Can exercise limits prevent post-exertional malaise in chronic fatigue syndrome? An uncontrolled clinical trial.
<b>Objective</b>: It was hypothesized that the use of exercise limits prevents symptom increases and worsening of their health status following a walking exercise in people with Chronic Fatigue Syndrome (CFS).
<b>Design</b>: An uncontrolled clinical trial (semi-experimental design).
<b>Setting</b>: Outpatient clinic of a university department.
<b>Subjects</b>: 24 patients with CFS.
<b>Interventions</b>: Subjects undertook a walking test with the two concurrent exercise limits. Each subject walked at an <i>intensity</i> where the maximum heart rate was determined by heart rate corresponding to the respiratory exchange ratio =1.0 derived from a previous sub-maximal exercise test and for a duration calculated from how long each patient felt they were able to walk.
<b>Main outcome measures</b>: The Short Form 36 Health Survey or SF-36, the CFS Symptom List, and the CFS-Activities and Participation Questionnaire were filled in prior to, immediately and 24 hours post-exercise.
<b>Results</b>: The fatigue increase observed immediately post-exercise (p=0.006) returned to pre-exercise levels 24 hours post-exercise. The increase in pain observed immediately post-exercise was retained at 24 hours post-exercise (p=0.03). Fourteen of 24 subjects experienced a clinically meaningful change in bodily pain (change of SF-36 bodily pain score Âł10). Six of 24 participants indicated that the exercise bout had slightly worsened their health status, and 2 of 24 had a clinically meaningful decrease in vitality (change of SF-36 vitality score Âł20). There was no change in activity limitations/participation restrictions.
<b>Conclusion</b>: It was shown that the use of exercise limits (limiting both the intensity and duration of exercise) prevents important health status changes following a walking exercise in people with CFS, but was unable to prevent short-term symptom increases

Finding large matchings in 1-planar graphs of minimum degree 3
A matching is a set of edges without common endpoint. It was recently shown
that every 1-planar graph (i.e., a graph that can be drawn in the plane with at
most one crossing per edge) that has minimum degree 3 has a matching of size at
least , and this is tight for some graphs. The proof did not
come with an algorithm to find the matching more efficiently than a
general-purpose maximum-matching algorithm. In this paper, we give such an
algorithm. More generally, we show that any matching that has no augmenting
paths of length 9 or less has size at least in a 1-planar
graph with minimum degree 3
Antioxidant intervention in rheumatoid arthritis: results of an open pilot study
There is evidence that reactive oxygen species play a causal role in auto-immune diseases, such as rheumatoid arthritis (RA). Despite the supporting evidence for a beneficial effect of antioxidants on clinical characteristics of RA, the right balance for optimal effectiveness of antioxidants is largely unknown. To determine the potential beneficial effects of an antioxidant intervention on clinical parameters for RA, an open pilot study was designed. Eight non-smoking female patients with rheumatoid factor + RA and a Disease Activity Score (DAS 28) higher than 2.5 were enrolled in the study. Patients had to be receiving stable non-steroidal anti-inflammatory drug treatment and/or âsecond lineâ medication for at least 3Â months. The pilot group consumed 20Â g of antioxidant-enriched spread daily during a period of 10Â weeks. The intervention was stopped after 10Â weeks and was followed by a âwash-outâ period of 4Â weeks. At tâ=â0, tâ=â10Â weeks and tâ=â14Â weeks, patientsâ condition was assessed by means of DAS. In addition, standard laboratory analyses were performed, and blood-samples for antioxidants were taken. The antioxidant-enriched spread was well tolerated. All laboratory measures of inflammatory activity and oxidative modification were generally unchanged. However, the number of swollen and painful joints were significantly decreased and general health significantly increased, as reflected by a significantly improved (1.6) DAS at tâ=â10Â weeks. The antioxidant effect was considered beneficial as, compared to the scores at tâ=â0, the DAS significantly reduced at tâ=â10Â weeks. Increase of the DAS (0.7) after the âwash-out periodâ at tâ=â14 confirmed a causal relation between changes in clinical condition and antioxidants. This open pilot study aimed to assess the clinical relevance of an antioxidant intervention as a first step in assessing potential beneficial effects of antioxidants on rheumatoid arthritis. These conclusions need to be validated in a larger controlled study population

