    At-risk and problem gambling among Finnish youth : The examination of risky alcohol consumption, tobacco smoking, mental health and loneliness as gender-specific correlates

    AIMS - The aims were to compare past-year at-risk and problem gambling (ARPG) and other at-risk behaviours (computer gaming, risky alcohol consumption, tobacco smoking) by age and gender, and to explore how ARPG is associated with risky alcohol consumption, tobacco smoking, poor mental health and loneliness in males and females. DESIGN - Data from respondents aged 15-28 (n = 822) were derived from a cross-sectional random sample of population-based data (n = 4484). The data were collected in 2011-2012 by telephone interviews. The Problem Gambling Severity Index (PGSI, score >= 2) was used to evaluate ARPG. Prevalence rates for risk behaviours were compared for within gender-specific age groups. Regression models were gender-specific. RESULTS - The proportion of at-risk and problem gamblers was higher among males than females in all age groups except among 18-21-year-olds, while frequent computer gaming was higher among males in all age groups. The odds ratio (95% CI) of being a male ARPGer was 2.57 (1.40-4.74) for risky alcohol consumption; 1.95 (1.07-3.56) for tobacco smoking; 2.63 (0.96-7.26) for poor mental health; and 4.41 (1.20-16.23) for feeling lonely. Likewise, the odds ratio (95% CI) of being a female ARPGer was 1.19 (0.45-3.12) for risky alcohol consumption; 4.01 (1.43-11.24) for tobacco smoking; 0.99 (0.18-5.39) for poor mental health; and 6.46 (1.42-29.34) for feeling lonely. All 95% CIs of ARPG correlates overlapped among males and females. CONCLUSIONS - Overall, past-year at-risk and problem gambling and computer gaming seem to be more common among males than females; however, for risky alcohol consumption similar gender differences were evident only for the older half of the sample. No clear gender differences were seen in correlates associated with ARPG.Peer reviewe

    Shaping the midwifery profession in Nepal - Uncovering actors' connections using a Complex Adaptive Systems framework.

    OBJECTIVES: To explore how actors connect in a system aiming at promoting the establishment of a midwifery profession in Nepal. METHODS: A qualitative explorative study based on the framework of Complex Adaptive Systems. Semi-structured interviews were conducted with 17 key people representing eight different organisations (actors) promoting the development of the midwifery profession. RESULTS: The actors' connections can be described with a complex set of facilitators for and barriers to promoting the establishment of a midwifery profession. The identified facilitators for this establishment in Nepal are (1) a common goal and (2) a desire to collaborate, whilst the barriers are (1) different political interests and priorities, (2) competing interests of the nursing profession and societal views, (3) divergent academic opinions on a midwifery profession, and (4) insufficient communication. The results also showed that Nepalese society cannot distinguish between nursing and midwifery and that the public support for a midwifery profession was hence minimal. CONCLUSION: The move of midwifery from an occupation to a profession in Nepal is an on-going, challenging process. The study indicates the importance of understanding the motivations of, and barriers perceived by, actors that can promote or obstruct the establishment of the midwifery profession. It also points to the importance of informing the wider public about the role and responsibility of an autonomous midwifery profession

    Alloying of C40-structured Mo(Si,Al)2 with Nb, Ta and V

    Alloying of Mo(Si,Al)2, prepared by sintering, with Nb, Ta or V has been studied. All alloyed materials retained the C40-structured matrix phase of the un-alloyed reference material. The concentration of alloying elements in the C40 phase reached 0.4–1.6 at.% (approximately 20–50 % of the intended levels), and the remaining alloying content was dissolved in the minority D8m- and D88-structured (Mo,X)5(Si,Al)3 phases. V-alloying also promoted the formation of C54-structured (Mo,V)(Si,Al)2

    High temperature compression of Mo(Si,Al)2-Al2O3 composites

    The aim of this study was to investigate the effect on high temperature of mechanical properties of adding Al2O3 particles to polycrystalline Mo(Si,Al)2. Mo(Si,Al)2-Al2O3 composites, containing 0–25 wt% Al2O3 particles have been compression tested at 1300 \ub0C, and the microstructure after deformation was studied using electron backscatter diffraction. It was shown that even small amounts (5 wt%) of Al2O3 particles resulted in a grain-refined material through inhibition of grain growth during sintering, which lead to lower flow stresses compared to the coarse-grained Al2O3-free material. The inverse grain size effect and post-test microstructure investigations suggest that creep-like deformation mechanisms dominate in fine grained Mo(Si,Al)2-Al2O3 composites at 1300 \ub0C. In the materials containing 5–15 wt% Al2O3, the maximum stress decreased with increasing Al2O3 content. In materials with higher Al2O3 additions, the maximum stress increased with the Al2O3 addition, but did not reach the strength levels in the Al2O3-free reference material. It is suggested that the deformation behaviour is affected by electroplasticity effects as resistive heating was used. Electroplasticity contributes to the decrease in maximum stress observed in the lower Al2O3 containing materials, while this is outweighed by particle strengthening at higher Al2O3 contents

    High temperature deformation of polycrystalline C40 Mo(Si,Al)2

    Polycrystalline Mo(Si,Al)2 with C40 crystal structure was deformed in compression with a strain rate of 10−4 s−1 at 1300 \ub0C. The specimens were deformed to a strain of 10%–15% and showed maximum stresses around 150 MPa prior to pronounced softening. No crack formation or significant increase in porosity could be observed. Post-test microstructure analysis revealed that the material was inhomogeneously deformed on both inter- and intragranular levels. Dynamic recrystallization occurred alongside low angle grain boundary formation in highly deformed grains. Furthermore, complex intragranular deformation fields suggest that slip systems other than 21̄1̄0[0001] may be active during deformation

    Gender comparison of online and land-based gamblers from a nationally representative sample : Does gambling online pose elevated risk?

