    Does Conscription Affect War Duration? A Study of Military Manpower Systems, Regime Type, and Interstate Wars

    While the causes of war have long been studied, the same scrutiny has not been applied to war duration. Recent empirical studies have found that similar variables correlate with both war outbreak and duration, thereby hinting at new areas for war research that can be conducted with a fresh perspective. One variable that stands out for this type of interrogation is conscription (especially when considered alongside regime type), as research on the subject has generated contradicting and inconclusive results. Given that many states around the world are replacing conscription with all volunteer forces, asking if conscription increases or decreases the lengths of wars has never been more salient. Using duration time models that assess hazard rates, I examine interstate wars from 1938-2007 to determine the risks democratic countries take on with military manpower systems. I conclude that war initiation and democracy are associated with shorter wars due to their hazard ratios. When democracies initiate wars, they are much shorter. Conscription is associated with longer wars regardless of regime type; however, this is not necessarily because conscription leads to longer wars. Rather, I argue, conscription is an indicator of state insecurity

    Phosphorus retention in a lowland Neotropical stream following an eight-year enrichment experiment

    Human alteration of the global P cycle has led to widespread P loading in freshwater ecosystems. Much research has been devoted to the capacity of wetlands and lakes to serve as long-term sinks for P inputs from the watershed, but we know much less about the potential of headwater streams to serve in this role. We assessed storage and retention of P in biotic and abiotic compartments after an 8-y experimental P addition to a 1st-order stream in a Neotropical wet forest. Sediment P extractions indicated that nearly all P storage was in the form of Fe- and Al-bound P (∼700 μg P/g dry sediment), similar to nearby naturally high-P streams. At the end of the enrichment, ∼25% of the total P added over the 8-y study was still present in sediments within 200 m of the injection site, consistent with water-column measurements showing sustained levels of high net P uptake throughout the experiment. Sediment P declined to baseline levels (∼100 μg P/g dry sediment) over 4 y after the enrichment ended. Leaf-litter P content increased nearly 2× over background levels during P enrichment and was associated with a 3× increase in microbial respiration rates, although these biotic responses were low compared to nearby naturally high- P streams. Biotic storage accounted for \u3c0.03% of retention of the added P. Our results suggest that the high sorption capacity of these sediments dampened the biotic effects of P loading and altered the timing and quantity of P exported downstream

    Dynamics of M-Theory Cosmology

    A complete global analysis of spatially-flat, four-dimensional cosmologies derived from the type IIA string and M-theory effective actions is presented. A non--trivial Ramond-Ramond sector is included. The governing equations are written as a dynamical system. Asymptotically, the form fields are dynamically negligible, but play a crucial role in determining the possible intermediate behaviour of the solutions (i.e. the nature of the equilibrium points). The only past-attracting solution (source in the system) may be interpreted in the eleven-dimensional setting in terms of flat space.     Exploring the challenges of accessing medication for patients with cystic fibrosis

    Reducing treatment burden in cystic fibrosis (CF) is the top research priority for patients and clinicians. Difficulty accessing medication is one aspect of treatment burden. We investigated this with an online survey available globally for patients with CF and healthcare professionals. Almost three quarters of patients with CF in our survey report difficulty getting repeat prescriptions on time, and most community pharmacists experience interrupted supplies of CF-specific medications.     The Influence of Body Mass Index on Clinical Interpretation of Established and Novel Biomarkers in Acute Heart Failure

