    Time course study of oxidative and nitrosative stress and antioxidant enzymes in K(2)Cr(2)O(7)-induced nephrotoxicity

    BACKGROUND: Potassium dichromate (K(2)Cr(2)O(7))-induced nephrotoxicity is associated with oxidative and nitrosative stress. In this study we investigated the relation between the time course of the oxidative and nitrosative stress with kidney damage and alterations in the following antioxidant enzymes: Cu, Zn superoxide dismutase (Cu, Zn-SOD), Mn-SOD, glutathione peroxidase (GPx), glutathione reductase (GR), and catalase (CAT). METHODS: Nephrotoxicity was induced in rats by a single injection of K(2)Cr(2)O(7). Groups of animals were sacrificed on days 1,2,3,4,6,8,10, and 12. Nephrotoxicity was evaluated by histological studies and by measuring creatinine clearance, serum creatinine, blood urea nitrogen (BUN), and urinary excretion of N-acetyl-β-D-glucosaminidase (NAG) and total protein. Oxidative and nitrosative stress were measured by immunohistochemical localization of protein carbonyls and 3-nitrotyrosine, respectively. Cu, Zn-SOD, Mn-SOD, and CAT were studied by immunohistochemical localization. The activity of total SOD, CAT, GPx, and GR was also measured as well as serum and kidney content of chromium and urinary excretion of NO(2 )(-)/NO(3)(-). Data were compared by two-way analysis of variance followed by a post hoc test. RESULTS: Serum and kidney chromium content increased reaching the highest value on day 1. Nephrotoxicity was made evident by the decrease in creatinine clearance (days 1–4) and by the increase in serum creatinine (days 1–4), BUN (days 1–6), urinary excretion of NAG (days 1–4), and total protein (day 1–6) and by the structural damage to the proximal tubules (days 1–6). Oxidative and nitrosative stress were clearly evident on days 1–8. Urinary excretion of NO(2)(-)/NO(3)(- )decreased on days 2–6. Mn-SOD and Cu, Zn-SOD, estimated by immunohistochemistry, and total SOD activity remained unchanged. Activity of GPx decreased on days 3–12 and those of GR and CAT on days 2–10. Similar findings were observed by immunohistochemistry of CAT. CONCLUSION: These data show the association between oxidative and nitrosative stress with functional and structural renal damage induced by K(2)Cr(2)O(7). Renal antioxidant enzymes were regulated differentially and were not closely associated with oxidative or nitrosative stress or with kidney damage. In addition, the decrease in the urinary excretion of NO(2)(-)/NO(3)(- )was associated with the renal nitrosative stress suggesting that nitric oxide was derived to the formation of reactive nitrogen species involved in protein nitration

    Mortalidade por Aids e indicadores sociais no Município de São Paulo, 1994 a 2002 AIDS mortality and socioeconomic indexes in the city of Sao Paulo, 1994-2002

    OBJETIVO: Analisar as correlações entre os coeficientes de mortalidade por Aids e os índices de inclusão/exclusão social, em homens e mulheres de 25 a 49 anos. MÉTODOS: O estudo foi realizado em 96 distritos administrativos do Município de São Paulo, no período de 1994 a 2002. Foram utilizados dados de óbitos do programa de aprimoramento das informações de mortalidade do Município e estimativas populacionais dos censos de 1991 e 2000 da Fundação Sistema Estadual de Análise de Dados (Seade) e Secretaria Municipal de Planejamento. Os índices foram obtidos do Mapa da Exclusão para a Cidade (1996 e 2000). Para a análise estatística foi utilizado o teste de correlação de Pearson (alfa=0,05). RESULTADOS: Entre os homens, observou-se correlação positiva significativa (p<0,05) entre a mortalidade por Aids e o índice de qualidade de vida dos distritos, de 1994 a 1998. Entre as mulheres, observou-se em todo o período correlação negativa significativa (p<0,05) entre a mortalidade por Aids e o índice de eqüidade, o qual mede a concentração de mulheres chefes de família/analfabetas. A partir de 2000, observou-se tendência à correlação negativa significativa (p<0,05) entre a mortalidade feminina por Aids e o índice global de exclusão social. CONCLUSÕES: Os resultados sugerem o deslocamento da mortalidade por Aids para áreas de exclusão e apontam para a hipótese de relação entre essa mortalidade e fatores socioeconômicos. Outros estudos epidemiológicos e no campo das ciências sociais são necessários para aprofundar essa investigação.<br>OBJECTIVE: To investigate the correlation between AIDS mortality rates and social inclusion/exclusion indexes among male and female populations aged between 25 to 49 years. METHODS: The study was carried out in 96 administrative districts of the municipality of São Paulo in the period 1994-2002. Mortality data were collected from the Program for Improving Local Mortality Data and from population estimates based on the 1991 and 2000 census of the State System Data Analysis (SEADE) Institute and the Municipal Department of Planning. The indicators were obtained from the city's map of exclusion (1996 and 2000). Statistical analysis was performed using Pearson's correlation test (at 5% level). RESULTS: A significant positive correlation was seen among men between AIDS mortality and the district life quality index from 1994 to 1998 (p<0.05). Among women, a significant negative correlation (p<0.05) was seen for the whole study period between AIDS mortality and the equity index, which measures the proportion of illiterate women who are family heads. From 2000, it was also observed a significant negative correlation (p<0.05) among women between AIDS mortality and the global social exclusion index. CONCLUSIONS: The study results suggest a shift of AIDS mortality to exclusion areas and might indicate a relationship between AIDS mortality and socioeconomic factors. Further epidemiological and social sciences studies are needed to investigate these associations

