    Distribution of monocarboxylate transporters in the peripheral nervous system suggests putative roles in lactate shuttling and myelination.

    Lactate, a product of glycolysis, has been shown to play a key role in the metabolic support of neurons/axons in the CNS by both astrocytes and oligodendrocytes through monocarboxylate transporters (MCTs). Despite such importance in the CNS, little is known about MCT expression and lactate function in the PNS. Here we show that mouse MCT1, MCT2, and MCT4 are expressed in the PNS. While DRG neurons express MCT1, myelinating Schwann cells (SCs) coexpress MCT1 and MCT4 in a domain-specific fashion, mainly in regions of noncompact myelin. Interestingly, SC-specific downregulation of MCT1 expression in rat neuron/SC cocultures led to increased myelination, while its downregulation in neurons resulted in a decreased amount of neurofilament. Finally, pure rat SCs grown in the presence of lactate exhibited an increase in the level of expression of the main myelin regulator gene Krox20/Egr2 and the myelin gene P0. These data indicate that lactate homeostasis participates in the regulation of the SC myelination program and reveal that similar to CNS, PNS axon-glial metabolic interactions are most likely mediated by MCTs

    Spontaneous reporting of adverse drug reactions in a pediatric population in a tertiary hospital

    The pediatric population is a vulnerable group for adverse drug reactions (ADRs), and data on spontaneous reporting of ADRs in the hospital setting are scarce. We conducted a retrospective analysis of ADRs in pediatric patients spontaneously reported by health care professionals to a Pharmacovigilance Program in a tertiary hospital between 2010 and 2020, and we compared characteristics of ADRs between pediatric age subgroups. From 1787 spontaneously reported ADRs in an 11-year period, 103 (5.85%) were pediatric ADRs. The median age of patients with ADRs was 8.4 years (range 1 day-17 years) and 57.3% were male. The most frequent ADRs reported were nervous system disorders (13.6%) and the most frequently involved drugs were antineoplastics and immunodulators (32.4%). A 59.2% of the ADRs were serious and 55.3% were classified as being type B reactions. Medication errors were involved in 7.8% of the ADRs and 11.9% of the suspected drugs were used off-label. Spontaneous reports of ADRs in newborns, infants, and toddlers were more serious and less often described in the product data sheet than in children and adolescents (p < 0.001 and p = 0.004 respectively). Medication errors were more frequent in patients under two years of age. These results should be interpreted with caution due to under-reporting and biases in spontaneous reporting of ADRs

    Interval cancers in a population-based screening program for colorectal cancer in Catalonia, Spain

    Objective. To analyze interval cancers among participants in a screening program for colorectal cancer (CRC) during four screening rounds. Methods. The study population consisted of participants of a fecal occult blood test-based screening program from February 2000 to September 2010, with a 30-month follow-up (n = 30,480). We used hospital administration data to identify CRC. An interval cancer was defined as an invasive cancer diagnosed within 30 months of a negative screening result and before the next recommended examination. Gender, age, stage, and site distribution of interval cancers were compared with those in the screen-detected group. Results. Within the study period, 97 tumors were screen-detected and 74 tumors were diagnosed after a negative screening. In addition, 17 CRC (18.3%) were found after an inconclusive result and 2 cases were diagnosed within the surveillance interval (2.1%). There was an increase of interval cancers over the four rounds (from 32.4% to 46.0%). When compared with screen-detected cancers, interval cancers were found predominantly in the rectum (OR: 3.66; 95% CI: 1.51-8.88) and at more advanced stages (P = 0.025). Conclusion. There are large numbers of cancer that are not detected through fecal occult blood test-based screening. The low sensitivity should be emphasized to ensure that individuals with symptoms are not falsely reassured

    Long-term outcome of patients wih distal ulcerative colitis and inflammation of the appendiceal orifice

    Background & Aims: Skip inflammation of the appendiceal orifice has been described in distal UC (UC-IAO) but long-term clinical outcomes are poorly established. Our aim was to evaluate the long-term clinical outcomes of UC-IAO as compared to classic distal UC. Methods: Patients with UC-IAO were identified from the local IBD database. Disease outcome and therapeutic requirements during follow-up were accurately collected, and compared with a control group of patients with distal UC without periappendiceal involvement matched by disease extent (proctitis/distal), smoking habit, and date and age at diagnosis. Results: Fourteen UC patients were found to have UC-IAO, most of them with initial extent of UC limited to the rectum. All patients were initially managed with mesalazine administered orally (28.5%), topically (28.5%), or in combination (43%). After a median follow-up of 78 months (interquartile range - IQR 45-123) most UC-IAO patients were successfully managed with oral and/or topical aminosalycilates. Only one of them developed proximal disease progression. As compared to controls, no differences in clinical outcomes or therapeutic requirements were found. Conclusions: Patients with UC-IAO tend to present a mild course, with a low probability to develop proximal progression of disease extent or to require immunosuppressive therapy or colectomy.CIBERehd of Fondo de Investigación Sanitaria of the Instituto de Salud Carlos III, from the Spanish Ministry of HealthMedicin

