Local Development and the Role of Small Towns in Space Organisation in Contemporary Poland

The choice of appropriate growth strategy based on the right diagnosis of the situation, taking advantage of the opportunities and mobility of the local community will be decisive for the position of each town in the settlement network. Activation of small towns must take place on many planes – direct efforts to forward socio-economic growth and improve spatial development should be accompanied by increased environmental awareness. The main positive effects of the past two decades of transformations in small towns include greater sense of responsibility for the neighbourhood and enhanced quality of urban space, particularly the improvement of urban infrastructure and housing conditions
Risk factors of ischemic stroke in children I. Selected biochemical and immunological risk factors of ischemic stroke in children
W grupie pacjentów będących pod opieką pediatrów udar mózgu występuje stosunkowo rzadko - u około 3 osób/100 000/rok, przy czym udar niedokrwienny stanowi niespełna 50%. Mimo że częstość udaru niedokrwiennego u dzieci jest wielokrotnie
mniejsza niż u dorosłych, to jednak incydenty ostrego niedokrwienia ośrodkowego układu nerwowego (OUN) są
przyczyną zaburzeń ruchowych oraz ekspresji mowy, opóźnienia rozwoju i trudności szkolnych, a także napadów padaczkowych,
a tym samym - istotnie wpływają na przyszłość chorego dziecka. Czynniki ryzyka chorób naczyniowych mózgu,
w tym udaru niedokrwiennego, w grupie pacjentów w wieku rozwojowym są znacznie mniej poznane niż u dorosłych. Mimo
przeprowadzenia szerokiej diagnostyki przyczyna udaru pozostaje nieznana u około 20–30% chorych. W ostatnich latach
oprócz tradycyjnych czynników związanych etiopatogenetycznie z występowaniem udaru mózgu u dzieci, takich jak wady
serca, nabyte choroby serca czy infekcje, podkreśla się znaczenie czynników biochemicznych i immunologicznych. Wśród
nich szczególną rolę odgrywają zaburzenia gospodarki lipidowej, koagulopatie związane z deficytem białek - naturalnych
antykoagulantów (białko C, białko S, antytrombina III), oporność na aktywną postać białka C (APCR, resistance to activated
protein C), lipoproteina (a) oraz zespół antyfosfolipidowy.Opposite to the adult group of patients, stroke is rather rare condition in pediatric patients. It occurs with the frequency of
about 3/100 000 children/year; ischemic stroke is less than 50% of general number. Although stroke occurs rare in children,
but accidents of acute brain ischemia are the reason of motor disturbances, speach problems, developmental delay, school
difficulties and epileptic seizures, and this is why it is so harmful for sick child’s future life. Risk factors for cerebrovascular
diseases in pediatric patients have not been known as well as in adult group. Even if the diagnostic procedures are wide still
about 20–30% of children the reason of stroke remains unknown. In recent years in spite of traditional risk factors of
ischemic stroke, e.g. heart defects, aquired heart diseases or infections, also biochemical and immunological risk factors are
concidered. The most important of them are: lipid metabolism disturbances, coagulopathies (deficiencies of protein C,
protein S, antithrombin III) APCR, lp(a) and antiphospholipid syndrome
Risk factors of ischemic stroke in children II. Lipid metabolism abnormalities in etiopathogenesis of ischemic stroke in children
Background. Ischemic stroke is rather rare condition in children. It occurs
with the frequency of 0.63/100 000/year. After that the consequences of stroke,
like motor disturbances, speach problems, developmental delay and epileptic seizures,
are significant for child’e future life.This is why the attempt to establish
of risk factors of stroke and its potential reccurence in every patient is so
important. In spite of „classical” risk factors, like inherited and aquired heart
diseases or infections also biochemical disturbances, e.g. lipid metabolism and
hiperlipoproteinemia (a), are concidered. The aim of the study was evaluation
of lipid metabolism parameters and concentration of lp(a) in children who underwent
ischemic stroke and comparison of the data with the group of healthy children.
Material and methods. The examination was conducted in 38 children (16
girls and 22 boys). The personal medical history profile concerning the actual
illness and directed towards the occurence of cerebrovascular diseases in patient’s
family was created for each patient. Physical and neurological examination, and
neuroimaging were performed in each case. Moreover the examination of blood serum
concentration of: total cholesterol and it’s fractions, triglicerides and lp(a)
was performed. The control group consisted of 38 healthy children of similiar
age.
Results. Statistically significant differences between both examined and
control groups has been proved in the concentration of total cholesterol and it’s
fractions, and lp(a).
