    Проведено аналіз розповсюдженості клініко-морфологічних ознак диспластичних змін у дітей 4−7 років. Установлено, що до полісистемних порушень сполучної тканини переважно мають схильність діти шести та семи років, порівняно з чотирічними та п’ятирічними. Серед головних ознак змін сполучної тканини в дітей найчастіше трапляються гіпермобільність суглобів (75 %) та зміни опорно-рухового апарату (56,78 %). The established that changes in connective tissue polysystem more inherent to children of six and seven years compared with children four and five years of age.     Effiectiveness of Etiotropic Therapy in Patients with Chronic Recurrent Oral Candidiasis: a Randomized Controlled Clinical Trial

    Background. Effective treatment of oral candidiasis is especially relevant due to the increase in patients with recurrent fungal lesions of the oral cavity caused by the growing number of Candidaspp. strains refractory to antimycotic drugs. The paper presents the results of the study on the clinical efficacy of etiotropic therapy in patients with chronic recurrent course of oral candidiasis.Objective. To assess the effectiveness of etiotropic therapy in patients with chronic recurrent oral candidiasis.Methods. A randomized controlled clinical trial was conducted in 56 patients with chronic recurrent oral candidiasis aged 45 to 74 years. Examination and treatment of patients was performed at the Department of Dentistry of Volgograd State Medical University and at Volgograd Regional Clinical Dental Polyclinic (Volgograd, Russia). Study timeline: September 2020 — November 2022. Patients were divided into 2 groups depending on the antimycotic drug used: in the main group, patients took voriconazole, in the control group — fluconazole. In addition to etiotropic therapy, patients of both groups were prescribed complex treatment aimed at stimulating the immune protection of the body, restoring vitamin and mineral deficiency, and correcting the imbalance of the oral ecosystem. The patients underwent local symptomatic therapy: antiseptic treatment with 0.06% chlorhexidine bigluconate solution, anesthesia with 15% lidocaine gel, and 3.44% retinol oil solution to stimulate epithelization. Oral sanitation and professional hygiene were recommended for patients to prevent oral candidiasis. In order to restore chewing function, patients were referred to prosthodontic treatment. The main reference point of the study was to assess the effectiveness of the therapy, both by clinical criteria and by evaluating the results of bacterial culture test in order to record the quantitative, species and susceptibility profile of Candidaspp. strains in 3 weeks, 6 and 12 months after the end of treatment. Analysis and statistical processing of the obtained data were performed using Excel program to MS Windows 10 (Microsoft Corp., USA).Results. The most common pathogenic agent of chronic recurrent forms of oral candidiasis is C. albicans (69.6%), followed by C. non-albicans species — C. krusei (10.7%), C. tropicalis (10.7%) and C. glabrata (8.9%). Analysis of the quantitative characteristics of Candidaspp. using bacterial culture test revealed a high and moderate number of Candidaspp. CFU in most clinical isolates before treatment, and no such cases in patients of the main group after treatment. The susceptibility test of yeast fungi to fluconazole and itraconazole showed the presence of isolates with dose-dependent susceptibility, and the resistance of individual samples C. krusei to fluconazole and C. glabrata (16.7% and 60%, respectively) and to itraconazole (33.3% and 100%, respectively). All Candidaspp. isolates were found to be highly susceptible to voriconazole. The efficacy of the combined treatment of an episode with voriconazole is 95.4%. Continued suppressive therapy with voriconazole resulted in relapse prevention in 98.3% of patients. Etiotropic therapy with voriconazole provided a stable clinical effect (92.7%) after a year of follow-up.Conclusion. The results of the study confirm the effectiveness of etiotropic therapy with voriconazole for chronic oral candidiasis complicated by a recurrent course, especially in case of detected C. non-albicans or fluconazole- and itraconazole-resistant C. albicans, or if it is impossible to identify the pathogenic agent

    Electronic structure of bcc Np, fcc Pu, Am, and Cm pure metals under pressure has been investigated employing the LDA+U method with spin-orbit coupling (LDA+U+SO). Magnetic state of the actinide ions was analyzed in both LS and jj coupling schemes to reveal the applicability of corresponding coupling bases. It was demonstrated that whereas Pu and Am are well described within the jj coupling scheme, Np and Cm can be described appropriately neither in {m-sigma}, nor in {jmj} basis, due to intermediate coupling scheme realizing in these metals that requires some finer treatment. The LDA+U+SO results for the considered transuranium metals reveal bands broadening and gradual 5f electron delocalization under pressure.Comment: 5 pages, 5 figure

