Aspectos clĂnicos e diagnĂłsticos da microsporidiose intestinal em pacientes com infecção pelo HIV e diarrĂ©ia crĂŽnica, no Rio de Janeiro, Brasil

Samples examined using transmission electron microscopy (n=3) or polymerase chain reaction (n=6) confirmed Enterocytozoon bieneusi as the causative agent. Microsporidia were the only potential enteric pathogens found in 5 of the 11 patients. Other pathogens were also detected in the intestinal tract of 21 patients, but diarrhea remained unexplained in 8. We concluded that microsporidial infection is frequently found in HIV infected persons in Rio de Janeiro, and it seems to be a marker of advanced stage of AIDS.Os objetivos deste estudo foram determinar a prevalĂȘncia e o prognĂłstico clĂnico da infecção por microsporĂdios em uma coorte de 40 pacientes com infecção pelo HIV e diarrĂ©ia crĂŽnica na cidade do Rio de Janeiro, Brasil. Cada paciente teve suas fezes e fragmentos de intestino examinados para a pesquisa de CMV, bactĂ©rias e parasitos. A prevalĂȘncia de microsporidiose foi de 27,5% (n=11). Esporos de microsporĂdios foram encontrados com maior frequĂȘncia no exame direto das fezes do que em biĂłpsias de intestino delgado. A microscopia eletrĂŽnica de transmissĂŁo e a reação de polimerase em cadeia (PCR) identificaram Enterocytozoon bieneusi, respectivamente, em 3 e 6 amostras examinadas, confirmando a espĂ©cie como Ășnico agente causal. Nenhum outro microrganismo patogĂȘnico, alĂ©m dos microsporĂdios, foi detectada em 5 dos pacientes com diarrĂ©ia. Outros parasitos foram encontrados no trato digestivo de 21 pacientes, enquanto que em 8 a etiologia da diarrĂ©ia nĂŁo foi definida. ConcluĂmos que a infecção por microsporĂdios pode ser freqĂŒentemente encontrada em indivĂduos infectados pelo HIV no Rio de Janeiro
The germline mutational landscape of BRCA1 and BRCA2 in Brazil
The detection of germline mutations in BRCA1 and BRCA2 is essential to the formulation of clinical management strategies, and in Brazil, there is limited access to these services, mainly due to the costs/availability of genetic testing. Aiming at the identification of recurrent mutations that could be included in a low-cost mutation panel, used as a first screening approach, we compiled the testing reports of 649 probands with pathogenic/likely pathogenic variants referred to 28 public and private health care centers distributed across 11 Brazilian States. Overall, 126 and 103 distinct mutations were identified in BRCA1 and BRCA2, respectively. Twenty-six novel variants were reported from both genes, and BRCA2 showed higher mutational heterogeneity. Some recurrent mutations were reported exclusively in certain geographic regions, suggesting a founder effect. Our findings confirm that there is significant molecular heterogeneity in these genes among Brazilian carriers, while also suggesting that this heterogeneity precludes the use of screening protocols that include recurrent mutation testing only. This is the first study to show that profiles of recurrent mutations may be unique to different Brazilian regions. Dislipidemia em pacientes HIV/AIDS em uso de anti-retrovirais num hospital universitĂĄrio, Rio de Janeiro, Brasil Dyslipidemia in HIV/AIDS patients in antiretroviral therapy in a university hospital, Rio de Janeiro, Brazil
INTRODUĂĂO: O uso contĂnuo da terapĂȘutica anti-retroviral (TARV) estĂĄ associado Ă ocorrĂȘncia de diversos distĂșrbios metabĂłlicos. OBJETIVO: O principal objetivo deste estudo foi determinar a prevalĂȘncia de dislipidemia em pacientes com vĂrus da imunodeficiĂȘncia humana/sĂndrome de imunodeficiĂȘncia adquirida (HIV/AIDS) atendidos no ambulatĂłrio de doenças infecciosas do Hospital UniversitĂĄrio Pedro Ernesto da Universidade do Estado do Rio de Janeiro (HUPE/UERJ). MATERIAL E MĂTODOS: No perĂodo de 1/10/2004 a 30/5/2005, os participantes do estudo responderam a uma entrevista sobre dados demogrĂĄficos e relativos Ă saĂșde. ApĂłs a entrevista foram verificadas as seguintes medidas: peso, altura, cintura e quadril. Foi coletado sangue para realização de dosagem plasmĂĄtica de colesterol total (CT), colesterol da lipoproteĂna de alta densidade (HDL-C) e triglicerĂdeos (TG). Dos 268 pacientes que compareceram Ă consulta no perĂodo de estudo, 23 nĂŁo se apresentaram para a