Sentimentos vivenciados por mulheres climatéricas: percepções de usuárias da atenção primária em saúde

Objective: To know the feelings of women who experience the climacteric about this phase of life. Method: Qualitative research with 18 women who experience the climacteric period carried out in a Family Health Strategy in a city in the southern region of Brazil. The data were collected through a semi-structured interview script in September 2016. The data analysis was based on Minayo’s Content Analysis. Results: The feelings most frequently cited by the interviewees were melancholy, low self-esteem and depression, as the climacteric is perceived as an unknown and mysterious period, which refers to aging, losses and denial. In this sense, nursing care needs to be embodied in the emotional, social, biological, sexual, environmental and cultural aspects that permeate the climacteric. Conclusion: In this perspective, for a qualified performance in the area of health care for climacteric women, it is essential to practice practices that value prior knowledge and experienced feelings, thus providing a space for the construction of knowledge and actions in this field of knowledge.Objetivo: Conhecer os sentimentos de mulheres que vivenciam o climatério acerca desta fase da vida. Método: Pesquisa qualitativa com 18 mulheres que vivenciam o período do climatério realizada em uma Estratégia em Saúde da Família de um município da região sul do Brasil. Os dados foram coletados por meio de um roteiro de entrevista semiestruturada em setembro de 2016. A análise dos dados foi a partir da Análise de Conteúdo de Minayo. Resultados: Os sentimentos citados pelas depoentes com maior frequência foram a melancolia, a baixa autoestima e depressão, pois o climatério é percebido como um período desconhecido e misterioso, que remete ao envelhecimento, a perdas e a negação. Nesse sentido, o cuidado de enfermagem precisa consubstanciar-se nos aspectos emocionais, sociais, biológicos, sexuais, ambientais e culturais que perpassam o climatério. Conclusão: Nessa perspectiva, para uma atuação qualificada na área da assistência à saúde da mulher climatérica, é essencial práticas que valorizem o conhecimento prévio e sentimentos experienciados, facultando, assim, um espaço de construção de saberes e fazeres neste campo de conhecimento

Rationale, study design, and analysis plan of the Alveolar Recruitment for ARDS Trial (ART): Study protocol for a randomized controlled trial

Background: Acute respiratory distress syndrome (ARDS) is associated with high in-hospital mortality. Alveolar recruitment followed by ventilation at optimal titrated PEEP may reduce ventilator-induced lung injury and improve oxygenation in patients with ARDS, but the effects on mortality and other clinical outcomes remain unknown. This article reports the rationale, study design, and analysis plan of the Alveolar Recruitment for ARDS Trial (ART). Methods/Design: ART is a pragmatic, multicenter, randomized (concealed), controlled trial, which aims to determine if maximum stepwise alveolar recruitment associated with PEEP titration is able to increase 28-day survival in patients with ARDS compared to conventional treatment (ARDSNet strategy). We will enroll adult patients with ARDS of less than 72 h duration. The intervention group will receive an alveolar recruitment maneuver, with stepwise increases of PEEP achieving 45 cmH(2)O and peak pressure of 60 cmH2O, followed by ventilation with optimal PEEP titrated according to the static compliance of the respiratory system. In the control group, mechanical ventilation will follow a conventional protocol (ARDSNet). In both groups, we will use controlled volume mode with low tidal volumes (4 to 6 mL/kg of predicted body weight) and targeting plateau pressure <= 30 cmH2O. The primary outcome is 28-day survival, and the secondary outcomes are: length of ICU stay; length of hospital stay; pneumothorax requiring chest tube during first 7 days; barotrauma during first 7 days; mechanical ventilation-free days from days 1 to 28; ICU, in-hospital, and 6-month survival. ART is an event-guided trial planned to last until 520 events (deaths within 28 days) are observed. These events allow detection of a hazard ratio of 0.75, with 90% power and two-tailed type I error of 5%. All analysis will follow the intention-to-treat principle. Discussion: If the ART strategy with maximum recruitment and PEEP titration improves 28-day survival, this will represent a notable advance to the care of ARDS patients. Brazilian recommendations of mechanical ventilation 2013. Part 2

O suporte ventilatório artificial invasivo e não invasivo ao paciente crítico tem evoluído e inúmeras evidências têm surgido, podendo ter impacto na melhora da sobrevida e da qualidade do atendimento oferecido nas unidades de terapia intensiva no Brasil. Isto posto, a Associação de Medicina Intensiva Brasileira (AMIB) e a Sociedade Brasileira de Pneumologia e Tisiologia (SBPT) - representadas pelo seus Comitê de Ventilação Mecânica e Comissão de Terapia Intensiva, respectivamente, decidiram revisar a literatura e preparar recomendações sobre ventilação mecânica objetivando oferecer aos associados um documento orientador das melhores práticas da ventilação mecânica na beira do leito, baseado nas evidencias existentes, sobre os 29 subtemas selecionados como mais relevantes no assunto. O projeto envolveu etapas visando distribuir os subtemas relevantes ao assunto entre experts indicados por ambas as sociedades que tivessem publicações recentes no assunto e/ou atividades relevantes em ensino e pesquisa no Brasil na área de ventilação mecânica. Esses profissionais, divididos por subtemas em duplas, responsabilizaram-se por fazer revisão extensa da literatura mundial sobre cada subtema. Reuniram-se todos no Forum de Ventilação Mecânica na sede da AMIB em São Paulo, em 03 e 04 de agosto de 2013 para finalização conjunta do texto de cada subtema e apresentação, apreciação, discussão e aprovação em plenária pelos 58 participantes, permitindo a elaboração de um documento final