    Background: Body mass index (BMI) is a known confounder for natriuretic peptides, but its influence on other biomarkers is less well described. We investigated whether BMI interacts with biomarkers’ association with prognosis in patients with acute heart failure (AHF).Methods and Results: B-type natriuretic peptide (BNP), high-sensitivity cardiac troponin I (hs-cTnI), galectin-3, serum neutrophil gelatinase-associated lipocalin (sNGAL), and urine NGAL were measured serially in patients with AHF during hospitalization in the AKINESIS (Acute Kidney Injury Neutrophil gelatinase-associated lipocalin Evaluation of Symptomatic Heart Failure) study. Cox regression analysis was used to determine the association of biomarkers and their interaction with BMI for 30-day, 90-day and 1-year composite outcomes of death or HF readmission. Among 866 patients, 21.2%, 29.7% and 46.8% had normal (18.5–24.9 kg/m2), overweight (25–29.9 kg/m2) or obese (≥ 30 kg/m2) BMIs on admission, respectively. Admission values of BNP and hs-cTnI were negatively associated with BMI, whereas galectin-3 and sNGAL were positively associated with BMI. Admission BNP and hs-cTnI levels were associated with the composite outcome within 30 days, 90 days and 1 year. Only BNP had a significant interaction with BMI. When BNP was analyzed by BMI category, its association with the composite outcome attenuated at higher BMIs and was no longer significant in obese individuals. Findings were similar when evaluated by the last-measured biomarkers and BMIs.Conclusions: In patients with AHF, only BNP had a significant interaction with BMI for the outcomes, with its association attenuating as BMI increased; hs-cTnI was prognostic, regardless of BMI.</p

    The top 10 research priorities in cystic fibrosis developed by a partnership between people with CF and healthcare providers

    There remain many treatment uncertainties in cystic fibrosis (CF). With limited resources, research should focus on questions which are most important to the CF community. We conducted a James Lind Alliance Priority Setting Partnership in CF. Research questions were elicited and then prioritised in successive surveys. A workshop agreed the final top 10. Online methods avoided cross infection and widened participation. The elicitation survey had 482 respondents (1080 questions) and prioritisation survey 677 respondents. Participants were drawn equally from the patient and clinical communities globally. We have achieved a consensus on 10 research priorities which will be attractive to funders

    Decongestion, kidney injury and prognosis in patients with acute heart failure

    Background: In patients with acute heart failure (AHF), the development of worsening renal function with appropriate decongestion is thought to be a benign functional change and not associated with poor prognosis. We investigated whether the benefit of decongestion outweighs the risk of concurrent kidney tubular damage and leads to better outcomes.& nbsp;Methods: We retrospectively analyzed data from the AKINESIS study, which enrolled AHF patients requiring intravenous diuretic therapy. Urine neutrophil gelatinase-associated lipocalin (uNGAL) and B-type natriuretic peptide (BNP) were serially measured during the hospitalization. Decongestion was defined as >= 30% BNP decrease at discharge compared to admission. Univariable and multivariable Cox models were assessed for oneyear mortality.& nbsp;Results: Among 736 patients, 53% had >= 30% BNP decrease at discharge. Levels of uNGAL and BNP at each collection time point had positive but weak correlations (r = 30% BNP decrease was a significant predictor after multivariable adjustment.& nbsp;Conclusions: Among AHF patients treated with diuretic therapy, decongestion was generally not associated with kidney tubular damage assessed by uNGAL. Kidney tubular damage with adequate decongestion does not impact outcomes; however, kidney injury without adequate decongestion is associated with a worse prognosis

    An in vitro spinal cord injury model to screen neuroregenerative materials

    Implantable 'structural bridges' based on nanofabricated polymer scaffolds have great promise to aid spinal cord regeneration. Their development (optimal formulations, surface functionalizations, safety, topographical influences and degradation profiles) is heavily reliant on live animal injury models. These have several disadvantages including invasive surgical procedures, ethical issues, high animal usage, technical complexity and expense. In vitro 3-D organotypic slice arrays could offer a solution to overcome these challenges, but their utility for nanomaterials testing is undetermined. We have developed an in vitro model of spinal cord injury that replicates stereotypical cellular responses to neurological injury in vivo, viz. reactive gliosis, microglial infiltration and limited nerve fibre outgrowth. We describe a facile method to safely incorporate aligned, poly-lactic acid nanofibre meshes (±poly-lysine + laminin coating) within injury sites using a lightweight construct. Patterns of nanotopography induced outgrowth/alignment of astrocytes and neurons in the in vitro model were strikingly similar to that induced by comparable materials in related studies in vivo. This highlights the value of our model in providing biologically-relevant readouts of the regeneration-promoting capacity of synthetic bridges within the complex environment of spinal cord lesions. Our approach can serve as a prototype to develop versatile bio-screening systems to identify materials/combinatorial strategies for regenerative medicine, whilst reducing live animal experimentation.EPSRC Doctoral Training Centre in regenerative medicine (EP/F500491/1
