    The effect of individual counseling, line follow-up, and free nicotine replacement therapy on smoking cessation in the samples of Iranian smokers: Examination of transtheoretical model

    Background: According to transtheoretical model(TTM), Stage matched interventions are more effective in quitting. The objective of current study was to investigate the effect of individual counseling, line follow-up, and free nicotine replacement therapy(NRT) on smoking cessation in smokers who are in preparation stage of smoking. Materials and Methods: In a randomized clinical trial design, through sending the short message system, potential participants in preparation stage of smoking were recruited and divided into control(n=60)and treatment(n=50) groups. The treatment group received an in-person counseling, line follow-up, and free NRT. TTM variables trend; pros and cons of smoking, behavioral and experiential processes, temptation, were assessed at baseline, 3 and 6months follow-up along with point prevalence and continuous abstinence. Results: Continuous abstinence at 6-month follow-up were 3.3%(n=2) in control group and 46%(n=23) in the treatment group(x 2 = 34.041, P < 0.001). Time � group analyses indicated that except cons of smoking (P > 0.05), all TTM constructs had significantly changed; temptation(F=36.864, P<0.001), pros (F=12.172, P<0.001), experiential processes(F=3.377, P<0.001), and behavioral processes(F=11.131, P<0.001). Conclusion: Interventions based on TTM variables increased the quite rate in prepared and motivated people. Our findings suggest that interventions through individual counseling along with free NRT and line follow-up in people who prepare for quitting are beneficial for our country

    Interest in rhinoplasty and awareness about its postoperative complications among female high school students

    Introduction: Rhinoplasty is a popular cosmetic surgical procedure. Informal statistics show that Iran has one of the highest rates of rhinoplasty in the world. However, rhinoplasty like any other surgery can have complications. Materials and Methods: In this cross-sectional study, 320 female students were selected by multistage cluster-stratified sampling from high schools in Kerman, Iran and each completed a questionnaire. Results: More than half of the students said they would like to undergo rhinoplasty. The main reasons for wanting rhinoplasty were beauty and because it is fashionable. However, more than half of the interested students did not know about the possible postoperative complications of rhinoplasty. There was no relation between interest in having rhinoplasty and parents' education, city of birth or economic status. Conclusion: Many teenagers are interested in having rhinoplasty in Iran. As the number of teenagers and young adults who choose to have cosmetic surgery increases, surgeons should consider their expectations, motivations and awareness of postoperative complications before surgery

    Comparison the self-concept and behavior in adolescents without criminal record with juvenile offenders

    Background and aims: impermanent and negative self-concept , disrupts personal and social adjustment. in this study compared self-concept and behavior in adolescents without a history of juvenile criminal and offenders. Methods: The study was cross-sectional . 79 person from centers for Upbringing and correction via census sample and 120 person that with first group in regard to age-and sex- matched controls from Isfahan City were enrolled. The data collection instrument consisted of three sections include demographic information , self-concept and behavior questions. Statistic examination include T-test, Fisher and K2, were use for data analyzed . Results: The mean and standard deviation of self-Concept in offenders was 58/94±10/30 and Behavior was 62/26±28/81. Adolescents with no history of criminal record, self-concept score was 63/02±9/75 and Behavior was 84/66±15/22. Self-concept and behavior scores were significantly higher in the group with no criminal record (p<0/001). Conclusion: The results suggest that the Self-concept and Behavior of juvenile offenders is lower than adolescents with no criminal record. It is also considered demographic factors will affect the incidence of antisocial behavior . It seems psychologists and counselors to be working in these centers must training , counseling and therapy and correction techniques with consideration to the establishment of such anomalies

    Survey of potential diagnostic metabolite markers in serum of the rat model of alzheimer�s disease using nuclear magnatic resonance (1H-nmr) technique

    Introduction: The high prevalence of Alzheimer's disease (AD) in today's societies indicates an urgent need for the development of methods that will help the early diagnosis of the disease. In this study, using proton nuclear magnetic resonance spectrometry (1H-NMR) metabolomics, identification of altered and distinct metabolites in serum of the rat model of AD was performed compared with healthy controls with the aim of introducing potential markers and to further understand the mechanisms of the AD. Materials and Methods: Serum samples from 25 adult male rats (10 healthy and 15 AD) were collected and their metabolites were extracted and analyzed using 1H-NMR technique. Differential metabolite profile was then determined by multivariate statistical analysis. The behavioral screening of the model rats was performed by the paired-associated learning method. Results: The results of the behavioral study showed the impairment of memory abilities in AD rats. Differential metabolites between the two groups were identified by multivariate analysis methods such as OPLS and Random Forest. Importantly, the results showed that there were differences in the pathways related to energy and amino acid metabolism between the control group and the Alzheimer's model. Conclusion: This research opens new horizons to identify biomarkers and physiological pathways involved in Alzheimer�s disease. The introduced metabolites must be confirmed by further studies and might be used as candidate biomarkers for early detection of the disease. © 2021, Semnan University of Medical Sciences.     α:Non-α and Gγ:Aγ globin chain ratios in thalassemia intermedia patients treated with hydroxyurea

