    Endoparasites in the feces of arctic foxes in a terrestrial ecosystem in Canada

    AbstractThe parasites of arctic foxes in the central Canadian Arctic have not been well described. Canada’s central Arctic is undergoing dramatic environmental change, which is predicted to cause shifts in parasite and wildlife species distributions, and trophic interactions, requiring that baselines be established to monitor future alterations. This study used conventional, immunological, and molecular fecal analysis techniques to survey the current gastrointestinal endoparasite fauna currently present in arctic foxes in central Nunavut, Canada. Ninety-five arctic fox fecal samples were collected from the terrestrial Karrak Lake ecosystem within the Queen Maud Gulf Migratory Bird Sanctuary. Samples were examined by fecal flotation to detect helminths and protozoa, immunofluorescent assay (IFA) to detect Cryptosporidium and Giardia, and quantitative PCR with melt-curve analysis (qPCR-MCA) to detect coccidia. Positive qPCR-MCA products were sequenced and analyzed phylogenetically. Arctic foxes from Karrak Lake were routinely shedding eggs from Toxascaris leonina (63%). Taeniid (15%), Capillarid (1%), and hookworm eggs (2%), Sarcocystis sp. sporocysts 3%), and Eimeria sp. (6%), and Cystoisospora sp. (5%) oocysts were present at a lower prevalence on fecal flotation. Cryptosporidium sp. (9%) and Giardia sp. (16%) were detected by IFA. PCR analysis detected Sarcocystis (15%), Cystoisospora (5%), Eimeria sp., and either Neospora sp. or Hammondia sp. (1%). Through molecular techniques and phylogenetic analysis, we identified two distinct lineages of Sarcocystis sp. present in arctic foxes, which probably derived from cervid and avian intermediate hosts. Additionally, we detected previously undescribed genotypes of Cystoisospora. Our survey of gastrointestinal endoparasites in arctic foxes from the central Canadian Arctic provides a unique record against which future comparisons can be made

    Transmission dynamics of Toxoplasma gondii, a parasite of importance for wildlife and human health, are enigmatic in the Arctic tundra, where free-ranging wild and domestic felid definitive hosts are absent and rarely observed, respectively. Through a multiyear mark-recapture study (2011– 17), serosurveillance was conducted to investigate transmission of T. gondii in Arctic foxes (Vulpes lagopus) in the Karrak Lake region, Nunavut, Canada. Sera from adult foxes and fox pups were tested for antibodies to T. gondii by using serologic methods, including the indirect fluorescent antibody test, direct agglutination test, and modified agglutination test. The overall seroprevalence was 39% in adults and 17% in pups. Mature foxes were more likely to be exposed (seroconvert) than young foxes (less than 1 yr old), with the highest level of seroprevalence in midaged foxes (2–4 yr old). Pups in two different litters were seropositive on emergence from the den, around 5 wk old, which could have been due to passive transfer of maternal antibody or vertical transmission of T. gondii from mother to offspring. The seropositive pups were born of seropositive mothers that were also seropositive the year before they gave birth, suggesting that vertical transmission might not be limited to litters from mothers exposed to T. gondii for the first time in pregnancy. All recaptured seropositive foxes remained seropositive on subsequent captures, suggesting that antibodies persist or foxes are constantly reexposed or a combination of both. The results of this study provided insights into how foxes were likely exposed to T. gondii, the dynamics of antibody persistence and immune response, and how the parasite was maintained in a terrestrial Arctic ecosystem in the absence of felid definitive hosts

    Assessing and Responding to Palliative Care Needs in Rural Sub-Saharan Africa: Results from a Model Intervention and Situation Analysis in Malawi

    Introduction: Palliative care is rarely accessible in rural sub-Saharan Africa. Partners In Health and the Malawi government established the Neno Palliative Care Program (NPCP) to provide palliative care in rural Neno district. We conducted a situation analysis to evaluate early NPCP outcomes and better understand palliative care needs, knowledge, and preferences. Methods: Employing rapid evaluation methodology, we collected data from 3 sources: 1) chart review of all adult patients from the NPCP’s first 9 months; 2) structured interviews with patients and caregivers; 3) semi-structured interviews with key stakeholders. Results: The NPCP enrolled 63 patients in its first 9 months. Frequent diagnoses were cancer (n = 50, 79%) and HIV/AIDS (n = 37 of 61, 61%). Nearly all (n = 31, 84%) patients with HIV/AIDS were on antiretroviral therapy. Providers registered 112 patient encounters, including 22 (20%) home visits. Most (n = 43, 68%) patients had documented pain at baseline, of whom 23 (53%) were treated with morphine. A majority (n = 35, 56%) had ≥1 follow-up encounter. Mean African Palliative Outcome Scale pain score decreased non-significantly between baseline and follow-up (3.0 vs. 2.7, p = 0.5) for patients with baseline pain and complete pain assessment documentation. Providers referred 48 (76%) patients for psychosocial services, including community health worker support, socioeconomic assistance, or both. We interviewed 36 patients referred to the NPCP after the chart review period. Most had cancer (n = 19, 53%) or HIV/AIDS (n = 10, 28%). Patients frequently reported needing income (n = 24, 67%) or food (n = 22, 61%). Stakeholders cited a need to make integrated palliative care widely available. Conclusions: We identified a high prevalence of pain and psychosocial needs among patients with serious chronic illnesses in rural Malawi. Early NPCP results suggest that comprehensive palliative care can be provided in rural Africa by integrating disease-modifying treatment and palliative care, linking hospital, clinic, and home-based services, and providing psychosocial support that includes socioeconomic assistance

    Adolescents with Congenital Heart Disease: a Patient and Parental Perspective of Genetic Information and Genetic Risk

    Congenital heart defects (CHDs) occur in 8 of 1000 live-born children, making them common birth defects in the adolescent population. CHDs may have single gene, chromosomal, or multifactorial causes. Despite evidence that patients with CHD want information on heritability and genetics, no studies have investigated the interest or knowledge base in the adolescent population. This information is necessary as patients in adolescence take greater ownership of their health care and discuss reproductive risks with their physicians. The objectives of this survey-based study were to determine adolescents' recall of their own heart condition, to assess patient and parent perception of the genetic contribution to the adolescent's CHD, and to obtain information about the preferred method(s) for education. The results show that adolescent patients had good recall of their type of CHD. Less than half of adolescents and parents believed their CHD had a genetic basis or was heritable; however, adolescents with a positive family history of CHD were more likely to believe that their condition was genetic (p = 0.0005). The majority of patients were interested in receiving additional genetics education and preferred education in-person and in consultation with both parents and a physician. The adolescents who felt most competent to have discussions with their doctors regarding potential causes of their heart defect previously had a school science course which covered topics in genetics. These results provide insight into adolescents' perceptions and understanding about their CHD and genetic risk and may inform the creation and provision of additional genetic education
