    Image quality of myocardial perfusion-gated studies: effect of ingestion of different fat content in the reduction of extra-myocardial abdominal signal

    Myocardial perfusion-gated-SPECT (MP-gated-SPECT) imaging often shows radiotracer uptake in abdominal organs. This accumulation interferes frequently with qualitative and quantitative assessment of the infero-septal region of myocardium. The objective of this study is to evaluate the effect of ingestion of different fat content on the reduction of extra-myocardial uptake and to improve MP-gated-SPECT image quality. In this study, 150 patients (65 ^ 18 years) who were referred for MP-gated-SPECT underwent a 1-day-protocol including imaging after stress (physical or pharmacological) and resting conditions. All patients gave written informed consent. Patients were subdivided into five groups: GI, GII, GIII, GIV and GV. In the first four groups, patients ate two chocolate bars with different fat content. Patients in GV – control group (CG) – had just water. Uptake indices (UI) of myocardium (M)/liver(L) and M/stomach–proximal bowel(S) revealed lower UI of M/S at rest in all groups. Both stress and rest studies using different food intake indicate that patients who ate chocolate with different fat content showed better UI of M/L than the CG. The UI of M/L and M/S of groups obtained under physical stress are clearly superior to that of groups obtained under pharmacological stress. These differences are only significant in patients who ate high-fat chocolate or drank water. The analysis of all stress studies together (GI, GII, GIII and GIV) in comparison with CG shows higher mean ranks of UI of M/L for those who ate high-fat chocolate. After pharmacological stress, the mean ranks of UI of M/L were higher for patients who ate high- and low-fat chocolate. In conclusion, eating food with fat content after radiotracer injection increases, respectively, the UI of M/L after stress and rest in MP-gated-SPECT studies. It is, therefore, recommended that patients eat a chocolate bar after radiotracer injection and before image acquisition

    Study of the influence of count’s number in myocardium in the determination of reproducible functional parameters in Gated-SPECT studies simulated with GATE

    Myocardial Perfusion Gated Single Photon Emission Tomography (Gated-SPET) imaging is used for the combined evaluation of myocardial perfusion and left ventricular (LV) function. But standard protocols of the Gated-SPECT studies require long acquisition times for each study. It is therefore important to reduce as much as possible the total duration of image acquisition. However, it is known that this reduction leads to decrease on counts statistics per projection and raises doubts about the validity of the functional parameters determined by Gated-SPECT. Considering that, it’s difficult to carry out this analysis in real patients. For ethical, logistical and economical matters, simulated studies could be required for this analysis.     Segmentation of pathological liver tissue with Dilated Fully Convolutional Networks: A Preliminary Study

    Early detection of liver cancer, whether from primary occurrence or from metastization is highly important to establish informed treatment decisions. Accurate delineation of the liver tissues of interest facilitates quantitative assessment of the regions of interest, treatment application, and prognosis. Segmentation of the liver in Computer Tomography (CT) images allows the extraction of the three-dimensional (3D) structure of the liver tissues in which the observation of their relative position to one another is particularly important in treatment scenarios of radiation therapy or interventional surgery planning. The adequate receptive field for the segmentation of such a big organ in CT images, from the remaining neighbouring organs was very successfully improved by the use of the state-of-the-art Convolutional Neural Networks (CNN) algorithms, however, certain issues still arise and are highly dependent of pre-or post-processing methods to refine the final segmentations. Here, the effects of Dilated Convolutional Networks is proposed, for the purpose of improving segmentation of liver tissues in CT. The introduction of a dilation module allowed the concatenation of feature maps with a richer contextual information. The hierarchical learning process given by different dilated convolutional layers is analysed quantitatively. Experiments on the MICCAI Lits challenge dataset are described achieving segmentations with a mean Dice coefficients of 95.57% and 59.36% for the liver and liver tumour, using a total number 30 CT test volumes.     OKThere is significant evidence linking a 'reward deficiency syndrome' (RDS), comprising decreased availability of striatal dopamine D2-like receptors (DD2lR) and addiction-like behaviors underlying substance use disorders and obesity. Regarding obesity, a systematic review of the literature with a meta-analysis of such data is lacking. Following a systematic review of the literature, we performed random-effects meta-analyses to determine group differences in case-control studies comparing DD2lR between individuals with obesity and non-obese controls and prospective studies of pre- to post-bariatric surgery DD2lR changes. Cohen's d was used to measure effect size. Additionally, we explored factors potentially associated with group differences in DD2lR availability, such as obesity severity, using univariate meta-regression. In a meta-analysis including positron emission tomography (PET) and single-photon emission computed tomography (SPECT) studies, striatal DD2lR availability did not significantly differ between obesity and controls. However, in studies comprising patients with class III obesity or higher, group differences were significant, favoring lower DD2lR availability in the obesity group. This effect of obesity severity was corroborated by meta-regressions showing inverse associations between the body mass index (BMI) of the obesity group and DD2lR availability. Post-bariatric changes in DD2lR availability were not found, although a limited number of studies were included in this meta-analysis. These results support lower DD2lR in higher classes of obesity which is a more targeted population to explore unanswered questions regarding the RDS.publishersversionpublishe

