    Archimedes: a feasibility study of an experiment to weigh the electromagnetic vacuum

    Archimedes is a feasibility study of a future experiment to ascertain the interaction of vacuum fluctuations with gravity. The experiment should measure the force that the earth's gravitational field exerts on a Casimir cavity by using a small force detector.     The Swift Supergiant Fast X-ray Transients outburst factory

    We present the Swift Supergiant Fast X-ray Transients project, which has been exploiting Swift's capabilities in a systematic study of SFXTs and classical supergiant X-ray binaries (SGXBs) since 2007. We performed an efficient long-term monitoring of 16 sources including both SFXTs and classical SGXBs and followed source activity across more than 4 orders of magnitude in X-ray luminosity, sampling the light curves on timescales spanning from few hundred seconds to years. We use our measurements of dynamic ranges, duty cycles as a function of luminosity, and luminosity distributions to highlight systematic differences that help discriminate between different theoretical models proposed to explain the differences between the wind accretion processes in SFXTs and classical SGXBs. Our follow-ups of the SFXT outbursts provide a steady advancement in the comprehension of the mechanisms triggering the high X-ray level emission of these sources. In particular, the recent observations of the outburst of the SFXT prototype IGR J17544-2619 on 2014 October 10, when the source reached a peak luminosity of 3x1038 erg s-1, challenged, for the first time, the maximum theoretical luminosity achievable by a wind-fed neutron star high mass X-ray binary.     The eEF1Îł Subunit Contacts RNA Polymerase II and Binds Vimentin Promoter Region

    Here, we show that the eukaryotic translation elongation factor 1 gamma (eEF1Îł) physically interacts with the RNA polymerase II (pol II) core subunit 3 (RPB3), both in isolation and in the context of the holo-enzyme. Importantly, eEF1Îł has been recently shown to bind Vimentin mRNA. By chromatin immunoprecipitation experiments, we demonstrate, for the first time, that eEF1Îł is also physically present on the genomic locus corresponding to the promoter region of human Vimentin gene. The eEF1Îł depletion causes the Vimentin protein to be incorrectly compartmentalised and to severely compromise cellular shape and mitochondria localisation. We demonstrate that eEF1Îł partially colocalises with the mitochondrial marker Tom20 and that eEF1Îł depletion increases mitochondrial superoxide generation as well as the total levels of carbonylated proteins. Finally, we hypothesise that eEF1Îł, in addition to its role in translation elongation complex, is involved in regulating Vimentin gene by contacting both pol II and the Vimentin promoter region and then shuttling/nursing the Vimentin mRNA from its gene locus to its appropriate cellular compartment for translation

    Management of hydrocele in adolescent patients

    Hydrocele is defined as an abnormal collection of serous fluid in the potential space between the parietal and visceral layers of the tunica vaginalis. In the majority of affected adolescents, hydrocele is acquired and is idiopathic in origin. The pathogenesis of idiopathic hydrocele is thought to be an imbalance in the normal process of fluid production and reabsorption. The diagnosis is usually clinical. Taking a thorough history is essential to rule out any fluctuation in size, which is an indication of a patent processus vaginalis. Scrotal ultrasonography is mandatory in nonpalpable testicles to rule out a subtending testicular solid mass requiring inguinal exploration. Otherwise, open hydrocelectomy via a scrotal incision is the standard treatment of idiopathic hydroceles. The second most common cause of hydrocele in adolescents is varicocelectomy. The risk of hydrocele formation is higher with non-artery-sparing procedures or those performed without microsurgical aid, and in surgery requiring cord dissection. If hydrocele occurs after varicocelectomy, initial management should include observation with or without hydrocele aspiration. Large persistent hydroceles are best served by open hydrocelectomy

    Cerium Oxide Nanoparticles Protect Cardiac Progenitor Cells from Oxidative Stress

    Cardiac progenitor cells (CPCs) are a promising autologous source of cells for cardiac regenerative medicine. However, CPC culture in vitro requires the presence of microenvironmental conditions (a complex array of bioactive substance concentration, mechanostructural factors, and physicochemical factors) closely mimicking the natural cell surrounding in vivo, including the capability to uphold reactive oxygen species (ROS) within physiological levels in vitro. Cerium oxide nanoparticles (nanoceria) are redox-active and could represent a potent tool to control the oxidative stress in isolated CPCs. Here, we report that 24 h exposure to 5, 10, and 50 !g/mL of nanoceria did not a!ect cell growth and function in cardiac progenitor cells, while being able to protect CPCs from H2O2-induced cytotoxicity for at least 7 days, indicating that nanoceria in an e!ective antioxidant. Therefore, these "ndings con"rm the great potential of nanoceria for controlling ROS-induced cell damage

    Global overview of the management of acute cholecystitis during the COVID-19 pandemic (CHOLECOVID study)

    Background: This study provides a global overview of the management of patients with acute cholecystitis during the initial phase of the COVID-19 pandemic. Methods: CHOLECOVID is an international, multicentre, observational comparative study of patients admitted to hospital with acute cholecystitis during the COVID-19 pandemic. Data on management were collected for a 2-month study interval coincident with the WHO declaration of the SARS-CoV-2 pandemic and compared with an equivalent pre-pandemic time interval. Mediation analysis examined the influence of SARS-COV-2 infection on 30-day mortality. Results: This study collected data on 9783 patients with acute cholecystitis admitted to 247 hospitals across the world. The pandemic was associated with reduced availability of surgical workforce and operating facilities globally, a significant shift to worse severity of disease, and increased use of conservative management. There was a reduction (both absolute and proportionate) in the number of patients undergoing cholecystectomy from 3095 patients (56.2 per cent) pre-pandemic to 1998 patients (46.2 per cent) during the pandemic but there was no difference in 30-day all-cause mortality after cholecystectomy comparing the pre-pandemic interval with the pandemic (13 patients (0.4 per cent) pre-pandemic to 13 patients (0.6 per cent) pandemic; P = 0.355). In mediation analysis, an admission with acute cholecystitis during the pandemic was associated with a non-significant increased risk of death (OR 1.29, 95 per cent c.i. 0.93 to 1.79, P = 0.121). Conclusion: CHOLECOVID provides a unique overview of the treatment of patients with cholecystitis across the globe during the first months of the SARS-CoV-2 pandemic. The study highlights the need for system resilience in retention of elective surgical activity. Cholecystectomy was associated with a low risk of mortality and deferral of treatment results in an increase in avoidable morbidity that represents the non-COVID cost of this pandemic