    Emergent Gauge Fields in Holographic Superconductors

    Holographic superconductors have been studied so far in the absence of dynamical electromagnetic fields, namely in the limit in which they coincide with holographic superfluids. It is possible, however, to introduce dynamical gauge fields if a Neumann-type boundary condition is imposed on the AdS-boundary. In 3+1 dimensions, the dual theory is a 2+1 dimensional CFT whose spectrum contains a massless gauge field, signaling the emergence of a gauge symmetry. We study the impact of a dynamical gauge field in vortex configurations where it is known to significantly affect the energetics and phase transitions. We calculate the critical magnetic fields H_c1 and H_c2, obtaining that holographic superconductors are of Type II (H_c1 < H_c2). We extend the study to 4+1 dimensions where the gauge field does not appear as an emergent phenomena, but can be introduced, by a proper renormalization, as an external dynamical field. We also compare our predictions with those arising from a Ginzburg-Landau theory and identify the generic properties of Abrikosov vortices in holographic models.Comment: 19 pages, 14 figures, few comments added, version published in JHE

    Ideación suicida y sintomatología obsesivo-compulsiva en una muestra de adolescentes

    El objetivo principal de este estudio es determinar la relación entre la sintomatologfa obsesivo-compulsiva (SOC) y la ideación suicida en una muestra elegida al azar (n=1525) proveniente de la población de estudiantes de 8° de EGB de la ciudad de Barcelona. Por otra parte, se estudiará la presencia de dicha síntomatología en la población origen de la muestra. La relación entre la alta SOC y la ideación suicida ha sido fuente de pocas investigaciones, las cuales frecuentemente se han centrado en el trastorno Obsesivo-compulsivo (TOC) y su relación con la conducta suicida. Es por ello que el estudio de la relación entre la alta SOC y ideación suicida no está exento de interés por el carácter novedoso del mismo. Los resultados indican que, tal y como se establecía en la hipótesis principal del trabajo, aquellos sujetos que cuyas puntuaciones son más elevadas en la escala que mide SOC presentan mayor ideación suicida. Se discute la posible implicación que dicho hallazgo supone en el marco de la práctica clínica y se establecen posibles estrategias para prevenir e identificar precozmente la conducta suicida en sujetos de alto riesgo

    Prevalence of major depression in preschool children

    The prevalence of preschool major depressive disorder (MDD) was studied in the community. The whole population of children between 3 and 6 years attending preschool nurseries in three areas (one urban, one rural and one suburban) in Spain (n = 1,427) were contacted. Selection was by a two-stage procedure. At stage I, the ESDM 3-6, a screening measure for preschool depression, was used to identify a sample for more intensive interviewing. Sensitivity and specificity of the cut-off point of the ESDM 3–6 had been previously tested in a pilot study (n = 229). During the first stage, 222 preschool children (15.6%) were found to be probable depressives, because they scored 27 or more, the cut-off used. At stage II, the children were interviewed and diagnosed by the consensus of two clinicians, blind to the ESDM 3-6 results. DSM-IV diagnostic criteria were used to define caseness. A total of 16 children (1.12%) met the MDD criteria. The prevalence by areas was urban 0.87%, rural 0.88%, suburban 1.43%. Sex distribution prevalence was 1:1.     Iron oxide-based nanostructured ceramics with tailored magnetic and mechanical properties: Development of mechanically robust, bulk superparamagnetic materials

    Nanostructured iron-oxide based materials with tailored mechanical and magnetic behavior are produced in bulk form. By applying ultra-fast heating routines via spark plasma sintering (SPS) to supercrystalline pellets, materials with an enhanced combination of elastic modulus, hardness and saturation magnetization are achieved. Supercrystallinity-namely the arrangement of the constituent nanoparticles into periodic structures-is achieved through self-assembly of the organically-functionalized iron oxide nanoparticles. The optimization of the following SPS regime allows the control of organics' removal, necking, iron oxide phase transformations and nano-grain size retention, and thus the fine-tuning of both mechanical properties and magnetic response, up until the production of bulk mm-size superparamagnetic materials.Deusche Forschungsgemeinschaft (DFG