Currently there is insufficient discussion of best practices regarding the construction process. This chapter introduces the artificial society analytics platform (ASAP) as a way to spark discussion of best practices. ASAP is designed to be an extensible architecture capable of functioning as the core of many different types of inquiries into social dynamics. Here we describe ASAP, focusing on design decisions in several key areas, thereby exposing our assumptions and reasoning to critical scrutiny, hoping for discussion that can advance debate over best practices in artificial society construction. Hedgehog Spin-texture and Berry's Phase tuning in a Magnetic Topological Insulator
Understanding and control of spin degrees of freedom on the surfaces of
topological materials are key to future applications as well as for realizing
novel physics such as the axion electrodynamics associated with time-reversal
(TR) symmetry breaking on the surface. We experimentally demonstrate
magnetically induced spin reorientation phenomena simultaneous with a
Dirac-metal to gapped-insulator transition on the surfaces of manganese-doped
Bi2Se3 thin films. The resulting electronic groundstate exhibits unique
hedgehog-like spin textures at low energies, which directly demonstrate the
mechanics of TR symmetry breaking on the surface. We further show that an
insulating gap induced by quantum tunnelling between surfaces exhibits spin
texture modulation at low energies but respects TR invariance. These spin
phenomena and the control of their Fermi surface geometrical phase first
demonstrated in our experiments pave the way for the future realization of many

Meeting the support and information needs of women with advanced breast cancer: a randomised controlled trial
Addressing psychosocial and quality of life needs is central to provision of excellent care for people with advanced cancer. This study tested a brief nurse-delivered intervention to address the needs of urban women with advanced breast cancer. This study was conducted at four large urban hospitals in Australia. One hundred and five women with advanced breast cancer were recruited and randomised to receive the intervention or usual care, then asked to complete the European Organisation of Research and Treatment of Quality of life Q-C30 version (2.0) (EORTC Q-C30) (version 2) and Supportive Care Needs Survey (SCNS) at 1 month and 3 months postrecruitment. No significant differences were detected between intervention and usual care groups in the SCNS or the EORTC Q-C30 subscale scores. However, when the groups were divided into high needs (score of above 50) and low baseline needs (score of 50 or below) for each SCNS subscale, a significant difference between intervention and usual care groups was found in the psychological/emotional subscale among women with high baseline needs. In conclusions, this study demonstrated that a face-to-face session and follow-up phone call with a breast care nurse significantly reduced the psychological and emotional needs of those with high initial needs. There was no evidence of the intervention influencing the quality of life; or perceived needs of women with low initial psychological/emotional needs or perceived needs in other domains. Possibly, the intervention was not sufficiently intense to achieve an effect
Fermi Large Area Telescope observations of PSR J1836+5925
The discovery of the gamma-ray pulsar PSR J1836+5925, powering the formerly
unidentified EGRET source 3EG J1835+5918, was one of the early accomplishments
of the Fermi Large Area Telescope (LAT). Sitting 25 degrees off the Galactic
plane, PSR J1836+5925 is a 173 ms pulsar with a characteristic age of 1.8
million years, a spindown luminosity of 1.1 erg s, and a
large off-peak emission component, making it quite unusual among the known
gamma-ray pulsar population. We present an analysis of one year of LAT data,
including an updated timing solution, detailed spectral results and a long-term
light curve showing no indication of variability. No evidence for a surrounding
pulsar wind nebula is seen and the spectral characteristics of the off-peak
emission indicate it is likely magnetospheric. Analysis of recent XMM
observations of the X-ray counterpart yields a detailed characterization of its
spectrum, which, like Geminga, is consistent with that of a neutron star
showing evidence for both magnetospheric and thermal emission.Comment: Accepted to Astrophysical Journa
