ESCADA: Efficient Safety and Context Aware Dose Allocation for Precision Medicine
Finding an optimal individualized treatment regimen is considered one of the
most challenging precision medicine problems. Various patient characteristics
influence the response to the treatment, and hence, there is no
one-size-fits-all regimen. Moreover, the administration of an unsafe dose
during the treatment can have adverse effects on health. Therefore, a treatment
model must ensure patient \emph{safety} while \emph{efficiently} optimizing the
course of therapy. We study a prevalent medical problem where the treatment
aims to keep a physiological variable in a safe range and preferably close to a
target level, which we refer to as \emph{leveling}. Such a task may be relevant
in numerous other domains as well. We propose ESCADA, a novel and generic
multi-armed bandit (MAB) algorithm tailored for the leveling task, to make
safe, personalized, and context-aware dose recommendations. We derive high
probability upper bounds on its cumulative regret and safety guarantees.
Following ESCADA's design, we also describe its Thompson sampling-based
counterpart. We discuss why the straightforward adaptations of the classical
MAB algorithms such as GP-UCB may not be a good fit for the leveling task.
Finally, we make \emph{in silico} experiments on the bolus-insulin dose
allocation problem in type-1 diabetes mellitus disease and compare our
algorithms against the famous GP-UCB algorithm, the rule-based dose
calculators, and a clinician.Comment: 23 pages, 12 figure
Traumatic Optic Neuropathy and Central Retinal Artery Occlusion Following Blunt Ocular Trauma
We present a case as a rare sign of traumatic optic neuropathy and central retinal artery occlusion following blunt ocular trauma. A 10-year-old child suffered complete loss of the vision of one eye following a blunt ocular injury. He sustained an occlusion of the central retinal artery and traumatic optic neurupathy of the affected eye. Isolated cases of central retinal vessel occlusions and traumatic optic neurapathy following ocular blunt trauma are rare conditions. Clinicians to be aware of the potential for blunt ocular trauma to cause optic nerve damage and retinal vessel occlusions
Gebelikte Akut Karın Nedeni Olarak Miyom Torsiyonu
Gebelikte akut karın tablosu, anatominin ve fi zyolojinin değişmesi nedeniyle,
tanı ve tedavi basamaklarında klinik zorluk yaratmaktadır. Belirtiler
değişmekte veya silikleşmekte, laboratuar incelemelerin sonucu gebelikteki
fi zyolojik yanıtlarla karışabilmektedir. Karın ağrısı, obstetrik nedenlerle
olabileceği kadar karın içi ve pelvis içi organlar nedeniyle de gelişebilmektedir.
Leiomiyomlar geç üreme dönenimde sık rastlanan benin jinekolojik tümörlerdir.
Çoğunlukla belirti vermezler. Miyom dejenerasyonu, saplı miyomun
torsiyone olması veya kitle etkisine bağlı uterusun torsiyonuna yol açarak
gebelikte ağrıya yol açabilir. Gebelikte akut karın tanısı ile cerrahi uygulanan
torsiyone miyom olgusu tartışılmıştır.
G1 P0 31 yaşında hasta, yirmi birinci gebelik haftasında karın ağrısı ile başvurdu.
Fizik muayenede batın sağ kadranda defans ve rebaund mevcuttu. Ultrasonografi
de ve Doppler incelemede her iki over normal olarak değerlendirildi.
Akut appendisit ön tanısı ile eksploratif laparotomi uygulandı. İntraoperatif
gözlemde uterus sağ lateral yüzde ince pediküllü subserozal miyomun torsiyone
olduğu tespit edildi. Miyomektomi uygulandı. Gebelik normal seyrinde
takip edildi
Post-traumatic stress disorder after terrorist attack in healthcare professionals
INTRODUCTION: On the date of 15 July 2016 a terrorist organization launched a terrorist attack using helicopters and heavy combat weapons in the city centers of Istanbul and Ankara simultaneously. Numerous civilian were hurt and many of them lost their lives during the attacks. Terrorism is a form of combat designed to cause the highest psychological influence on the masses. Post-traumatic stress disorder (PTSD) is one of the most common psychological disorders after such disasters. The aim of this study is to determine the associated risk factors and PTSD rates in healthcare professionals who were on call during the 15 July 2016 terrorist attacks.
METHOD: Since the hospital is in a neighbouring the street to where terror attacks occurred, all healthcare professionals ≥ 18 years of age who were on duty that night in the Dr. Ridvan Ege Training and Research Hospital and were auditory or visual witnesses of the event were included in the study as the first group while healthcare professionals who work in the same hospital but were not on duty that night were included as the control group.
RESULTS: The mean post-traumatic diagnostic scale stress score of the control group was 11.87. The mean post-traumatic diagnostic scale stress score of the Group 1 was 21.91. There was a significant difference between the groups in terms of posttraumatic diagnostic scale stress score (p < 0.05). While the healthcare professionals on duty on July 15, 2016 (Group 1) had moderate-severe (21.91 ± 5.11) stress disorder, the healthcare professionals who were not at the hospital on July 15, 2016 (Control Group) had moderate stress disorder (11.87 ± 6.86).
