    Fracturas de extremidad proximal de cubito en el niño

    Las fracturas y epifisiolisis proximales de cubito, son lesiones raras y cuando se producen suelen estar mínimamente ó no desplazadas, pudiendo ser tratadas de forma conservadoras. En los casos en que existe desplazamiento significativo, requieren reducción abierta y osteosíntesis. Independientemente del tipo de tratamiento utilizado, el resultado final suele ser satisfactorio. Realizamos un estudio retrospectivo de 13 casos de fractura-epifisiolisis de extremo proximal de cubito, ocurridas durante el periodo 1992-2001. Nuestro objetivo es analizar los resultados obtenidos en el tratamiento de las lesiones y realizar una revisión bibliográfica, teniendo en cuenta los distintos métodos de clasificación, valoración de resultados y las valoraciones descritas para la iniciación de tratamiento quirúrgico de estas lesiones.The epiphysiolisis and proximal lunar fractures in children rare injuries and usually the aren't displaced. In these cases they can be managed nonoperatively. If there's a marked displacement, open reduction and internal fixation is mandatory.     Epifisiolisis de cabeza de radio asociadas a luxación de codo y reducción espontánea

    Realizamos un estudio retrospectivo de 10 casos de la epifisiolisis de cabeza de radio ocurridas durante el periodo 1992 a 1997, con un desplazamiento mayor de 60º que según los criterios de O'Brien corresponderían a un grado III. De todas las revisadas seleccionamos 6 casos en los que había una angulación mayor de 90º, creemos ocurren después de luxación o reducción espontánea de luxación de codo; por ser las más frecuentes, las que más problemas presentan en el tratamiento y las que producen más secuelas. Nuestro objetivo es valorar los resultados obtenidos con nuestros tratamientos y realizar una revisión bibliográfica.We have done a retrospective study of ten cases of radial head epiphysiolysis occurred between 1992 and 1997, displaced more than 60º. (O'Brien's grade III). Amongst them we have selected those which were displaced more than 90º (six cases) probably occurred after an elbow luxation or after the spontaneous reduction of an elbow luxation; because the are more infrequent, more troublesome and they give more secuelae.     The TRPV4 channel links calcium influx to DDX3X activity and viral infectivity

    Ion channels are well placed to transduce environmental cues into signals used by cells to generate a wide range of responses, but little is known about their role in the regulation of RNA metabolism. Here we show that the TRPV4 cation channel binds the DEAD-box RNA helicase DDX3X and regulates its function. TRPV4-mediated Ca2+ influx releases DDX3X from the channel and drives DDX3X nuclear translocation, a process that involves calmodulin (CaM) and the CaM-dependent kinase II. Genetic depletion or pharmacological inhibition of TRPV4 diminishes DDX3X-dependent functions, including nuclear viral export and translation. Furthermore, TRPV4 mediates Ca2+ influx and nuclear accumulation of DDX3X in cells exposed to the Zika virus or the purified viral envelope protein. Consequently, targeting of TRPV4 reduces infectivity of dengue, hepatitis C and Zika viruses. Together, our results highlight the role of TRPV4 in the regulation of DDX3X-dependent control of RNA metabolism and viral infectivity

    Injections of the low diffusion retrograde tracer rhodamine latex microspheres were made in the pontine grey matter of the cat in order to study the cortical convergence to the pons. We found a different distribution of cells in the convex surface of the brain hemisphere making ventral os dorsolateral injections. In the first case, cells were grouped in the top of the gyri. In the second case, cells were more frequent in the bottom of the sulci. Our results show a possible retinotopic organization of this projection

    Longitudinal strain in remote non-infarcted myocardium by tissue tracking CMR: characterization, dynamics, structural and prognostic implications

    [EN] Purpose In ST-segment elevation myocardial infarction (STEMI) patients, longitudinal strain (LS) in remote non-infarcted myocardium (RNM) has not yet been characterized by tissue tracking (TT) cardiovascular magnetic resonance (CMR). In STEMI patients, we aimed to characterize RNM-LS by TT-CMR and to assess both its dynamics and its structural and prognostic implications. Methods We recruited 271 patients with a first STEMI studied with TT-CMR 1 week after infarction. Of these patients, 145 underwent 1-week and 6-month TT-CMR and were used to characterize both the dynamics and the short-term and long-term structural implications of RNM-LS. Based on previously validated data, RNM areas were defined depending on the culprit coronary artery. Results Reduced RNM-LS at 1 week (n = 70, 48%) was associated with larger infarct size and more depressed left ventricular ejection fraction (LVEF) at both the 1-week and 6-month TT-CMR (p value < 0.001). Late normalization of RNM-LS was frequent (28/70, 40%) and independently related to late recovery of LVEF (p value = 0.002). Patients with reduced RNM-LS at 1-week TT-CMR had more major adverse cardiac events (death, heart failure or re-infarction) in both the 271 patients included in the study group (26% vs. 11%, p value = 0.002) and in an external validation cohort made up of 177 STEMI patients (57% vs. 13%, p value < 0.001). Conclusion After STEMI, reduced RNM-LS by TT-CMR is common and is associated with more severe short- and long-term structural damage. There is a beneficial tendency towards recovery of RNM-LS that parallels late recovery of LVEF.
