    Predictors of child stunting in Fitche town, North Shewa Zone, Ethiopia

    Stunting is a well-known child health indicator for chronic malnutrition. The nutritional status of children is a good gauge for the nutritional well- being of the population, and it is, nowadays, one of the key issues in interrupting generational poverty transmissions as they bear the brunt of poverty. There has been scarcity of disaggregated survey to explore covariates of child stunting. This study attempted to identify socio-economic,demographic and other proximate factors associated with stunting of children under the age of five in Fitche town. The survey was cross- sectional, conducted using multistage sampling procedures to collect information from 160 children and caregivers. The anthropometric, descriptive and logistic analyses to determine the prevalence and identify covariates of child stunting were applied using Epi info and STATA software. Logistic analysis of the effects of selected child and maternal characteristics, child caring practices, and environmental health conditions on child stunting was done. Therefore, birth order, family size, maternal education, remittances, farm income, income from women employment and size of cultivated land were found to be the strongest predictors. However, the estimated coefficients of vaccine status of the child, Low Birth Weight, latrine facility in the house, morbidity status of the child, child birth interval, distance to  the nearest health center, antenatal care visit of the mother and household’s source of water indicated theoretically inconsistent and statistically insignificant effect on the likelihood of child stunting. The prevalence of child stunting was 45%, which is significantly higher than the regional and national averages. Among children who were stunted, 11.9 % were moderately stunted compared to 33.1% who were severely stunted. The proportion of stunted children for male was greater than female. There is a need to increase maternity care and education as this would make them amenable to health interventions that help improve not only their nutritional status and that of their child but also improve their income earning potential and food purchasing power. Addressing poverty may lead to improvements in the nutrition status of children as household economic status significantly determines nutritional status in the town.     Forecasting malaria incidence from historical morbidity patterns in epidemic-prone areas of Ethiopia: a simple seasonal adjustment method performs best.

    The aim of this study was to assess the accuracy of different methods of forecasting malaria incidence from historical morbidity patterns in areas with unstable transmission. We tested five methods using incidence data reported from health facilities in 20 areas in central and north-western Ethiopia. The accuracy of each method was determined by calculating errors resulting from the difference between observed incidence and corresponding forecasts obtained for prediction intervals of up to 12 months. Simple seasonal adjustment methods outperformed a statistically more advanced autoregressive integrated moving average method. In particular, a seasonal adjustment method that uses mean deviation of the last three observations from expected seasonal values consistently produced the best forecasts. Using 3 years' observation to generate forecasts with this method gave lower errors than shorter or longer periods. Incidence during the rainy months of June-August was the most predictable with this method. Forecasts for the normally dry months, particularly December-February, were less accurate. The study shows the limitations of forecasting incidence from historical morbidity patterns alone, and indicates the need for improved epidemic early warning by incorporating external predictors such as meteorological factors

    The Concept of Planetary Boundaries for Sustainable Development: A Review

    The concept of the planetary boundaries (PB) is a reasonable scientific approach towards achieving the sustainable development goals (SDGs). It started in 2009 with a group of scientists at the Stockholm Resilience Center (SRC) in Sweden with the aim of showing a safe operating space for human development and wellbeing through scientific research by quantifying what they refer to as “planetary boundaries”. But in other for the concept of the PB to thoroughly address the matters of the SDGs which does not only include environmental sustainability but also social issues, human prosperity, and governance, the concept of the PB has in combination with social boundaries and governance approaches created a sphere of reasonable ideas towards achieving the SDGs. The sustainable development trajectories of the PB comprising of six major structural transformations ensures that the world continues to develop economically while staying within planetary boundaries. Therefore, it has to take the cooperative efforts of all nations in the world to tackle, and to mitigate the overwhelming effects we have caused on our “good planet”. Humans should realize that our planet has tried in all its “resilient capabilities “to condone all our exploitations, but it seems we have squeezed it too much and it is deflating. We need to build resilience to the unexpected (resilience thinking); we need to be prepared for the unexpected, both to be able to cope with “shocks” and to take advantage of “potentially positive surprises.” This review tries to put together the different research ideas towards achieving the aim of the concept of the PB.     Expanding the Repertoire of Carbapenem-Hydrolyzing Metallo-ß-Lactamases by Functional Metagenomic Analysis of Soil Microbiota

