

Kinematics and deformation structures in a crustal-scale shear zone on Kea (W. Cyclades, Greece)

It is generally agreed upon that the exhumation of metamorphic rocks in the Aegean is caused by post orogenic extension in the late Oligocene to early Miocene. This extension is in principle largely accommodated by low-angle crustal detachment faulting possibly resulting in the formation of metamorphic core complexes (MCC). Here, we present data from recent structural investigations on the island of Kea in the W. Cyclades, Greece. Our work focussed in the north of the island. Of the ca. 270m total structural thickness that was mapped, the entire section of rocks are highly strained. Exhumation during progressive deformation is recorded by the transition from ductile to brittle/ductile to brittle conditions. Late stage evolution of the Serifos Metamorphic Core Complex (Cyclades, Greece)

The island of Serifos is located in the Western Cyclades within the Attic- Cycladic metamorphic belt. It represents the westward continuation of an arcuate belt of Metamorphic Core Complexes with intrusions of late syn-post tectonic intrusions younging from East (e.g. Naxos main activity ca. 12Ma) to West (e.g. Serifos with 9–8Ma). In scientific discussions the dominance of probably continuous extension since ca. 30Ma (e.g. Jolivet & Faccenna, 2000) and the presence of Metamorphic Core Complexes (Lister et al. 1984) is accepted. The speculated roll-back of the subducting plate possibly started due to the slowing down of absolute plate convergence rate between Africa and Eurasia. This model is attractive, because it would also explain the shift from a compressional Andean-type regime to an extensional Mariana-type regime (Jolivet & Faccenna 2000). Contrary to the kinematic directions reported from the Central and Eastern Cyclades, the movement of the hanging wall of the Serifos Metamorphic Core Complex is south directed. The island’s main part is occupied by an undeformed granodiorite. Early granitic intrusions intruded into low-grade M2-crystalline rocks that have been overprinted to as high as amphibolite facies conditions due to contact metamorphism. Anomalous electron trapping by localized magnetic fields

We consider an electron with an anomalous magnetic moment g>2 confined to a plane and interacting with a nonzero magnetic field B perpendicular to the plane. We show that if B has compact support and the magnetic flux in the natural units is F\ge 0, the corresponding Pauli Hamiltonian has at least 1+[F] bound states, without making any assumptions about the field profile. Furthermore, in the zero-flux case there is a pair of bound states with opposite spin orientations. Using a Birman-Schwinger technique, we extend the last claim to a weak rotationally symmetric field with B(r) = O(r^{-2-\delta}) correcting thus a recent result. Infection prevention during anaesthesia ventilation by the use of breathing system filters (BSF): Joint recommendation by German Society of Hospital Hygiene (DGKH) and German Society for Anaesthesiology and Intensive Care (DGAI)

An interdisciplinary working group from the German Society of Hospital Hygiene (DGKH) and the German Society for Anaesthesiology and Intensive Care (DGAI) worked out the following recommendations for infection prevention during anaesthesia by using breathing system filters (BSF). The BSF shall be changed after each patient. The filter retention efficiency for airborne particles is recommended to be >99% (II). The retention performance of BSF for liquids is recommended to be at pressures of at least 60 hPa (=60 mbar) or 20 hPa above the selected maximum ventilation pressure in the anaesthetic system
