    Avaliação comparativa das causas básicas de morte processadas pelos Sistemas "Automated Classification of Medical Entities" e de Seleção de Causa Básica

    INTRODUCTION: The correct identification of the underlying cause of death and its precise assignment to a code from the International Classification of Diseases are important issues to achieve accurate and universally comparable mortality statistics These factors, among other ones, led to the development of computer software programs in order to automatically identify the underlying cause of death. OBJECTIVE: This work was conceived to compare the underlying causes of death processed respectively by the Automated Classification of Medical Entities (ACME) and the "Sistema de Seleção de Causa Básica de Morte" (SCB) programs. MATERIAL AND METHOD: The comparative evaluation of the underlying causes of death processed respectively by ACME and SCB systems was performed using the input data file for the ACME system that included deaths which occurred in the State of S. Paulo from June to December 1993, totalling 129,104 records of the corresponding death certificates. The differences between underlying causes selected by ACME and SCB systems verified in the month of June, when considered as SCB errors, were used to correct and improve SCB processing logic and its decision tables. RESULTS: The processing of the underlying causes of death by the ACME and SCB systems resulted in 3,278 differences, that were analysed and ascribed to lack of answer to dialogue boxes during processing, to deaths due to human immunodeficiency virus [HIV] disease for which there was no specific provision in any of the systems, to coding and/or keying errors and to actual problems. The detailed analysis of these latter disclosed that the majority of the underlying causes of death processed by the SCB system were correct and that different interpretations were given to the mortality coding rules by each system, that some particular problems could not be explained with the available documentation and that a smaller proportion of problems were identified as SCB errors. CONCLUSION: These results, disclosing a very low and insignificant number of actual problems, guarantees the use of the version of the SCB system for the Ninth Revision of the International Classification of Diseases and assures the continuity of the work which is being undertaken for the Tenth Revision version.INTRODUÇÃO: A identificação correta da causa básica de morte e a atribuição de código preciso da Classificação Internacional de Doença à mesma são importantes para a obtenção de estatísticas de mortalidade confiáveis e passíveis de comparabilidade universal. Estes fatores, dentre outros, levaram ao desenvolvimento de programas de computador para identificar automaticamente a causa básica de morte. OBJETIVO: Este trabalho teve a finalidade de comparar a causa básica de morte identificada respectivamente pelos programas Automated Classification of Medical Entities (ACME) e pelo Sistema de Seleção de Causa Básica de Morte (SCB). MATERIAL E MÉTODO: O arquivo para a entrada de dados sobre causas de morte (input file) para o Sistema ACME contendo registros de 129.104 declarações de óbito de mortes ocorridas no estado de São Paulo de junho a dezembro de 1993 foi utilizado para o processamento da causa básica pelo SCB. Os problemas identificados pelo processamento dos registros do mês de junho foram considerados para o aprimoramento do sistema SCB. RESULTADOS: Foram encontras 3.278 causas básicas de morte identificadas de modo diferente pelos programs ACME e SCB. Essas diferenças foram atribuídas à falta de resposta adequada a janelas de diálogo durante o processamento pelo SCB, a óbitos por doenças devida a vírus da imunodeficiência adquirida para os quais não havia tabelas de decisão específicas, a erros de codificação e/ou digitação e a problemas propriamente ditos. A análise pormenorizada deste últimos mostrou que, em sua maioria, as causas básicas processadas pelo sistema SCB estavam corretas, que diferentes interpretações das regras de mortalidade foram dadas pelos sistemas comparados, que alguns problemas particulares não tiveram explicação adequada por falta de documentação sobre os mesmos e que uma menor proporção de problemas consistia de erros do SCB. CONCLUSÕES: O número pequeno e praticamente insignificante de problemas encontrados garante o uso da versão do SCB para a Nona Revisão da Classificação Internacional de Doenças e assegura a continuidade dos trabalhos relativos à sua versão para a Décima Revisão

    Bioconversion of agro-industrial by-products in rhamnolipids toward applications in enhanced oil recovery and bioremediation

