Lymphogranuloma venereum: első hazai esetek
Lymphogranuloma venereum is a sexually transmitted infection caused by the Chlamydia trachomatis serovars L1-3. It has been found to be endemic in tropical countries. In the last decades several cases have been reported in Western Europe, particularly in men who have sex with men population infected with human immunodeficiency virus. The authors present three cases of lymphogranuloma venereum infections, observed at their department in 2013 and 2014. The three human immunodeficiency virus infected patients who belonged to men who have sex with men population had casual sexual contacts in Western Europe. The symptoms included urethral discharge, discomfort and inguinal lymphadenomegaly in two patients, and rectal pain, discharge and perianal ulceration in one patient. The diagnosis was confirmed by nucleic acid amplification test performed in samples obtained from urethral discharge and exudate of perianal ulcer; lymphogranuloma venereum 2b serovars were demonstrated in two patients and serovar 2 in one patient. Doxycyclin (daily dose of two times 100 mg for 21 days) resolved the symptoms in all cases. The authors conclude that lymphogranuloma venereum is a diagnostic challenge in Hungary, too. It is important to be aware of the altered clinical features of this disease to prevent complications and spreading
A szelektív monoaminoxidáz-B-gátlók helye a Parkinson-kór kezelési stratégiájában a marosvásárhelyi ideggyógyászati klinikák gyakorlatában = The role of selective monoamine oxidase B inhibitors in the therapeutic strategy of Parkinson’s disease in the neurology clinics of Tirgu Mures County Emergency Clinical Hospital
Absztrakt:
Bevezetés: A Parkinson-kór kezelési stratégiájában a szelektív
monoaminoxidáz-B-gátlóknak a betegség minden stádiumában jól meghatározott
helyük van. Enyhe esetekben, főleg fiatal betegeknél, a szubsztitúciós terápia
késleltetésének egyik hatékony eszközeként számolhatunk velük; előrehaladott
Parkinson-kórban, a motoros komplikációk ellátásában, a levodopaterápia
kiegészítői. Célkitűzés: Annak felmérése, hogy a
marosvásárhelyi ideggyógyászati klinikákon alkalmazott terápiás stratégiákban
mekkora szerep jut a szelektív monoaminoxidáz-B-gátlóknak.
Módszer: Retrospektív tanulmányunkban 2003. január 1. és
2016. december 31. között a klinikákon vizsgált összes Parkinson-kóros beteg
adatait elemeztük. A 2194 beteg zárójelentésében rögzített terápiás ajánlások
alapján tanulmányoztuk a monoaminoxidáz-B-gátlók alkalmazásának sajátosságait. A
Parkinson-kór megállapítása óta eltelt idő szerint öt éve, illetve több mint öt
éve tartó betegségcsoportokat alkottunk. Eredmények: A vizsgált
időszakban az öt éve vagy ennél rövidebb ideje diagnosztizált csoportban 1183
betegből 243 esetben szerepelt a kezelési stratégiában monoaminoxidáz-B-gátló:
12 esetben monoterápia, 52 esetben dopaminagonistával, illetve 61 esetben
levodopával kombinálva. A többi 118 betegnél levodopa és dopaminagonista
kombinációjához társítva kerültek alkalmazásra a monoaminoxidáz-B-gátlók. A több
mint öt éve ismert 582 esetből 195-nél egészítették ki a terápiás stratégiát
monoaminoxidáz-B-gátlóval (10 esetben szelegilin, 185 esetben rasagilin). Nem
volt felhasználható adat a betegség kezdetét illetően 429 esetben (ezek közül öt
esetben szelegilint, illetve 93 esetben rasagilint alkalmaztak).
Következtetés: A vizsgált periódusban a
monoaminoxidáz-B-gátlók alkalmazásának aránya hasonló az irodalomban talált
adatokhoz. A betegséggel foglalkozó szakorvosoknak nagyobb bátorsággal kellene
alkalmazniuk a rendelkezésre álló és az ajánlásokban szereplő készítményeket,
jobban kihasználni a különböző gyógyszertársítások előnyeit, különösen, ha ez

Abstract:
Introduction: Selective monoamine oxidase B inhibitors have an
accurate place in therapeutical strategy of Parkinsons’s disease. In the early
stages of the disease, especially in younger patients with milder symptoms, the
introduction of levodopa substitution could be efficacious in delaying; in
advanced stages they are mainly used to treat motor complications, as an adjunct
to levodopa. Aim: The evaluation of therapeutical strategies
used in the neurology clinics of Tirgu Mures County Emergency Clinical Hospital
in order to define the role of monoamine oxidase B inhibitors.