CONCLUSION: Our country is at risk from the fact that such attacks may be experienced again because of its unique conditions. The results of our work support the data on the high level of exposure to PTSD when exposed to a terrorist attack. For this reason, the identification of the characteristics of pre-traumatic health individuals at risk is useful in planning the presentation of preventive and curative health services. At the same time, there is a need for longer-term work and wider samples to reveal the psychological consequences of such attacks
Evaluation of applications and services for the elderly during the covid-19 outbreak in Turkey
İnsanlık ortaya çıktığı günden bugüne kadar savaşlar, göçler ve salgınlar gibi birçok felaketle karşılaşmıştır. Bu felaketlerden birisi de hiç şüphesiz Covid-19 salgınıdır. Çin’in Wuhan şehrinde ilk kez karşılaşılan bu yeni tür koronavirüs dünya genelinde milyonlarca kişiyi etkilemiş, bütün ülkelere yayılmış
ve Dünya Sağlık Örgütü (DSÖ) tarafından pandemi ilan edilmiştir. Bütün yaşlardaki insanlar için bu
yeni tip virüsün bulaşma riski olmakla beraber yaşlılarda, yaşlılığa eşlik eden kronik hastalıklar nedeniyle ölüm oranları diğer yaş gruplarına göre daha yüksektir. Yaşlılık döneminde bireyler diğer yaş
gruplarına göre daha fazla fizyolojik, psikolojik, sosyal ve ekonomik sorunlar ile karşı karşıya olabilmektedirler. Yaşlılığın çok boyutlu yapısı ve kendine özgü sorunları ile birleşen Covid-19 salgını, yaşlılara
özel uygulamaların ve yeni yaklaşımların ortaya çıkmasını zorunlu hale getirmiştir. Bu sebeplerle salgının başından itibaren dünya genelinde hükümetler yaşlı gruba yönelik önlemler, kısıtlamalar ve yeni
hizmet modelleri geliştirmişlerdir. Salgın sürecinde Türkiye’nin yaşlılara yönelik yaptığı çalışmaların
ve hizmetlerin incelenmesi ve değerlendirilmesi, gelecekte yaşanabilecek yeni salgınlara hazırlıklı olmak
için önem taşımaktadır. Bu nedenlerle çalışmanın amacı Covid-19 salgını süresince Türkiye’de yaşlılara
yönelik uygulamaların ve hizmetlerin incelenmesi ve değerlendirilmesidir. Araştırmada yaşlılara yönelik uygulamalar; hizmetler, kısıtlamalar, ekonomik destek, izin/ayrıcalıklar ve tıbbi hizmetler boyutlarında ele alınmıştır. Sonuç olarak, ele alınan boyutlarda yaşlıların psiko-sosyal ve ekonomik açıdan desteklenmeye çalışıldığı anlaşılmıştır. Ancak hizmetlerin planlı, çok boyutlu ve sürdürülebilir olmaması
nedeniyle uygulamaların istenilen düzeyde etkiyi yaratmadığı söylenebilmektedir. Bu bağlamda yaşlılara sunulan hizmetlerin yaşlının merkezde olduğu çok boyutlu ve çözüme yönelik olması gerekmektedir.Humanity has been faced with many disasters such as wars, migrations and outbreak since the day it
emerged. One of these disasters is undoubtedly Covid-19 outbreak. This new type of coronavirus, which
was encountered for the first time in Wuhan, China, affected millions of people worldwide, spread to all
countries and was declared an outbreak by the World Health Organization (WHO). Although there is
a risk of transmission of this new type of virus for people of all ages, mortality rates in the elderly are
higher than other age groups due to chronic diseases accompanying old age. In old age, individuals may
face more physiological, psychological, social and economic problems than other age groups. Combining
with the multi-dimensional structure of old age and its unique problems, the The Covid-19 outbreak has
made the emergence of applications and new approaches specific to the elderly mandatory. For these
reasons, since the beginning of the outbreak, governments around the world have developed measures,
restrictions and new service models for the elderly group. In the Covid-19 outbreak, during work for
elderly people in Turkey, and to determine whether services are what are important to be ready for a new
outbreak in the future. It is shown as an example by WHO with the studies conducted by Turkey in the
Covid-19 outbreak for the elderly. Examining and evaluating the work and services of Turkey for the
elderly during the outbreak process is important to be prepared for new outbreaks that may occur in the
future. For these reasons, the purpose of the study is to evaluate and examine practices for the elderly in
Turkey during the Covid-19 outbreak. Applications for the elderly; services, restrictions, economic support, permission/ privileges and medical services has been examined in this research. As a result, it was
understood that the elderly people were tried to be supported psycho-socially and economically in the
dimensions discussed. However, it can be said that the applications do not create the desired level of