    In this work, biosurfactant production by a Pseudomonas aeruginosa strain was optimized using low-cost substrates. The highest biosurfactant production (3.2 g/l) was obtained using a culture medium containing corn step liquor (10% (v/v)) and molasses (10% (w/v)). The biosurfactant reduced the surface tension of water up to 30 mN/m, and exhibited a high emulsifying activity (E24= 60%), with a critical micelle concentration as low as 50 mg/l. The biosurfactant produced in this alternative medium was characterized as a mixture of eight different rhamnolipid congeners, being the most abundant the mono-rhamnolipid Rha-C10-C10. However, using LB medium, nine different rhamnolipid congeners were identified, being the most abundant the di-rhamnolipid Rha-Rha-C10-C10.     O presente trabalho apresenta uma metodologia num?rica e experimental para determina??o dos par?metros de controle da temperatura do a?o l?quido contido na panela de aciaria durante o vazamento para o distribuidor, por ocasi?o dos seus ciclos normais de opera??o. Inicialmente os modelos idealizados s?o resolvidos numericamente com a aplica??o da t?cnica do m?todo de elementos finitos, obtendo-se, como principais resultados, o perfil de temperatura no a?o l?quido juntamente com a parede refrat?ria e o perfil de velocidade do banho met?lico. Os resultados obtidos da simula??o s?o comparados com os retirados de experimentos na ind?stria realizados pelos autores. Pode-se, assim, identificar os pontos cr?ticos na perda de temperatura do banho met?lico e empregar medidas preventivas, de modo a evitar um superaquecimento ou um aquecimento prematuro que resulta em uma s?rie de problemas durante o lingotamento cont?nuo.The control parameters which establish the liquid steel temperature inside the steel making ladle during teeming (when liquid steel flows from the ladle into the distributor) have been analysed, by means of numerical and experimental procedures. Initially the idealized models are solved with the use of finite element techniques obtaining as main results: the liquid steel temperature profile, the refractory wall temperature profile, and the velocity profile of the melt. The results obtained from the computational simulation are then compared with actual steel plant results measured by the authors. It is possible in this way to obtain the critical points where major temperature losses occur, helping to obtain preventive measures to lessen overheating and premature heating which result in a series of problems during continuous casting

    Characterization of kefir produced in household conditions: Physicochemical and nutritional profile, and storage stability

    Kefir, a traditional fermented food, has numerous health benefits due to its unique chemical composition, which is reflected in its excellent nutritional value. Physicochemical and microbial composition of kefir obtained from fermented milk are influenced by the type of the milk, grain to milk ratio, time and temperature of fermentation, and storage conditions. It is crucial that kefir characteristics are maintained during storage since continuous metabolic activities of residual kefir microbiota may occur. This study aimed to examine the nutritional profile of kefir produced in traditional in use conditions by fermentation of ultra-high temperature pasteurized (UHT) semi-skimmed cow milk using argentinean kefir grains and compare the stability and nutritional compliance of freshly made and refrigerated kefir. Results indicate that kefir produced under home use conditions maintains the expected characteristics with respect to the physicochemical parameters and composition, both after fermentation and after refrigerated storage.     Papilomavírus humano e anormalidades cervicais em mulheres do sistema de saúde privado e público no Estado do Rio de Janeiro, Brasil

