    What happens when oral tuberculosis is not treated?

    Pulmonary tuberculosis (TB) is the most important form of the disease, although infection may also occur by way of the intestinal tract, tonsils and skin. Oral lesions consist of persistent ulcers or granulomatous masses. A 50 year old man had been diagnosed “necrotising granulomatous inflammation” following a biopsy of a lesion on lower lip, 21 months before at a medical centre. A chest-X-ray had not been performed and he had not been given any advise in respect of treatment. He was admitted to the hospital with cough, sputum, weakness, weight loss and lesions on his lower lip. In radiology, it was detected that he had supraclavicular, submental, cervical, mediastinal lymphadenopathies, pulmonary infiltrations with cavities, thickening and roughness on left oropharengial tonsil, thickenning on inner parts of larynx and bilateral surrenal thickening. The biopsy of lesions on larynx, tonsil and epiglottis revealed “necrotising granulomatous inflammation” and histopathology supported TB infection. Sputum acid-fast bacilli was positive and culture was positive for Mycobacterium tuberculosis complex. Two months of combination treatment resulted in a gradual relief of the symptoms, radiological response, disappearing of neck swelling and healing of lesions on lip, tonsil and larynx.     We intended to identify mycobacterium and search the incidence of atypical mycobacterium infection in patients diagnosed active pulmonary tuberculosis using biochemical tests and Gen Probe nucleic acide multiplication method. We carried out Gen Probe identification for 78 cases, additional niacin and catalase tests on 31 cases. About 31 strain; 25 (80.6%) of them were niacin (+), 6 (19.4%) were niacin (-), 26 (83.9%) of them were catalase (-), 5 (16.1%) were catalase (+). We concluded Gen Probe identification test to be more dependable than the biochemical tests for the exact identification of mycobacterium.Aktif akciğer tüberkülozu tanısı alan hastalarda biyokimyasal testler ve Gen Probe nükleik asit çoğaltma yöntemi kullanarak mikobakteri identifikasyonu yapmak ve atipik mikobakteri infeksiyonu insidansını araştırmayı amaçladık.     Evaluation of antimicrobial susceptibilities of non-tuberculous mycobacteria against linezolid and tigecycline

    Mycobacterial susceptibility testing is important for the management of nontuberculous mycobacteria (NTM) infections. The aim of the study is to determine the susceptibilities of tigecycline (TGC) and linezolid (LZD) against NTM. The study was carried out using stocks of NTM strains in the tuberculosis department of the microbiology laboratory. It was designed a retrospective study. LZD and TGC sensitivities of study isolates were analyzed by microdilution. Forty NTM isolates have been studied. LZD and TGC sensitivities varied according to the NTM type.     2-s2.0-85045192544The EURO-QOL (EQ-5D), a generic quality of life measure, is a self-report scale. The aim was to study the reliability, validity and feasibility of the Turkish version of the EQ-5D as an instrument to evaluate quality of life in lung cancer patients in Turkish society. A prospective cohort study carried out within the framework of the multicenter Lung Cancer Association Project which was conducted in collaboration with Turkish Thoracic Society, Lung and Pleural Malignancies working group and Health and Quality of Life Society. A total of 266 patients were included in the study with a mean age of 61.3 years. 75,9% of the patients had chemotherapy alone; 9% received sequential chemotherapy and radiotherapy, 8,6% were given concurrent chemotherapy and radiotherapy, and 6,4% of them received radical radiotherapy alone. EQ-5D score measures at the moment of diagnosis were found to be moderately reliable while measures at the 3rd chemotherapy cycle, 5th chemotherapy cycle and after the radiotherapy were highly reliable. The items of the EQ-5D were found to be consistent among themselves. EQ-5D Visual Analogue Scale scores at diagnosis, the 3rd cycle, the 5th cycle and following radiotherapy were assessed to be correlated with each other. EQ-5D scale was found to be valid as it meets the conditions of 6 fit indices in our study design. The psychometric properties of the Turkish version of the EQ-5D questionnaire is a valid and reliable scale for patients with lung cancer in Turkey and it can be safely used in clinical practices. © 2018, UHOD - Uluslararasi Hematoloji Onkoloji Dergisi.     Objectives:psychometric properties of the Turkish version of the Functional Assessment of Cancer Therapy-Lung (FACT-L) scale and the sensitivity to changes in clinical indicators.Patients and Methods:This study was conducted within the framework of a national multicentre project. Patients with either stage IIIB or IV primary lung cancer diagnosed after April 2010were included in thisstudy. A classical confirmatory approach was used for both the reliability and validity analyses. Internal consistency was tested using Cronbach's alpha value, and the validity analysis was performed using construct validity and clinical validity.Results:276 (92.3%) patients were male and the average age was 60.8 +/- 9.4 years. The most frequently observed histological type was squamous cell carcinoma (36.8%), and 61.5% of these stage IV tumours. Cronbachs alpha values for the subscales ranged from 0.60 to 0.84. The majority of the FACT-L subscales revealed inter-scale correlation coefficients greater than 0.35. All sub-dimensions, except that of the social/family well-being scale, are able to significantly discriminate between stages IIIB and IV. Significantly lower scale scores were detected in patients with stage IV than stage IIIB. The comparative fit index was 0.917, and the root mean square error of approximation was 0.091.