impact because the services are not planned, multi-dimensional and sustainable. In this context, the
services provided to the elderly should be multidimensional and solution oriented, with the elderly at
the centre
Yapı İşlerinde Sağlık Ve Güvenlik Planının Önemi
Ülkemizde SGK istatistikleri incelendiğinde en çok iş kazasının yaşandığı ilk 3 sektör arasında inşaat işleri yer almaktadır. İş salığı ve güvenliği düzenlemelerinin yer aldığı 6331 sayılı İş Sağlığı ve Güvenliği kanunun 30 maddesinde dayanarak yapı işleri için çıkartılmış Yapı İşlerinde İş Sağlığı ve Güvenliği Yönetmeliği bu kapsamsa yer alan işler için düzenlemeleri içermektedir. Yapı işleri dinamik yapısı nedeniyle diğer sektörlerden farklı olarak değerlendirilmelidir. Buda Yapı İşlerinde İş Sağlığı ve Güvenliği Yönetmeliği Mad.10.b de belirtildiği gibi iş ve işlemler öncesi yani proje aşamasında Sağlık Güvenlik Planı hazırlanması gerektiğini belirtmiştir. Ayrıca yapı işleri kapsamındaki işler genel olarak aynı gibi görünse de bulunduğu bölge, yapı işinin niteliği gibi sebeplerden ötürü farklılar göstermektedir. Bu yüzden yapı işlerinde en önemli husus plan ve proje aşamalarıdır. Çalışmalara başlamadan önce planlama ve organizasyon aşamasında bir çok faktör göz önünde bulundurularak yapılmalıdır (Zemin, hava koşulları, çevresel etkenler, insan taşıt yoğunluğu vb.). Bu çalışmalar yapılırken sağlık ve güvenlik önlemleri organizasyon ve planlamaya dahil edilmelidir
Evaluation of the factors that cause delay in the application to health services in hemorrhoidal disease
Amaç: Hemoroidal hastalıkta demografik, etiyolojik, psikososyal
faktörlerin irdelenmesi ve başvuru gecikmesine
olan etkilerinin araştırılması.
Gereç ve Yöntem: Sağlık Bakanlığı Ankara Eğitim ve
Araştırma Hastanesi Genel Cerrahi polikliniğinde hemoroidal
hastalık tanısı konulan 100 hasta ile görüşme yapılarak bir anket formu dolduruldu.
Bulgular: Çalışma grubu yaşları 18 ile 76 arasında değişen 100 hastayı kapsamaktaydı. Çalışmaya alınan hastaların
%48’i erkek, %52’si kadın idi. Hastalarımızın %38’i
ilk semptomu hissettiğinde 7 gün içerisinde polikliniğe
başvurmuş, %62’si ise çeşitli nedenlerden dolayı başvuruda
gecikmişlerdi. Hastaların %4’ünün sağlık güvencesi
yoktu, Bu hastaların hepsi başvuruda gecikmişti. Hastaların
eğitim düzeyleri yükseldikçe hemoroidlerin evresi
düşüyordu (p=0,033). Hekime başvurmada gecikme ile
eğitim düzeyleri arasında negatif bir anlamlı ilişki bulundu
(p=0,05). Kanama, ağrı ve kaşıntı şikâyeti olan hastaların
ilk şikâyetleri ile başvuru tarihleri arasındaki süre bu
semptomları olmayanlara göre daha uzundu. Karşı cinsiyetteki
hekimlerden çekinen hastalar ile çekinmeyenler
arasında başvuruda gecikme bakımından önemli bir
farklılık yoktu.
Sonuç: Ailede anorektal hastalık öyküsü, meslekler, ayrıca gebelik ve obezite hemoroidal hastalık etiyolojisinde
etkili bulundu. Eğitim düzeyi yüksek olan hastalarda başvuruda gecikme oranı ve hemoroidlerin evresi daha düşüktü. Sağlık güvencesi bulunmaması başvuruda gecikmeye
neden olmaktaydı. Ağrı, kanama ve kaşıntı şikâyeti
olan hastalar başvuruda gecikmişlerdi.Objective: To reveal the demographic, etiologic, psychosocial
factors in the hemorrhoidal disease and the effects
of delay in application.
Material and Methods: We interviewed 100 patients
ranging in age from 18 to 76 diagnosed with hemorrhoidal
disease in the General Surgery Outpatient Clinics at
the Ministry of Health Ankara Training and Research Hospital.
Results: Overall, 48% of the patients were men, 52% were
women. Additionally, 38% of the patients were admitted
to the clinic within 7 days of experiencing the first symptoms,
while 62%were admitted with a delay for several
reasons. The application was delayed for 4% of the patients
who did not have health insurance. The grades of
hemorrhoidal disease were decreasing as the education
levels were increasing (p=0,033). A statistically significant
relationship was found between delay in application and
education levels (p=0, 05). The period between the first
complaints and the application were longer for patients
suffering from bleeding, pain, and itching compared to
those without these symptoms. We found no significant
difference in delayed application between the patients
who were and were not ashamed to see a doctor of the
opposite sex.
Conclusion: An anorectal disease history in the family,
the occupations, pregnancy, and obesity were found effective
in the etiology of the hemorrhoidal disease. The
grades of hemorrhoidal disease and the ratio of delay in
application was low among the more educated patients.