Conclusions. It is necessary to perform the analysis of serum lipids in
each pediatric patient after an acute ischemic event of CNS even if "traditional"
risk factors of stroke were found.Wstęp. Udar niedokrwienny mózgu u dzieci to rzadka patologia - jej częstość
wynosi 0,63/100 000/rok. Jednak jej następstwa, czyli zaburzenia motoryki oraz
mowy, pogorszenie funkcjonowania intelektualnego, a także napady padaczkowe, mają
istotne znaczenie w rozwoju i dalszym życiu dziecka po przebytym ostrym niedokrwieniu
ośrodkowego układu nerwowego (OUN). Dlatego tak istotne jest dążenie do wykrycia
czynników ryzyka udaru oraz jego ewentualnego nawrotu u każdego pacjenta. Oprócz
znaczenia niekwestionowanych, „klasycznych” czynników ryzyka coraz częściej podkreśla
się rolę zaburzeń metabolicznych, w tym dyslipidemii oraz hiperlipoproteinemii
(a).Celem opisanego niżej badania była ocena parametrów gospodarki lipidowej
oraz stężenia lipoproteiny (a) [lp(a)] w grupie dzieci po przebytym udarze niedokrwiennym
mózgu oraz w grupie kontrolnej, a następnie ustalenie, które spośród ocenianych
parametrów wiążą się z największym zagrożeniem wystąpienia udaru niedokrwiennego
mózgu u dzieci.
Materiał i metody. Badaniem objęto 38 dzieci (16 dziewcząt, 22 chłopców)
po przebytym udarze niedokrwiennym mózgu. W każdym przypadku przeprowadzono dokładny
wywiad, badanie przedmiotowe i neurologiczne, a także neuroobrazowanie morfologiczne
i czynnościowe. Ponadto oceniono stężenia: cholesterolu całkowitego i jego frakcji,
triglicerydów, a także lp(a). Grupę porównawczą stanowiło 38 dzieci nieobciążonych
chorobami naczyniowymi OUN, odpowiadających pod względem wieku i płci pacjentom
z grupy badanej.
Wyniki. Między dwiema badanymi grupami stwierdzono różnice znamienne statystycznie
w zakresie stężeń: cholesterolu całkowitego i jego frakcji oraz lp(a).
Wnioski. U każdego pacjenta w wieku rozwojowym po przebytym incydencie
ostrego niedokrwienia OUN konieczne jest zbadanie profilu lipidowego, nawet jeśli
stwierdzono "tradycyjne" czynniki ryzyka udaru
Malformations of cortical development in children : clinical manifestation, neuroimaging and neuropathology in selected cases
Cerebral cortical development can be divided into three steps: cellular proliferation, neuronal migration and organization.
Based on known pathologic, genetic and neuroimaging features a classification for malformations of cortical
development was proposed by Barkovich in 2001, and updated in 2005. Malformations of cerebral cortex development
(MCCD) often demonstrate epileptic seizures and delay in psychomotor development. About 20-40% of children with
epilepsy are drug-resistant and there is a large paediatric population requiring epilepsy surgery operations. In our work
we performed clinical analysis of 68 children with MCCD treated in our hospital between 2000 and 2006. In our work to
consider the type of MCCD we used the updated classification scheme proposed by Barkovich et al. We analyzed
epilepsy, gestational and perinatal history, initial symptoms, time to establishing full diagnosis and
neurodevelopmental/IQ status. In our results we found that despite similar clinical manifestation neuropathological
basis could be significantly different, and vice versa: children with nearly identical neuropathological findings could have
completely different neurological and radiological symptoms. Children with drug-resistant epilepsy are potential
candidates for neurosurgical treatment; especially lesionectomies in such cases could be very promising in terms of
epilepsy management and quality of life as well
Clinical and neuropathological picture of familial encephalopathy with bifunctional protein deficiency
Peroxisomal diseases are a heterogeneous group of genetic metabolic disorders which are caused by incorrect
biogenesis of peroxisomes or a defect in activity of particular enzymes located in those organelles.
D-bifunctional protein (D-BP) deficiency belongs to the second group of peroxisomal diseases characterised by
dysfunction of a single peroxisomal enzyme. Bifunctional protein is a catalyst in the second and third stage of the
\beta-oxidation of fatty acids. Gene locus of bifunctional protein deficiency comprises chromosomes 5q2 and 3p23-p22.
The authors present two siblings with progressing family encephalopathy. In the younger brother the diagnosis of
a bifunctional protein deficiency was made. The girl died before a diagnosis was made; however, due to the presence
of a very similar clinical condition a suspicion arises that the girl had a peroxisomal disease. In the siblings were
ascertained characteristic dysmorphic features, delayed psychomotor development, polymorphic epileptic seizures
and generalized muscular hypotonia with areflexia.
