    Advantages and disadvantages of intravenous total anesthesia for one day surgery in children.

    The choice of an anesthetic support option in the condi- tions of "one-day surgery" in children should be carried out taking into account a number of the following factors. First of all, it is a smooth introduction into anesthesia and rapid withdrawal from it immediately after the operation. We conducted a study to assess the clinical course of an- esthesia with ketamine in combination with midazolam and fentanyl, to study central hemodynamics in 37 chil- dren aged 0 to 16 years who underwent surgical interven- tions for inguinal and umbilical hernia, dropsy of the tes- ticles, phimosis, paraphimosis, cryptorchidism, rectal polyp. Intravenous anesthesia with ketamine in combination with midazolam and fentanyl was characterized by the stability of the main hemodynamic parameters, no critical changes leading to death were recorded. The most pro- nounced fluctuations in hemodynamic parameters during anesthesia were noted at the traumatic moment of the op- eration. At this stage, total peripheral vascular resistance increased significantly, with a corresponding change in mean arterial pressure. There was a decrease in stroke volume and minute volume of blood circulation, despite compensatory tachycardia. It is most likely that these changes are associated with a lack of anesthesia. The data obtained during the study indicate the insufficient depth of this anesthesia regimen (ketamine + midazolam + fen- tanyl) to prevent reflex reactions and pain during opera- tions. During the awakening period, a slow stabilization of blood circulation parameters was noted. The greatest number of critical incidents was noted with intravenous total anesthesia with a combination of ketamine, midazo- lam and fentanyl. Therefore, this scheme of anesthesia does not fully meet the requirements for surgical inter- ventions in a one-day hospital

    Nonlinear process when the moisture has a stationary position from the point of the analytical research is needed for theoretical and applied problems. In this paper we investigate the distribution of the heat provided stationary moisture where heat distribution has a pattern differs from the linear interaction between moisture and heat. Permanence moisture sampling is the result of a relatively steady drying mode. In the drying chamber we have a closed volume, it is particularly noticeable for the vacuum chambers, and therefore there is almost stationary process between stable heating and evaporation. Obtained linear problem of parabolic type to accomplish this task with the appropriate boundary conditions, we can apply for the interval (0, l), separation of variables.     To achieve the necessary quality properties of the products of leguminous crops, along with the use of leguminous seeds flour, various processing tools are used: extrusion, micronization. Micronization of leguminous seeds changes their functional and technological properties and can significantly reduce the time of heat treatment. These changes depend on the mode of micronized substance. The article presents the modes of micronization of legumes, such as chickpeas, lentils and peas, in order to obtain on their basis a food additive for adjusting the recipes of bakery products in order to enrich wheat bread with valuable micronutrients useful for human health

    Association of vascular endothelial growth factor B (VEGFВ) gene polymorphisms with intracranial aneurysms

    Intracranial aneurysm (IA) is a complex disease resulting in subarachnoid hemorrhage (SAH) due to a rupture. The average worldwide prevalence of this disease is about 2–5 %, with 50 % of them ending in death or neurological disorders of varying severity, with a high probability of recurrence of hemorrhage during the frst half of the year after rupture. Subarachnoid hemorrhage is annually registered in at least 18 thousand people in Russia. Associations of polymorphic variants rs594942 and rs11603042 of the VEGFB gene in intracranial aneurysm development in the Volga-Ural region of the Russian Federation with the presence of the symptom complex of undifferentiated connective tissue dysplasia (uDST) and arterial hypertension (AH) were investigated. The C* allele rs594942 and rs11603042 of the VEGFB gene is a marker of an increased risk of IA as a whole (p = 0.025; χ2 = 5.052; OR = 1.32) in women as a whole (p = 0.001; χ2 = 10.124; OR = 1.70) and in comorbid state with uDCT (p = 0.002; χ2 = 9.501; OR = 2.34) and AG (p = 0.006; χ2 = 7.385; OR = 2.109). We found that the genotype *C*C of locus rs594942 of the VEGFB gene is a marker of an increased risk of intracranial aneurysm in general (p = 0.017; χ2 = 5.702; OR = 1.49) and among women in general (p = 0.0005; χ2 = 12.078; OR = 2.25) and with the symptomatic complex uCTD (p = 0.007; χ2 = 7.173; OR = 2.67) and AH (p = 0.010; χ2 = 6.471; OR = 2.51). We have obtained new results on the role of polymorphic variants of the VEGFB gene in the formation of intracranial aneurysm, taking into account the presence of the symptom complex uDCT and AH among the residents of the Volga-Ural region of Russia. A burdened comorbid background and the presence of undifferentiated connective tissue dysplasia and arterial hypertension can contribute to an increased risk of intracranial aneurysm, as evidenced by the results of our study

