    Evaluation of Formal posterior distributions via Markov chain arguments

    We consider evaluation of proper posterior distributions obtained from improper prior distributions. Our context is estimating a bounded function ϕ\phi of a parameter when the loss is quadratic. If the posterior mean of ϕ\phi is admissible for all bounded ϕ\phi, the posterior is strongly admissible. We give sufficient conditions for strong admissibility. These conditions involve the recurrence of a Markov chain associated with the estimation problem. We develop general sufficient conditions for recurrence of general state space Markov chains that are also of independent interest. Our main example concerns the pp-dimensional multivariate normal distribution with mean vector θ\theta when the prior distribution has the form g(θ2)dθg(\|\theta\|^2) d\theta on the parameter space Rp\mathbb{R}^p.     Wetting-induced volumetric collapse of UO2 powder beds and the consequence on transient nuclear criticality excursions

    Mathematical and computational models are proposed to simulate wetting-induced volumetric collapse of fissile powder beds. Slumping, nuclear thermal hydraulics, radiolytic gas, and steam production models are coupled with point neutron kinetics to investigate transient nuclear criticality excursions in two 5-wt% enriched UO2 fissile powder beds with varying levels of wetting-induced volumetric collapse. The two beds are distinguished by their mean powder particle size of 30 μm and 100 μm. For the UO2 powder beds modelled, the re-distribution of UO2 powder and moderator due to slumping introduced a negative reactivity into the system. This increased the amount of time taken for a delayed critical state to be reached once infiltration began, and also reduced the total fission energy generated over the course of the simulated transient. The total fission energy generated ranged from 42 MJ to 48 MJ 100 seconds after the initial nuclear criticality excursion was observed for the 30 μm sized UO2 powder bed. The fission energy of the larger sized powder bed (100 μm), varied from 42 MJ to 57 MJ. Larger discrepancies between the slumped and un-slumped initial peak power are predicted. Peak powers varied from 29.2 MW to 106 MW for the smaller-sized powder particles, whereas for larger particles, the peak powers varied from 255 MW to 501 MW

    Transient nuclear criticality excursion analysis of highly dispersed particulate three-phase fluidised systems

    The aim of this study was to perform sensitivity analysis, investigating how different fluidisation and sedimentation characteristics of three-phase wetted UO2 powder beds, may affect a transient nuclear criticality excursion initiated through the addition of water into a fissile powder bed. This type of postulated nuclear criticality accident scenario may occur in nuclear fuel fabrication facilities when a fire is fought through the use of water, supplied via an automatic or manual fire-suppression system. A similar scenario may also develop as a result of water leaks or flooding of the process area housing UO2 powder. The article introduces a model for gas-bubble induced fluidisation of a UO2 powder bed and examines how this phenomenon may affect the neutron kinetic response of a three-phase fluidised fissile powder system. Empirical analysis has shown that fissile suspensions form agglomerated structures when suspended in water, at agglomerate sizes that range from 18 to 40 . Simulation results indicate that both the critical gas velocity and rate of fluidisation may significantly affect transient nuclear criticality excursion dynamics. The re-distribution of fissile mass into a highly dispersed suspension generally reduces the reactivity of the system, however, depending on the H/U ratio, a positive reactivity may be added to the system. Low Péclet numbers in the suspension suggest that gas-bubble induced motion of the suspension causes a highly dispersive flow field. An oscillatory power response is predicted for low critical gas velocities where the reactivity of the system is predominantly governed by the re-distribution of fissile mass within the system. The frequency of these oscillations is greater for a higher hindered settling rate of powder particles. At a higher critical gas velocity, the transient nuclear criticality excursion is governed by the voidage reactivity feedback, making the response quite independent of fluidisation. In all cases, large volumes of UO2 powder may leave the domain due to overflowing of the suspension. At first sight this is counter-intuitive. It seems improbable that reducing the availability of a substrate that enables high-energy phosphate production via oxidative phosphorylation would be of benefit. But recent clinical data beyond the field of inherited metabolic disease support this approach

