    Beyond linear regression: A reference for analyzing common data types in discipline based education research

    [This paper is part of the Focused Collection on Quantitative Methods in PER: A Critical Examination.] A common goal in discipline-based education research (DBER) is to determine how to improve student outcomes. Linear regression is a common technique used to test hypotheses about the effects of interventions on continuous outcomes (such as exam score) as well as control for student nonequivalence in quasirandom experimental designs. (In quasirandom designs, subjects are not randomly assigned to treatments. For example, when treatment is assigned by classroom, and observations are made on students, the design is quasirandom because treatment is assigned to classroom, not subject (students).) However, many types of outcome data cannot be appropriately analyzed with linear regression. In these instances, researchers must move beyond linear regression and implement alternative regression techniques. For example, student outcomes can be measured on binary scales (e.g., pass or fail), tightly bound scales (e.g., strongly agree to strongly disagree), or nominal scales (i.e., different discrete choices for example multiple tracks within a physics major), each necessitating alternative regression techniques. Here, we review extensions of linear modeling—generalized linear models (glms)—and specifically compare five glms that are useful for analyzing DBER data: logistic, binomial, proportional odds (also called ordinal; including censored regression), multinomial, and Poisson (including negative binomial, hurdle, and zero-inflated) regression. We introduce a diagnostic tool to facilitate a researcher’s identification of the most appropriate glm for their own data. For each model type, we explain when, why, and how to implement the regression approach. When: we provide examples of the types of research questions and outcome data that would motivate this regression approach, including citations to articles in the DBER literature. Why: we name which linear regression assumption is violated by the data type. How: we detail implementation and interpretation of this modeling approach in R, including R syntax and code, and how to discuss the regression output in research papers. Code accompanying each analysis can be found in the online github repository that is associated with this paper (https://github.com/ejtheobald/BeyondLinearRegression). This paper is not an exhaustive review of regression techniques, nor does it review nonregression-based analyses. Rather, it aims to compile and summarize regression techniques useful for the most common types of DBER data and provide examples, citations, and heavily annotated R code so that researchers can easily implement the technique in their work

    Efficacy of a High-Iron Dietary Intervention in Women with Celiac Disease and Iron Deficiency without Anemia : A Clinical Trial

    Background and aim: Iron deficiency without anemia (IDWA) is a common finding in celiac disease (CD) and can also persist in case of good compliance and clinical response to a strict gluten-free diet (GFD). This scenario usually presents in CD women of child-bearing age in whom the imbalance between menstrual iron loss and inadequate iron intake from their diet plays the major role. A recommended approach to this condition is yet to be established. This study aimed to compare, in this subset of patients, the efficacy of a dietary approach consisting of an iron-rich diet against the traditional pharmacological oral-replacement therapy. Material and methods: Between February and December 2016, consecutive CD female patients of child-bearing age as referred to our outpatient center with evidence of IDWA (ferritin <15 ng/mL or 15-20 ng/L with transferrin saturation <15%) were enrolled. After the completion of a 7-day weighed food intake recording to assess the usual iron dietary intake, the patients were randomized in two arms to receive a 12-week iron-rich diet (iron intake >20 mg/die) versus oral iron supplementation with ferrous sulfate (FS) (105 mg/day). Blood tests and dietary assessments were repeated at the end of treatment. The degree of compliance and tolerability to the treatments were assessed every month by means of specific questionnaires and symptoms evaluation. Results: A total of 22 women were enrolled and divided in the diet group (n = 10, age 37 \ub1 8 years) and in the FS group (n = 12, age 38 \ub1 10 years). The food intake records demonstrated an inadequate daily intake of iron in all the enrolled subjects. At the end of the treatments, ferritin levels were higher in the FS group (8.5 (5) versus 34 (30.8), p = 0.002). Compliance and tolerability were similar in both treatment groups (89% versus 87%, p = ns). Conclusions: These findings did not support any equivalent efficacy of an iron-rich diet compared to a FS supplementation in non-anemic iron-deficient women affected by CD. However, the diet appeared a well-tolerated approach, and adequate dietary instructions could effectively increase the daily iron consumption, suggesting a role in the long-term management of IDWA, especially in patients who do not tolerate pharmacological supplementation

