    ASSESSMENT OF PREVALENCE OF DEPRESSION IN CARDIAC PATIENTS AND ITS ASSOCIATION WITH THE USE OF BETA-BLOCKERS AND STATINS

    Objective: The objective of the study is to assess the prevalence of depression among patients with cardiovascular disease and its association with the use of β-blockers and statins.Methods: This is a prospective observational study conducted at a corporate hospital, Hyderabad, Telangana, India, for a period of 6 months. 250 cardiac patients above 16 years are included in the study. The required data are collected from the patients through direct interview using standard questionnaires and also from patients' respective case sheets. The acquired data are evaluated based on the standard questionnaires Patient Health Questionnaire-9 (PHQ-9) and Beck Depression Inventory-II (BDI-II) scales; used to diagnose the severity of depression in cardiac patients.Results: Prevalence of minor to major depressive symptoms according to BDI-II was found to be 17.2%. Prevalence of minor to major depressive symptoms according to PHQ-9 was found to be 19.2%. Among male patients, 13% showed depressive symptoms, whereas among female patients 25% showed depressive symptoms. Among the patients coadministering beta-blockers and statins, 15% were depressed according to BDI-II, and 16% were depressed according to PHQ-9 at visit. After 1 month (first follow-up), the percentage increased by 8% (for BDI-II)-12% (for PHQ-9) and remains almost the same at the second follow-up. As per BDI-II and PHQ-9 scores, the percentage of patients with minor to major depression among the patients using only beta-blockers decreased significantly from the time of visit to the second follow-up. The percentage of patients with minor to major depression among the patients using only statins increased significantly from the time of visit to second follow-up.Conclusion: Prevalence of minor to major depression according to BDI-II was found to be 17%, whereas according to PHQ-9, it was found to be 20% in patients with cardiovascular disease. Cardiovascular diseases have been more prevalent in men than in women, whereas depressive symptoms have been more prevalent in women than in men. Patients using only β-blockers showed a decrease in symptoms of depression. Whereas statins have shown to increase the chances of depression slightly which is often negligible, atorvastatin was associated with a higher level of depression when compared to rosuvastatin. Controversies still exist that statins decrease risk of depression.Â

    nocaps: novel object captioning at scale

    Image captioning models have achieved impressive results on datasets containing limited visual concepts and large amounts of paired image-caption training data. However, if these models are to ever function in the wild, a much larger variety of visual concepts must be learned, ideally from less supervision. To encourage the development of image captioning models that can learn visual concepts from alternative data sources, such as object detection datasets, we present the first large-scale benchmark for this task. Dubbed 'nocaps', for novel object captioning at scale, our benchmark consists of 166,100 human-generated captions describing 15,100 images from the OpenImages validation and test sets. The associated training data consists of COCO image-caption pairs, plus OpenImages image-level labels and object bounding boxes. Since OpenImages contains many more classes than COCO, nearly 400 object classes seen in test images have no or very few associated training captions (hence, nocaps). We extend existing novel object captioning models to establish strong baselines for this benchmark and provide analysis to guide future work on this task

    Establishment of reference CD4+ T cell values for adult Indian population

    <p>Abstract</p> <p>Background</p> <p>CD4+ T lymphocyte counts are the most important indicator of disease progression and success of antiretroviral treatment in HIV infection in resource limited settings. The nationwide reference range of CD4+ T lymphocytes was not available in India. This study was conducted to determine reference values of absolute CD4+ T cell counts and percentages for adult Indian population.</p> <p>Methods</p> <p>A multicentric study was conducted involving eight sites across the country. A total of 1206 (approximately 150 per/centre) healthy participants were enrolled in the study. The ratio of male (N = 645) to female (N = 561) of 1.14:1. The healthy status of the participants was assessed by a pre-decided questionnaire. At all centers the CD4+ T cell count, percentages and absolute CD3+ T cell count and percentages were estimated using a single platform strategy and lyse no wash technique. The data was analyzed using the Statistical Package for the Social Scientist (SPSS), version 15) and Prism software version 5.</p> <p>Results</p> <p>The absolute CD4+ T cell counts and percentages in female participants were significantly higher than the values obtained in male participants indicating the true difference in the CD4+ T cell subsets. The reference range for absolute CD4 count for Indian male population was 381-1565 cells/μL and for female population was 447-1846 cells/μL. The reference range for CD4% was 25-49% for male and 27-54% for female population. The reference values for CD3 counts were 776-2785 cells/μL for Indian male population and 826-2997 cells/μL for female population.</p> <p>Conclusion</p> <p>The study used stringent procedures for controlling the technical variation in the CD4 counts across the sites and thus could establish the robust national reference ranges for CD4 counts and percentages. These ranges will be helpful in staging the disease progression and monitoring antiretroviral therapy in HIV infection in India.</p