    Anterior Segment Optical Coherence Tomography for the Quantitative Evaluation of the Anterior Segment Following Boston Keratoprosthesis

    OBJECTIVE: To quantitatively evaluate the anterior segment using anterior segment optical coherence tomography (AS-OCT) following Boston keratoprosthesis type 1. METHODS: A retrospective study consisted of AS-OCT imaging at a single time point postoperatively in 52 eyes. Main outcomes measures include anatomical and functional anterior chamber depth (ACD), angle (ACA) and peripheral and proximal synechiae. RESULTS: The mean time point of imaging was 19.3 months postoperatively. Average anatomical and functional ACD was 2.0 and 0.21 mm respectively, and mean ACA ranged from 2.5° to 6.14° in representative meridians. An average of 8.7 clock hours of angle closure was observed in the 25 eyes in which all meridians were imaged. The majority of eyes showed peripheral (86.5%) and proximal (67.3%) synechiae. CONCLUSIONS: AS-OCT is a useful tool for quantitative evaluation of anterior segment and angle after keratoprosthesis, which is otherwise poorly visible.     Prevalência do tracoma em pré-escolares e escolares no Município de São Paulo Prevalence of trachoma in preschool and schoolchildren in the city of São Paulo

    OBJETIVO: Conhecer a prevalência de tracoma em pré-escolares e escolares das escolas públicas para redirecionar as atividades de seu controle. MÉTODOS: Realizou-se inquérito epidemiológico no Município de São Paulo, em 1999. A seleção das crianças com idade entre quatro e 14 anos foi feita por meio de amostragem por conglomerados, sendo o turno de estudo a unidade amostral. Foi realizado exame ocular externo para detectar a presença de sinais clínicos de tracoma. RESULTADOS: Das 27.091 crianças examinadas foram diagnosticados 597 casos de tracoma (2,2%; IC 95%: 1,86-2,55). A prevalência variou de 0,4% a 4,2% entre as 10 regiões do Município de São Paulo. A taxa de detecção entre os comunicantes foi de 8,7%. Tracoma folicular foi encontrado em 99,0% dos casos e tracoma intenso em 1,0% dos casos. Verificou-se que 22,5% dos casos eram assintomáticos. CONCLUSÕES: Embora a prevalência tenha sido baixa, a presença de formas graves aponta para a possibilidade da existência de casos cicatriciais no futuro, se não houver tratamento e controle adequado. A grande diferença entre as taxas encontradas para cada uma das regiões da cidade, indica a necessidade de intensificação das ações de vigilância epidemiológica do tracoma.<br>OBJECTIVE: To assess the prevalence of trachoma among preschool and school children of public schools to give new focus to control programs. METHODS: An epidemiological survey was carried out in São Paulo City in 1999. Children between four and 14 years old were selected by a cluster sampling where school shift was the sampling unit. External eye examination was conducted to detect trachoma. RESULTS: A total of 27,091 children were examined and 597 cases of trachoma were found (2.2%; 95% CI: 1.86-2.55). The prevalence ranged from 0.4% to 4.2% in 10 city areas. The trachoma detection rate in the household contacts examined was 8.7%. Follicular trachoma was found in 99% of the cases and intense trachoma in 1.0%. It was observed that 21.8% of the cases were asymptomatic. CONCLUSIONS: Though the trachoma prevalence was low, the occurrence of severe cases points out to the likelihood of cicatricial trachoma cases in the future if they are not adequately treated and controlled. The great difference in the prevalences in different city areas indicates the need for strengthening epidemiological surveillance activities

    Hypertensive portal colopathy in schistosomiasis mansoni: proposal for a classification

    Portal hypertension is a frequent complication of chronic liver disease, detected not only in schistosomiasis, but also in cirrhosis of any etiology. Vascular alterations in the colonic mucosa are a potential source for acute or chronic bleeding and have been observed in patients with portal hypertension. The purpose of this prospective study was to describe and propose a classification for the vascular alterations of portal hypertension in the colonic mucosa among patients with hepatosplenic schistosomiasis mansoni. One or more alterations of portal colopathy were observed in all patients and they were classified according to their intensity, obeying the classification proposed by the authors. Portal colopathy is an important finding in hepatosplenic schistosomiasis and might be the cause of lower gastrointestinal bleeding in patients with severe portal hypertension