    This article reports the HPV status and cervical cytological abnormalities in patients attended at public and private gynecological services from Rio de Janeiro State. It also comments the performance of each HPV DNA tests used. A set of 454 women from private health clinics was tested by routine Capture Hybrid II HPV DNA assay. Among these, 58.4% presented HPV and nearly 90% of them were infected by high risk HPV types. However, this group presented few premalignant cervical lesions and no invasive cervical cancer was registered. We also studied 220 women from low income class attended at public health system. They were HPV tested by polymerase chain reaction using My09/11 primers followed by HPV typing with E6 specific primers. The overall HPV prevalence was 77.3%. They also showed a high percentage of high squamous intraepithelial lesion-HSIL (26.3%), and invasive cervical carcinoma (16.3%). HPV infection was found in 93.1% and 94.4% of them, respectively. The mean ages in both groups were 31.5 and 38 years, respectively. In series 1, HPV prevalence declined with age, data consistent with viral transient infection. In series 2, HPV prevalence did not decline, independent of age interval, supporting not only the idea of viral persistence into this group, but also regional epidemiological variations in the same geographic area. Significant cytological differences were seen between both groups. Normal and benign cases were the most prevalent cytological findings in series 1 while pre-malignant lesions were the most common diagnosis in the series 2. HPV prevalence in normal cases were statistically higher than those from series 1 (p < 0.001), indicating a higher exposure to HPV infection. Women from both samples were referred for previous abnormal cytology. However, socio-demographic evidence shows that women from series 1 have access to treatment more easily and faster than women from series 2 before the development of pre-malignant lesions. These data provides baseline support for the role of social inequalities linked to high risk HPV infection leading to cervical cancer. Broadly screening programs and the development of safe and effective vaccines against HPV would diminish the toll of this disease that affect mainly poor women.Este artigo analisa a infecção por HPV e anormalidades citológicas cervicais encontradas em pacientes atendidas em serviços ginecológicos dos sistemas de saúde público e privado do estado do Rio de Janeiro. O trabalho também avalia os testes utilizados para detecção de DNA do HPV em cada população estudada. Um grupo de 454 mulheres oriundas de serviços da rede privada de saúde foi testado por Captura do Híbrido II. Destas, 58,4% apresentaram infecção por HPV e cerca de 90% delas estavam infectadas por HPV de alto risco. Este grupo, entretanto, apresentava poucos casos de lesões cervicais pré-malígnas e nenhum caso de câncer. Estudamos, também, 220 mulheres de baixo nível econômico atendidas no serviço de saúde pública que foram testadas para HPV pela reação da polimerase em cadeia utilizando-se os oligonucleotídeos My09/My11. A identificação dos tipos foi efetuada por amplificação com oligonucleotídeos específicos para a região E6 do genoma viral. A prevalência de HPV nesta população foi de 77.3%, observando-se uma alta porcentagem de casos de neoplasias intraepiteliais cervicais de alto grau (26,3%) e de carcinoma cervical invasivo (16,3%). A infecção por HPV foi achada em, respectivamente, 93,1% e 94,4% destes casos. A média de idade em ambos os grupos era de 31,5 e 38 anos, respectivamente. Na série 1, a prevalência da infecção por HPV decresce com a idade, enquanto na série 2 ela não desaparece, dando suporte não só à idéia de persistência viral neste grupo, mas também a variações epidemiológicas na mesma área geográfica. Diferenças significativas foram vistas nos dois grupos. Casos normais e benignos foram incidentes na série 1, enquanto as lesões malígnas predominaram na série 2. Ao contrário, casos normais infectados por HPV eram prevalentes na série 2 (p < 0.001), indicando maior exposição ao vírus. Embora as mulheres de ambos os grupos tenham sido incluídas no estudo por apresentarem citologia anormal, evidências sócio-demográficas demonstram que mulheres da série 1 tem acesso mais fácil e rápido ao tratamento do que as mulheres da série 2 antes que as lesões pré-malígnas se desenvolvam. Estes resultados fornecem dados sobre o papel das desigualdades sociais associadas à infecção por HPV de alto risco na progressão do câncer cervical. Programas de prevenção abrangentes e o desenvolvimento de vacinas eficazes e seguras contra o HPV poderiam reduzir o tributo desta doença que afeta principalmente mulheres pobres

    Hereditary angioedema: quality of life in Brazilian patients

    OBJECTIVE: Hereditary angioedema is a serious medical condition caused by a rare autosomal dominant genetic disorder and it is associated with deficient production or dysfunction of the C1 esterase inhibitor. In most cases, affected patients experience unexpected and recurrent crises of subcutaneous, gastrointestinal and laryngeal edema. The unpredictability, intensity and other factors associated with the disease impact the quality of life of hereditary angioedema patients. We evaluated the quality of life in Brazilian hereditary angioedema patients. METHODS: Patients older than 15 years with any severity of hereditary angioedema and laboratory confirmation of C1 inhibitor deficiency were included. Two questionnaires were used: a clinical questionnaire and the SF-36 (a generic questionnaire). This protocol was approved by the Ethics Committee of Hospital das Clínicas da Faculdade de Medicina da Universidade de São Paulo. RESULTS: The SF-36 showed that 90.4% (mean) of all the patients had a score below 70 and 9.6% had scores equal to or higher than 70. The scores of the eight dimensions ranged from 51.03 to 75.95; vitality and social aspects were more affected than other arenas. The internal consistency of the evaluation was demonstrated by a Cronbach's alpha value above 0.7 in seven of the eight domains. CONCLUSIONS: In this study, Brazilian patients demonstrated an impaired quality of life, as measured by the SF-36. The most affected domains were those related to vitality and social characteristics. The generic SF-36 questionnaire was relevant to the evaluation of quality of life; however, there is a need for more specific instruments for better evaluation

