    The use of a cluster analysis in across herd genetic evaluation for beef cattle

    To investigate the possibility of a genotype x environment interaction in Bonsmara cattle, a cluster analysis was performed on weaning weight records of 72 811 Bonsmara calves, the progeny of 1 434 sires and 24 186 dams in 35 herds. The following environmental factors were used to classify herds into clusters: solution for herd effects corrected for year-season, sex, age of dam and age at weaning (indicative of the management level in a herd), herd size and average temperature and rainfall. Two different genetic analyses were performed. Breeding values obtained in a univariate analysis were used as basis for comparison to breeding values obtained from a multivariate analysis where weaning weight in each cluster was considered as a separate trait. Direct additive, maternal additive, permanent maternal environment and Herd-Year-Season x Sire (HYSxS) interaction were included as random effects in both analyses. The direct genetic correlations between the clusters varied between 0.51 and 1.00. The low correlation estimates between some of the clusters indicate a possible genotype x environment interaction. Substantial reranking of sires between clusters did occur. However, further research is needed to identify and prioritize variables that can describe the genetics, management and climate of each herd more accurately.     Genetic parameters and trends for birth weight, birth coat score and weaning weight in Merino lines divergently selected for ewe multiple rearing ability

    Data were recorded for birth weight, birth coat score and weaning weight of between 2018 and 4235 Merino lambs born over a 17-year interval from 1986 to 2002. The resource population was divergently selected from the same base population since 1986, either for (H line) or against (L line) maternal multiple rearing ability. Direct additive genetic variances - expressed as a ratio of the total phenotypic variance (h2, direct heritability) - were estimated at 0.12 + 0.03 for birth weight, 0.70 + 0.05 for birth coat score and 0.13 + 0.04 for weaning weight. Maternal genetic variance ratios (m2) were estimated at 0.23 + 0.04 for birth weight and 0.10 + 0.04 for weaning weight. It was not significant for birth coat score. Maternal permanent environmental variance ratios (c2) for the respective traits amounted to 0.08 + 0.03 for birth weight, 0.04 + 0.02 for birth coat score and 0.08 + 0.03 for weaning weight. No consistent phenotypic or genetic differences in favour of either line were found for birth weight or birth coat score. Compared to L line lambs, phenotypic means for weaning weight were heavier in favour of H line lambs from 1995. This trend was confirmed by divergence in yearly averaged direct breeding values in the H and L lines. Expressed as percentage of the overall phenotypic mean for weaning weight, genetic changes amounted to +0.23% in the H line and -0.26% in the L line. A corresponding maternal genetic trend amounting to -0.19% of the overall phenotypic mean for weaning weight was found in the L line, while no maternal genetic change could be discerned in the H line. Divergent selection for multiple rearing ability thus did not influence birth weight and birth coat score, while the correlated response in weaning weight was favourable.     Imaging-guided chest biopsies: techniques and clinical results

    Background This article aims to comprehensively describe indications, contraindications, technical aspects, diagnostic accuracy and complications of percutaneous lung biopsy. Methods Imaging-guided biopsy currently represents one of the predominant methods for obtaining tissue specimens in patients with lung nodules; in many cases treatment protocols are based on histological information; thus, biopsy is frequently performed, when technically feasible, or in case other techniques (such as bronchoscopy with lavage) are inconclusive. Results Although a coaxial system is suitable in any case, two categories of needles can be used: fine-needle aspiration biopsy (FNAB) and core-needle biopsy (CNB), with the latter demonstrated to have a slightly higher overall sensitivity, specificity and accuracy. Conclusion Percutaneous lung biopsy is a safe procedure even though a few complications are possible: pneumothorax, pulmonary haemorrhage and haemoptysis are common complications, while air embolism and seeding are rare, but potentially fatal complications

    Does the availability of positron emission tomography modify diagnostic strategies for solitary pulmonary nodules? An observational study in France

