    Deep brain stimulation for dystonia in Finland during 2007-2016

    BackgroundDystonia is a movement disorder substantially affecting the quality of life and the ability to work. A proportion of patients does not respond to first line pharmacotherapy. Deep brain stimulation (DBS) is established as a primary operative treatment option for severe drug resistant dystonia. We studied dystonia patients treated with DBS in Finland between the years 2007-2016 to evaluate the use and outcomes of DBS treatment.MethodsWe analysed the hospital records of dystonia patients, who underwent DBS operation during 2007-2016 in Finland. The clinical and technical parameters were recorded as well as preoperative assessments and treatments. The response to DBS was evaluated retrospectively using the Global Dystonia Rating Scale (GDS).ResultsOut of 585dB implantations during the study period, 37 were done for dystonia. The clinical response improved significantly with time in the isolated focal dystonia group, and at 12months, 22 of 32 patients had over 50% alleviation of the GDS score. There was only one subclinical intracerebral haemorrhage, and four infections leading to revision. Speech impairment and limb coordination problems were common stimulation- related adverse events and were mostly resolved or relieved with the adjustment of stimulation parameters.ConclusionsDBS seems to be beneficial in dystonia. Although DBS is indicated for dystonia in Finland, the number of operations did not increase at the same rate as DBS operations in general. DBS appears to be a safe and effective treatment for focal as well as generalized dystonia.Peer reviewe

    Atmospheric humidity affects global variation of bat echolocation via indirect effects

    The peak frequency of bat echolocation is a species-specific functional trait linked to foraging ecology. It is tailored via evolution to suit conditions within the distribution range of each species, but the evolutionary drivers are not yet well-understood. Global patterns of humidity correlate with many aspects of bat ecology. We hypothesized that atmospheric absolute humidity could explain global peak frequency variation directly and indirectly via increasing species body size and bat species richness. These hypotheses were tested using Bayesian phylogenetic path analysis on 226 tropical and subtropical bat species. In line with our predictions, we found a positive total effect of humidity on peak frequency, which was dominated by the positive indirect effects via body size and bat species richness. We did not observe the negative direct effect of humidity on peak frequency, which was hypothesized based on atmospheric attenuation of sound. In line with our expectations, excluding the predominantly clutter foraging bat families from our dataset downplayed the importance of the richness-mediated route. To conclude, our findings suggest that indirect effects, owing to ecology and biogeography of bat taxa, play a major role in the global relationship between peak frequency and atmospheric humidity.Peer reviewe

    Calculating the 3D magnetic field of ITER for European TBM studies

    The magnetic perturbation due to the ferromagnetic test blanket modules (TBMs) may deteriorate fast ion confinement in ITER. This effect must be quantified by numerical studies in 3D. We have implemented a combined finite element method (FEM) -- Biot-Savart law integrator method (BSLIM) to calculate the ITER 3D magnetic field and vector potential in detail. Unavoidable geometry simplifications changed the mass of the TBMs and ferritic inserts (FIs) up to 26%. This has been compensated for by modifying the nonlinear ferromagnetic material properties accordingly. Despite the simplifications, the computation geometry and the calculated fields are highly detailed. The combination of careful FEM mesh design and using BSLIM enables the use of the fields unsmoothed for particle orbit-following simulations. The magnetic field was found to agree with earlier calculations and revealed finer details. Unmanned mobile technology implementation for soil sampling automation is significantly increasing the efficiency of the process. This automated and remotely controlled technology is enabling more frequent sample collection than traditional human operated manual methods. In this publication universal mobile robotic platform is adapted and modified to collect and store soil s amples from fields and measure soil parameters simultaneously.     Connectivity of larval cod in the transition area between North Sea and Baltic Sea and potential implications for fisheries management

    Connectivity of pelagic, early life stages via transport by ocean currents may affect survival chances of offspring, recruitment success, and mixing of stocks across management units. Based on drift model studies, transport patterns of particles representing exogenously feeding cod larvae in the transition area between North Sea and Baltic were investigated to (i) determine long-term trends and variability in advective transport of larvae from spawning grounds to juvenile nursery areas, (ii) estimate the degree of exchange between different management areas, and (iii) compare the results with spatial distributions of juvenile cod. The transport of particles showed considerable intra- and interannual variability, but also some general patterns of retention within and dispersion to different management areas. Good spatial overlap of particle end positions, representing potential juvenile settlement areas, with observed distributions of juveniles in bottom trawl surveys suggests that the drift simulations provide reasonable estimates of early life stage connectivity between cod populations in the investigated areas. High exchange rates of particles between management areas of up to ca. 70% suggest that cod populations in the investigated areas are demographically correlated.     Effect of hydrochlorothiazide on serum uric acid concentration : a genome-wide association study

