    Gastric carcinoma in Durban's Indian population

    Histologlcally proven gastric carcinoma was studied to establish the incidence and pattern of the disease In the Indian population of Durban. The Incidence In this population wasfound to be low, 6,9/100000/year. Over a 7-year period (1980 - 1986) 115 patients were treated for gastric carcinoma at R. K. Khan Hospital. There was a male preponderance, and the average age at presentation was 56 years. The commonest presenting symptoms were dyspepsia and vomiting, and the majority of patients presented with advanced disease. Only a third underwent resection, a third had no treatment, and a third underwent palliative bypass or laparotomy only. The majority of patients who had a palliative bypass or no treatment died within 9 months. The 5-year survival rate for patients undergoing curative resection was 38% and for palliative resection 9%. To Improve survival, emphasis must be on early diagnosis and it is recommended that any patient with dyspepsia who is over the age of 30 years should have an endoscopic investigation

    Assessment patient satisfaction after total knee replacement in Indian population

    Background: We aim to assess the satisfaction of patients after total Knee replacement in Indian population. Tоtаl knee Аrthrорlаsty is most commonly performed for osteoarthritis, and also for other knee diseases such as rheumatoid arthritis and psoriatic arthritis. Knee reрlасement surgeries helрs tо restоre nоrmаl meсhаniсs during level wаlking аs well аs mоre demanding аnd соmрlex асtivities suсh аs stаir negоtiаtiоn. Раtient sаtisfасtiоn rаtes fоr the рrосedure hаve been reроrted between 81-97%. However, this leаves а signifiсаnt роrtiоn оf the TKR рорulаtiоn аs dissаtisfied with the оutсоmes оf the reрlасed knee. Methods: Patients were assessed at opd visit or with telephonic conversation and scores are calculated and mentioned in excel sheet. Post surgery satisfaction (PSS) score is the score created to measure the satisfaction after total hip replacement. This score contains different questionnaires. These questions include inquiry about relief of pain, ability to do daily routine activities, requirement of walking aids etc. Score contains total 7 questions. Results: Out of 55 patients we found through this study that about 92.7% were classified as satisfied while 7.3% patients were dissatisfied. Satisfaction rate is more as compared to dissatisfaction rate. Conclusions: In present study we have tried to eliminate other factors responsible for dissatisfaction such as hospital ward cleanliness, hospital administration, patient and hospital staff communication. we recommend total knee replacement surgery for end stage arthritis and it is more beneficial to the patients in terms of improvement in post-operative pain and function

    A Singular Perturbation Analysis for \\Unstable Systems with Convective Nonlinearity

    We use a singular perturbation method to study the interface dynamics of a non-conserved order parameter (NCOP) system, of the reaction-diffusion type, for the case where an external bias field or convection is present. We find that this method, developed by Kawasaki, Yalabik and Gunton for the time-dependant Ginzburg-Landau equation and used successfully on other NCOP systems, breaks down for our system when the strength of bias/convection gets large enough.Comment: 5 pages, PostScript forma

    Radiological and functional outcome of conservatively managed middle one third clavicle fractures

    Background: We aim to assess radiological and functional outcome of conservatively managed middle one third clavicle fractures in Indian population. Clavicle fractures represent 2% to 10% of all fractures. In middle third clavicle fractures, conservative management is the commonest approach. In uncomplicated non displaced midshaft fractures, patients treated non operatively with these conservative measures have fewer complications and a timely recovery as those treated operatively. Conservative management is a simpler yet effective mode of management in middle one third clavicle fractures. Methods: Patients managed conservatively for clavicle fractures were assessed on OPD basis with follow up chest X-ray with bilateral shoulder AP view for radiological signs of union and assessing percent shortening and two questionnaires. Constant shoulder score which assess pain, degree of function, range of motion and muscular force and, simple shoulder test, self administered questionnaire defining one or more affirmative answers to find the satisfaction after treatment modality which were tabulated in Microsoft excel sheets. Results: Out of the 55 patients enrolled in the study 52 showed signs of union at 6 months follow up with mean fracture union time of 13.9 weeks. At 6 months follow up Mean Constant Shoulder score was 87.1 with excellent outcome in 67.67 percent patients and using Simple Shoulder Test satisfaction rate was found to be 78.18 percent. Conclusions: Solid evidence in favour of non-operative treatment for fractures with remaining contact of the bone fragments. Non-surgical management of middle third clavicle fractures yield excellent results. Clear indications for conservative treatment versus surgical fixation of displaced midshaft fractures have not finally been established yet. We recommend conservative management for uncomplicated middle one third clavicle fractures

