Energy Relaxation in Nonlinear One-Dimensional Lattices
We study energy relaxation in thermalized one-dimensional nonlinear arrays of
the Fermi-Pasta-Ulam type. The ends of the thermalized systems are placed in
contact with a zero-temperature reservoir via damping forces. Harmonic arrays
relax by sequential phonon decay into the cold reservoir, the lower frequency
modes relaxing first. The relaxation pathway for purely anharmonic arrays
involves the degradation of higher-energy nonlinear modes into lower energy
ones. The lowest energy modes are absorbed by the cold reservoir, but a small
amount of energy is persistently left behind in the array in the form of almost
stationary low-frequency localized modes. Arrays with interactions that contain
both a harmonic and an anharmonic contribution exhibit behavior that involves
the interplay of phonon modes and breather modes. At long times relaxation is
extremely slow due to the spontaneous appearance and persistence of energetic
high-frequency stationary breathers. Breather behavior is further ascertained
by explicitly injecting a localized excitation into the thermalized array and
observing the relaxation behavior
Elastic constant dishomogeneity and dependence of the broadening of the dynamical structure factor in disordered systems
We propose an explanation for the quadratic dependence on the momentum ,
of the broadening of the acoustic excitation peak recently found in the study
of the dynamic structure factor of many real and simulated glasses. We ascribe
the observed law to the spatial fluctuations of the local wavelength of
the collective vibrational modes, in turn produced by the dishomegeneity of the
inter-particle elastic constants. This explanation is analitically shown to
hold for 1-dimensional disordered chains and satisfatorily numerically tested

Arrhythmic risk profile and outcomes of patients undergoing cardiac sympathetic denervation for recurrent monomorphic ventricular tachycardia after ablation
Background Cardiac sympathetic denervation (CSD) has been used as a bailout strategy for refractory ventricular tachycardia (VT). Risk of VT recurrence in patients with scar-related monomorphic VT referred for CSD and the extent to which CSD can modify this risk is unknown. We aimed to quantify arrhythmia recurrence risk and impact of CSD in this population. Methods and Results Adjusted competing risk time to event models were developed to adjust for risk of VT recurrence and sustained VT/implantable cardioverter-defibrillator shocks after VT ablation based on patient comorbidities at the time of VT ablation. Adjusted VT and implantable cardioverter-defibrillator shock recurrence rates were estimated for the subgroup who subsequently required CSD after ablation. The expected adjusted recurrence rates were then compared with the observed rates after CSD. Data from 381 patients with scar-mediated monomorphic VT who underwent VT ablation were analyzed, excluding patients with polymorphic VT. Sixty eight patients underwent CSD for recurrent VT. CSD reduced the expected adjusted VT recurrence rate by 36% (expected rate of 5.61 versus observed rate of 3.58 per 100 person-months, P=0.01) and the sustained VT/implantable cardioverter-defibrillator shock rates by 34% (expected rate of 4.34 versus observed 2.85 per 100 person-months, P=0.03). The median number of sustained VT/implantable cardioverter-defibrillator shocks in the year before versus the year after CSD was reduced by 90% (10 versus 1, P<0.0001). Conclusions Patients referred for CSD for refractory scar-mediated monomorphic VT are at a higher risk of VT recurrence after ablation as compared with those not requiring CSD, mostly because of their cardiac comorbidities. CSD significantly reduced both the expected risk of recurrences and VT burden
Preparo de meristemas apicais de cupuaçuzeiro (Theobroma grandiflorum) para estudos de expressão de genes por hibridização in situ.
