    Frederick A. Milan died on 28 January 1995 after a series of strokes and related illness of several years. ... His activities around the polar regions of the world are well known and fondly remembered. Fred was Professor Emeritus of Human Ecology and Anthropology at the University of Alaska, Fairbanks. ... His work as an assistant on the Harvard Peabody Museum Anthropological Expedition to the Aleutian Islands in 1949 heightened his interest in the Arctic. It also impressed upon him the utility of studying Russian for understanding the history of the region. Fred worked in the summers of 1950 and 1952 as an archaeological assistant at Deering and Kodiak, Alaska. He was a weather observer on the Juneau Icefield Research Project in the summer of 1951. His major scholastic interests were in anthropology and linguistics, and he graduated in 1952. In 1953 Fred began several years of association with the former United States Air Force Arctic Aeromedical Laboratory near Fairbanks. ... [In 1954] he and other Arctic Aeromedical Laboratory personnel participated in the Mount Wrangell Research Project, a high-latitude cosmic ray study. His role was as a collaborator in a study of high altitude acclimatization of the team members. ... Fred undertook graduate study at several institutions: the Universities of Oregon, Wisconsin and Copenhagen, and the London School of Economics. His linguistic skills became well developed, and he was especially interested in the Inuit language and culture. He spent one winter in northern Sweden traveling and living with a nomadic Sami family. Back in Alaska in 1956, he began his close association with the Inupiat Eskimo village of Wainwright, Alaska. His ability to speak the language enabled him to gain the villagers' confidence and elicit genealogical information, and endeared him to the people. These activities led eventually to successful completion of his M.S. and Ph.D. degrees at the University of Wisconsin. ... In the 1960s Fred played an important role in establishing a worldwide study of northern people as part of the Human Adaptability section of the International Biological Program. ... He was a longtime advocate for multidisciplinary study of the needs for health care delivery and for the international exchange of information about circumpolar health problems and research efforts. In 1967 Fred, Dr. C. Earl Albrecht and colleagues in Alaska and other parts of the United States, Canada, Sweden, Norway, Denmark, Iceland, Greenland and the Union of Soviet Socialist Republics organized, with the support of the Arctic Institute of North America, the First International Symposium on Circumpolar Health. The symposium was held at Fairbanks in July 1967, and these meetings have continued at three-year intervals at various circumpolar locations. Fred was the president of the Sixth International Congress on Circumpolar Health held at Anchorage in 1984. ... During the 1970s and 1980s Fred was a faculty member at the University of Alaska, Fairbanks. He was a much-respected teacher, and he was noted for conveying his enthusiasm for scholarly work and his respect for the people of anthropological study to students and colleagues. The Fred Milan we knew was a genuine free spirit--he looked at the world through eyes wide with perceptive curiosity and interpreted it with kindness and good humor. We have been made richer and wiser by sharing in some of his generous and stimulating life

    Postgraduate palliative care education and curricular issues in Central Asia, Eastern and South-Eastern Europe: Results from a quantitative study

    Introduction. The WHO Europe Office, together with the European Association for Palliative Care, issupporting the development of an interdisciplinary core curriculum for health care professionals in theEuropean Region, which is to be given to the countries as a recommendation.Material and methods. Between April and September 2018, a research journey to Central Asia, Easternand South-Eastern Europe took place. It covered two main tasks: collecting quantitative data using a surveyand collecting qualitative data by conducting on-site interviews. This article contains the quantitativepart. Experts in palliative care education in 23 countries were invited to fill out a questionnaire. The datareceived from 23 questions were statistically evaluated by IBM SPSS Statistics 25.Results. 27 surveys received from 21 countries were evaluated. In one-third of the surveyed countries, thereis still no postgraduate training in palliative care. The main barriers to the development of educational workare limited political interest, followed by limited educational structures, missing curricula, lack of trainersand the limited healthcare system. For 92.6% of all respondents, a WHO-recommended interdisciplinarypostgraduate core curriculum would help promote palliative care in their country.Conclusion. There is a high need for postgraduate education in general and the intended core curriculumin particular. The results of the survey, along with those from the interviews in a later second part of thepublication, are essential for the development of a core curriculum

    Long-term variations of North Atlantic tropical cyclone activity downscaled from a coupled model simulation of the last millennium

    The observed historical record of North Atlantic tropical cyclones (TCs) is relatively short and is subject to potential biases owing to a lack of observation platforms such as aircraft reconnaissance and satellite imagery in earlier decades. Therefore, studies of long-term variability in TC activity are hindered by the limitations and uncertainty within the historical data. An alternative approach is to study long-term Atlantic TC variability within the framework of a coupled ocean-atmosphere climate model simulation. We have taken such an approach using a simulation of the National Center for Atmospheric Research Climate System Model 1.4 forced with estimated natural and anthropogenic forcing over the past millennium. Atmospheric variables from the long-term model simulation are used to drive a recently developed downscaling relationship that simulates TC genesis and tracking over the course of the 1150 year model simulation. This downscaling process generates a long-term synthetic TC track data set, free of observational biases, though subject to limitations in the model climatology. The synthetic TC data are used to perform an analysis of long-term variability in Atlantic TCs, specifically focusing on TC landfalls, within the context of the coupled model simulation. Ultimately, analysis of various TC time series reveals that counts of landfalling TCs and even landfalling hurricanes track relatively well with the total basin-wide TC activity on multidecadal and longer timescales

    Background: Femoral fracture rates of up to 30% have been reported following lengthening procedures using fixators. “Lengthening then rodding” uses one or two titanium elastic nails (TENs) for prophylactic intramedullary nailing to reduce this complication. The aim of the study was to decide if usage of only one TEN is safe or has it a higher risk of getting a fracture? And we asked if there is a difference between patients with monolateral or bilateral lengthening procedures according to their fracture rate? Methods: One or two TENs were implanted in two groups of patients (monolateral and bilateral) after femoral lengthening procedures. The regenerate quality was classified using the Li system and fractures were categorized using the Simpson and Kenwright classification. The follow-up period was at least 1 year after removal of the frame. Results: Sixty-seven patients with 101 femoral lengthening procedures were included in 2007–2011. Group A included 34 patients with bilateral lengthening due to congenital short stature. Group B consisted of 33 patients with congenital disorders with leg length discrepancies. Seven fractures in six patients were seen in group A and five fractures in group B. One patient had residual shortening of 1 cm, and 11 fractures healed without relevant deviation (< 5°) or shortening (< 5 mm). A soft-tissue infection in one patient led to early removal of one TEN. Conclusions: Fractures occurred in both groups of patients in total in 12 of the 101 cases (12%). The rate of secondary interventions was markedly reduced. Usage of one or two TENs did not influence the fracture rate.<br