The neuropathological findings were consistent in general with MRI findings showing features of hypomyelination.
Also neuron heterotopias that were found in autopsy are a form of pathology typical for D-BP
Efekt steroidoterapii u 16-letniej pacjentki obserwowanej w kierunku limfocytarnego zapalenia przysadki – opis przypadku.
Not required for Clinical Vignette.Wstęp: Autoimmunologiczne zapalenie przysadki (AZP) występuje u dzieci bardzo rzadko, opisano mniej niż 100 przypadków. Najczęstszą postacią AZP jest limfocytarne zapalenie przysadki (LZP). Zwykle LZP prowadzi do niedoczynności i powiększenia przysadki. Najczęstszymi objawami u młodzieży oprócz niedoboru wazopresyny argininowej są niedoczynność nadnerczy, hipogonadyzm, bóle głowy i dwojenie.
Opis przypadku: 16-letnia, dotychczas zdrowa pacjentka, została przyjęta do szpitala z powodu bólu głowy, wymiotów, w wywiadzie dwumiesięczny okres polidypsji i poliurii. Przy przyjęciu obserwowano nadmierną senność, osłabienie i zez obuoczny.
Badania endokrynologiczne wykazały centralną niedoczynność tarczycy i niedobór wazopresyny argininowej. Stężenia siarczanu dehydroepiandrosteronu (DHEA-S) i hormonu adrenokortykotropowego (ACTH) były poniżej normy. Stwierdzono prawidłowy poziom hormonu folikulotropowego (FSH), hormonu luteinizującego (LH) i prolaktyny, obniżone stężenie estradiolu. W badaniu rezonansu magnetycznego (MRI) głowy w sekwencji T2 wykazano hiperintensywną zmianę siodłową/nadsiodłową o wymiarach 10x9x21mm z grubolinijnym wzmocnieniem kontrastowym na jej obwodzie oraz pogrubienie szypuły przysadki do 4,5mm. W ocenie okulistycznej ostrość i pole widzenia były w normie.
Leczenie rozpoczęto od deksametazonu stosowanego dożylnie (kolejno prednizon doustnie), a następnie zastosowano lewotyroksynę i desmopresynę. Bóle głowy, zez oraz osłabienie całkowicie ustąpiły. Po 2 miesiącach steroidoterapii badanie MRI mózgu nie ujawniło opisanej wcześniej zmiany ogniskowej przysadki, a jedynie niejednorodny obraz przysadki i szypuły, wskazujący na przebyty proces zapalny.
Wnioski: LZP jest rzadką przyczyną wielohormonalnej niedoczynności przysadki u dzieci i młodzieży. W leczeniu farmakologicznym stosuje się steroidy, a z powodu niedoczynności przysadki hormonalne leczenie substytucyjne. Konieczna długofalowa obserwacja pacjentów, ze względu na ryzyko nawrotu zmian zapalnych przysadki
Cloninger's Psychobiological Model of Personality and Strelau's Regulative Theory of Temperament - analysis of their associations in a Polish sample
The present study explores the relationship between Cloninger scales and Strelau's Regulative Theory of Temperament
(RTT ) traits. Cloninger's psychobiological model identifies four dimensions of temperament (Novelty seeking, Harm
Avoidance, Reward Dependence and Persistence) and three dimensions of character (Self-Directedness, Cooperativeness
and Self-Transcendence). RTT proposes the traits of Briskness, Perseveration, Sensory Sensitivity, Emotional Reactivity,
Endurance and Activity as the basic dimensions underlying individual differences. The relationships between the
dimensions of Cloninger's Temperament and Character and Strelau's Regulative Theory of Temperament are investigated
in a sample of 282 participants. Data analysis demonstrated some significant correlations between the two models. The
strongest associations were found between the dimension of Harm Avoidance from Cloninger’s concept and Strelau’s
RTT traits (positive correlations from r = .73 for Emotional Reactivity to r = .48 for Perseveration, and negative
correlations from r = -.57 for Endurance to r = -.51 for Briskness). However, there is not enough evidence to suggest
that the two models offer an alternative way of explaining individual differences. According to RTT, temperamental traits
are expressed in formal characteristics of behavior (energetic and temporal) and every kind of behavior (irrespective of
its content) can be described in the same formal categories. In the Psychobiological Model of Personality the traits are
characterized rather by the content or goals of behavior. In this model, in addition to underlining the biological variation
of heritable traits, the social, cultural and phenotypical levels of behavior are reflected
