Psychometric Properties of the Malay Language Version of Knee Injury and Osteoarthritis Outcome Score (KOOS) Questionnaire among Knee Osteoarthritis Patients: A Confirmatory Factor Analysis
Introduction: This study aimed to cross-culturally adapt a
Malay version of Knee Injury and Osteoarthritis Outcome
Score (KOOS) and to evaluate its psychometric properties in
patients with knee osteoarthritis (OA).
Materials and Methods: The English version KOOS was
translated into a Malay version using forward and backward
translation process, followed by face validity and content
validity. Two hundred and twenty-six knee OA patients
attending the Outpatient and Orthopaedic Clinics, Universiti
Sains Malaysia Hospital, completed the Malay version
KOOS. Construct validity using confirmatory factor analysis
and internal reliability assessment were performed.
Results: The results showed that the original five-factor
model with 42 items failed to achieve acceptable values of
the goodness of fit indices, indicating poor model fit. A new
five-factor model of 26 items demonstrated acceptable level
of goodness of fit (comparative fit index= 0.929, incremental
fit index= 0.930, Tucker Lewis fit index= 0.920, root mean
square error of approximation= 0.073 and Chisquared/
degree of freedom= 2.183) indices to signify a
model fit. The Cronbach’s alpha value for the new model
ranged from 0.776 to 0.946. The composite reliability values
of each construct ranged between 0.819 and 0.921,
indicating satisfactory to high level of convergent validity.
Conclusion: The five-factor model with 26 items in the
Malay version of KOOS questionnaire demonstrated a good
degree of goodness of fit and was found to be valid, reliable
and simple as an assessment tool for symptoms, pain,
activity of daily living, sports and recreational activity and
quality of life for Malaysian adults suffering from knee
osteoarthritis
A low energy core-collapse supernova without a hydrogen envelope
The final fate of massive stars depends on many factors, including mass,
rotation rate, magnetic fields and metallicity. Theory suggests that some
massive stars (initially greater than 25-30 solar masses) end up as Wolf-Rayet
stars which are deficient in hydrogen because of mass loss through strong
stellar winds. The most massive of these stars have cores which may form a
black hole and theory predicts that the resulting explosion produces ejecta of
low kinetic energy, a faint optical display and a small mass fraction of
radioactive nickel(1,2,3). An alternative origin for low energy supernovae is
the collapse of the oxygen-neon core of a relatively lowmass star (7-9 solar
masses) through electron capture(4,5). However no weak, hydrogen deficient,
core-collapse supernovae are known. Here we report that such faint, low energy
core-collapse supernovae do exist, and show that SN2008ha is the faintest
hydrogen poor supernova ever observed. We propose that other similar events
have been observed but they have been misclassified as peculiar thermonuclear
supernovae (sometimes labelled SN2002cx-like events(6)). This discovery could
link these faint supernovae to some long duration gamma-ray bursts. Extremely
faint, hydrogen-stripped core-collapse supernovae have been proposed to produce
those long gamma-ray bursts whose afterglows do not show evidence of

Inverse Association between trans Isomeric and Long-Chain Polyunsaturated Fatty Acids in Pregnant Women and Their Newborns: Data from Three European Countries
Background: trans unsaturated fatty acids are thought to interfere with essential fatty acid metabolism. To extend our knowledge of this phenomenon, we investigated the relationship between trans isomeric and long-chain polyunsaturated fatty acids (LCPUFA) in mothers during pregnancy and in their infants at birth. Methods: Fatty acid composition of erythrocyte phosphatidylcholine (PC) and phosphatidylethanolamine (PE) was determined in Spanish (n = 120), German (n = 78) and Hungarian (n = 43) women at the 20th and 30th week of gestation, at delivery and in their newborns. Results: At the 20th week of gestation, the sum of trans fatty acids in PE was significantly (p < 0.01) lower in Hungarian [0.73 (0.51), % wt/wt, median (IQR)] than in Spanish [1.42 (1.36)] and German [1.30 (1.21)] women. Docosahexaenoic acid (DHA) values in PE were significantly (p < 0.01) higher in Hungarian {[}5.65 (2.09)] than in Spanish [4.37 (2.60)] or German [4.39 (3.3.2)] women. The sum of trans fatty acids significantly inversely correlated to DHA in PCs in Spanish (r = -0.37, p < 0.001), German (n = -0.77, p < 0.001) and Hungarian (r = -0.35, p < 0.05) women, and in PEs in Spanish (r = -0.67, p < 0.001) and German (r = -0.71, p < 0.001), but not in Hungarian (r = -0.02) women. Significant inverse correlations were seen between trans fatty acids and DHA in PEs at the 30th week of gestation (n = 241, r = -0.52, p < 0.001), at delivery (n = 241, r = -0.40, p < 0.001) and in cord lipids (n = 218, r = -0.28, p < 0.001). Conclusion: Because humans cannot synthesize trans isomeric fatty acids, the data obtained in the present study support the concept that high maternal trans isomeric fatty acid intake may interfere with the availability of LCPUFA both for the mother and the fetus. An asymmetric explosion as the origin of spectral evolution diversity in type Ia supernovae