Method: This retrospective study includes all records of
patients with Parkinson’s disease hospitalized between 1 January 2003 and 31
December 2016. From the 2194 reports we used data focusing on the therapeutic
recommendations. Regarding disease duration, we divided the patients in two
groups: less than or equal to 5 years and more than 5 years.
Results: From the 1183 patients in first group, 243
received monoamine oxidase inhibitors: 12 as monotherapy, 52 together with
dopamine agonists, in 61 cases combined with levodopa. In 118 cases monoamine
oxidase inhibitors were combined with levodopa and dopamine agonists. From 582
cases whith Parkinson’s disease for more than 5 years, 195 received monoamine
oxidase B inhibitors (selegiline: 10 cases, rasagiline: 185 cases). In 429 cases
we did not find accurate data regarding disease duration (selegiline: 5 cases,
rasagiline: 93 cases). Conclusion: The use of monoamine oxidase
B inhibitors was similar to those found in literature. The treating physicians
should utilise more confidently the available therapeutical combinations. Orv

Szomatikus onkogén mutációk összehasonlító vizsgálata egészséges és tumoros pajzsmirigyszövetmintákban = Comprehensive examination of somatic oncogene mutation in normal and pathologic thyroid tissues
Az elmúlt években több munkacsoportnak sikerült olyan szomatikus mutációkat (BRAF, NRAS, HRAS, KRAS génekben) és génátrendeződéseket (RET/PTC, PAX8/PPAR-gamma) azonosítani, amelyek összefüggést mutatnak a pajzsmirigydaganatok kialakulásával. Jelen vizsgálatban 11 személy 22 (11 kóros és 11 betegségmentes) intraoperatív pajzsmirigy-szövetmintáit elemezték. A RAS géncsalád és a BRAF gének szomatikus egypontos nukleotid polimorfizmusait LigthCycler olvadáspontanalízis-módszerrel, míg a génátrendeződéseket valós idejű polimeráz láncreakció módszerével vizsgálták. A daganatos mintákban 3 BRAF-, 2 NRAS-, 1 HRAS-mutációt, valamint 1 RET/PTC1 átrendeződést találtak. Az eredmények megerősítik a nemzetközi adatokat, miszerint ezek az egypontos nukleotidpolimorfizmusok és génátrendeződések megtalálhatók a daganatos pajzsmirigyszövetekben. Valószínűsíthető, hogy ezen genetikai vizsgálatokkal kiegészült citológiai vizsgálat segítheti a malignus göbök azonosítását, illetve elképzelhető, hogy prognosztikai faktorként előre jelezhetik a későbbi daganatos átalakulást. It is established that numerous somatic oncogene mutation (BRAF, NRAS, HRAS, KRAS) and gene translocations (RET/PTC, PAX8/PPAR-gamma) are associated with the development of thyroid cancer. In this study 22 intraoperative thyroid tissue samples (11 pathologic and 11 normal) were examined. Somatic single nucleotide polymorphisms were analyzed by LigthCycler melting method, while translocations were identified by real-time polymerase chain reaction technique. In tumorous sample 3 BRAF, 2 NRAS and one HRAS mutations were found, as well as one RET/PTC1 translocation. Results confirm international data showing that these oncogene mutations and translocations are linked to thyroid cancer. Cytological examination completed with genetic data may support the diagnosis of thyroid malignancies. The anti-inflammatory effect of dimethyl trisulfide in experimental acute pancreatitis