O cupuaçuzeiro, Theobroma grandiflorum (Willd. ex Spreng.) Schum., é uma espécie nativa da Amazônia que produz uma das frutas mais populares da região, o cupuaçu. Sua polpa tem sabor e aroma muito apreciados, sendo utilizada na fabricação de uma grande diversidade de doces e na preparação de sucos. A partir das amêndoas é possível produzir o cupulate, produto similar ao chocolate. A cultura do cupuaçuzeiro tem relevante importância econômica para a região amazônica. Um dos entraves para esta cultura, no entanto, é a doença vassoura-de-bruxa (VB), causada pelo fungo Moniliophthora perniciosa, que compromete o desenvolvimento e a produção da planta. Este fungo acomete inicialmente as regiões meristemáticas do cupuaçuzeiro, particularmente o meristema da gema apical, causando superbrotamento, hipertrofia e hiperplasia dos tecidos, que acabam morrendo. Para maior entendimento do processo de infecção, genes relacionados à patogênese, particularmente os genes que codificam para as proteínas PR (pathogenesis-related), foram identificados em cupuaçuzeiro. Estudos de expressão temporal e espacial destes genes nos tecidos-alvo da infecção são necessários para acrescentar informações sobre a função de cada um no desenvolvimento da doença. Para tanto, torna-se necessário o estabelecimento de um método eficiente de hibridização in situ que permita identificar os locais e momentos específicos da expressão dos genes de interesse. Neste trabalho foi ajustada metodologia de hibridização in situ em gemas apicais de cupuaçuzeiros, desde a fixação e preparo do material vegetal até as condições de hibridização e pós-hibridização. Gemas apicais de galhos de cupuaçuzeiro foram coletadas e processadas para avaliação morfo-anatômica por microscopia de luz. Fragmento de cDNA de um gene de proteína do tipo PR foi clonado em pGEM®-T Easy e sondas de mRNA senso e antisenso marcadas com digoxigenina, produzidas por transcrição, foram utilizadas para a hibridização in situ em meristemas de cupuaçu. Heart Rate Recovery After Exercise Is Associated With Arrhythmic Events in Patients With Catecholaminergic Polymorphic Ventricular Tachycardia
BACKGROUND: Risk stratification in catecholaminergic polymorphic ventricular tachycardia remains ill defined. Heart rate recovery (HRR) immediately after exercise is regulated by autonomic reflexes, particularly vagal tone, and may be associated with symptoms and ventricular arrhythmias in patients with catecholaminergic polymorphic ventricular tachycardia. Our objective was to evaluate whether HRR after maximal exercise on the exercise stress test (EST) is associated with symptoms and ventricular arrhythmias. METHODS: In this retrospective observational study, we included patients ≤65 years of age with an EST without antiarrhythmic drugs who attained at least 80% of their age- and sex-predicted maximal HR. HRR in the recovery phase was calculated as the difference in heart rate (HR) at maximal exercise and at 1 minute in the recovery phase (ΔHRR1'). RESULTS: We included 187 patients (median age, 36 years; 68 [36%] symptomatic before diagnosis). Pre-EST HR and maximal HR were equal among symptomatic and asymptomatic patients. Patients who were symptomatic before diagnosis had a greater ΔHRR1' after maximal exercise (43 [interquartile range, 25-58] versus 25 [interquartile range, 19-34] beats/min; P<0.001). Corrected for age, sex, and relatedness, patients in the upper tertile for ΔHRR1' had an odds ratio of 3.4 (95% CI, 1.6-7.4) of being symptomatic before diagnosis (P<0.001). In addition, ΔHRR1' was higher in patients with complex ventricular arrhythmias at EST off antiarrhythmic drugs (33 [interquartile range, 22-48] versus 27 [interquartile range, 20-36] beats/min; P=0.01). After diagnosis, patients with a ΔHRR1' in the upper tertile of its distribution had significantly more arrhythmic events as compared with patients in the other tertiles (P=0.045). CONCLUSIONS: Catecholaminergic polymorphic ventricular tachycardia patients with a larger HRR following exercise are more likely to be symptomatic and have complex ventricular arrhythmias during the first EST off antiarrhythmic drug
Antidepressant and antipsychotic use in an Italian pediatric population
<p>Abstract</p> <p>Background</p> <p>The safety and effectiveness of psychotropic drug use in the paediatric population is widely debated, in particular because of the lack of data concerning long term effects.</p> <p>In Italy the prevalence of psychotropic drug prescriptions increased in the early 2000s and decreased afterwards. In such a context, a study with the aim to estimate the incidence and prevalence of psychotropic drug prescription in the paediatric population and to describe diagnostic and therapeutic approaches was performed.</p> <p>Methods</p> <p>The study population was composed of 76,000 youths less than 18 years and living in the area covered by the local health unit of Verona, Italy. The data source was the Verona local health unit's administrative prescription database. Prevalence and incidence of antidepressant and/or antipsychotic drug prescriptions in the 2004-2008 period were estimated. Children and adolescents receiving antidepressant and/or antipsychotic drug prescriptions between 1 January 2005 and 31 December 2006 were identified and questionnaires were sent to the prescribers with the aim to collect data concerning diagnostic and therapeutic approaches, and care strategies.</p> <p>Results</p> <p>The prevalence of psychotropic drug prescriptions did not change in the 2004-2008 period, while incidence slightly increased (from 7.0 in 2005 to 8.3 per 10,000 in 2008). Between 1 January 2005 and 31 December 2006, 111 youths received at least one psychotropic drug prescription, 91 of whom received antidepressants. Only 28 patients attended child and adolescent psychiatry services. Information concerning diagnostic and therapeutic approaches, and care strategies was collected for 52 patients (47%). Anxiety-depressive syndrome and attention disorders were the diseases for which psychotropic drugs were most commonly prescribed. In all, 75% youths also received psychological support and 20% were prescribed drugs for 2 or more years.</p> <p>Conclusions</p> <p>Despite the low drug prescription prevalence, the finding that most children were not cared for by child and adolescent psychiatric services is of concern and calls for a systematic, continuous monitoring of psychopharmacological treatments.</p