Type Ia Supernovae (SNe Ia) form an observationally uniform class of stellar
explosions, in that more luminous objects have smaller decline-rates. This
one-parameter behavior allows SNe Ia to be calibrated as cosmological `standard
candles', and led to the discovery of an accelerating Universe. Recent
investigations, however, have revealed that the true nature of SNe Ia is more
complicated. Theoretically, it has been suggested that the initial
thermonuclear sparks are ignited at an offset from the centre of the
white-dwarf (WD) progenitor, possibly as a result of convection before the
explosion. Observationally, the diversity seen in the spectral evolution of SNe
Ia beyond the luminosity decline-rate relation is an unresolved issue. Here we
report that the spectral diversity is a consequence of random directions from
which an asymmetric explosion is viewed. Our findings suggest that the spectral
evolution diversity is no longer a concern in using SNe Ia as cosmological
standard candles. Furthermore, this indicates that ignition at an offset from

Mapping an atlas of tissue-specific drosophila melanogaster metabolomes by high resolution mass spectrometry
Metabolomics can provide exciting insights into organismal function, but most work on simple models has focussed on the whole organism metabolome, so missing the contributions of individual tissues. Comprehensive metabolite profiles for ten tissues from adult Drosophila melanogaster were obtained here by two chromatographic methods, a hydrophilic interaction (HILIC) method for polar metabolites and a lipid profiling method also based on HILIC, in combination with an Orbitrap Exactive instrument. Two hundred and forty two polar metabolites were putatively identified in the various tissues, and 251 lipids were observed in positive ion mode and 61 in negative ion mode. Although many metabolites were detected in all tissues, every tissue showed characteristically abundant metabolites which could be rationalised against specific tissue functions. For example, the cuticle contained high levels of glutathione, reflecting a role in oxidative defence; the alimentary canal (like vertebrate gut) had high levels of acylcarnitines for fatty acid metabolism, and the head contained high levels of ether lipids. The male accessory gland uniquely contained decarboxylated S-adenosylmethionine. These data thus both provide valuable insights into tissue function, and a reference baseline, compatible with the FlyAtlas.org transcriptomic resource, for further metabolomic analysis of this important model organism, for example in the modelling of human inborn errors of metabolism, aging or metabolic imbalances such as diabetes
A Cross-Sectional Survey on Knowledge and Perceptions of Health Risks Associated with Arsenic and Mercury Contamination from Artisanal Gold mining in Tanzania.
An estimated 0.5 to 1.5 million informal miners, of whom 30-50% are women, rely on artisanal mining for their livelihood in Tanzania. Mercury, used in the processing gold ore, and arsenic, which is a constituent of some ores, are common occupational exposures that frequently result in widespread environmental contamination. Frequently, the mining activities are conducted haphazardly without regard for environmental, occupational, or community exposure. The primary objective of this study was to assess community risk knowledge and perception of potential mercury and arsenic toxicity and/or exposure from artisanal gold mining in Rwamagasa in northwestern Tanzania. A cross-sectional survey of respondents in five sub-villages in the Rwamagasa Village located in Geita District in northwestern Tanzania near Lake Victoria was conducted. This area has a history of artisanal gold mining and many of the population continue to work as miners. Using a clustered random selection approach for recruitment, a total of 160 individuals over 18 years of age completed a structured interview. The interviews revealed wide variations in knowledge and risk perceptions concerning mercury and arsenic exposure, with 40.6% (n=65) and 89.4% (n=143) not aware of the health effects of mercury and arsenic exposure respectively. Males were significantly more knowledgeable (n=59, 36.9%) than females (n=36, 22.5%) with regard to mercury (x²=3.99, p<0.05). An individual's occupation category was associated with level of knowledge (x²=22.82, p=<0.001). Individuals involved in mining (n=63, 73.2%) were more knowledgeable about the negative health effects of mercury than individuals in other occupations. Of the few individuals (n=17, 10.6%) who knew about arsenic toxicity, the majority (n=10, 58.8%) were miners. The knowledge of individuals living in Rwamagasa, Tanzania, an area with a history of artisanal gold mining, varied widely with regard to the health hazards of mercury and arsenic. In these communities there was limited awareness of the threats to health associated with exposure to mercury and arsenic. This lack of knowledge, combined with minimal environmental monitoring and controlled waste management practices, highlights the need for health education, surveillance, and policy changes
