    Self-Reported Health Status in Primary Health Care: The Influence of Immigration and Other Associated Factors

    OBJECTIVE: The aims of this study are to compare self-reported health status between Spanish-born and Latin American-born Spanish residents, adjusted by length of residence in the host country; and additionally, to analyse sociodemographic and psychosocial variables associated with a better health status. DESIGN: This is a cross-sectional population based study of Latin American-born (n = 691) and Spanish-born (n = 903) in 15 urban primary health care centres in Madrid (Spain), carried out between 2007 and 2009. The participants provided information, through an interview, about self-reported health status, socioeconomic characteristics, psychosocial factors and migration conditions. Descriptive and multiple logistic regression analyses were conducted. RESULTS: The Spanish-born participants reported a better health status than the Latin America-born participants (79.8% versus 69.3%, p<0.001). Different patterns of self-reported health status were observed depending on the length of residence in the host country. The proportion of immigrants with a better health status is greater in those who have been in Spain for less than five years compared to those who have stayed longer. Better health status is significantly associated with being men, under 34 years old, being Spanish-born, having a monthly incomes of over 1000 euros, and having considerable social support and low stress. CONCLUSIONS: Better self-reported health status is associated with being Spanish-born, men, under 34 years old, having an uppermiddle-socioeconomic status, adequate social support, and low stress. Additionally, length of residence in the host country is seen as a related factor in the self-reported health status of immigrants

    Propiciar la mutua comunicación de conocimientos e investigaciones en torno al tema propuesto de historia y sistema. Comunicación de métodos de investigación, formas de planteamiento y explicación de los problemas. Reflexión educativa a partir del tema estudiado. Estímulo, ocasión y marco adecuado para el estudio, la investigación y el perfeccionamiento mutuo. 20 profesores universitarios. La presente publicación reúne los informes, guiones y material bibliográfico utilizados en el seminario. El tratamiento didáctico ha intentado responder a una metodología activa y participativa. Fases. A) Problemática epistemológica: tradiciones históricas en las que se genera esta problemática, aplicaciones de la investigación al espacio de dos ciencias, Economía y Lingüística, teoría del aprendizaje. B) Implicaciones pedagógicas en la problemática estudiada con fundamentación teórica activa. Textos de Hegel, Chomsky, Zubiri, Marx, etc. Análisis teórico. Es una teoría del sujeto, la única que puede hacer posible la solución de los problemas que se plantean hoy a nivel epistemológico (teoría de la ciencia) y pedagógico (teoría del aprendizaje). Una característica fundamental del individuo, en cuanto sujeto epistemológico es la capacidad para construir a través de sucesivas abstracciones reflexivas o formalizantes estructuras cada vez más complejas en los diversos campos en que se mueve. Cuando se trata de estructuras de índole lógico matemático, predominan las relaciones de implicación, mientras que en el caso de estructuras psicológicas, sociológicas, históricas, se trata sobre todo de relaciones causales y genéticas con vistas a la articulación orgánica de las mismas. En definitiva, la articulación de la historia y el sistema, tanto a nivel epistemológico como pedagógico, solamente es posible si se dispone de una adecuada teoría del sujeto, cosa que pensamos no se ha logrado por el momento.     High medium-term incidence of major cardiovascular events in discharged patients with unstable angina

    The introduction of high-sensitivity troponin (hsTn) assays has reduced the diagnosis of unstable angina (UA) in favor of non-ST elevation myocardial infarction (NSTEMI) in the context of non-ST elevation acute coronary syndrome (NSTEACS). It is unclear whether the detection of these hsTn levels affects the prognosis and therefore whether a different therapeutic approach is warranted. This study aims to determine whether using hsTn results in medium-term prognostic differences in patients with UA and NSTEMI. Methods: This multicenter, prospective registry study included consecutive patients who underwent hsTn assays and were discharged with a diagnosis of NSTEACS. Patients were followed for two years. Outcomes were the occurrence of major adverse cardiovascular events (MACE: cardiovascular death, non-fatal myocardial infarction, and non-fatal ischemic stroke), major bleeding, and all-cause mortality. Results: Patients with UA and NSTEMI did not show differences in terms of the invasive interventions received, the coronary artery disease diagnosed, the type of revascularization performed, or the proportion presenting MACE (UA 18.1% vs. NSTEMI 18.9%; p = 0.79). However, patients with NSTEMI had higher cardiovascular mortality at two years (UA 4% vs. NSTEMI 9.2%; p = 0.012), as well as, all-cause mortality (UA vs. 7.9% vs. NSTEMI 16.4%; p = 0.002). Conclusions: Medium-term incidence of MACE was similar in patients with UA and NSTEMI, but cardiovascular and all-cause mortality in NSTEMI patients was over twice that of patients with UA

    PubMed ID: 28578163Background Patients with spinal cord injury (SCI) who carry indwelling urinary catheters have an increased risk of urinary tract infection (UTI). Antiseptic silver alloy-coated (SAC) silicone urinary catheters prove to be a promising intervention to reduce UTIs; however, current evidence cannot be extrapolated to patients with SCI. Purpose This study aimed to assess the efficacy of SAC urinary catheters for preventing catheter-associated urinary tract infections. Design/Setting This is an open-label, multicenter (developed in Spain, Portugal, Chile, Turkey, and Italy), randomized clinical trial conducted in 14 hospitals from November 2012 to December 2015. Patient Sample Eligible patients were men or women with traumatic or medical SCI, aged ?18 years, requiring an indwelling urinary catheter for at least 7 days. Outcome Measures The primary outcome was the incidence of symptomatic UTIs. The secondary outcome included bacteremia in the urinary tract and adverse events. Materials and Methods Patients were randomized to receive a SAC urinary catheter (experimental group) or a standard catheter (control group) for at least 7 days. Data were compared using chi-squared test and also calculating the absolute risk difference with a 95% confidence interval. An adjusted analysis including different risk factors of UTI was performed. This study was mainly funded by La Marató de TV3 Foundation (grant number # 112210) and the European Clinical Research Infrastructures Network organization. The funders had no role in the interpretation or reporting of results. Results A total of 489 patients were included in the study, aged 55 years in the experimental group and aged 57 in the control group (p=.870); 72% were men; 43% were hospitalized patients, and 57% were outpatients (p=1.0). The most frequent cause of SCI was traumatic (73.75%), and the localization was mainly the cervical spine (42.74%). Most of the patients had an A score (complete spinal injury and no motor and sensory is preserved) on the ASIA scale (62.37%). The median time of urethral catheterization was 27 days in the experimental group and 28 days in the control group (p=.202). Eighteen patients (7.41%) in the experimental group and 19 in the control (7.72%) group had a symptomatic UTI (odds ratio [OR] 0.96 [0.49–1.87]). The adjusted analysis revealed no change in the results. Only three patients in the experimental group had bacteremia within the urinary tract. The experimental group presented more adverse events related to the use of a catheter than the control group (OR 0.03 [0.00–0.06]). Conclusions The results of this study do not support the routine use of indwelling antiseptic SAC silicone urinary catheters in patients with SCI.