    Objectives: To elucidate the possible ways by which hydroxyurea molecules affect globin chain (α or β-like) synthesis. Methods: A total of 23 thalassemia intermedia patients (13 male and 10 female) aged between 5 and 26 years were treated for five months with 15 mg/(kg-day) of hydroxyurea. Hemoglobins electrophoresis and globin chain electrophoresis was performed on each sample at different time points before and during the treatment. Results: Fetal hemoglobin increased significantly in most patients and average episodes of transfusion decreased. Both Gγ and Aγ-globin chains increased significantly and α-globin:Nonα-globin chain as well as Gγ-globin:Aγ globin chains ratios decreased. Conclusions: Improvement in α:non-α ratio and consequent decrease of free α-globin chain might be the cause of beneficial effects of hydroxyurea therapy. Two patients who felt better didn't show significant increase in their fetal hemoglobin level, and this is in contradiction with the hypothesis claiming that the HbF level increase is the cause of such therapeutic effect. In spite of the unclear mechanism of action of this drug, hydroxyurea therapy had noticeable impacts on thalassemia intermedia and also sickle cell disease and even patients suffering from thalassemia major. © 2014 by the Asian Pacific Journal of Tropical Biomedicine

    Rearing of common carp (Cyprinus carpio) in fiber glass tanks with pellet feeding

    The intensive culture of common carp (Cyprinus carpio) evaluated in fibreglass tanks (2*2m) with feeding on pellet during 8 weeks. The fish (n=201)of common carp selected from three treatments along with three replicates as the average of fish weight was less than 100(g )which divided into 0.05).The common pellet food was chosen for feeding from market as fish were fed by the mean of 4% of their body weight a day. The result indicated that the mean of particular growth ratio at first treatment (0.54) has higher than second treatment (0.34) but in comparison with third treatment (0.46), there was no statistically significant difference (P<0.05).Also, the conversion ratio at first treatment (5.28) and third treatment (6.09) was lower than second treatment (35.66) and third treatment (20.91) was lower than second treatment (24.24), (P<0.05). This study showed that monoculture in small scale is feasible by utilizing artificial diet, if the water quality is exposed to change. In this survey, the conversion ratio of the fish was relatively high whereas the growth rate was low

    Global burden and strength of evidence for 88 risk factors in 204 countries and 811 subnational locations, 1990–2021: a systematic analysis for the Global Burden of Disease Study 2021