    A trouble shared is a trouble halved : the role of family identification and identification with humankind in well-being during the COVID-19 pandemic

The data collection for this study was supported by a grant from the association of friends and supporters (Freunde & Förderer) at Goethe University.The COVID-19 pandemic has triggered health-related anxiety in ways that undermine peoples’ mental and physical health. Contextual factors such as living in a high-risk area might further increase the risk of health deterioration. Based on the Social Identity Approach, we argue that social identities can not only be local that are characterized by social interactions, but also be global that are characterized by a symbolic sense of togetherness and that both of these can be a basis for health. In line with these ideas, we tested how identification with one’s family and with humankind relates to stress and physical symptoms while experiencing health-related anxiety and being exposed to contextual risk factors. We tested our assumptions in a representative sample (N = 974) two-wave survey study with a 4-week time lag. The results show that anxiety at Time 1 was positively related to stress and physical symptoms at Time 2. Feeling exposed to risk factors related to lower physical health, but was unrelated to stress. Family identification and identification with humankind were both negatively associated with subsequent stress and family identification was negatively associated with subsequent physical symptoms. These findings suggest that for social identities to be beneficial for mental health, they can be embodied as well as symbolic.Publisher PDFPeer reviewe

    How national leaders keep ‘us’ safe : a longitudinal, four-nation study exploring the role of identity leadership as a predictor of adherence to COVID-19 non-pharmaceutical interventions

    This research was supported by a grant from the German Research Foundation awarded to RvD, NMJ and JAH (DI 848/15-1 and HA 6455/4-1).Objectives : To investigate whether citizens’ adherence to health-protective non-pharmaceutical interventions (NPIs) during the COVID-19 pandemic is predicted by identity leadership, wherein leaders are perceived to create a sense of shared national identity. Design : Observational two-wave study. Hypotheses testing was conducted with structural equation modelling. Setting : Data collection during the COVID-19 pandemic in China, Germany, Israel and the USA in April/May 2020 and four weeks later. Participants : Adults in China (n=548, 66.6% women), Germany (n=182, 78% women), Israel (n=198, 51.0% women) and the USA (n=108, 58.3% women). Measures : Identity leadership (assessed by the four-item Identity Leadership Inventory Short-Form) at Time 1, perceived shared national identification (PSNI; assessed with four items) and adherence to health-protective NPIs (assessed with 10 items that describe different health-protective interventions; for example, wearing face masks) at Time 2. Results : Identity leadership was positively associated with PSNI (95% CI 0.11 to 0.30, p<0.001) in all countries. This, in turn, was related to more adherence to health-protective NPIs in all countries (95% CI 0.03 to 0.36, 0.001≤p≤0.017) except Israel (95% CI −0.03 to 0.27, p=0.119). In Germany, the more people saw Chancellor Merkel as engaging in identity leadership, the more they adhered to health-protective NPIs (95% CI 0.04 to 0.18, p=0.002). In the USA, in contrast, the more people perceived President Trump as engaging in identity leadership, the less they adhered to health-protective NPIs (95% CI −0.17 to −0.04, p=0.002). Conclusions : National leaders can make a difference by promoting a sense of shared identity among their citizens because people are more inclined to follow health-protective NPIs to the extent that they feel part of a united ‘us’. However, the content of identity leadership (perceptions of what it means to be a nation’s citizen) is essential, because this can also encourage people to disregard such recommendations.Publisher PDFPeer reviewe