    Non-celiac gluten sensitivity : time for sifting the grain

    In the last few years, a new nomenclature has been proposed for the disease induced by the ingestion of gluten, a protein present in wheat, rice, barley and oats. Besides celiac disease and wheat allergy, the most studied forms of gluten-related disorders characterized by an evident immune mechanism (autoimmune in celiac disease and IgE-mediated in wheat allergy), a new entity has been included, apparently not driven by an aberrant immune response: the non-celiac gluten sensitivity (NCGS). NCGS is characterized by a heterogeneous clinical picture with intestinal and extraintestinal symptoms arising after gluten ingestion and rapidly improving after its withdrawal from the diet. The pathogenesis of NCGS is largely unknown, but a mixture of factors such as the stimulation of the innate immune system, the direct cytotoxic effects of gluten, and probably the synergy with other wheat molecules, are clues for the complicated puzzle. In addition, the diagnostic procedures still remain problematic due to the absence of efficient diagnostic markers; thus, diagnosis is based upon the symptomatic response to a gluten-free diet and the recurrence of symptoms after gluten reintroduction with the possibility of an important involvement of a placebo effect. The temporary withdrawal of gluten seems a reasonable therapy, but the timing of gluten reintroduction and the correct patient management approach are have not yet been determined

    Endoscopy during the Covid-19 outbreak : experience and recommendations from a single center in a high-incidence scenario

    A dramatic SARS-Cov-2 outbreak is hitting Italy hard. To face the new scenario all the hospitals have been re-organised in order to reduce all the outpatient services and to devote almost all their personnel and resources to the management of Covid-19 patients. As a matter of fact, all the services have undergone a deep re-organization guided by: the necessity to reduce exams, to create an environment that helps reduce the virus spread, and to preserve the medical personnel from infection. In these days a re-organization of the endoscopic unit, sited in a high-incidence area, has been adopted, with changes to logistics, work organization and patients selection. With the present manuscript, we want to support gastroenterologists and endoscopists in the organization of a \u201cnew\u201d endoscopy unit that responds to the \u201cnew\u201d scenario, while remaining fully aware that resources, availability and local circumstances may extremely vary from unit to unit

    Covid-19 vaccine: A survey of hesitancy in patients with celiac disease

    (1) Background: COVID-19 vaccination campaigns offer the best hope of controlling the pandemic. However, the fast production of COVID-19 vaccines has caused concern among the general public regarding their safety and efficacy. In particular, patients with chronic illnesses, such as celiac disease (CD), may be more fearful. Information on vaccine hesitancy plays a pivotal role in the development of an efficient vaccination campaign. In our study, we aimed to evaluate COVID-19 vaccine hesitancy among Italian CD patients. (2) Methods: an anonymous questionnaire was sent to CD patients followed at our tertiary referral center for CD in Milan, Italy. Patients were defined as willing, hesitant and refusing. We evaluated the reasons for hesitancy/refusal and the possible determinants, calculating crude and adjusted odds ratios [AdjORs] with 95% confidence intervals [CIs]. (3) Results: the questionnaire was sent to 346 patients with a response rate of 29.8%. Twenty-six (25.2%) of the 103 respondents were hesitant, with a total refusal rate of 4.8%. The main reason was fear of adverse events related to vaccination (68.2%). Among hesitant patients, 23% declared that their opinion was influenced by their CD. The determinants positively influencing willingness to be vaccinated against COVID-19 were adherence to a GFD, perception of good knowledge about COVID-19 and its vaccines, and a positive attitude to previous vaccines (AdjOR 12.71, 95% CI 1.82–88.58, AdjOR 6.50, 95% CI 1.44–29.22, AdjOR 0.70, 95% CI 0.11–4.34, respectively). (4) Conclusions: CD patients should be vaccinated against COVID-19 and a specific campaign to address the determinants of hesitancy should be developed

    Are treated celiac patients at risk for mycotoxins? An Italian case-study

    Urinary biomarkers of mycotoxin exposure were evaluated in a group of celiac patients (n = 55) and in a control group of healthy subjects (n = 50) following their habitual diet. Deoxynivalenol (DON), zearalenone (ZEN), and fumonisin B1 (FB1) were monitored in 105 urinary samples collected from the two groups. Dietary habits were also recorded through compilation of a seven-day weighed dietary diary. Biomarkers of mycotoxin exposure were detected in 21 celiac patients and in 15 control subjects, corresponding to about 34% of total participants. In particular, ZEN was the most detected mycotoxin among all the studied subjects with a total of 19 positive cases. Results did not show a statistically significant difference in mycotoxin exposure between the two groups, and the presence of specific mycotoxins was not related to the intake of any particular food category. Our findings suggest little urgency of specific regulation for gluten free products, although the prevalence of exposure observed in free-living diets of both celiac and healthy subjects underlines the need of a constant surveillance on mycotoxins occurrence at large