Various organosulfur compounds, such as dimethyl trisulfide (DMTS), display anti-inflammatory properties. We aimed to examine the effects of DMTS on acute pancreatitis (AP) and its mechanism of action in both in vivo and in vitro studies. AP was induced in FVB/n mice or Wistar rats by caerulein, ethanol-palmitoleic acid, or L-ornithine-HCl. DMTS treatments were administered subcutaneously. AP severity was assessed by pancreatic histological scoring, pancreatic water content, and myeloperoxidase activity measurements. The behaviour of animals was followed. Pancreatic heat shock protein 72 (HSP72) expression, sulfide, and protein persulfidation were measured. In vitro acinar viability, intracellular Ca 2+ concentration, and reactive oxygen species production were determined. DMTS dose-dependently decreased the severity of AP. It declined the pancreatic infiltration of leukocytes and cellular damage in mice. DMTS upregulated the HSP72 expression during AP and elevated serum sulfide and low molecular weight persulfide levels. DMTS exhibited cytoprotection against hydrogen peroxide and AP-inducing agents. It has antioxidant properties and modulates physiological but not pathophysiological Ca 2+ signalling. Generally, DMTS ameliorated AP severity and protected pancreatic acinar cells. Our findings indicate that DMTS is a sulfur donor with anti-inflammatory and antioxidant effects, and organosulfur compounds require further investigation into this potentially lethal disease
A pajzsmirigygöbök genetikai vizsgálata újgenerációs szekvenáláson alapuló platformon kifejlesztett génpanel segítségével = Genetic testing of thyroid nodules using a gene panel developed on a new generation sequencing platform
Absztrakt:
Bevezetés: Vékonytű-biopsziás pajzsmirigygöbminták 25%-ánál
bizonytalan citológiai eredményt kapunk. A göbök genetikai vizsgálata
hozzájárulhat a pontos diagnózishoz. Célkitűzés: Európában az
első és a legtöbb, 23 releváns pajzsmirigyonkogént (568 mutációval) tartalmazó
génpanel kialakítása. Módszer: A biopsziás mintákból izolált
DNS vizsgálata Ion Torrent újgenerációs szekvenálással történt.
Eredmények: Módszerünk validációját tumorszövetmintákon
végeztük, ennek során 127, a pajzsmirigydaganatokban eddig nem ismert
eltéréseket azonosítottunk. Az AXIN1 a legpolimorfabb génnek,
míg a BRAF c.1799T>A (V600E) a leggyakrabban azonosított
mutációnak bizonyult. A vékonytű-biopsziás mintáinkban 36-féle, klinikailag
releváns variánst detektáltunk, melyek 75%-a az irodalomban még nincs leírva. A
citológiailag malignus nyolc mintánk közül hatban, a bizonytalan citológiájú
tizennégy mintánk közül nyolcban, míg a citológiailag benignus huszonnyolc
mintánk közül húszban azonosítottunk patogén variánst valamely driver génben
(BRAF c.1799T>A, NRAS c.181C>A).
Következtetés: Olyan validált, megbízhatóan működő
újgenerációs szekvenáláson alapú módszert fejlesztettünk ki, amely nagy pozitív
prediktív értékkel (89%) és szenzitivitással (79%) képes a pajzsmirigy
rosszindulatú elváltozásainak korai felismerésére. Orv Hetil. 2019; 160(36):

Abstract:
Introduction: Twenty-five percent of fine-needle aspiration
biopsy samples of thyroid nodules produce indeterminate cytological results.
Genetic testing of nodules can contribute to accurate diagnosis.
Aim: Developing the first gene panel in Europe utilizing
the 23 most relevant thyroid oncogenes with 568 mutations.
Method: Examination of the isolated DNA from biopsy samples
by Ion Torrent new generation sequencing. Results: The
validation of our method was performed on tumor tissue samples, in which 127
genetic variations were identified, yet unknown in thyroid tumors.
AXIN1 was the most polymorphic gene, while
BRAF c.1799T>A (V600E) was the most frequently
identified mutation. We detected 36 clinically relevant variants, 75% of which
have not been described in the literature. Six of our 8 cytologically malignant
and 8 of our 14 indeterminate as well as 20 of our 28 cytologically benign
samples were identified as containing pathologic variants in a driver gene
(BRAF c.1799T>A, NRAS c.181C>A).
Conclusion: We have developed a validated, reliable new
generation sequencing-based method with high positive predictive value (89%) and
sensitivity (79%), suitable for the early detection of malignant lesions in the