    Earth‐Moon‐Mars Radiation Environment Module framework

    [1] We are preparing to return humans to the Moon and setting the stage for exploration to Mars and beyond. However, it is unclear if long missions outside of low-Earth orbit can be accomplished with acceptable risk. The central objective of a new modeling project, the Earth-Moon-Mars Radiation Exposure Module (EMMREM), is to develop and validate a numerical module for characterizing time-dependent radiation exposure in the Earth-Moon-Mars and interplanetary space environments. EMMREM is being designed for broad use by researchers to predict radiation exposure by integrating over almost any incident particle distribution from interplanetary space. We detail here the overall structure of the EMMREM module and study the dose histories of the 2003 Halloween storm event and a June 2004 event. We show both the event histories measured at 1 AU and the evolution of these events at observer locations beyond 1 AU. The results are compared to observations at Ulysses. The model allows us to predict how the radiation environment evolves with radial distance from the Sun. The model comparison also suggests areas in which our understanding of the physics of particle propagation and energization needs to be improved to better forecast the radiation environment. Thus, we introduce the suite of EMMREM tools, which will be used to improve risk assessment models so that future human exploration missions can be adequately planned for

    Edge and Mean Based Image Compression

    In this paper, we present a static image compression algorithm for very low bit rate applications. The algorithm reduces spatial redundancy present in images by extracting and encoding edge and mean information. Since the human visual system is highly sensitive to edges, an edge-based compression scheme can produce intelligible images at high compression ratios. We present good quality results for facial as well as textured, 256~x~256 color images at 0.1 to 0.3 bpp. The algorithm described in this paper was designed for high performance, keeping hardware implementation issues in mind. In the next phase of the project, which is currently underway, this algorithm will be implemented in hardware, and new edge-based color image sequence compression algorithms will be developed to achieve compression ratios of over 100, i.e., less than 0.12 bpp from 12 bpp. Potential applications include low power, portable video telephones

    Emerging methods in therapeutics using multifunctional nanoparticles

    Clinical translation of nanoparticle‐based drug delivery systems is hindered by an array of challenges including poor circulation time and limited targeting. Novel approaches including designing multifunctional particles, cell‐mediated delivery systems, and fabrications of protein‐based nanoparticles have gained attention to provide new perspectives to current drug delivery obstacles in the interdisciplinary field of nanomedicine. Collectively, these nanoparticle devices are currently being investigated for applications spanning from drug delivery and cancer therapy to medical imaging and immunotherapy.     Valsartan for attenuating disease evolution in early sarcomeric hypertrophic cardiomyopathy: the design of the Valsartan for Attenuating Disease Evolution in Early Sarcomeric Hypertrophic Cardiomyopathy (VANISH) trial

    Background: Hypertrophic cardiomyopathy (HCM) is often caused by sarcomere gene mutations, resulting in left ventricular hypertrophy (LVH), myocardial fibrosis, and increased risk of sudden cardiac death and heart failure. Studies in mouse models of sarcomeric HCM demonstrated that early treatment with an angiotensin receptor blocker (ARB) reduced development of LVH and fibrosis. In contrast, prior human studies using ARBs for HCM have targeted heterogeneous adult cohorts with well-established disease. The VANISH trial is testing the safety and feasibility of disease-modifying therapy with an ARB in genotyped HCM patients with early disease. Methods: A randomized, placebo-controlled, double-blind clinical trial is being conducted in sarcomere mutation carriers, 8 to 45 years old, with HCM and no/minimal symptoms, or those with early phenotypic manifestations but no LVH. Participants are randomly assigned to receive valsartan 80 to 320 mg daily (depending on age and weight) or placebo. The primary endpoint is a composite of 9 z-scores in domains representing myocardial injury/hemodynamic stress, cardiac morphology, and function. Total z-scores reflecting change from baseline to final visits will be compared between treatment groups. Secondary endpoints will assess the impact of treatment on mutation carriers without LVH, and analyze the influence of age, sex, and genotype. Conclusions: The VANISH trial is testing a new strategy of disease modification for treating sarcomere mutation carriers with early HCM, and those at risk for its development. In addition, further insight into disease mechanisms, response to therapy, and phenotypic evolution will be gained
