    This study uses the theory of values which bases on Schwartz (1992)’s 10 core life values to explore the life value directions of 448 Certified Public Accountants from Union of Chambers of Certified Public Accountants of Turkiye (TÜRMOB) registered 31 different chambers. A questionnaire is applied to the sample and SPSS 13 packaged software is respectively used in data entrance-analyze and factor analysis (Principle Components Analysis, Equamax Rotation) processes. We find that certified public accountants in Turkiye have 8 life value directions as Conformity, Power, Universalism, Stimulation, Tradition, Belonging, Benevolence and Hedonism.Life values; accounting profession; certified public accountant’s life values in Turkiye

    Factors associated with increased carotid intima-media thickness and being nondipper in nonobese and normotensive young patients affected by PCOS

    Polycystic ovary syndrome (PCOS) is characterized by chronic unovulation, hyperandrogenism, and insulin resistance. We evaluated factors that affect nondipper status during 24-hour ambulatory blood pressure monitoring (ABPM) and carotid intima-media thickness (cIMT) in PCOS. Forty-two nonobese women newly diagnosed as PCOS and 32 healthy women were included. After biochemical and hormonal measurements, the ovaries were imaged by pelvic ultrasonography and cIMT was measured by B-mode ultrasonography. A 24-hour ABPM was performed thereafter. Carotid IMT and the ratio of nondippers were elevated compared with controls. Homeostasis model assessment insulin resistance index (HOMA-IR) and low-density lipoprotein cholesterol (LDL-C) were found to be related with being a nondipper in PCOS. None of the parameters evaluated were found to correlate with cIMT. In conclusion, patients with PCOS had increased nondipping ratios and cIMT when compared with controls. Insulin resistance and LDL cholesterol are factors that are related to diurnal variation in normotensive and young patients with PCOS

    Short-term outcome of early electrical cardioversion for atrial fibrillation in hyperthyroid versus euthyroid patients*

    Background: Atrial fibrillation (AF) is the most common cardiac complication of hyperthyroidism. The influence of the time of cardioversion on hyperthyroidism-induced AF remains unclear. The aim of this study was to compare short-term outcomes of early electrical cardioversion for AF in hyperthyroid and euthyroid patients. Methods and Results: Sixty-seven subjects with persistent AF (duration, 10 days–12 months) were divided into two groups according to thyroid function: Euthyroid (Group 1, n = 36, female/male: 23/13, mean age: 61.77 ± 10.45 years) and hyperthyroid (Group 2, n = 31, female/male: 10/21, mean age: 65.43 ± 6.40 years). Two patients were excluded for unsuccessful cardioversion (one in each group). In Group 2, 19 patients had clinical and 11 had subclinical hyperthyroidism. Following transthoracic and transesophageal echocardiography, cardioversion was performed until the highest energy was reached (270 J) or until sinus rhythm was achieved. AF recurrence was detected in 13 of 35 patients (37.1%) in Group 1 and in 11 of 30 patients (36.9%) in Group 2 (p = 0.96) at one month. Recurrence rate was higher in the clinical hyperthyroid patients than in the subclinical hyperthyroid patients (52.6% vs 9.1%, p = 0.021), but neither the clinical nor the subclinical hyperthyroid subgroups were significantly different from Group 1 in terms of recurrence rate (p = 0.27 and p = 0.13, respectively).     Early assessment of percutaneous coronary interventions for chronic total occlusions analyzed by novel echocardiographic techniques

    OBJECTIVE: Successful revascularization of chronic total occlusions has been associated with improved left ventricular systolic function, reduced anginal symptoms, increased exercise capacity, and increased survival. This study was conducted to determine the impact of revascularization in chronic total occlusion on left ventricular function using novel echocardiographic techniques. METHODS: A total of 129 patients with chronic total occlusion who underwent revascularization between April 2011 and November 2012 were included in this study. Echocardiographic assessments with two-dimensional speckle tracking echocardiography and real-time three-dimensional echocardiography were performed before the procedure and one month after the procedure. The left ventricular ejection fraction, left ventricular volumes, and three-dimensional systolic dyssynchrony index were quantified. RESULTS: An immediate procedural success was obtained in 118 patients (91.5%). There were no acute or subacute stent thromboses during follow-up.     Delirium and Extrapyramidal Symptoms Due to a Lithium-Olanzapine Combination Therapy: A Case Report

    We report an elderly patient who developed severe delirium and extrapyramidal signs after initiation of lithium-olanzapine combination. On hospital admission, serum levels of lithium were found to be 3.0 mM/L which were far above toxic level. Immediate discontinuation of both drugs resulted in complete resolution of most of the symptoms except for perioral dyskinesia which persisted for three more months. We critically discussed the differential diagnosis of lithium intoxication and assessed confounding factors which induce delirium and extrapyramidal signs related with combination therapy of lithium and olanzapine
