Basic physical exam skills versus technology: a case of undiagnosed scleroderma
A 50-year-old female with no previous known medical history was transferred to our tertiary hospital for undiagnosed chronic abdominal pain, bloating, nausea, recurrent vomiting, and extreme unintentional weight loss ongoing for the past three years. During that time, she had many emergency department visits and evaluated by multiple specialists. Previous workups include numerous CT scans, MRIs, EGDs and colonoscopies without a clear clinical diagnosis. She had been prescribed multiple courses of PPIs and antibiotics. Also, she had undergone two abdominal surgeries for suspected bowel obstruction and pseudo-obstruction. Upon encounter, the patient was noted to be severely cachectic after being on TPN for the last three months due to severe esophageal reflux, heartburn, and food intolerance. A more careful history revealed symptoms of Raynaud´s for the last 10 years. Also, when specifically asked and examined, the patient stated that her skin is hard and shiny, as if it was waxed. Based on the clinical presentation, diagnosis of CREST syndrome was suspected. That was further supported by a positive anti-centromere antibody test. Treatment options were limited due to the extent of disease progression by the time of diagnosis. Systemic sclerosis diagnosis can be challenging and requires a high level of suspicion. This case accents the gravity and value of a thorough history and physical examination as the prime foundation to reaching a diagnosis and it reminds us that sophisticated investigations are not a substitution for the essential skills of history taking and physical exam, especially in the setting of a long-standing undiagnosed illness
Serum 25-hydroxyvitamin D and bone turnover markers in Palestinian postmenopausal osteoporosis and normal women
Summary This study evaluated the association of vitaminD and
bone markers with the development osteoporosis in Palestinian
postmenopausal women. Even though vitamin D deficiency
was very high for the recruited subjects, it was not associated
with osteoporosis except for bones of the hip. Age and obesity
were the strongest determining factors of the disease.
Purpose The purpose of this study was to investigate the association
of bone mineral density (BMD) with serum vitamin
D levels, parathyroid hormone (PTH), calcium, obesity, and
bone turnover markers in Palestinian postmenopausal women.
Methods Three hundred eighty-two postmenopausal women
(≥45 years) were recruited from various women clinics for
BMD assessment (131 women had osteoporosis and 251 were
normal and served as controls). Blood samples were obtained
for serum calcium, PTH, 25(OH)D, bone formation (N-terminal
propeptide (PINP)), and bone resorption (serum Cterminal
telopeptide of type I collagen (CTX1)) markers.
Results Women with osteoporosis had statistically significant
lower mean weight, height, body mass index (BMI), and serum
calcium (p < 0.05) compared to controls. No significant
differences were detected between the mean values of bone
turnover markers (CTX and PINP), 25(OH)D, and PTH of
the two groups. Women with vitamin D deficiency (severe
and insufficiency) represented 85.9% of the study subjects.
Multiple and logistic regression showed that age and BMI
significantly affected BMD and vitamin D had a significant
association with BMD only at the lumbar spine. BMI was
positively correlated with BMD and PTH but negatively
correlated with vitamin D. Logistic regression showed that
the odds ratio (OR) for having osteoporosis decreased with
increasing BMI (overweight OR = 0.11, p = 0.053; obese
OR = 0.05, p = 0.007).
Conclusions There was no direct correlation between BMD
and PTH, bone turnover markers, and vitamin D except at the
lumbar spine. A negative correlation between BMD and age
and a positive correlation with BMI were observed. The protective
effect of obesity on osteoporosis was complicated by
the effect of obesity on vitamin D and PTH.This work was supported in part by a special grant

Geometrical frustration and incommensurate magnetic order in Na3RuO4 with two triangular motifs
Incommensurate magnetic order in the spin-3/2 antiferromagnet Na3RuO4 is uncovered by neutron diffraction combined with ab initio calculations. The crystal structure of Na3RuO4 contains two triangular motifs on different length scales. The magnetic Ru5+ ions form a lozenge (diamond) configuration, with tetramers composed of two isosceles triangles. These tetramers are further arranged in layers, such that an effective triangular lattice is formed. The tetramers are nearly antiferromagnetic but frustration between them leads to an incommensurately modulated magnetic structure described by the propagation vector →k=(0.242(1),0,0.313(1)). We show that the long-range Ru-O-O-Ru couplings between the tetramers play a major role in Na3RuO4 and suggest an effective description in terms of the spatially anisotropic triangular lattice if the tetramers are treated as single sites
A Unifying Model of Genome Evolution Under Parsimony
We present a data structure called a history graph that offers a practical
basis for the analysis of genome evolution. It conceptually simplifies the
study of parsimonious evolutionary histories by representing both substitutions
and double cut and join (DCJ) rearrangements in the presence of duplications.