    Long Term Running Biphasically Improves Methylglyoxal-Related Metabolism, Redox Homeostasis and Neurotrophic Support within Adult Mouse Brain Cortex

    Oxidative stress and neurotrophic support decline seem to be crucially involved in brain aging. Emerging evidences indicate the pro-oxidant methylglyoxal (MG) as a key player in the age-related dicarbonyl stress and molecular damage within the central nervous system. Although exercise promotes the overproduction of reactive oxygen species, habitual exercise may retard cellular aging and reduce the age-dependent cognitive decline through hormetic adaptations, yet molecular mechanisms underlying beneficial effects of exercise are still largely unclear. In particular, whereas adaptive responses induced by exercise initiated in youth have been broadly investigated, the effects of chronic and moderate exercise begun in adult age on biochemical hallmarks of very early senescence in mammal brains have not been extensively studied. This research investigated whether a long-term, forced and moderate running initiated in adult age may affect the interplay between the redox-related profile and the oxidative-/MG-dependent molecular damage patterns in CD1 female mice cortices; as well, we investigated possible exercise-induced effects on the activity of the brain derived neurotrophic factor (BDNF)-dependent pathway. Our findings suggested that after a transient imbalance in almost all parameters investigated, the lately-initiated exercise regimen strongly reduced molecular damage profiles in brains of adult mice, by enhancing activities of the main ROS- and MG-targeting scavenging systems, as well as by preserving the BDNF-dependent signaling through the transition from adult to middle age

    SARS-CoV-2 B.1.617.2 Delta variant replication and immune evasion

    The B.1.617.2 (Delta) variant of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) was first identified in the state of Maharashtra in late 2020 and spread throughout India, outcompeting pre-existing lineages including B.1.617.1 (Kappa) and B.1.1.7 (Alpha)1. In vitro, B.1.617.2 is sixfold less sensitive to serum neutralizing antibodies from recovered individuals, and eightfold less sensitive to vaccine-elicited antibodies, compared with wild-type Wuhan-1 bearing D614G. Serum neutralizing titres against B.1.617.2 were lower in ChAdOx1 vaccinees than in BNT162b2 vaccinees. B.1.617.2 spike pseudotyped viruses exhibited compromised sensitivity to monoclonal antibodies to the receptor-binding domain and the amino-terminal domain. B.1.617.2 demonstrated higher replication efficiency than B.1.1.7 in both airway organoid and human airway epithelial systems, associated with B.1.617.2 spike being in a predominantly cleaved state compared with B.1.1.7 spike. The B.1.617.2 spike protein was able to mediate highly efficient syncytium formation that was less sensitive to inhibition by neutralizing antibody, compared with that of wild-type spike. We also observed that B.1.617.2 had higher replication and spike-mediated entry than B.1.617.1, potentially explaining the B.1.617.2 dominance. In an analysis of more than 130 SARS-CoV-2-infected health care workers across three centres in India during a period of mixed lineage circulation, we observed reduced ChAdOx1 vaccine effectiveness against B.1.617.2 relative to non-B.1.617.2, with the caveat of possible residual confounding. Compromised vaccine efficacy against the highly fit and immune-evasive B.1.617.2 Delta variant warrants continued infection control measures in the post-vaccination era
