A qualitative study of older patients’ and family caregivers’ perspectives of transitional care from hospital to home

Differences in Importance Attached to Drug Effects Between Patients With Type 2 Diabetes From the Netherlands and Turkey:A Preference Study
Objective: The aim of this study was to compare the importance that patients with type 2 diabetes mellitus from the Netherlands and Turkey attach to certain drug effects of oral anti-diabetic drugs. Methods: Data were collected through a cross-sectional survey containing demographic questions and a discrete choice experiment assessing preferences for oral anti-diabetic drugs. Adults from the Netherlands and Turkey were included if they had type 2 diabetes mellitus and had received a prescription of an oral anti-diabetic drug in the last 4 months. The oral anti-diabetic drugs in the discrete choice experiment were described in terms of six attributes: effects on HbA1c, cardiovascular diseases, weight change, gastrointestinal adverse drug events hypoglycemic events, and bladder cancer. Multinomial logit models with country as an interaction factor were fitted. Results: In total, 381 patients were included, 199 from the Netherlands and 182 from Turkey. Patients' preferences toward drug effects varied between the countries. Turkish patients attached the highest importance to reducing the risk of cardiovascular diseases (relative weight: 0.51, 95% CI 0.45-0.55), followed by reducing hypoglycemic events (relative weight: 0.16, 95% CI 0.11-0.22), and reducing gastrointestinal adverse drug events (relative weight: 0.11, 95% CI 0.07-0.18). Patients from the Netherlands attached the highest importance to gastrointestinal ADEs (relative weight: 0.22, 95% CI 0.14-0.39), followed by reducing hypoglycemic events (relative weight: 0.22, 95% CI 0.16-0.25), and reducing the risk of cardiovascular diseases (relative weight: 0.20, 95% CI 0.13-0.23). Conclusion: Patient preferences may differ across countries. Such differences should be acknowledged in regulatory decisions and clinical practice

Akılcı ilaç kullanımı: Hollanda örneği
Toplam sağlık harcamaları içerisinde ilaca ayrılan pay giderek büyümekte ve politikacılar kaliteli sağlık hizmetlerinden ödün vermeden ilaç harcamalarını kontrol altına almak amacıyla yeni çözüm arayışlarını sürdürmektedirler. Ülkeler için sağlık politikasının önemli bileşenlerinden biri olan akılcı ilaç kullanımı hastaların klinik ihtiyaçlarına uygun, bireysel gereksinimlerini karşılayacak dozda, yeterli süre boyunca, kendileri ve yaşadıkları toplum için karşılanabilir fiyatta ilaçları almalarını gerektirir. Akılcı olmayan ilaç kullanımı dünya genelinde önemli halk sağlığı problemlerinden biridir. Polifarmasi, antibiyotiklerin yetersiz dozda veya nonbakteriyel enfeksiyonlarda kullanımı, ilaçların oral formları daha uygun olabilecekken enjektabl formlarının kullanımı, klinik tanı ve tedavi rehberlerine uygun olmayan ilaç reçete edilmesi, hastaların muayene olmadan ilaç alıp kullanmaları veya ilaçlarını hekim tarafından tavsiye edildiği şekilde kullanmamaları akılcı olmayan ilaç kullanımına örnek olarak verilebilir3. Dünya Sağlık Örgütü (DSÖ)’nün tahminlerine göre ilaçların yarısından fazlası uygunsuz biçimde reçete edilmekte, satılmakta ve hastaların yarısı ilaçlarını tedavilerine uygun şekilde almamaktadır. İlaçların gerektiği miktarın altında, üzerinde veya yanlış kullanımı insan sağlığına zarar vermesinin yanı sıra kısıtlı kaynakların da boşa harcanmasına neden olmaktadır. YASLI BİREYLERİN İLAÇ KULLANIMIYLA İLGİLİ BİLGİ VE UYGULAMALARIN BELİRLENMESİ
Bu arastırma yaslı bireylerin ilaç kullanım bilgi ve uygulamalarını
belirlemek amacıyla planlanmıs tanımlayıcı bir çalısmadır.
Arastırmanın örneklemini Eryaman 3 No’lu Saglık Ocagı bölgesinde
yasayan 65 yas ve üzeri 960 birey arasından rastgele örnekleme
yöntemiyle seçilen 300 kisi olusturmustur. Veri toplanmasında anket formu
kullanılmıstır. Elde edilen verilerin dagılımları SPSS 11.5 paket
programında, sayı, yüzde ve ki- kare testleri kullanılarak
degerlendirilmistir.
Arastırmaya katılan bireylerin %50.7’si 65- 69 yas grubunda,
%61.0’i kadın ve %76.3’ü ilkokul ve üzeri egitim düzeyindeydi. Ayrıca
bireylerin %44.1’i birden fazla hastalıga sahip ve %84.3’ü birden fazla ilaç
kullanmaktaydı. Çalısmaya katılan bireylerde en fazla görülen hastalık
hipertansiyon ve en fazla kullanılan ilaç grubu kardiyovasküler sistem
ilaçlarıydı.
Bireylerin %17.0’si reçete edilen ilacı kullanmadıklarını, %16.7’si de
reçetesiz ilaç aldıklarını ifade etmislerdir. Çalısma sonucuna göre
bireylerin %19.3’ünde kullandıkları ilaca baglı yan etki görülmüs ve en sık
mide agrısı- yanması sikayeti görülmüstür (%36.2).