    International audienceBACKGROUND: Previous studies showed that at the individual level, positron emission tomography (PET) has some benefits for patients and physicians in terms of cancer management and staging. We aimed to describe the benefits of (PET) in the management of solitary pulmonary nodules (SPNs) in a population level, in terms of the number of diagnostic and invasive tests performed, time to diagnosis and factors determining PET utilization. METHODS: In an observational study, we examined reports of computed tomography (CT) performed and mentioning "spherical lesion", "nodule" or synonymous terms. We found 11,515 reports in a before-PET period, 2002-2003, and 20,075 in an after-PET period, 2004-2005. Patients were followed through their physician, who was responsible for diagnostic management. RESULTS: We had complete data for 112 patients (73.7%) with new cases of SPN in the before-PET period and 250 (81.4%) in the after-PET period. Patients did not differ in mean age (64.9 vs. 64.8 years). The before-PET patients underwent a mean of 4 tests as compared with 3 tests for the after-PET patients (p = 0.08). Patients in the before-PET period had to wait 41.4 days, on average, before receiving a diagnosis as compared with 24.0 days, on average, for patients in the after-PET period who did not undergo PET (p < 0.001). In the after-PET period, 11% of patients underwent PET during the diagnostic process. A spiculated nodule was more likely to determine prescription for PET (p < 0.001). Multivariate analysis revealed that patients in both periods underwent fewer tests when PET was prescribed by general practitioners (p < 0.001) and if the nodule was not spiculated (p < 0.001). The proportion of unnecessary invasive approaches prescribed (47% vs. 49%) did not differ between the groups. CONCLUSION: In our study, 1 year after the availability of PET, the technology was not the first choice for diagnostic management of SPN. Even though we observed a tendency for reduced number of tests and mean time to diagnosis with PET, these phenomena did not fully relate to PET availability in health communities. In addition, the availability of PET in the management of SPN diagnosis did not reduce the overall rate of unnecessary invasive approaches

    A review of African horse sickness and its implications for Ireland

    African horse sickness is an economically highly important non-contagious but infectious Orbivirus disease that is transmitted by various species of Culicoides midges. The equids most severely affected by the virus are horses, ponies, and European donkeys; mules are somewhat less susceptible, and African donkeys and zebra are refractory to the devastating consequences of infection. In recent years, Bluetongue virus, an Orbivirus similar to African horse sickness, which also utilises Culicoides spp. as its vector, has drastically increased its range into previously unaffected regions in northern Europe, utilising indigenous vector species, and causing widespread economic damage to the agricultural sector. Considering these events, the current review outlines the history of African horse sickness, including information concerning virus structure, transmission, viraemia, overwintering ability, and the potential implications that an outbreak would have for Ireland. While the current risk for the introduction of African horse sickness to Ireland is considered at worst ‘very low’, it is important to note that prior to the 2006 outbreak of Bluetongue in northern Europe, both diseases were considered to be of equal risk to the United Kingdom (‘medium-risk’). It is therefore likely that any outbreak of this disease would have serious socio-economic consequences for Ireland due to the high density of vulnerable equids and the prevalence of Culicoides species, potentially capable of vectoring the virus

    Clinical practice: Swallowing problems in cerebral palsy

    Cerebral palsy (CP) is the most common physical disability in early childhood. The worldwide prevalence of CP is approximately 2–2.5 per 1,000 live births. It has been clinically defined as a group of motor, cognitive, and perceptive impairments secondary to a non-progressive defect or lesion of the developing brain. Children with CP can have swallowing problems with severe drooling as one of the consequences. Malnutrition and recurrent aspiration pneumonia can increase the risk of morbidity and mortality. Early attention should be given to dysphagia and excessive drooling and their substantial contribution to the burden of a child with CP and his/her family. This review displays the important functional and anatomical issues related to swallowing problems in children with CP based on relevant literature and expert opinion. Furthermore, based on our experience, we describe a plan for approach of investigation and treatment of swallowing problems in cerebral palsy

    Cost-effectiveness of minimal interventional procedures for chronic mechanical low back pain: design of four randomised controlled trials with an economic evaluation

    Background: Minimal interventional procedures are frequently applied in patients with mechanical low back pain which is defined as pain presumably resulting from single sources: facet, disc, sacroiliac joint or a combination of these. Usually, these minimal interventional procedures are an integral part of a multidisciplinary pain programme. A recent systematic review issued by the Dutch Health Insurance Council showed that the effectiveness of these procedures for the total group of patients with chronic low back pain is yet unclear and cost-effectiveness unknown. The aim of the study is to evaluate whether a multidisciplinary pain programme with minimal interventional procedures is cost-effective compared to the multidisciplinary pain programme alone for patients with chronic mechanical low back pain who did not respond to conservative primary care and were referred to a pain clinic. Methods. All patients with chronic low back pain who are referred to one of the 13 participating pain clinics will be asked to participate in an observational study. Patients with a suspected diagnosis of facet, disc or sacroiliac joint problems will receive a diagnostic block to confirm this diagnosis. If confirmed, they will be asked to participate in a Randomized Controlled Trial (RCT). For each single source a separate RCT will be conducted. Patients with a combination of facet, disc or sacroiliac joint problems will be invited for participation in a RCT as well. An economic evaluation from a societal perspective will be performed alongside these four RCTs. Patients will complete questionnaires at baseline, 3 and 6 weeks, 3, 6, 9 and 12 months after start of the treatment

    Using transformation algorithms to estimate (co)variance components by REML in models with equal design matrices

    The reduction of computational demands on Restricted Maximum Likelihood (REML) procedures by a diagonalization approach is extended to multiple traits by the use of canonical transformations. A computing strategy is developed for use on large data sets employing two different REML algorithms for the estimation of (co)variance components.