    The expansion of online gambling opportunities calls for better comprehension of online gambling, including relevant gender specific correlates. This study compared online and land-based gamblers among males and females separately, utilizing a nationally representative Finnish survey sample of 18-74 year olds. Online gamblers were younger than land-based gamblers and had full-time working status more often than land-based gamblers, with partial indication of land-based gamblers' monthly income being lower. Online gambling was associated with participation in computer or video gaming more strongly than with land-based gambling. Results show that the strongest predictors of online gambling common to both genders were younger age, computer gaming and gambling on multiple gambling types. Risky alcohol consumption and tobacco smoking were not associated to gambling mode when controlling for other factors. Results indicate that particularly for females online gambling may be related to higher relative expenditure and at-risk and problem gambling, providing implications for tailored interventions. The continued study of subgroups of gamblers is necessary to comprehensively understand the altering gambling milieu. (C) 2017 Elsevier Ltd. All rights reserved.Peer reviewe

    A Strategy For Identifying Putative Causes Of Gene Expression Variation In Human Cancer

    There is often a need to predict the impact of alterations in one variable on another variable. This is especially the case in cancer research, where much effort has been made to carry out large-scale gene expression screening by microarray techniques. However, the causes of this variability from one cancer to another and from one gene to another often remain unknown. In this study we present a systematic procedure for finding genes whose expression is altered by an intrinsic or extrinsic explanatory phenomenon. The procedure has three stages: preprocessing, data integration and statistical analysis. We tested and verified the utility of this approach in a study, where expression and copy number of 13,824 genes were determined in 14 breast cancer samples. The expression of 270 genes could be explained by the variability of gene copy number. These genes may represent an important set of primary, genetically "damaged" genes that drive cancer progression

    Intracerebral Hemorrhage among Blood Donors and Their Transfusion Recipients

    Importance: Recent reports have suggested that cerebral amyloid angiopathy, a common cause of multiple spontaneous intracerebral hemorrhages (ICHs), may be transmissible through parenteral injection of contaminated cadaveric pituitary hormone in humans. Objective: To determine whether spontaneous ICH in blood donors after blood donation is associated with development of spontaneous ICH in transfusion recipients. Design, Setting, and Participants: Exploratory retrospective cohort study using nationwide blood bank and health register data from Sweden (main cohort) and Denmark (validation cohort) and including all 1089370 patients aged 5 to 80 years recorded to have received a red blood cell transfusion from January 1, 1970 (Sweden), or January 1, 1980 (Denmark), until December 31, 2017. Exposures: Receipt of red blood cell transfusions from blood donors who subsequently developed (1) a single spontaneous ICH, (2) multiple spontaneous ICHs, or (3) no spontaneous ICH. Main Outcomes and Measures: Spontaneous ICH in transfusion recipients; ischemic stroke was a negative control outcome. Results: A total of 759858 patients from Sweden (median age, 65 [IQR, 48-73] years; 59% female) and 329512 from Denmark (median age, 64 [IQR, 50-73] years; 58% female) were included, with a median follow-up of 5.8 (IQR, 1.4-12.5) years and 6.1 (IQR, 1.5-11.6) years, respectively. Patients who underwent transfusion with red blood cell units from donors who developed multiple spontaneous ICHs had a significantly higher risk of a single spontaneous ICH themselves, compared with patients receiving transfusions from donors who did not develop spontaneous ICH, in both the Swedish cohort (unadjusted incidence rate [IR], 3.16 vs 1.12 per 1000 person-years; adjusted hazard ratio [HR], 2.73; 95% CI, 1.72-4.35; P <.001) and the Danish cohort (unadjusted IR, 2.82 vs 1.09 per 1000 person-years; adjusted HR, 2.32; 95% CI, 1.04-5.19; P =.04). No significant difference was found for patients receiving transfusions from donors who developed a single spontaneous ICH in the Swedish cohort (unadjusted IR, 1.35 vs 1.12 per 1000 person-years; adjusted HR, 1.06; 95% CI, 0.84-1.36; P =.62) nor the Danish cohort (unadjusted IR, 1.36 vs 1.09 per 1000 person-years; adjusted HR, 1.06; 95% CI, 0.70-1.60; P =.73), nor for ischemic stroke as a negative control outcome. Conclusions and Relevance: In an exploratory analysis of patients who received red blood cell transfusions, patients who underwent transfusion with red blood cells from donors who later developed multiple spontaneous ICHs were at significantly increased risk of spontaneous ICH themselves. This may suggest a transfusion-transmissible agent associated with some types of spontaneous ICH, although the findings may be susceptible to selection bias and residual confounding, and further research is needed to investigate if transfusion transmission of cerebral amyloid angiopathy might explain this association.