    The results of the analysis of lung disease in community-acquired pneumonia in the militaries (including among the mobilized soldiers) who were treated in clinics NVMKTS «GVKG» are displayed. The features of localization, severity of community-acquired pneumonia, depending on the categories of patients and the season are illustrated. Most cases of all patients with severe pleurisy were recorded in the period from January to April. Severe and complicated forms were recorded more from mobilized troops compared with other categories of patients.Наведено результати аналізу ураження легень при негоспітальній пневмонії у військовослужбовців (у т.ч. серед мобілізованих військовослужбовців), які перебували на лікуванні у клініках НВМКЦ «ГВКГ». Встановлені особливості локалізації, тяжкості негоспітальної пневмонії залежно від категорій хворих і сезону року. Найбільше хворих з тяжким перебігом і з плевритом спостерігалося в період із січня по квітень. Тяжких та ускладнених форм реєструвалося більше у мобілізованих військовослужбовців порівняно з іншими категоріями хворих.

Проаналізовано вплив медіа-технологій на соціальне середовище і показано, що розробки в області нанотехнологій, робототехніки, мемрісторной електроніки, що пронизують всі рівні живої і неживої матерії, стають глобальними загрозами. Технології NBICS задають нову стратегію розвитку цивілізації, і їх потенційні наслідки вимагають гуманітарного осмислення як перспектив, так і загроз її реалізації. Сама людина стає свідком нового еволюційного стану цивілізації і трансформується під впливом комплексу сучасних медіа-технологій. Доведено, що медіа-технології змінюють повсякденне життя людей, їх спілкування, спосіб їх безпосереднього особистого буття, а світ людини під впливом медіа-технологій, медіасоціума і медіакультури трансформується в його Медіасвіт. Розкрито соціогуманітарна функціональність цифрових медіа-мереж як цифрових посередників і охарактеризовано їх вплив на соціально-природне середовище проживання людини.The article shows that the modern media environment is a qualitatively new stage in the transformation of the media environment, since there is not only an intensification of the level of development of science and technology, new structures of social life and ways of organizing production, but also the formation of new forms of sociality, understanding of human nature and his transformation. The influence of media technologies on the social environment is analyzed and it is shown that developments in the field of nanotechnology, robotics, memristor electronics, which penetrate all levels of living and non-living matter, become global threats. NBICS technologies set a new strategy for the development of civilization, and their potential consequences require a humanitarian reflection on both the prospects and the threats to its implementation. A human himself becomes a witness to a new evolutionary state of civilization and is transformed under the influence of a complex of modern media technologies. It is proved that media technologies are changing the everyday life of people, their communication, the way of their direct personal being, and the human world under the influence of media technologies, media socium and media culture is transformed into his media world. The socio-humanitarian functionality of digital media networks as digital mediators is revealed and their influence on the social-natural environment of human habitation is characterized.В статье показано, что современная медиасреда представляет собой качественно новый этап трансформации медиасреды, поскольку происходит не только интенсификация уровня развития науки и технологий, новых структур общественной жизни и способов организации производства, но и также формирование новых форм социальности, понимание природы человека и его трансформация. Проанализировано влияние медиа-технологий на социальную среду и показано, что разработки в области нанотехнологий, робототехники, мемристорной электроники, пронизывающие все уровни живой и неживой материи, становятся глобальными угрозами. Технологии NBICS задают новую стратегию развития цивилизации, и их потенциальные последствия требуют гуманитарного осмысления как перспектив, так и угроз ее реализации.     Poloidal inhomogeneity of the particle fluctuation induced fluxes near of the LCFS at lower hybrid heating and improved confinement transition at the FT-2 tokamak

    This paper deals with the new spectral and microturbulence experimental data and their analysis, which show, that the radial electric field Er generated at the LH heating (LHH) in the FT-2 is high enough to form the transport barriers. The ETB is formed when LHH is switched off. The radial fluctuation-induced EB drift flux densities near LCFS in SOL are measured at two different poloidal angles. For this purpose two Langmuir probes located at low and high field sides of the torus are used. Registration of the poloidal and radial components of the electric field and density fluctuations at the same time during one discharge permits to measure the poloidal asymmetry of the transport reduction mechanism of the radial and poloidal particle fluxes in the SOL. The absolute E(~) fluctuation levels show dependence on the sign of Er shear. The modification of the microscale turbulence by the poloidal Er x B rotation shear EB at the L - H transition near LCFS is also studied by X-mode fluctuation Reflectometry.
