Preclinical/subclinical rheumatoid arthritis-associated interstitial lung disease: misleading terms with potentially deleterious consequences
Interstitial lung disease (ILD) is a leading cause of mortality in patients with rheumatic diseases, including rheumatoid arthritis. The 5-year mortality rate is twice as high in patients with rheumatoid arthritis-associated ILD than in patients with rheumatoid arthritis without ILD. Moreover, a report showed that mortality rates in patients with disease codes for rheumatoid arthritis-associated ILD remained unchanged from 2005–18, even though the overall rheumatoid arthritis mortality rate declined during this time period. Despite the evidence that ILD contributes to premature death in rheumatoid arthritis, screening for ILD in patients with rheumatoid arthritis is not routinely performed in clinical practice and numerous questions remain regarding the management of rheumatoid arthritis-associated ILD
Heterogeneous detection probabilities for imperiled Missouri River fishes: implications for large-river monitoring programs
Occupancy modeling was used to determine (1) if detection probabilities (p) for 7 regionally imperiled Missouri River fishes (Scaphirhynchus albus, Scaphirhynchus platorynchus, Cycleptus elongatus, Sander canadensis, Macrhybopsis aestivalis, Macrhybopsis gelida, and Macrhybopsis meeki) differed among gear types (i.e. stationary gill nets, drifted trammel nets, and otter trawls), and (2) how detection probabilities were affected by habitat (i.e. pool, bar, and open water), longitudinal position (five 189 to 367 rkm long segments), sampling year (2003 to 2006), and season (July 1 to October 30 and October 31 to June 30). Adult, large-bodied fishes were best detected with gill nets (p: 0.02–0.74), but most juvenile large-bodied and all small-bodied species were best detected with otter trawls (p: 0.02–0.58). Trammel nets may be a redundant sampling gear for imperiled fishes in the lower Missouri River because most species had greater detection probabilities with gill nets or otter trawls. Detection probabilities varied with river segment for S. platorynchus, C. elongatus, and all small-bodied fishes, suggesting that changes in habitat influenced gear efficiency or abundance changes among river segments. Detection probabilities varied by habitat for adult S. albus and S. canadensis, year for juvenile S. albus, C. elongatus, and S. canadensis, and season for adult S. albus. Concentrating sampling effort on gears with the greatest detection probabilities may increase species detections to better monitor a population’s response to environmental change and the effects of management actions on large-river fishes
Sleeping arrangements and mass distribution of bed nets in six districts in central and northern Mozambique
OBJECTIVE: Universal coverage with insecticide-treated bed nets
is a cornerstone of modern malaria control. Mozambique has
developed a novel bed net allocation strategy, where the number
of bed nets allocated per household is calculated on the basis
of household composition and assumptions about who sleeps with
whom. We set out to evaluate the performance of the novel
allocation strategy. METHODS: 1,994 households were visited
during household surveys following two universal coverage bed
net distribution campaigns in Sofala and Nampula Provinces in
2010-2013. Each sleeping space was observed for the presence of
a bed net, and the sleeping patterns for each household were
recorded. The observed coverage and efficiency were compared to
a simulated coverage and efficiency had conventional allocation
strategies been used. A composite indicator, the product of
coverage and efficiency, was calculated. Observed sleeping
patterns were compared with the sleeping pattern assumptions.
RESULTS: In households reached by the campaign, 93% (95% CI:
93-94%) of sleeping spaces in Sofala and 84% (82-86%) in Nampula
were covered by campaign bed nets. The achieved efficiency was
high, with 92% (91-93%) of distributed bed nets in Sofala and
93% (91-95%) in Nampula covering a sleeping space. Using the
composite indicator, the novel allocation strategy outperformed
all conventional strategies in Sofala and was tied for best in
Nampula. The sleeping pattern assumptions were completely
satisfied in 66% of households in Sofala and 56% of households
in Nampula. The most common violation of the sleeping pattern
assumptions was that male children 3-10 years of age tended not
to share sleeping spaces with female children 3-10 or 10-16
years of age. CONCLUSIONS: The sleeping pattern assumptions
underlying the novel bed net allocation strategy are generally
valid, and net allocation using these assumptions can achieve
high coverage and compare favorably with conventional allocation

Differences in Respiratory Symptoms and Lung Structure Between Hispanic and Non-Hispanic White Smokers: A Comparative Study
Background: Prior studies have demonstrated that U.S. Hispanic
smokers have a lower risk of decline in lung function and
chronic obstructive pulmonary disease (COPD) compared with
non-Hispanic whites (NHW). This suggests there might be
racial-ethnic differences in susceptibility in cigarette
smoke-induced respiratory symptoms, lung parenchymal
destruction, and airway and vascular disease, as well as in
extra-pulmonary manifestations of COPD. Therefore, we aimed to
explore respiratory symptoms, lung function, and pulmonary and
extra-pulmonary structural changes in Hispanic and NHW smokers.