    Background: Understanding the health consequences associated with exposure to risk factors is necessary to inform public health policy and practice. To systematically quantify the contributions of risk factor exposures to specific health outcomes, the Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) 2021 aims to provide comprehensive estimates of exposure levels, relative health risks, and attributable burden of disease for 88 risk factors in 204 countries and territories and 811 subnational locations, from 1990 to 2021. Methods: The GBD 2021 risk factor analysis used data from 54 561 total distinct sources to produce epidemiological estimates for 88 risk factors and their associated health outcomes for a total of 631 risk–outcome pairs. Pairs were included on the basis of data-driven determination of a risk–outcome association. Age-sex-location-year-specific estimates were generated at global, regional, and national levels. Our approach followed the comparative risk assessment framework predicated on a causal web of hierarchically organised, potentially combinative, modifiable risks. Relative risks (RRs) of a given outcome occurring as a function of risk factor exposure were estimated separately for each risk–outcome pair, and summary exposure values (SEVs), representing risk-weighted exposure prevalence, and theoretical minimum risk exposure levels (TMRELs) were estimated for each risk factor. These estimates were used to calculate the population attributable fraction (PAF; ie, the proportional change in health risk that would occur if exposure to a risk factor were reduced to the TMREL). The product of PAFs and disease burden associated with a given outcome, measured in disability-adjusted life-years (DALYs), yielded measures of attributable burden (ie, the proportion of total disease burden attributable to a particular risk factor or combination of risk factors). Adjustments for mediation were applied to account for relationships involving risk factors that act indirectly on outcomes via intermediate risks. Attributable burden estimates were stratified by Socio-demographic Index (SDI) quintile and presented as counts, age-standardised rates, and rankings. To complement estimates of RR and attributable burden, newly developed burden of proof risk function (BPRF) methods were applied to yield supplementary, conservative interpretations of risk–outcome associations based on the consistency of underlying evidence, accounting for unexplained heterogeneity between input data from different studies. Estimates reported represent the mean value across 500 draws from the estimate's distribution, with 95% uncertainty intervals (UIs) calculated as the 2·5th and 97·5th percentile values across the draws. Findings: Among the specific risk factors analysed for this study, particulate matter air pollution was the leading contributor to the global disease burden in 2021, contributing 8·0% (95% UI 6·7–9·4) of total DALYs, followed by high systolic blood pressure (SBP; 7·8% [6·4–9·2]), smoking (5·7% [4·7–6·8]), low birthweight and short gestation (5·6% [4·8–6·3]), and high fasting plasma glucose (FPG; 5·4% [4·8–6·0]). For younger demographics (ie, those aged 0–4 years and 5–14 years), risks such as low birthweight and short gestation and unsafe water, sanitation, and handwashing (WaSH) were among the leading risk factors, while for older age groups, metabolic risks such as high SBP, high body-mass index (BMI), high FPG, and high LDL cholesterol had a greater impact. From 2000 to 2021, there was an observable shift in global health challenges, marked by a decline in the number of all-age DALYs broadly attributable to behavioural risks (decrease of 20·7% [13·9–27·7]) and environmental and occupational risks (decrease of 22·0% [15·5–28·8]), coupled with a 49·4% (42·3–56·9) increase in DALYs attributable to metabolic risks, all reflecting ageing populations and changing lifestyles on a global scale. Age-standardised global DALY rates attributable to high BMI and high FPG rose considerably (15·7% [9·9–21·7] for high BMI and 7·9% [3·3–12·9] for high FPG) over this period, with exposure to these risks increasing annually at rates of 1·8% (1·6–1·9) for high BMI and 1·3% (1·1–1·5) for high FPG. By contrast, the global risk-attributable burden and exposure to many other risk factors declined, notably for risks such as child growth failure and unsafe water source, with age-standardised attributable DALYs decreasing by 71·5% (64·4–78·8) for child growth failure and 66·3% (60·2–72·0) for unsafe water source. We separated risk factors into three groups according to trajectory over time: those with a decreasing attributable burden, due largely to declining risk exposure (eg, diet high in trans-fat and household air pollution) but also to proportionally smaller child and youth populations (eg, child and maternal malnutrition); those for which the burden increased moderately in spite of declining risk exposure, due largely to population ageing (eg, smoking); and those for which the burden increased considerably due to both increasing risk exposure and population ageing (eg, ambient particulate matter air pollution, high BMI, high FPG, and high SBP). Interpretation: Substantial progress has been made in reducing the global disease burden attributable to a range of risk factors, particularly those related to maternal and child health, WaSH, and household air pollution. Maintaining efforts to minimise the impact of these risk factors, especially in low SDI locations, is necessary to sustain progress. Successes in moderating the smoking-related burden by reducing risk exposure highlight the need to advance policies that reduce exposure to other leading risk factors such as ambient particulate matter air pollution and high SBP. Troubling increases in high FPG, high BMI, and other risk factors related to obesity and metabolic syndrome indicate an urgent need to identify and implement interventions.     Repositioning of the global epicentre of non-optimal cholesterol

    High blood cholesterol is typically considered a feature of wealthy western countries1,2. However, dietary and behavioural determinants of blood cholesterol are changing rapidly throughout the world3 and countries are using lipid-lowering medications at varying rates. These changes can have distinct effects on the levels of high-density lipoprotein (HDL) cholesterol and non-HDL cholesterol, which have different effects on human health4,5. However, the trends of HDL and non-HDL cholesterol levels over time have not been previously reported in a global analysis. Here we pooled 1,127 population-based studies that measured blood lipids in 102.6 million individuals aged 18 years and older to estimate trends from 1980 to 2018 in mean total, non-HDL and HDL cholesterol levels for 200 countries. Globally, there was little change in total or non-HDL cholesterol from 1980 to 2018. This was a net effect of increases in low- and middle-income countries, especially in east and southeast Asia, and decreases in high-income western countries, especially those in northwestern Europe, and in central and eastern Europe. As a result, countries with the highest level of non-HDL cholesterol—which is a marker of cardiovascular risk—changed from those in western Europe such as Belgium, Finland, Greenland, Iceland, Norway, Sweden, Switzerland and Malta in 1980 to those in Asia and the Pacific, such as Tokelau, Malaysia, The Philippines and Thailand. In 2017, high non-HDL cholesterol was responsible for an estimated 3.9 million (95% credible interval 3.7 million–4.2 million) worldwide deaths, half of which occurred in east, southeast and south Asia. The global repositioning of lipid-related risk, with non-optimal cholesterol shifting from a distinct feature of high-income countries in northwestern Europe, north America and Australasia to one that affects countries in east and southeast Asia and Oceania should motivate the use of population-based policies and personal interventions to improve nutrition and enhance access to treatment throughout the world.</p