    Carbapenemases are bacterial enzymes that hydrolyze carbapenems, a group of last-resort β-lactam antibiotics used for treatment of severe bacterial infections. They belong to three β-lactamase classes based amino acid sequence (A, B, and D). The aim of this study was to elucidate occurrence, diversity and functionality of carbapenemase-encoding genes in soil microbiota by functional metagenomics. Ten plasmid libraries were generated by cloning metagenomic DNA from agricultural (n = 6) and grassland (n = 4) soil into Escherichia coli. The libraries were cultured on amoxicillin-containing agar and up to 100 colonies per library were screened for carbapenemase production by CarbaNP test. Presumptive carbapenemases were characterized with regard to DNA sequence, minimum inhibitory concentration (MIC) of β-lactams, and imipenem hydrolysis. Nine distinct class B carbapenemases, also known as metallo-beta-lactamases (MBLs), were identified in six soil samples, including two subclass B1 (GRD23-1 and SPN79-1) and seven subclass B3 (CRD3-1, PEDO-1, GRD33-1, ESP-2, ALG6-1, ALG11-1, and DHT2-1). Except PEDO-1 and ESP-2, these enzymes were distantly related to any previously described MBLs (33 to 59% identity). RAIphy analysis indicated that six enzymes (CRD3-1, GRD23-1, DHT2-1, SPN79-1, ALG6-1, and ALG11-1) originated from Proteobacteria, two (PEDO-1 and ESP-2) from Bacteroidetes and one (GRD33-1) from Gemmatimonadetes. All MBLs detected in soil microbiota were functional when expressed in E. coli, resulting in detectable imipenem-hydrolyzing activity and significantly increased MICs of clinically relevant β-lactams. Interestingly, the MBLs yielded by functional metagenomics generally differed from those detected in the same soil samples by antibiotic selective culture, showing that the two approaches targeted different subpopulations in soil microbiota.     Nuclear Localized LSR: A Novel Regulator of Breast Cancer Behavior and Tumorigenesis

    Lipolysis Stimulated Lipoprotein Receptor (LSR) has been found in the plasma membrane and is believed to function in lipoprotein endocytosis and tight junctions. Given the impact of cellular metabolism and junction signaling pathways on tumor phenotypes and patient outcome, it is important to understand how LSR cellular localization mediates its functions. We conducted localization studies, evaluated DNA binding, and examined the effects of nuclear LSR in cells, xenografts, and clinical specimens. We found LSR within the membrane, cytoplasm, and the nucleus of breast cancer cells representing multiple intrinsic subtypes. Chromatin immunoprecipitation (ChIP) showed direct binding of LSR to DNA, and sequence analysis identified putative functional motifs and post-translational modifications of the LSR protein. While neither overexpression of transcript variants, nor pharmacological manipulation of post-translational modification significantly altered localization, inhibition of nuclear export enhanced nuclear localization, suggesting a mechanism for nuclear retention. Co-immunoprecipitation and proximal ligation assays indicated LSR-pericentrin interactions, presenting potential mechanisms for nuclear-localized LSR. The clinical significance of LSR was evaluated using data from over 1,100 primary breast tumors, which showed high LSR levels in basal-like tumors and tumors from African-Americans. In tumors histosections, nuclear localization was significantly associated with poor outcomes. Finally, in vivo xenograft studies revealed that basal-like breast cancer cells that over-express LSR exhibited both membrane and nuclear localization, and developed tumors with 100% penetrance, while control cells lacking LSR developed no tumors. These results show that nuclear LSR alters gene expression and may promote aggressive cancer phenotypes