Çalısmaya katılan bireylerin %51.3’ü soruların tamamını dogru
olarak yanıtlamıstır. Çalısmamız sonucunda bireylerin egitim durumunun
reçetesiz ilaç alma ve bilgi sorularını dogru yanıtlama durumlarını etkiledigi
bulunmustur (p<0.05).
Yaslı bireylere kullandıkları ilaçların %88.7’si ile ilgili doktorlar, %
2.2’si ile ilgili hemsireler bilgi vermislerdir.
Bu sonuçlara göre hemsirelerin ilaç kullanan yaslıların farkında
olmaları, bu bireyleri yakından izlemeli, ilaç kullanımı hakkında bireyleri
sürekli olarak bilgilendirmeleri ve desteklemeleri önerilmektedir.Determining the practise and knowledge of medicine usage in older people
is the aim of this descriptive study.
The aim of this descriptive research was to determine drug use and
information in the older people.
The study sample was comprised of 300 people chosen from 960 old
people over 65 years of age living in Eryaman Health Centre No. 3
neighbourhood by random sampling. Data which had been gathered with
questionnaire survey were analyzed with the SPSS 11.5 and frequency, percent
and chi-square tests.
In this study, 50.7% of the persons are in the age group of 65-69, 61.0%
are female and 76.3% of the survey group graduated from a primary school or
higher schools. Additionally 44.1% of persons have more than one chronic
disease and 84.3% of them use more than one medicine. The most common seen
disease in the persons who participated in this research is hypertension and the
most common used medicine group is cardiovascular system medicine.
According to the older persons expressions, 17.0% of the persons
abandoned using doctor’s prescription and 16.7% of them have been using overthe-
counter medicine. As a result of the this study, adverse medicine reactions
occurred in the 19.3% of persons and most common seen adverse medicine
reactions are gastric problems (36.2%).
Replies to the questions concerning medicine knowledge shows that 51.3%
of the persons know the right answers. This research presents that education level
of the older persons is found to be effective in the usage of over-the-counter
medicines and medicine knowledge (p<0.05).
88.7% of the survey group stated that physicians informed them about the
usage of the medicines and 2.2% of them were informed by nurses.
Taking into consideration, these findings suggest that the nurses should be
aware of the education level, habits of the older persons who use medicines, and
they should observe them closely and inform and support them with the
appropriate usage of the medicines
Impact of a nurse‐led sleep programme on the sleep quality and depressive symptomatology among older adults in nursing homes: A non‐randomised controlled study
Sleep problems are much more prevalent in nursing home residents than in their community‐dwelling counterparts. Cognitive behavioural therapy is likely to improve sleep quality in older adults. However, these interventions have a narrow scope to generalise to nursing home practice, and there are minimal data on the effectiveness of the integration of cognitive behavioural therapy and motivational interviewing in nursing home settings. Aim: The purpose of this study was to investigate the impact of a nurse‐led sleep programme on the sleep quality and depressive symptomatology in cognitively intact nursing home residents. Methods: A non‐randomised controlled study design was applied to 52 older adults living in three different nursing homes in the same locality. Then, we categorised our study cohort into the following two groups: intervention group in which the older adults received the nurse‐led sleep programme and control group in which the older adults received regular nursing home services. One nursing home was assigned to the intervention, and two were assigned to control. The intervention group participated in four weekly 1‐hr sessions performed by the first author. Results: Follow‐up assessments conducted at 8 and 12 weeks after the intervention ended revealed that participants in the intervention group experienced a significant improvement in some objective sleep parameters, including awakenings, total wake time, wake after sleep onset and sleep efficiency, as well as a significant decrease in depressive symptomatology. Conclusion: To summarise, the results of this study showed that the nurse‐led sleep programme improved the sleep quality and reduced depressive symptomatology in cognitively intact nursing home residents. In this context, we can assert that interventions for sleep problems should be individualised by addressing sleep experience and sleep management strategies of older adults living in nursing homes and should be integrated with motivational interviewing techniques
Feasibility of the duration of actigraphy data to illustrate circadian rhythm among cognitively intact older people in nursing home: cosinor analysis
Assessment of sleep is an important determinant of health. Moreover, sleep assessment is quite difficult in elderly people;
getting consent for participation in the study, spending more time due to cognitive and communicative disruptions, having
difficulties in accessing to individuals, and higher withdrawal rate of elderly people than adults occur as additional difficulties.
The objective of the study was to determine whether 1-day measurement could be enough to determine the circadian rhythm
of cognitively intact older adults in nursing home. A sample of the study consisted of individuals living in three nursing
homes with similar environmental conditions and who were independent in daily living activities. The exclusion criteria
were severe health problems, severe neurological and mental diseases, and severe hearing loss and cognitive impairment.
Continuous actigraphy monitoring was performed in 52 older adults who met the sampling criteria over a 4-day period.
Cycle mesor, amplitude, acrophase, and R2, calculated using 1 to 4 days of data and each day of data, were compared. For
amplitude, acrophase and R2 parameters, there was no statistically significant difference between 4-day measurement and
1-day measurement (p > 0.05). However, for mesor parameter, there was no significant difference between 1-day, 2-day,
and 3-day measurements, and differentiation was observed in 4-day measurements. A daily measurement could be used to
determine the circadian rhythm of actigraphy in cognitively intact older adults in nursing home. However, further studies
are required to better understand and support our findings