The problem of constructing parsimonious history graphs thus subsumes related
maximum parsimony problems in the fields of phylogenetic reconstruction and
genome rearrangement. We show that tractable functions can be used to define
upper and lower bounds on the minimum number of substitutions and DCJ
rearrangements needed to explain any history graph. These bounds become tight
for a special type of unambiguous history graph called an ancestral variation
graph (AVG), which constrains in its combinatorial structure the number of
operations required. We finally demonstrate that for a given history graph ,
a finite set of AVGs describe all parsimonious interpretations of , and this
set can be explored with a few sampling moves.Comment: 52 pages, 24 figure
Sorting by reversals, block interchanges, tandem duplications, and deletions
<p>Abstract</p> <p>Background</p> <p>Finding sequences of evolutionary operations that transform one genome into another is a classic problem in comparative genomics. While most of the genome rearrangement algorithms assume that there is exactly one copy of each gene in both genomes, this does not reflect the biological reality very well – most of the studied genomes contain duplicated gene content, which has to be removed before applying those algorithms. However, dealing with unequal gene content is a very challenging task, and only few algorithms allow operations like duplications and deletions. Almost all of these algorithms restrict these operations to have a fixed size.</p> <p>Results</p> <p>In this paper, we present a heuristic algorithm to sort an ancestral genome (with unique gene content) into a genome of a descendant (with arbitrary gene content) by reversals, block interchanges, tandem duplications, and deletions, where tandem duplications and deletions are of arbitrary size.</p> <p>Conclusion</p> <p>Experimental results show that our algorithm finds sorting sequences that are close to an optimal sorting sequence when the ancestor and the descendant are closely related. The quality of the results decreases when the genomes get more diverged or the genome size increases. Nevertheless, the calculated distances give a good approximation of the true evolutionary distances.</p
Yellow fever vaccine induces integrated multilineage and polyfunctional immune responses
Correlates of immune-mediated protection to most viral and cancer vaccines are still unknown. This impedes the development of novel vaccines to incurable diseases such as HIV and cancer. In this study, we have used functional genomics and polychromatic flow cytometry to define the signature of the immune response to the yellow fever (YF) vaccine 17D (YF17D) in a cohort of 40 volunteers followed for up to 1 yr after vaccination. We show that immunization with YF17D leads to an integrated immune response that includes several effector arms of innate immunity, including complement, the inflammasome, and interferons, as well as adaptive immunity as shown by an early T cell response followed by a brisk and variable B cell response. Development of these responses is preceded, as demonstrated in three independent vaccination trials and in a novel in vitro system of primary immune responses (modular immune in vitro construct [MIMIC] system), by the coordinated up-regulation of transcripts for specific transcription factors, including STAT1, IRF7, and ETS2, which are upstream of the different effector arms of the immune response. These results clearly show that the immune response to a strong vaccine is preceded by coordinated induction of master transcription factors that lead to the development of a broad, polyfunctional, and persistent immune response that integrates all effector cells of the immune system
Convergence of TCR and cytokine signaling leads to FOXO3a phosphorylation and drives the survival of CD4+ central memory T cells