    An epigenome-wide association study of child appetitive traits and DNA methylation

    The etiology of childhood appetitive traits is poorly understood. Early-life epigenetic processes may be involved in the developmental programming of appetite regulation in childhood. One such process is DNA methylation (DNAm), whereby a methyl group is added to a specific part of DNA, where a cytosine base is next to a guanine base, a CpG site. We meta-analyzed epigenome-wide association studies (EWASs) of cord blood DNAm and early-childhood appetitive traits. Data were from two independent cohorts: the Generation R Study (n = 1,086, Rotterdam, the Netherlands) and the Healthy Start study (n = 236, Colorado, USA). DNAm at autosomal methylation sites in cord blood was measured using the Illumina Infinium HumanMethylation450 BeadChip. Parents reported on their child's food responsiveness, emotional undereating, satiety responsiveness and food fussiness using the Children's Eating Behaviour Questionnaire at age 4–5 years. Multiple regression models were used to examine the association of DNAm (predictor) at the individual site- and regional-level (using DMRff) with each appetitive trait (outcome), adjusting for covariates. Bonferroni-correction was applied to adjust for multiple testing. There were no associations of DNAm and any appetitive trait when examining individual CpG-sites. However, when examining multiple CpGs jointly in so-called differentially methylated regions, we identified 45 associations of DNAm with food responsiveness, 7 associations of DNAm with emotional undereating, 13 associations of DNAm with satiety responsiveness, and 9 associations of DNAm with food fussiness. This study shows that DNAm in the newborn may partially explain variation in appetitive traits expressed in early childhood and provides preliminary support for early programming of child appetitive traits through DNAm. Investigating differential DNAm associated with appetitive traits could be an important first step in identifying biological pathways underlying the development of these behaviors.</p

    Sustainable management practices for durum wheat production: Analyzing specific agronomic interventions on productivity, grain micronutrient content, and quality

    As compared with single agronomic crop management practices during grain formation, knowledge about integrated agronomic management practices on grain mineral composition and grain technological properties in durum wheat is limited. This knowledge is important for determining management strategies aimed at increasing grain yield without affecting grain nutritional quality. Integrated agronomic practices such as foliar nutrient application × seeding rate × varieties combined with growing locations were investigated to evaluate the dynamics of yield and grain quality traits. Two durum wheat varieties, three-level of micronutrients (i.e. control, FeSO4, and ZnSO4), and four levels of seeding rate (i.e. 100, 125, 150, and 175 kg ha−1) were arranged in split-split plot design under two different growing locations (environments). The main plots were assigned to the varieties, subplots to micronutrients, and sub-sub plots to the seeding rate treatments. Zinc and iron were applied in a form of ZnSO4 and FeSO4 at the early flowering stage, both at a rate of 25 kg ha−1. Results showed a linear increment in biomass (21.5%) and grain yield (23.5%) under a high seeding rate, even though the 1000-grain weight, the number of grains spike−1, spike length, and the number of grains spike−1 were decreased. Higher varietal and environmental response of seeding rate was observed between varieties. The grain protein content, gluten, and zeleyn index decreased as the seeding rate increased. Grain micronutrient content was significantly influenced by seeding rate and varietal difference. The grain protein content was higher in a dryland environment than in a wet environment. A combined use of density-tolerant varieties, high seeding rate, and foliar-based iron application can improve the grain yield from 2.01 to 3.20 t ha−1 under a potential environment. Hence, all stakeholders should consider the genotype (G), environment (E), management (M), and their synergies, as far as grain yield and quality are considered simultaneously

    Insecticide resistance in Anopheles arabiensis from Ethiopia (2012-2016): a nationwide study for insecticide resistance monitoring.