    Knowledge and attitudes of men to prostate cancer

    Objective: To ascertain the current level of understanding about prostate cancer (PCa), including treatment options and potential side effects of treatment, among older men. Design and Setting: Questionnaires administered by general practitioners (GPs) in 5 general practices in the Perth metropolitan and regional areas of Western Australia. Participants: Convenience sample of men aged 40-80 years (n=503) with or without prostate cancer presenting for routine consultations. Main outcome measures: Knowledge and attitudes of men to prostate cancer Results: Eighty percent of men did not know the function of the prostate and 48% failed to identify PCa as the most common internal cancer in men. Thirty-five percent had no knowledge of the treatments for PCa and 53% had no knowledge of the side effects of treatments. Asked how they would arrive at a decision about treatment, 70% stated they would ask the GP/specialist for all their options and then decide themselves. Conclusion: This study confirms a deficit in knowledge of the disease among men in the at risk age group. Lack of knowledge encompassed areas which could delay diagnosis and hence treatment. Overall the population preferred some GP/specialist involvement in treatment decision making

    Joint inversion of long-period magnetotelluric data and surface-wave dispersion curves for anisotropic structure: Application to data from Central Germany

    Geophysical datasets sensitive to different physical parameters can be used to improve resolution of Earth's internal structure. Herein, we jointly invert long-period magnetotelluric (MT) data and surface-wave dispersion curves. Our approach is based on a joint inversion using a genetic algorithm for a one-dimensional (1-D) isotropic structure, which we extend to 1-D anisotropic media. We apply our new anisotropic joint inversion to datasets from Central Germany demonstrating the capacity of our joint inversion algorithm to establish a 1-D anisotropic model that fits MT and seismic datasets simultaneously and providing new information regarding the deep structure in Central Germany. The lithosphere/asthenosphere boundary is found at approx. 84 km depth and two main anisotropic layers with coincident most conductive/seismic fast-axis direction are resolved at lower crustal and asthenospheric depths. We also quantify the amount of seismic and electrical anisotropy in the asthenosphere showing an emerging agreement between the two anisotropic coefficients

    Regular appraisal and revalidation are now a routine part of professional life for doctors in the UK. For British-trained psychiatrists working abroad (in either development/humanitarian or academic fields) this is a cause of insecurity, as most of the processes of revalidation are tailored to those working in the standard structures of the National Health Service. This article explores the degree to which a peer group for psychiatrists working abroad has achieved its aim of helping its members to fulfil their revalidation requirements. It gives recommendations for how those considering work abroad can maximise their chances of remaining recognised under the revalidation system

    Fiber depolymerization

    Depolymerization is, by definition, a crucial process in the reversible assembly of various biopolymers. It may also be an important factor in the pathology of sickle cell disease. If sickle hemoglobin fibers fail to depolymerize fully during passage through the lungs then they will reintroduce aggregates into the systemic circulation and eliminate or shorten the protective delay (nucleation) time for the subsequent growth of fibers. We study how depolymerization depends on the rates of end- and side-depolymerization, kend and kside, which are, respectively, the rates at which fiber length is lost at each end and the rate at which new breaks appear per unit fiber length. We present both an analytic mean field theory and supporting simulations showing that the characteristic fiber depolymerization time View the MathML source depends on both rates, but not on the fiber length L, in a large intermediate regime 1 much less-than ksideL2/kend much less-than (L/d)2, with d the fiber diameter. We present new experimental data which confirms that both mechanisms are important and shows how the rate of side depolymerization depends strongly on the concentration of CO, acting as a proxy for oxygen. Our theory remains rather general and could be applied to the depolymerization of an entire class of linear aggregates, not just sickle hemoglobin fibers

    Patient level costing analysis of paediatric short bowel syndrome care in a specialist tertiary centre

    PURPOSE: To undertake a pilot study estimating patient-level costs of care for paediatric short bowel syndrome (SBS) from the healthcare provider perspective. METHODS: A pilot group of patients with anatomical SBS was selected at a single specialist tertiary centre in the United Kingdom. The Patient Level Information and Costing System (PLICS) was used to extract costing data for all hospital-based activities related to SBS, from the implementation of PLICS in 2016 to April 2021. Patient-specific and pooled data were reported descriptively in per patient-year terms. RESULTS: Five patients had full PLICS data available for the 5-year study period and 2 patients had 4 years of data. The median cost for hospital care of SBS was £52,834 per patient-year (range £1804–£331,489). The key cost drivers were inpatient beds, pharmacy, and staffing costs, which made up > 60% of annual costs. In the first 3 years following index admission (n = 2), there was a steady decline in the annual cost of care to a level comparable with patients with established SBS. CONCLUSION: Patient-level cost of care analysis for SBS is feasible using PLICS. Hospital-related costs vary widely between and within individual patients over time. Key drivers of cost are related to complications of SBS