    Extraction, selection and comparison of features for an effective automated computer-aided diagnosis of Parkinson's disease based on [123I]FP-CIT SPECT images

    Purpose This work aimed to assess the potential of a set of features extracted from [I-123] FP-CIT SPECT brain images to be used in the computer-aided "in vivo" confirmation of dopaminergic degeneration and therefore to assist clinical decision to diagnose Parkinson's disease.Methods Seven features were computed from each brain hemisphere: five standard features related to uptake ratios on the striatum and two features related to the estimated volume and length of the striatal region with normal uptake. The features were tested on a dataset of 652 [I-123] FP-CIT SPECT brain images from the Parkinson's Progression Markers Initiative. The discrimination capacities of each feature individually and groups of features were assessed using three different machine learning techniques: support vector machines (SVM), k-nearest neighbors and logistic regression.Results Cross-validation results based on SVM have shown that, individually, the features that generated the highest accuracies were the length of the striatal region (96.5%), the putaminal binding potential (95.4%) and the striatal binding potential (93.9%) with no statistically significant differences among them. The highest classification accuracy was obtained using all features simultaneously (accuracy 97.9%, sensitivity 98% and specificity 97.6%). Generally, slightly better results were obtained using the SVM with no statistically significant difference to the other classifiers for most of the features.Conclusions The length of the striatal region uptake is clinically useful and highly valuable to confirm dopaminergic degeneration "in vivo" as an aid to the diagnosis of Parkinson's disease. It compares fairly well to the standard uptake ratio-based features, reaching, at least, similar accuracies and is easier to obtain automatically. Thus, we propose its day to day clinical use, jointly with the uptake ratio-based features, in the computer-aided diagnosis of dopaminergic degeneration in Parkinson's disease

    Estudo da influência do número de ciclos por projecção no cálculo da fracção de ejecção ventricular esquerda em estudos Gated-SPECT

    Introdução - Gated-Single Photon Emission Computed Tomography (Gated-SPECT) do miocárdio usa-se cada vez mais na avaliação conjunta da perfusão do miocárdio e da função ventricular esquerda. O objectivo deste estudo é analisar a possível interferência do número de ciclos cardíacos (ciclos/projecção) e contagens totais por aquisição no cálculo da FEVE (fracção de ejecção do ventrículo esquerdo). Material e métodos - Foram incluídos 35 indivíduos a quem foram realizados estudos Gated-SPECT do miocárdio por indicação clínica. Em todos os pacientes foram adquiridos dois estudos em esforço: um com 25 ciclos! projecção (ES-25) e outro com 50 ciclos! projecção (ES-50), e dois estudos em repouso: um com 25 ciclos/projecção (ER-25) e outro com 12 ciclos! projecção (ER-1 2). Os valores da FEVE obtidos nos diferentes estudos foram tratados estatisticamente com o recurso à versão 14 SPSS. Resultados - Não houve diferenças estatisticamente significativas entre os valores de FEVE ES-25 e ES-50 (p=0.504) nem entre os valores de FEVE ER-25 e ER-12 (p=O.243). Conclusão - No caso da nossa amostra, o número de ciclos!projecção e consequentemente as contagens totais por aquisição não parecem ter influência de forma significativa no cálculo da FEVE%, nem em repouso nem após esforço

    Simulación de Monte Carlo de los rasgos básicos de la cámara gamma SPECT GE Millennium MG