    BACKGROUND: Indoor residual spraying (IRS) and long-lasting insecticidal nets (LLINs) remain the cornerstones of malaria vector control. However, the development of insecticide resistance and its implications for operational failure of preventative strategies are of concern. The aim of this study was to characterize insecticide resistance among Anopheles arabiensis populations in Ethiopia and describe temporal and spatial patterns of resistance between 2012 and 2016. METHODS: Between 2012 and 2016, resistance status of An. arabiensis was assessed annually during the long rainy seasons in study sites from seven of the nine regions in Ethiopia. Insecticide resistance levels were measured with WHO susceptibility tests and CDC bottle bioassays using insecticides from four chemical classes (organochlorines, pyrethroids, organophosphates and carbamates), with minor variations in insecticides tested and assays conducted between years. In selected sites, CDC synergist assays were performed by pre-exposing mosquitoes to piperonyl butoxide (PBO). In 2015 and 2016, mosquitoes from DDT and deltamethrin bioassays were randomly selected, identified to species-level and screened for knockdown resistance (kdr) by PCR. RESULTS: Intense resistance to DDT and pyrethroids was pervasive across Ethiopia, consistent with historic use of DDT for IRS and concomitant increases in insecticide-treated net coverage over the last 15 years. Longitudinal resistance trends to malathion, bendiocarb, propoxur and pirimiphos-methyl corresponded to shifts in the national insecticide policy. By 2016, resistance to the latter two insecticides had emerged, with the potential to jeopardize future long-term effectiveness of vector control activities in these areas. Between 2015 and 2016, the West African (L1014F) kdr allele was detected in 74.1% (n = 686/926) of specimens, with frequencies ranging from 31 to 100% and 33 to 100% in survivors from DDT and deltamethrin bioassays, respectively. Restoration of mosquito susceptibility, following pre-exposure to PBO, along with a lack of association between kdr allele frequency and An. arabiensis mortality rate, both indicate metabolic and target-site mutation mechanisms are contributing to insecticide resistance. CONCLUSIONS: Data generated by this study will strengthen the National Malaria Control Programme's insecticide resistance management strategy to safeguard continued efficacy of IRS and other malaria control methods in Ethiopia

    Electroencephalographic Findings, Antiepileptic Drugs and Risk Factors of 433 Individuals Referred to a Tertiary Care Hospital in Ethiopia

    Background: Little is known about the characteristics of electroencephalogram (EEG) findings in epileptic patients in Ethiopia. The objective of this&nbsp; study was to characterize the EEG patterns, indications, antiepileptic drugs (AEDs), and epilepsy risk factors.Methods: A retrospective observational review of EEG test records of 433 patients referred to our electrophysiology unit between July 01, 2020 and&nbsp; December 31, 2021.Results: The age distribution in the study participants was right skewed unipolar age distribution for both sexes and the mean age of 33.8 (SD=15.7) years. Male accounted for 51.7%. Generalized tonic clonic seizure was the most common seizure type. The commonest indication for EEG was&nbsp; abnormal body movement with loss of consciousness (35.2%). Abnormal EEG findings were observed in 55.2%; more than half of them were Interictal epileptiform discharges, followed by focal/or generalized slowing. Phenobarbitone was the commonest AEDs. A quarter (20.1%) of the&nbsp; patients were getting a combination of two AEDs and 5.2% were on 3 different AEDs. Individuals taking the older AEDs and those on 2 or more AEDs&nbsp; tended to have abnormal EEG findings. A cerebrovascular disorder (27.4%) is the prevalent risk factor identified followed by brain tumor, HIV&nbsp; infection, and traumatic head injury respectively.Conclusion: High burden of abnormal EEG findings among epileptic patients referred to our unit. The proportion of abnormal EEG patterns was&nbsp; higher in patients taking older generation AEDs and in those on 2 or more AEDs. Stroke, brain tumor, HIV infection and traumatic head injury were&nbsp; the commonest identified epilepsy risk factors
