Structure and Composition of Two Transitional Circumstellar Disks in Corona Australis
The late stages of evolution of the primordial circumstellar disks
surrounding young stars are poorly understood, yet vital to constrain theories
of planet formation. We consider basic structural models for the disks around
two ~10 Myr-old members of the nearby RCrA association, RX J1842.9-3532 and RX
J1852.3-3700. We present new arcsecond-resolution maps of their 230 GHz
continuum emission from the Submillimeter Array and unresolved CO(3-2) spectra
from the Atacama Submillimeter Telescope Experiment. By combining these data
with broadband fluxes from the literature and infrared fluxes and spectra from
the catalog of the Formation and Evolution of Planetary Systems (FEPS) Legacy
program on the Spitzer Space Telescope, we assemble a multiwavelength data set
probing the gas and dust disks. Using the Monte Carlo radiative transfer code
RADMC to model simultaneously the SED and millimeter continuum visibilities, we
derive basic dust disk properties and identify an inner cavity of radius 16 AU
in the disk around RX J1852.3-3700. We also identify an optically thin 5 AU
cavity in the disk around RX J1842.9-3532, with a small amount of optically
thick material close to the star. The molecular line observations suggest an
intermediate disk inclination in RX J1842.9-3532, consistent with the continuum
emission. In combination with the dust models, the molecular data allow us to
derive a lower CO content than expected, suggesting that the process of gas
clearing is likely underway in both systems, perhaps simultaneously with planet

Towards a global analysis of polarized parton distributions
We present a technique for implementing in a fast way, and without any
approximations, higher-order calculations of partonic cross sections into
global analyses of parton distribution functions. The approach, which is set up
in Mellin-moment space, is particularly suited for analyses of future data from
polarized proton-proton collisions, but not limited to this case. The
usefulness and practicability of this method is demonstrated for the
semi-inclusive production of hadrons in deep-inelastic scattering and the
transverse momentum distribution of ``prompt'' photons in pp collisions, and a
case study for a future global analysis of polarized parton densities is

The scientific potential of space-based gravitational wave detectors
The millihertz gravitational wave band can only be accessed with a
space-based interferometer, but it is one of the richest in potential sources.
Observations in this band have amazing scientific potential. The mergers
between massive black holes with mass in the range 10 thousand to 10 million
solar masses, which are expected to occur following the mergers of their host
galaxies, produce strong millihertz gravitational radiation. Observations of
these systems will trace the hierarchical assembly of structure in the Universe
in a mass range that is very difficult to probe electromagnetically. Stellar
mass compact objects falling into such black holes in the centres of galaxies
generate detectable gravitational radiation for several years prior to the
final plunge and merger with the central black hole. Measurements of these
systems offer an unprecedented opportunity to probe the predictions of general
relativity in the strong-field and dynamical regime. Millihertz gravitational
waves are also generated by millions of ultra-compact binaries in the Milky
Way, providing a new way to probe galactic stellar populations. ESA has
recognised this great scientific potential by selecting The Gravitational
Universe as its theme for the L3 large satellite mission, scheduled for launch
in ~2034. In this article we will review the likely sources for millihertz
gravitational wave detectors and describe the wide applications that
observations of these sources could have for astrophysics, cosmology and

An NIH intramural percubator as a model of academic-industry partnerships: from the beginning of life through the valley of death
In 2009 the NIH publicly announced five strategic goals for the institutes that included the critical need to translate research discoveries into public benefit at an accelerated pace, with a commitment to find novel ways to engage academic investigators in the process. The emphasis on moving scientific advancements from the laboratory to the clinic is an opportune time to discuss how the NIH intramural program in Bethesda, the largest biomedical research center in the world, can participate in this endeavor. Proposed here for consideration is a percolator-incubator program, a 'percubator' designed to enable NIH intramural investigators to develop new medical interventions as quickly and efficiently as possible
‘What are you going to do, confiscate their passports?’ Professional perspectives on cross-border reproductive travel
Objective: This article reports findings from a UK-based study which explored the phenomenon of overseas travel for fertility treatment. The first phase of this project aimed to explore how infertility clinicians and others professionally involved in fertility treatment understand the nature and consequences of cross-border reproductive travel. Background: There are indications that, for a variety of reasons, people from the UK are increasingly travelling across national borders to access assisted reproductive technologies. While research with patients is growing, little is known about how ‘fertility tourism’ is perceived by health professionals and others with a close association with infertility patients. Methods: Using an interpretivist approach, this exploratory research included focussed discussions with 20 people professionally knowledgeable about patients who had either been abroad or were considering having treatment outside the UK. Semi-structured interviews were recorded, transcribed verbatim and subjected to a thematic analysis. Results: Three conceptual categories are developed from the data: ‘the autonomous patient’; ‘cross-border travel as risk’, and ‘professional responsibilities in harm minimisation’. Professionals construct nuanced, complex and sometimes contradictory narratives of the ‘fertility traveller’, as vulnerable and knowledgeable; as engaged in risky behaviour and in its active minimisation. Conclusions: There is little support for the suggestion that states should seek to prevent cross-border treatment. Rather, an argument is made for less direct strategies to safeguard patient interests. Further research is required to assess the impact of professional views and actions on patient choices and patient experiences of treatment, before, during and after travelling abroad