Methods: We compared respiratory symptoms, lung function, and
computed tomography (CT) measures of emphysema-like tissue,
airway disease, the branching generation number (BGN) to reach a
2-mm-lumen-diameter airway, and vascular pruning as well as
muscle and fat mass between 39 Hispanic and 39 sex-, age- and
smoking exposure-matched NHW smokers. Results: Hispanic smokers
had higher odds of dyspnea than NHW after adjustment for COPD
and asthma statuses (odds ratio[OR] = 2.96; 95% confidence
interval [CI] 1.09-8.04), but no significant differences were
found in lung function and CT measurements. Conclusions: While
lung function and CT measures of the lung structure were
similar, dyspnea is reported more frequently by Hispanic than
matched-NHW smokers. It seems to be an impossible puzzle but
it's easy to solve a Rubik' Cube using a few algorithms
<i>RPA3-UMAD1</i> rs12702634 and rheumatoid arthritis-associated interstitial lung disease in European ancestry
Objective Recently, a genome-wide association study identified an association between RA-associated interstitial lung disease (ILD) and RPA3-UMAD1 rs12702634 in the Japanese population, especially for patients with a usual interstitial pneumonia (UIP) pattern. We aimed to replicate this association in a European population and test for interaction with MUC5B rs35705950.Methods In this genetic case-control association study, patients with RA and ILD and controls with RA and no ILD were included from France, the USA and the Netherlands. Only cases and controls from European genetic ancestries determined by principal components analysis were included in the analyses. RA was defined by the 1987 ACR or 2010 ACR/EULAR criteria and ILD by chest high-resolution CT scan, except in the control dataset from the Netherlands, where the absence of ILD was determined by chart review. Patients were genotyped for RPA3-UMAD1 rs12702634 and MUC5B rs35705950. Associations were tested using logistic regression adjusted for sex, age at RA onset, age at ILD onset or at certified absence of ILD, tobacco smoking status and country of origin.Results Among the 883 patients included, 322 were RA-ILD cases (36.5%). MUC5B rs35705950 was strongly associated with RA-ILD in all datasets {combined adjusted odds ratio [OR] 2.9 [95% CI 2.1, 3.9], P = 1.1 x 10-11. No association between RPA3-UMAD1 rs12702634 and RA-ILD was observed [combined OR 1.2 (95% CI 0.8, 1.6), P = 0.31. No interaction was found between RPA3-UMAD1 rs12702634 and MUC5B rs35705950 (P = 0.70).Conclusion Our findings did not support a contribution of RPA3-UMAD1 rs12702634 to the overall RA-ILD susceptibility in the European population.What does this mean for patients?Interstitial lung disease (ILD) can develop in 10-60% of patients with rheumatoid arthritis (RA) and is associated with an increased risk of death. We do not yet fully understand why RA-ILD occurs, but risk factors include genetics and environmental factors such as tobacco smoking. Identifying new genetic risk factors for RA-ILD may improve our understanding of how this disease occurs, help us categorize patients in terms of their risk level and help us to potentially identify new drug targets. A previous Japanese genetic study identified the RPA3-UMAD1 rs12702634 common genetic variant as a risk factor for RA-ILD. However, a second Japanese study failed to replicate these findings. In this international study including patients with European ancestry, we did not find that RPA3-UMAD1 rs12702634 contributed to the overall risk of RA-ILD. Our findings highlight the importance of conducting analyses that try to replicate the results of a study. We also emphasize that genetic associations-even those already reported-require rigorous testing in different groups of people before we can conclude that they contribute to disease risk. Ongoing collaboration and multi-ancestry genetic studies are essential in order to advance our understanding of the complex genetics underlying RA-ILD
Effects of antiplatelet therapy on stroke risk by brain imaging features of intracerebral haemorrhage and cerebral small vessel diseases: subgroup analyses of the RESTART randomised, open-label trial
