    MitraClip after failed surgical mitral valve repair: an international multicenter study

    BackgroundRecurrence of mitral regurgitation (MR) after surgical mitral valve repair (SMVR) varies and may require reoperation. Redo mitral valve surgery can be technically challenging and is associated with increased risk of mortality and morbidity. We aimed to assess the feasibility and safety of MitraClip as a treatment strategy after failed SMVR and identify procedure modifications to overcome technical challenges.Methods and ResultsThis international multicenter observational retrospective study collected information for all patients from 16 high-volume hospitals who were treated with MitraClip after failed SMVR from October 29, 2009, until August 1, 2017. Data were anonymously collected. Technical and device success were recorded per modified Mitral Valve Academic Research Consortium criteria. Overall, 104 consecutive patients were included. Median Society of Thoracic Surgeons score was 4.5% and median age was 73 years. At baseline, the majority of patients (82%) were in New York Heart Association class >= III and MR was moderate or higher in 86% of patients. The cause of MR pre-SMVR was degenerative in 50%, functional in 35%, mixed in 8%, and missing/unknown in 8% of patients. The median time between SMVR and MitraClip was 5.3 (1.9-9.7) years. Technical and device success were 90% and 89%, respectively. Additional/modified imaging was applied in 21% of cases. An MR reduction of >= 1 grade was achieved in 94% of patients and residual MR was moderate or less in 90% of patients. In-hospital all-cause mortality was 2%, and 86% of patients were in New York Heart Association class <= II.ConclusionsMitraClip is a safe and less invasive treatment option for patients with recurrent MR after failed SMVR. Additional/modified imaging may help overcome technical challenges during leaflet grasping.Cardiolog

    Resumen: La publicación de esta entrevista es un homenaje agradecido a quien fuera el inspirador y fundador del Seminario Interdisciplinario Permanente de Literatura, Estética y Teología (Siplet). En ella reflexiona en torno a los mutuos aportes de la teología a la literatura y de la literatura a la teología en el marco de las modificaciones que se producen en ambas como resultado del diálogo entre la palabra lógica y la palabra estética. El entrevistado destaca el papel de la experiencia como elemento común a ambas disciplinas cuyo papel es decisivo para los tiempos actuales.Abstract: The publication of this interview is a grateful tribute to who was the inspiration and founder of Permanent Interdisciplinary Seminar in Literature, Aesthetics and Theology (Siplet). It reflects on the mutual contributions of theology to literature and literature to theology in the context of the changes that occur both as a result of the dialogue between the logic word and the aesthetic word. En esta oportunidad, se asumen aspectos biográficos del entrevistado, relativos al método interdisciplinario, de diálogo teológico con los clásicos y el teatro, sobre el lugar de la literatura en la teología y otros temas que abren las reflexiones hacia nuevas orillas en estas latitudes.The authors return once again to the question about the interdisciplinary dialogue between theology and literature through an interview with Lucio Gera, who has given a significant impulse to this particular research. In this oportunity, biographical aspects relating to the interdisciplinary method of theological dialogue with the classics and theater and the place of literature in theology are examined.     Resumen: La presente entrevista a Eugenio Guasta pertenece al proyecto de reconstrucción de la historia del diálogo entre literatura y teología en la Argentina, emprendido por el Seminario Interdisciplinario Permanente Literatura, Estética y Teología de la Facultad de Teología (UCA). En la misma línea de las entrevistas ya realizadas a Lucio Gera y Juan Carlos Scannone (cf. Teología 99 y 102), también ésta busca recoger la trama viva del interés de la teología por la literatura. El caso del actual entrevistado se distingue de los casos anteriores en el hecho de que se trata de un sacerdote y formador que proviene del ámbito literario y que mantiene una explícita vinculación con los escritores y académicos argentinos que signaron la cultura literaria de la segunda mitad del siglo XX.Abstract: This interview with Eugenio Guasta is part of a project, undertaken by the Permanent Interdisciplinary Seminary on Literature, Aesthetics and Theology of the Faculty of Theology (UCA), to reconstruct the history of the dialogue between literature and theology in Argentina. Following the line of the interviews with Lucio Gera and Juan Carlos Scannone, (cf. Teología, 99 & 102) this one attempts to tap into the real interest that exists in theology for literature. This present interview differs from the previous ones in that the subject is a priest and formator who comes from a literary background and who has a very clear connection with Argentinean writers and academics who marked the literary culture of the second half of the XX century