coleta de sangue e 10 nĂŁo quiseram participar. O modelo de regressĂŁo de Poisson foi empregado para encontrar variĂĄveis associadas Ă dislipidemia. RESULTADOS: Foram incluĂdos 235 pacientes, dos quais 182 (77,5%) tinham dislipidemia; houve prevalĂȘncia maior no sexo masculino (69,8%) do que no feminino (30,2%); e 26,9% tinham antecedentes familiares de dislipidemia contra 15,1% sem este antecedente. Para o tempo de uso de TARV, tanto a mĂ©dia quanto a mediana foram maiores no grupo de pacientes com dislipidemia. CONCLUSĂO: Em nosso estudo, a prevalĂȘncia de dislipidemia em portadores de HIV/AIDS foi alta (77,5%) e foram identificados sexo masculino, histĂłria familiar de dislipidemia e tempo de uso de TARV como fatores associados.<br>BACKGROUND: The continuous use of antiretroviral therapy (ART) is associated with several metabolic disturbances. OBJECTIVES: The main objective of this study was to determine the prevalence of dyslipidemia in human immunodeficiency virus/acquired immunological deficiency syndrome (HIV/AIDS) patients followed in the infectious diseases outpatient clinic of Hospital UniversitĂĄrio Pedro Ernesto, of Universidade do Estado do Rio de Janeiro (HUPE/UERJ). MATERIAL AND METHODS: From 10/1/2004 to 5/30/2005, the participants of this study answered a survey about demographic and health data. Afterwards, the following measurements were checked: weight, height, waist and hips. Blood samples were collected for total cholesterol, high-density lipoprotein cholesterol (HDL-C) and triglycerides tests. Out of 268 patients who came to the appointment during the study period, 23 did not attend the blood collection, and 10 did not want to participate. Poisson regression model was used to find the variables associated with dyslipidemia. RESULTS: Out of the 235 patients included in the study 182 (77.5%) had dyslipidemia, with prevalence of male (69.8%) over female ones (30.2%). Among the patients with dyslipidemia, 26.9% had family history of dyslipidemia against 15.1% who did not. Regarding ART duration, both mean and media were higher in the group of patients with dyslipidemia. CONCLUSION: In our study the prevalence of dyslipidemia in HIV/AIDS patients was high (77.5%), and the associated factors were male sex, family history of dyslipidemia and ART duration
ResistĂȘncia aos medicamentos anti-tuberculose de cepas de Mycobacterium tuberculosis isoladas de pacientes atendidos em hospital geral de referĂȘncia para tratamento de AIDS no Rio de Janeiro
INTRODUĂĂO: A tuberculose tem trazido importantes desafios ao setor de saĂșde. O Brasil tem alta prevalĂȘncia da doença e o Estado do Rio de Janeiro possui as maiores taxas de incidĂȘncia do paĂs. O municĂpio do Rio de Janeiro Ă© responsĂĄvel pela maior notificação do estado e cerca de 30% dos casos diagnosticados tĂȘm origem em hospitais. OBJETIVO: Estimar a prevalĂȘncia de resistĂȘncia inicial e adquirida e identificar fatores associados Ă ocorrĂȘncia de resistĂȘncia em hospital geral referĂȘncia para tratamento de AIDS no Rio de Janeiro. MĂTODO: Foram analisadas cepas de Mycobacterium tuberculosis de 165 pacientes, entre agosto de 1.996 e fevereiro de 1.998. RESULTADOS: Vinte por cento (33/165) apresentaram resistĂȘncia a pelo menos um medicamento: 13% (22/165) Ă isoniazida, 1,8% (3/165) Ă rifampicina e 3,6% (6/165) a ambas. Entre pacientes infectados pelo HIV (52/165), 28,85% (15/52) apresentaram resistĂȘncia a pelo menos um fĂĄrmaco. ResistĂȘncia adquirida ocorreu em 12,5% dos pacientes que referiram tratamento anterior (3/24), com associação significativa com imagem radiolĂłgica nĂŁo cavitĂĄria (p = 0,05). ResistĂȘncia inicial ocorreu em 18,4% dos pacientes virgens de tratamento (26/141), com associação significativa com ser profissional de saĂșde (p = 0,004), desemprego (p = 0,03) e diarrĂ©ia (p = 0,01) na anĂĄlise bivariada. Na anĂĄlise multivariada, ser profissional de saĂșde manteve-se fortemente associado com a ocorrĂȘncia de resistĂȘncia inicial (p = 0,002). CONCLUSĂO: Taxas de resistĂȘncia elevadas foram encontradas nesta sĂ©rie. Estes achados corroboram a necessidade de que atenção maior seja dada Ă tuberculose em ambiente hospitalar, em especial no que diz respeito Ă preocupante associação entre resistĂȘncia nas cepas isoladas e profissionais de saĂșde