Background
Findings from the RESTART trial suggest that starting antiplatelet therapy might reduce the risk of recurrent symptomatic intracerebral haemorrhage compared with avoiding antiplatelet therapy. Brain imaging features of intracerebral haemorrhage and cerebral small vessel diseases (such as cerebral microbleeds) are associated with greater risks of recurrent intracerebral haemorrhage. Themis2/ICB1 Is a Signaling Scaffold That Selectively Regulates Macrophage Toll-Like Receptor Signaling and Cytokine Production
BACKGROUND: Thymocyte expressed molecule involved in selection 1 (Themis1, SwissProt accession number Q8BGW0) is the recently characterised founder member of a novel family of proteins. A second member of this family, Themis2 (Q91YX0), also known as ICB1 (Induced on contact with basement membrane 1), remains unreported at the protein level despite microarray and EST databases reporting Themis2 mRNA expression in B cells and macrophages. METHODOLOGY/PRINCIPAL FINDINGS: Here we characterise Themis2 protein for the first time and show that it acts as a macrophage signalling scaffold, exerting a receptor-, mediator- and signalling pathway-specific effect on TLR responses in RAW 264.7 macrophages. Themis2 over-expression enhanced the LPS-induced production of TNF but not IL-6 or Cox-2, nor TNF production induced by ligands for TLR2 (PAM3) or TLR3 (poly IratioC). Moreover, LPS-induced activation of the MAP kinases ERK and p38 was enhanced in cells over-expressing Themis2 whereas the activation of JNK, IRF3 or NF-kappaB p65, was unaffected. Depletion of Themis2 protein by RNA inteference inhibited LPS-induced TNF production in primary human macrophages demonstrating a requirement for Themis2 in this event. Themis2 was inducibly tyrosine phosphorylated upon LPS challenge and interacted with Lyn kinase (P25911), the Rho guanine nucleotide exchange factor, Vav (P27870), and the adaptor protein Grb2 (Q60631). Mutation of either tyrosine 660 or a proline-rich sequence (PPPRPPK) simultaneously interrupted this complex and reduced by approximately 50% the capacity of Themis2 to promote LPS-induced TNF production. Finally, Themis2 protein expression was induced during macrophage development from murine bone marrow precursors and was regulated by inflammatory stimuli both in vitro and in vivo. CONCLUSIONS/SIGNIFICANCE: We hypothesise that Themis2 may constitute a novel, physiological control point in macrophage inflammatory responses

Non-equilibrium statistical mechanics: From a paradigmatic model to biological transport
Unlike equilibrium statistical mechanics, with its well-established
foundations, a similar widely-accepted framework for non-equilibrium
statistical mechanics (NESM) remains elusive. Here, we review some of the many
recent activities on NESM, focusing on some of the fundamental issues and
general aspects. Using the language of stochastic Markov processes, we
emphasize general properties of the evolution of configurational probabilities,
as described by master equations. Of particular interest are systems in which
the dynamics violate detailed balance, since such systems serve to model a wide
variety of phenomena in nature. We next review two distinct approaches for
investigating such problems. One approach focuses on models sufficiently simple
to allow us to find exact, analytic, non-trivial results. We provide detailed
mathematical analyses of a one-dimensional continuous-time lattice gas, the
totally asymmetric exclusion process (TASEP). It is regarded as a paradigmatic
model for NESM, much like the role the Ising model played for equilibrium
statistical mechanics. It is also the starting point for the second approach,
which attempts to include more realistic ingredients in order to be more
applicable to systems in nature. Restricting ourselves to the area of
biophysics and cellular biology, we review a number of models that are relevant
for transport phenomena. Successes and limitations of these simple models are