The molecular events involved in the establishment and maintenance of CD4+ central memory and effector memory T cells (TCM and TEM, respectively) are poorly understood. In this study, we demonstrate that ex vivo isolated TCM are more resistant to both spontaneous and Fas-induced apoptosis than TEM and have an increased capacity to proliferate and persist in vitro. Using global gene expression profiling, single cell proteomics, and functional assays, we show that the survival of CD4+ TCM depends, at least in part, on the activation and phosphorylation of signal transducer and activator of transcription 5a (STAT5a) and forkhead box O3a (FOXO3a). TCM showed a significant increase in the levels of phosphorylation of STAT5a compared with TEM in response to both IL-2 (P < 0.04) and IL-7 (P < 0.002); the latter is well known for its capacity to enhance T cell survival. Moreover, ex vivo TCM express higher levels of the transcriptionally inactive phosphorylated forms of FOXO3a and concomitantly lower levels of the proapoptotic FOXO3a target, Bim. Experiments aimed at blocking FOXO3a phosphorylation confirmed the role of this phosphoprotein in protecting TCM from apoptosis. Our results provide, for the first time in humans, an insight into molecular mechanisms that could be responsible for the longevity and persistence of CD4+ TCM
Newt-omics: a comprehensive repository for omics data from the newt Notophthalmus viridescens
Notophthalmus viridescens, a member of the salamander family is an excellent model organism to study regenerative processes due to its unique ability to replace lost appendages and to repair internal organs. Molecular insights into regenerative events have been severely hampered by the lack of genomic, transcriptomic and proteomic data, as well as an appropriate database to store such novel information. Here, we describe ‘Newt-omics’ (http://newt-omics.mpi-bn.mpg.de), a database, which enables researchers to locate, retrieve and store data sets dedicated to the molecular characterization of newts. Newt-omics is a transcript-centred database, based on an Expressed Sequence Tag (EST) data set from the newt, covering ∼50 000 Sanger sequenced transcripts and a set of high-density microarray data, generated from regenerating hearts. Newt-omics also contains a large set of peptides identified by mass spectrometry, which was used to validate 13 810 ESTs as true protein coding. Newt-omics is open to implement additional high-throughput data sets without changing the database structure. Via a user-friendly interface Newt-omics allows access to a huge set of molecular data without the need for prior bioinformatical expertise
Oral health status of adults in Southern Vietnam - a cross-sectional epidemiological study

89929.pdf (publisher's version ) (Open Access)BACKGROUND: Before strategies or protocols for oral health care can be advised at population level, epidemiological information on tooth decay patterns and its effects on oral function are indispensable. The aim of this study was to investigate influences of socio-demographic variables on the prevalence of decayed, missing, filled (DMF) and sound teeth (St) and to determine the relative risk of teeth in different dental regions for D, M, and F, of adults living in urban and rural areas in Southern Vietnam. METHODS: Cross-sectional DMF and St data of 2965 dentate subjects aged 20 to 95 living in urban and rural areas in three provinces were collected by means of a self-administered questionnaire and an oral examination. The sample was stratified by age, gender, residence and province. RESULTS: The percentage of subjects having missing teeth was high for all ages while it was low for subjects with decayed and filled teeth. The mean number of missing teeth increased gradually by age from approximately 1 in each jaw at the age of 20 to 8 at the age of 80. The number of decayed teeth was relative low at all ages, being highest in molars at young ages. The mean number of filled teeth was extremely low at all ages in all dental regions. Every additional year of age gives a significantly lower chance for decay, a higher chance for missing, and a lower chance for filled teeth. Molars had a significantly higher risk for decay, missing and filled than premolars and anterior teeth. Females had significantly higher risk for decayed and filled teeth, and less chance for missing teeth than males. Urban subjects presented lower risk for decay, but approximately 4 times greater chance for having fillings than rural subjects. Low socio-economic status (SES) significantly increased the chance for missing anterior and molar teeth; subjects with high SES had more often fillings. CONCLUSIONS: The majority of adults of Southern Vietnam presented a reduced dentition. The combination of low numbers of filled teeth and relative high numbers of decayed and missing teeth indicates that the main treatment for decay is extraction.