    Objective - To describe and validate the simulation of the basic features of GE Millennium MG gamma camera using the GATE Monte Carlo platform. Material and methods - Crystal size and thickness, parallel-hole collimation and a realistic energy acquisition window were simulated in the GATE platform. GATE results were compared to experimental data in the following imaging conditions: a point source of 99mTc at different positions during static imaging and tomographic acquisitions using two different energy windows. The accuracy between the events expected and detected by simulation was obtained with the Mann–Whitney–Wilcoxon test. Comparisons were made regarding the measurement of sensitivity and spatial resolution, static and tomographic. Simulated and experimental spatial resolutions for tomographic data were compared with the Kruskal–Wallis test to assess simulation accuracy for this parameter. Results - There was good agreement between simulated and experimental data. The number of decays expected when compared with the number of decays registered, showed small deviation (≤0.007%). The sensitivity comparisons between static acquisitions for different distances from source to collimator (1, 5, 10, 20, 30cm) with energy windows of 126–154 keV and 130–158 keV showed differences of 4.4%, 5.5%, 4.2%, 5.5%, 4.5% and 5.4%, 6.3%, 6.3%, 5.8%, 5.3%, respectively. For the tomographic acquisitions, the mean differences were 7.5% and 9.8% for the energy window 126–154 keV and 130–158 keV. Comparison of simulated and experimental spatial resolutions for tomographic data showed no statistically significant differences with 95% confidence interval. Conclusions - Adequate simulation of the system basic features using GATE Monte Carlo simulation platform was achieved and validated.RESUMEN. Objetivo - Describir y validar la simulación de características básicas de la cámara gamma GE Millennium MG utilizando la plataforma GATE Monte Carlo. Material y métodos - El tamaño y espesor del cristal, la colimación de agujeros paralelos y una ventana de adquisición de energía realista se simularon en la plataforma GATE. Los resultados GATE se compararon con los datos experimentales en las siguientes condiciones de formación de imágenes: fuente puntual 99mTc en diferentes posiciones durante la adquisición de imágenes estáticas y tomográficas utilizando 2 diferentes ventanas de energía. La precisión entre los eventos esperados y detectados por simulación se realizó utilizando la prueba de Mann-Whitney-Wilcoxon. Las comparaciones se hicieron con respecto a las medidas de los parámetros sensibilidad y resolución espacial, estáticas y tomográficas. Las resoluciones espaciales simulada y experimental de los datos tomográficos se compararon con la prueba de Kruskal-Wallis. Resultados - Hubo buena concordancia entre los datos simulados y experimentales. El número de decaimientos esperado en comparación con los registrados, ha revelado una pequeña desviación (≤0,007%). Las comparaciones de sensibilidad entre las adquisiciones estáticas, para diferentes distancias desde la fuente al colimador (1, 5, 10, 20, 30cm) con ventanas de energía de 126-154 keV y 130-158 keV, mostraron diferencias de 4,4; 5,5; 4,2; 5,5; 4,5 y 5,4; 6,3; 6,3; 5,8, y 5,3%, respectivamente. Las comparaciones entre sensibilidad tomográfica fueron 7,5 y 9,8% para la ventana de energía 126-154 keV y 130-158 eV. La comparación de resoluciones espaciales simuladas y experimentales para los datos tomográficos no ha mostrado diferencias estadísticamente significativas con un intervalo de confianza del 95%.     Do Xpert MTB/RIF Cycle Threshold Values Provide Information about Patient Delays for Tuberculosis Diagnosis?

    INTRODUCTION: Early diagnosis and initiation to appropriate treatment is vital for tuberculosis (TB) control. The XpertMTB/RIF (Xpert) assay offers rapid TB diagnosis and quantitative estimation of bacterial burden through Cycle threshold (Ct) values. We assessed whether the Xpert Ct value is associated with delayed TB diagnosis as a potential monitoring tool for TB control programme performance. MATERIALS AND METHODS: This analysis was nested in a prospective study under the routine TB surveillance procedures of the National TB Control Program in Manhica district, Maputo province, Mozambique. Presumptive TB patients were tested using smear microscopy and Xpert. We explored the association between Xpert Ct values and self-reported delay of Xpert-positive TB patients as recorded at the time of diagnosis enrolment. Patients with >60 days of TB symptoms were considered to have long delays. RESULTS: Of 1,483 presumptive TB cases, 580 were diagnosed as TB of whom 505 (87.0%) were due to pulmonary TB and 302 (94.1%) were Xpert positive. Ct values (range, 9.7-46.4) showed a multimodal distribution. The median (IQR) delay was 30 (30-45) days. Ct values showed no correlation with delay (R2 = 0.001, p = 0.621), nor any association with long delays: adjusted odds ratios (AOR) (95% confidence interval [CI]) comparing to >28 cycles 0.99 (0.50-1.96; p = 0.987) for 23-28 cycles, 0.93 (0.50-1.74; p = 0.828) for 16-22 cycles; and 1.05 (0.47-2.36; p = 0.897) for <16 cycles. Being HIV-negative (AOR [95% CI]), 2.05 (1.19-3.51, p = 0.009) and rural residence 1.74 (1.08-2.81, p = 0.023), were independent predictors of long delays. CONCLUSION: Xpert Ct values were not associated with patient delay for TB diagnosis and cannot be used as an indicator of TB control program performance