Observational study of the development and evaluation of a fertility preservation patient decision aid for teenage and adult women diagnosed with cancer: The Cancer, Fertility and Me research protocol
Introduction: Women diagnosed with cancer and facing potentially sterilising cancer treatment have to make time-pressured decisions regarding fertility preservation with specialist fertility services whilst undergoing treatment of their cancer with oncology services. Oncologists identify a need for resources enabling them to support women’s fertility preservation decisions more effectively; women report wanting more specialist information to make these decisions. The overall aim of the ‘Cancer, Fertility and Me’ study is to develop and evaluate a new evidence-based patient decision aid (ptDA) for women with any cancer considering fertility preservation to address this unmet need. Methods and analysis: This is a prospective mixed-method observational study including women of reproductive age (16 years +) with a new diagnosis of any cancer across two regional cancer and fertility centres in Yorkshire, UK. The research involves three stages. In Stage 1 the aim is to develop the ptDA using a systematic method of evidence synthesis and multidisciplinary expert review of current clinical practice and patient information. In Stage 2, the aim is to assess the face validity of the ptDA. Feedback on its content and format will be ascertained using both questionnaires and interviews with patients, user groups and key stakeholders. Finally, in Stage 3 the acceptability of using this resource when integrated into usual cancer care pathways at the point of cancer diagnosis and treatment planning will be evaluated. This will involve a quantitative and qualitative evaluation of the ptDA in clinical practice. Measures chosen include using count data of the ptDAs administered in clinics and accessed online, decisional and patient-reported outcome measures and qualitative feedback. Quantitative data will be analysed using descriptive statistics, paired sample t tests and confidence intervals; interviews will be analysed using thematic analysis. Ethics and dissemination: Research Ethics Committee approval (Ref: 16/EM/0122) and Health Research Authority approval (Ref: 194751) has been granted. Findings will be published in open access peer-reviewed journals, presented at conferences for academic and health professional audiences, with feedback to health professionals and program managers. The Cancer, Fertility and Me ptDA will be disseminated via a diverse range of open-access media, study and charity websites, professional organisations and academic sources. External endorsement will be sought from the International Patient Decision Aid Standards (IPDAS) Collaboration inventory of ptDAs and other relevant professional organisations e.g. the British Fertility Society. We are all one together : peer educators\u27 views about falls prevention education for community-dwelling older adults - a qualitative study
Background: Falls are common in older people. Despite strong evidence for effective falls prevention strategies, there appears to be limited translation of these strategies from research to clinical practice. Use of peers in delivering falls prevention education messages has been proposed to improve uptake of falls prevention strategies and facilitate translation to practice. Volunteer peer educators often deliver educational presentations on falls prevention to community-dwelling older adults. However, research evaluating the effectiveness of peer-led education approaches in falls prevention has been limited and no known study has evaluated such a program from the perspective of peer educators involved in delivering the message. The purpose of this study was to explore peer educators’ perspective about their role in delivering peer-led falls prevention education for community-dwelling older adults.
Methods: A two-stage qualitative inductive constant comparative design was used.In stage one (core component) focus group interviews involving a total of eleven participants were conducted. During stage two (supplementary component) semi-structured interviews with two participants were conducted. Data were analysed thematically by two researchers independently. Key themes were identified and findings were displayed in a conceptual framework.
Results: Peer educators were motivated to deliver educational presentations and importantly, to reach an optimal peer connection with their audience. Key themes identified included both personal and organisational factors that impact on educators’ capacity to facilitate their peers’ engagement with the message. Personal factors that facilitated message delivery and engagement included peer-to-peer connection and perceived credibility, while barriers included a reluctance to accept the message that they were at risk of falling by some members in the audience. Organisational factors, including ongoing training for peer educators and formative feedback following presentations, were perceived as essential because they affect successful message delivery.
Conclusions: Peer educators have the potential to effectively deliver falls prevention education to older adults and influence acceptance of the message as they possess the peer-to-peer connection that facilitates optimal engagement. There is a need to consider incorporating learnings from this research into a formal large scale evaluation of the effectiveness of the peer education approach in reducing falls in older adults