Type Ia Supernovae as Stellar Endpoints and Cosmological Tools
Empirically, Type Ia supernovae are the most useful, precise, and mature
tools for determining astronomical distances. Acting as calibrated candles they
revealed the presence of dark energy and are being used to measure its
properties. However, the nature of the SN Ia explosion, and the progenitors
involved, have remained elusive, even after seven decades of research. But now
new large surveys are bringing about a paradigm shift --- we can finally
compare samples of hundreds of supernovae to isolate critical variables. As a
result of this, and advances in modeling, breakthroughs in understanding all

Cognitive and autonomic dysfunction measures in normal controls, white coat and borderline hypertension
<p>Abstract</p> <p>Background</p> <p>White coat hypertension (WCHT) is a significant clinical condition with haemodynamic differences and presence of functional changes. We aim to compare cognitive and autonomic dysfunction variables (heart rate variability) between subjects with normal blood pressure (controls), WCHT, and borderline hypertension (BLH).</p> <p>Methods</p> <p>We performed a cross-sectional study in a cohort of 69 subjects (mean age ± SD; 38.2 ±10.8 years) comprising comparable number of normal controls, WCHT, and BLH. We measured clinic and 24-hour ambulatory blood pressure monitoring (ABPM), cognitive function parameters, and heart rate variability (HRV). All subjects underwent 24-hour ambulatory electrocardiography monitoring which was analyzed for HRV measurements. We performed a routine echocardiography (ECHO) for all subjects.</p> <p>Results</p> <p>Multiple comparison between the three groups revealed significant (p < 0.04) differences in mean day-time ABPM (systolic and diastolic). In the state anxiety inventory (SAI), both subjects with WCHT and BLH had significantly (p < 0.006) higher anxiety levels than the control group. In memory tasks WCHT subjects scored significantly (p < 0.004) lower in comparison with the other two groups. WCHT significantly (p < 0.001) performed less in memory tests, whereas BLH subjects had significantly (p < 0.001) lower reaction time. We found a significant (p < 0.05) difference in the 24-hour RMSSD and SDNN between the three groups. There was significant correlation between 24-hour RMSSD and computer CANTAB scores. The Echocardiography assessment revealed no significant differences in LV mass indices and diastolic function.</p> <p>Conclusions</p> <p>WCHT and BLH subjects showed lower cognitive performance and higher levels of anxiety when compared to controls. Autonomic function reflected by HRV indices was lower in WCHT and BLH in contrast to control, though not significantly. Our results suggest that WCHT may not be a benign condition as it may contribute to the overall risk for cardiovascular disease and LV damage. Longitudinal studies of patients with WCHT should clarify the transient, persistent or the progressive nature of this condition.</p
Observational and Physical Classification of Supernovae
This chapter describes the current classification scheme of supernovae (SNe).
This scheme has evolved over many decades and now includes numerous SN Types
and sub-types. Many of these are universally recognized, while there are
controversies regarding the definitions, membership and even the names of some
sub-classes; we will try to review here the commonly-used nomenclature, noting
the main variants when possible. SN Types are defined according to
observational properties; mostly visible-light spectra near maximum light, as
well as according to their photometric properties. However, a long-term goal of
SN classification is to associate observationally-defined classes with specific
physical explosive phenomena. We show here that this aspiration is now finally
coming to fruition, and we establish the SN classification scheme upon direct
observational evidence connecting SN groups with specific progenitor stars.
Observationally, the broad class of Type II SNe contains objects showing strong
spectroscopic signatures of hydrogen, while objects lacking such signatures are
of Type I, which is further divided to numerous subclasses. Recently a class of
super-luminous SNe (SLSNe, typically 10 times more luminous than standard
events) has been identified, and it is discussed. We end this chapter by
briefly describing a proposed alternative classification scheme that is
inspired by the stellar classification system. This system presents our
emerging physical understanding of SN explosions, while clearly separating
robust observational properties from physical inferences that can be debated.
This new system is quantitative, and naturally deals with events distributed
along a continuum, rather than being strictly divided into discrete classes.
Thus, it may be more suitable to the coming era where SN numbers will quickly