    Presence of virulence genes and pathogenicity islands in extraintestinal pathogenic Escherichia coli isolates from Brazil.

    International audienceExtraintestinal pathogenic Escherichia coli (ExPEC) is associated with various diseases such as urinary tract infections, neonatal meningitis and septicemia. There are many virulence factors (VF) encoded by genes in ExPEC, including papC, papG, ecpA, iroN, fyuA, iutA, ompTp, tsh, hlyF, hlyA and iss. These virulence genes may be present in pathogenicity islands (PAI) or plasmids. In this study, we analyzed the presence of VF encoding genes, PAI sequences and phylogenetic groups of 96 ExPEC strains isolated from the urine and blood of patients at the University Hospital of Londrina, and we compared them with 50 faecal commensal strains from healthy individuals. The VF fyuA (65.60%) was detected in pathogenic strains and commensal strains (46%). A comparison of the distribution of ExPEC and commensal strains in the phylogenetic groups showed that more ExPEC strains belonged to group B2 whereas more of the commensal isolates belonged to group A. The distribution of the seven PAI sequences between commensal strains and ExPEC strains showed that PAI IV536 was common in both ExPEC and commensal isolates. These results showed that the ExPEC strains that belonged to group B2 had more PAI sequences compared to those of the other groups, especially group B1, which had virulence genes but the lowest percentage of PAI sequences, which leads us to conclude that the virulence of ExPEC strains characterized as B2 is likely attributed to PAI encoded genes, whereas the virulence of ExPEC strains belonging to phylogenetic group B1 is likely due to plasmid encoded virulence genes

    Nanopharmaceuticals for eye administration: sterilization, depyrogenation and clinical applications

    As an immune-privileged target organ, the eyes have important superficial and internal barriers, protecting them from physical and chemical damage from exogenous and/or endogenous origins that would cause injury to visual acuity or even vision loss. These anatomic, physiological and histologic barriers are thus a challenge for drug access and entry into the eye. Novel therapeutic concepts are highly desirable for eye treatment. The design of an efficient ocular drug delivery system still remains a challenge. Although nanotechnology may offer the ability to detect and treat eye diseases, successful treatment approaches are still in demand. The growing interest in nanopharmaceuticals offers the opportunity to improve ophthalmic treatments. Besides their size, which needs to be critically monitored, nanopharmaceuticals for ophthalmic applications have to be produced under sterilized conditions. In this work, we have revised the different sterilization and depyrogenation methods for ophthalmic nanopharmaceuticals with their merits and drawbacks.     Otimização da extração sólido-líquido com partição em baixa temperatura para determinação de carbofurano em Cucurbita pepo l (“abobrinha”) por cromatografia líquida de alta eficiência

    According to results of the Program of Analysis of Pesticide Residues in Foods of the National Agency of Sanitary in Brazil (2013 to 2015) the zucchini, Cucurbita pepo L is one of the non-hardy vegetables, that presented higher contamination index by pesticides residues. One of these agrochemicals is carbofuran, which although not authorized for this crop, is commonly and illegally used for whitefly control. The objective of this study was to optimize the solid-liquid extraction method with low temperature partitioning (SLE/LTP) for the determination of carbofuran residues in zucchini by high performance liquid chromatography with ultraviolet detector (HPLC-UV). The optimization was performed with a complete factorial design, 23 to evaluate the simultaneous performance of three factors in two levels, in triplicate. The optimized SLE/LTP-HPLC/UV method showed to be selective, simple and efficient, consuming a small amount of sample and solvent extractor. The average recovery percentage was 75.88%. Samples of zucchini purchased in the Itumbiara-GO trade, Brazil, and submitted to the optimized method did not present carbofuran residues
