    Role and application of three-dimensional transthoracic echocardiography in the assessment of left and right ventricular volumes and ejection fraction: a UK nationwide survey

    Three-dimensional echocardiography (3DE) imaging has permitted advancements in the quantification of left ventricular (LV) and right ventricular (RV) volumes and ejection fraction. We evaluated the availability of 3DE equipment / analysis software, the integration of 3DE assessment of the LV and RV in routine clinical practice, current training provisions in 3DE, and aimed to ascertain barriers preventing the routine use of 3DE for volumetric analysis. Through the British Society of Echocardiography (BSE) regional representatives’ network, echocardiographers were invited to participate in an open online survey. A total of 181 participants from echocardiography departments in the United Kingdom (UK), the majority from tertiary centres (61%), completed the 28-question survey. For 3DE quantification, 3DE-LV was adopted more frequently than 3DE-RV (48% vs 11%, respectively). Imaging feasibility was a recognised factor in 3DE RV and LV adoption. Many respondents had access to 3D probes (93%). The largest observed barriers to 3DE routine use were training deficiencies, with 83% reporting they would benefit from additional training opportunities and the duration of time permitted for the scan, with 68% of responders reporting allowances of less than the BSE standard of 45–60 min per patient (8% < 30-min). Furthermore, of those respondents who had undertaken professional accreditation, competence in 3DE was not formally assessed in 89%. This UK survey also reported good accessibility to magnetic resonance imaging (72%), which was related to overall 3DE adoption. In summary, although 3DE is now readily available, it remains underutilised. Further training opportunities, integrated formal assessment, improved adoption of BSE minimum recommended scanning times, alongside industry and societal support, may increase 3DE utilisation in routine practice

    Effect of orientation on broadband acoustic scattering of Antarctic krill Euphausia superba : implications for inverting zooplankton spectral

The definitive version was published in Journal of the Acoustical Society of America 104 (1998): 2121-2135, doi:10.1121/1.423726.Acoustic scattering experiments involving simultaneous acquisition of broadband echoes and video footage from several Antarctic krill were carried out to determine the effect of animal orientation on echo spectral structure. A novel video analysis technique, applied to extract krill angle of orientation corresponding to each insonification, revealed that echo spectra from krill near broadside incidence relative to the incident acoustic wave exhibited widely spaced, deep nulls, whereas off-broadside echo spectra had a more erratic structure, with several closely spaced nulls of variable depth. The pattern of changes in echo spectra with orientation for the experimentally measured acoustic returns was very similar to theoretically predicted patterns based on a distorted wave Born approximation (DWBA) model. Information contained in the broadband echo spectra of the krill was exploited to invert the acoustic returns for angle of orientation by applying a newly developed Covariance Mean Variance Classification (CMVC) approach, using generic and animal-specific theoretical and empirical model spaces. The animal-specific empirical model space was best able to invert for angle of orientation.     Spatial and seasonal variation of peatland-fed riverine macroinvertebrate and benthic diatom assemblages and implications for assessment: a case study from Ireland

    Blanket peat catchments are important biodiversity refugia. Key pressures on peatland catchment water bodies include artificial drainage, forestry, over-grazing, wind farm development and climate change, and assessment of these pressures requires sensitive monitoring programmes. This study, undertaken in two neighbouring blanket peat catchments, examined the variability in macroinvertebrate and diatom assemblages and related indices in response to spatial and seasonal variability. Multivariate analysis revealed significant trends in the taxa distribution of both groups and the indices downstream and away from the constraining influence of the peat. However, the ecological quality ratios and status assessments for the associated water bodies were consistent irrespective of spatial variability in assemblages and raw indices. Significant seasonal trends emerged only in the macroinvertebrate assemblages and indices. This study contributes to the understanding of sources of uncertainty in ecological assessment and thus provides valuable information for the calibration of assessment protocols for sensitive peatland catchments

    Simple elbow dislocations: a systematic review of the literature

    Objective: To identify if functional treatment is the best available treatment for simple elbow dislocations. Search strategy: Electronic databases MEDLINE, EMBASE, LILACS, and the Cochrane Central Register of Controlled Trials. Selection criteria: Studies were eligible for inclusion if they were trials comparing different techniques for the treatment of simple elbow dislocations. Data analysis: Results were expressed as relative risk for dichotomous outcomes and weighted mean difference for continuous outcomes with 95% confidence intervals. Main results: This review has included data from two trials and three observational comparative studies. Important data were missing from three observational comparative studies and the results from these studies were extracted for this review. No difference was found between surgical treatment of the collateral ligaments and plaster immobilisation of the elbow joint. Better range of movement, less pain, better functional scores, shorter disability and shorter treatment time were seen after functional treatment versus plaster immobilisation

    Effect of different cleaning procedures on water use and bacterial levels in weaner pig pens

    peer reviewedPork is one of the most globally eaten meats and the pig production chain contributes significantly to the water footprint of livestock production. However, very little knowledge is available about the on-farm factors that influence freshwater use in the pig production chain. An experiment was conducted to quantify the effect of three different washing treatments on freshwater use, bacterial levels [(total bacterial counts; TBC), Enterobacteriaceae and Staphylococcus] and cleaning time in washing of pens for weaning pigs. Three weaner rooms were selected with each room having 10 pens and a capacity to hold up to 14 pigs each. Pigs were weaned and kept in the pens for 7 weeks. Finally, the pens were cleaned before the next batch of pigs moved in. The washing treatments used were power washing and disinfection (WASH); presoaking followed by power washing and disinfection (SOAK), and presoaking followed by detergent, power washing and disinfection (SOAK + DETER). A water meter was used to collect water use data and swab samples were taken to determine the bacterial levels. The results showed that there was no overall effect of washing treatments on water use. However, there was an effect of treatment on the washing time (p<0.01) with SOAK and SOAK+DETER reducing the washing time per pen by 2.3 minutes (14%) and 4.2 minutes (27%) compared to WASH. Nonetheless, there was an effect of sampling time (before or after washing) (p<0.001) on the levels of TBC and Staphylococcus, but no effect was seen on Enterobacteriaceae levels. Thus, the washing treatments used in this study had no effect on the water use of the pork production chain. Although there was no difference in both water use and bacterial load, from a producer perspective, presoaking and detergent use can save time and labour costs, so this would be the preferred option