    Aim: To recognize genetic associations of hydrochlorothiazide-induced change in serum uric acid (SUA) concentration. Patients & methods: We conducted a genome-wide association study on hydrochlorothiazide-induced change in SUA in 214 Finnish men from the GENRES study. Replication analyses were performed in 465 Finns from the LIFE study. Results: In GENRES, we identified 31 loci associated with hydrochlorothiazide-induced change in SUA at p <5 x 10(-5). rs1002976 near VEGFC associated with the change in GENRES and in LIFE. rs950569 near BRINP3 associated with the change in SUA in GENRES and LIFE. The analysis of previously reported SNPs and candidate genes provided some proof for PADI4 and ABCC4. Conclusion: We report genetic markers that may predict the increase in SUA concentration during thiazide treatment.Peer reviewe

    Levodopa-Induced Changes in Electromyographic Patterns in Patients with Advanced Parkinson's Disease

    Levodopa medication is the most efficient treatment for motor symptoms of Parkinson's disease (PD). Levodopa significantly alleviates rigidity, rest tremor, and bradykinesia in PD. The severity of motor symptoms can be graded with UPDRS-III scale. Levodopa challenge test is routinely used to assess patients' eligibility to deep-brain stimulation (DBS) in PD. Feasible and objective measurements to assess motor symptoms of PD during levodopa challenge test would be helpful in unifying the treatment. Twelve patients with advanced PD who were candidates for DBS treatment were recruited to the study. Measurements were done in four phases before and after levodopa challenge test. Rest tremor and rigidity were evaluated using UPDRS-III score. Electromyographic (EMG) signals from biceps brachii and kinematic signals from forearm were recorded with wireless measurement setup. The patients performed two different tasks: arm isometric tension and arm passive flexion-extension. The electromyographic and the kinematic signals were analyzed with parametric, principal component, and spectrum-based approaches. The principal component approach for isometric tension EMG signals showed significant decline in characteristics related to PD during levodopa challenge test. The spectral approach on passive flexion-extension EMG signals showed a significant decrease on involuntary muscle activity during the levodopa challenge test. Both effects were stronger during the levodopa challenge test compared to that of patients' personal medication. There were no significant changes in the parametric approach for EMG and kinematic signals during the measurement. The results show that a wireless and wearable measurement and analysis can be used to study the effect of levodopa medication in advanced Parkinson's disease.Peer reviewe

    Signal features of surface electromyography in advanced Parkinson's disease during different settings of deep brain stimulation

    Objective: Electromyography (EMG) and acceleration (ACC) measurements are potential methods for quantifying efficacy of deep brain stimulation (DBS) treatment in Parkinson's disease (PD). The treatment efficacy depends on the settings of DBS parameters (pulse amplitude, frequency and width). This study quantified, if EMG and ACC signal features differ between different DBS settings and if DBS effect is unequal between different muscles. Methods: EMGs were measured from biceps brachii (BB) and tibialis anterior (TA) muscles of 13 PD patients. ACCs were measured from wrists. Measurements were performed during seven different settings of DBS and analyzed using methods based on spectral analysis, signal morphology and nonlinear dynamics. Results: The results showed significant within-subject differences in the EMG signal kurtosis, correlation dimension, recurrence rate and EMG-ACC coherence between different DBS settings for BB but not for TA muscles. Correlations between EMG feature values and clinical rest tremor and rigidity scores were weak but significant. Conclusions: Surface EMG features differed between different DBS settings and DBS effect was unequal between upper and lower limb muscles. Significance: EMG changes pointed to previously defined optimal settings in most of patients, which should be quantified even more deeply in the upcoming studies.     Physical activity as a protective factor for dementia and Alzheimer's disease : systematic review, meta-analysis and quality assessment of cohort and case-control studies

    Objective Physical activity (PA) is associated with a decreased incidence of dementia, but much of the evidence comes from short follow-ups prone to reverse causation. This meta-analysis investigates the effect of study length on the association. Design A systematic review and meta-analysis. Pooled effect sizes, dose-response analysis and funnel plots were used to synthesise the results. Data sources CINAHL (last search 19 October 2021), PsycInfo, Scopus, PubMed, Web of Science (21 October 2021) and SPORTDiscus (26 October 2021). Eligibility criteria Studies of adults with a prospective follow-up of at least 1 year, a valid cognitive measure or cohort in mid-life at baseline and an estimate of the association between baseline PA and follow-up all-cause dementia, Alzheimer's disease or vascular dementia were included (n=58). Results PA was associated with a decreased risk of all-cause dementia (pooled relative risk 0.80, 95% CI 0.77 to 0.84, n=257 983), Alzheimer's disease (0.86, 95% CI 0.80 to 0.93, n=128 261) and vascular dementia (0.79, 95% CI 0.66 to 0.95, n=33 870), even in longer follow-ups (>= 20 years) for all-cause dementia and Alzheimer's disease. Neither baseline age, follow-up length nor study quality significantly moderated the associations. Dose-response meta-analyses revealed significant linear, spline and quadratic trends within estimates for all-cause dementia incidence, but only a significant spline trend for Alzheimer's disease. Funnel plots showed possible publication bias for all-cause dementia and Alzheimer's disease. Conclusion PA was associated with lower incidence of all-cause dementia and Alzheimer's disease, even in longer follow-ups, supporting PA as a modifiable protective lifestyle factor, even after reducing the effects of reverse causation.Peer reviewe
