    K-CPT in a Brazilian sample: description of performance and comparison with North American norms

    INTRODUCTION: This study investigated the performance of a sample of Brazilian children aged 4-5 years on the Conners' Kiddie CPT (K-CPT), a computer-based task used to assess attention and inhibitory control in children. METHODS: Scores of Brazilian children pre-screened for attention disorders were obtained using the K-CPT, and data were compared with North American norms. RESULTS: Age and gender effects on the Brazilian sample were similar to those previously described. However, the lack of screening for attention deficits might have distorted normative scores of the K-CPT in the USA because Brazilian boys and girls had better scores than the North American children in almost all measures. CONCLUSION: The screening procedures used in Brazil, in addition to the previously described age and gender effects on sustained attention, indicate that the results described in the present study constitute appropriate local performance scores and, as such, are useful in the evaluation of pre-school children in Brazil until national norms are established.INTRODUÇÃO: O presente estudo investigou o desempenho de uma amostra de crianças brasileiras de 4 a 5 anos de idade no Conners' Kiddie CPT (K-CPT), uma tarefa computadorizada utilizada na avaliação da atenção e do controle inibitório em crianças. MÉTODO: Os escores de crianças brasileiras, que passaram por uma triagem para excluir déficit de atenção, foram comparados com as normas norte-americanas. RESULTADOS: Os efeitos de gênero e idade na nossa amostra foram similares aos descritos na literatura. Entretanto, a falta de triagem para déficit de atenção pode ter distorcido os dados normativos norte-americanos, pois meninos e meninas brasileiros tiveram melhores escores em quase todas as medidas. CONCLUSÕES: Os procedimentos de triagem aqui empregados, juntamente com a obtenção dos efeitos de idade e gênero previamente descritos, confirmam a adequação dos escores obtidos como referência local de desempenho, que podem ser úteis na avaliação de crianças pré-escolares no Brasil até que normas nacionais sejam estabelecidas.Universidade Federal de São Paulo (UNIFESP)UNIFESPSciEL

    Este estudo teve como objetivo analisar o uso do NEUPSILIN-Inf num modelo de avaliação neuropsicológica breve em crianças atendidas em um serviço assistencial. As etapas seguidas no modelo de avaliação neuropsicológica breve são: sala de espera, grupo de crianças e avaliação neuropsicológica individual. A análise da aplicabilidade do NEUPSILIN-Inf na detecção de déficits cognitivos foi realizada comparando o desempenho de 171 crianças com possíveis transtornos do neurodesenvolvimento a 358 crianças da amostra de normatização, tendo a média marginal do escore ajustado pela idade. Em todas as tarefas do NEUPSILIN-Inf, o grupo clínico mostrou desempenho inferior. O conjunto de dados do modelo de avaliação pode gerar hipóteses diagnósticas, como deficiência intelectual, possíveis quadros de transtornos da aprendizagem ou específicos de linguagem. Frente à alta escassez de atendimento público para crianças com possíveis transtornos do neurodesenvolvimento, este pode ser um modelo economicamente eficiente para centros de saúde em diversas regiões do Brasil.This study aimed to analyze the use of NEUPSILIN-Inf in a model of brief neuropsychological assessment for children attending a helthcare service. The set of data derived from the evaluation model can underlie diagnostic hypotheses, such as intellectual disabilities, learning disorders or specific language impairment.     Perfil clínico e neuropsicológico de crianças com transtorno do deficit de atenção e hiperatividade

    The aim of this study was to evaluate clinical and neuropsychological findings in children with suspicion of attention deficity hyperactivity disorder (ADHD). The assessment involved 150 children aged 7 to 14 referred to NANI at UNIFESP. RESULTS: 75 children (55 M and 20 F) fulfilled the criteria for ADHD, among which 35 were of the inattentive type, 28 of combined type and 12 were hyperactive/impulsive. There was negative correlation between the digit score and the Corsi test. Children with hyperactivity and impulsivity had a low performance for functional memory. Children with oppositional defiant disorder presented pattern changes in adaptability when there was a change in the rhythm the stimuli were presented and lower adaptation to time variability (Hit RT), in addition to higher rates of omission in the continuous performance test. CONCLUSION: This study suggests multiple interrelations between the scores of neuropsychological battery useful for detailed delimitation of the clinical profile of children with ADHD.O objetivo deste trabalho foi delimitar indicadores clínicos e neuropsicológicos em crianças com suspeita de transtorno do déficit de atenção e hiperatividade (TDA/H). Foram avaliadas 150 crianças (idade de 7 a 14 anos) encaminhados ao NANI da UNIFESP. RESULTADOS: 75 crianças (55 M e 20 F) preenchiam os critérios para o TDA/H, dentre os quais 35 (46,6%) pacientes eram desatentos, 28 do tipo combinado e 12 do tipo hiperatividade/impulsivo. Observou-se correlação negativa com o escore de dígitos e no escore do teste de Corsi. Crianças com hiperatividade e impulsividade apresentaram baixo desempenho nas funções relacionadas à memória operacional. Crianças com transtorno opositor desafiante apresentaram alterações nos padrões de adaptação às mudanças do ritmo com menor adaptação às variações do tempo de exposição aos estímulos (Hit RT), além de maiores taxas de omissão no teste contínuo de performance.     A model for pediatric and neuropsychological screening assessment of children with learning disabilities

    ABSTRACT Objectives: The high frequency of learning difficulties, attention disorders or developmental delay in children in the early years of schooling has resulted in a greater demand for pediatric services. Such services generally include assessments covering various specialties, are lengthy and often inaccessible to families due to prohibitively high cost. This paper presents an economically efficient model of interdisciplinary diagnosis. Methods: A group of 109 Brazilian students from public schools aged between 5 and 14 years old, referred by teachers for a history of learning disabilities, behavioral changes or language problems, was evaluated at the NANI (Nucleo de Atendimento Neuropsicologico Infantil). Assessments were performed simultaneously during a single day's attendance and comprised clinical-genetic examination, behavioral assessment and neuropsychological screening, specially developed for the process. The multiaxial system of DSM-IV was adopted for diagnostic description. Results: The results revealed heterogeneity in diagnoses which included specific learning disorders (25.7%), mild intellectual disabilities (17.43%), as well as suspected dysmorphic features (11.93%). Logistic regression showed good sensitivity of neuropsychological screening in the detection of predictive factors for specific developmental disorders, while working memory (p=0.05) and language (p=0.02) problems were found to be higher risk. Conclusions: The model adopted proved to be useful for defining the diagnosis of several conditions in infancy, and can be incorporated into specialized clinics such as psychiatric or developmental pediatric services

    Performance patterns in Conners' CPT among children with attention deficit hyperactivity disorder and dyslexia

    This study investigated the performance of children with attention deficit hyperactivity disorder (ADHD) and dyslexia using Conners' Continuous Performance Test (CCPT). The clinical groups were composed of 52 children with ADHD and 32 children with dyslexia. Performance in the CCPT was evaluated using ANCOVA to compare the clinical groups with the normative Brazilian sample. The ADHD group performed worse than the normative sample in almost all of the measurements, except for reaction time and response style. The dyslexia group scored higher on commissions, variability, perseverations and inconsistency in the reaction time over the six time blocks (Hit SE Block Change) than the children in the normative Brazilian sample. The ADHD and dyslexia groups differed in omission measurements, Hit RT SE, variability, perseverations, Hit RT Interstimulus Intervals (ISI) Change and Hit SE ISI Change. We thus found that the dyslexia group had specific deficit patterns, with greater response to non-target stimuli, greater perseveration and response variability, and difficulties in hit reaction time as the test progressed
