    Viscous fingering of miscible slices

    Viscous fingering of a miscible high viscosity slice of fluid displaced by a lower viscosity fluid is studied in porous media by direct numerical simulations of Darcy's law coupled to the evolution equation for the concentration of a solute controlling the viscosity of miscible solutions. In contrast with fingering between two semi-infinite regions, fingering of finite slices is a transient phenomenon due to the decrease in time of the viscosity ratio across the interface induced by fingering and dispersion processes. We show that fingering contributes transiently to the broadening of the peak in time by increasing its variance. A quantitative analysis of the asymptotic contribution of fingering to this variance is conducted as a function of the four relevant parameters of the problem i.e. the log-mobility ratio R, the length of the slice l, the Peclet number Pe and the ratio between transverse and axial dispersion coefficients Ï”\epsilon. Relevance of the results is discussed in relation with transport of viscous samples in chromatographic columns and propagation of contaminants in porous media.Comment: 10 pages, 13 figure

    Influence of Langmuir adsorption and viscous fingering on transport of finite size samples in porous media

    We examine the transport in a homogeneous porous medium of a finite slice of a solute which adsorbs on the porous matrix following a Langmuir adsorption isotherm and can influence the dynamic viscosity of the solution. In the absence of any viscosity variation, the Langmuir adsorption induces the formation of a shock layer wave at the frontal interface and of a rarefaction wave at the rear interface of the sample. For a finite width sample, these waves interact after a given time that varies nonlinearly with the adsorption properties to give a triangle-like concentration profile in which the mixing efficiency of the solute is larger in comparison to the linear or no-adsorption cases. In the presence of a viscosity contrast such that a less viscous carrier fluid displaces the more viscous finite slice, viscous fingers are formed at the rear rarefaction interface. The fingers propagate through the finite sample to preempt the shock layer at the viscously stable front. In the reverse case i.e. when the shock layer front features viscous fingering, the fingers are unable to intrude through the rarefaction zone and the qualitative properties of the expanding rear wave are preserved. A non-monotonic dependence with respect to the Langmuir adsorption parameter bb is observed in the onset time of interaction between the nonlinear waves and viscous fingering. The coupled effect of viscous fingering at the rear interface and of Langmuir adsorption provides a powerful mechanism to enhance the mixing efficiency of the adsorbed solute.Comment: 12 pages 11 figure

    Hot spots in density fingering of exothermic autocatalytic chemical fronts

    A light field is commonly described by a two-plane representation with four dimensions. Refocused three-dimensional contents can be rendered from light field images. A method for capturing these images is by using cameras with microlens arrays. A dense sampling of the light field results in large amounts of redundant data. Therefore, an efficient compression is vital for a practical use of these data. In this paper, we propose a displacement intra prediction scheme with a maximum of two hypotheses for the compression of plenoptic contents from focused plenoptic cameras. The proposed scheme is further implemented into HEVC. The work is aiming at coding plenoptic captured contents efficiently without knowing underlying camera geometries. In addition, the theoretical analysis of the displacement intra prediction for plenoptic images is explained; the relationship between the compressed captured images and their rendered quality is also analyzed. Evaluation results show that plenoptic contents can be efficiently compressed by the proposed scheme. Bit rate reduction up to 60 percent over HEVC is obtained for plenoptic images, and more than 30 percent is achieved for the tested video sequences

    Establishing the values for patient engagement (PE) in health-related quality of life (HRQoL) research: an international, multiple-stakeholder perspective

    PurposeActive patient engagement is increasingly viewed as essential to ensuring that patient-driven perspectives are considered throughout the research process. However, guidance for patient engagement (PE) in HRQoL research does not exist, the evidence-base for practice is limited, and we know relatively little about underpinning values that can impact on PE practice. This is the first study to explore the values that should underpin PE in contemporary HRQoL research to help inform future good practice guidance. MethodsA modified ‘World Café’ was hosted as a collaborative activity between patient partners, clinicians and researchers: self-nominated conference delegates participated in group discussions to explore values associated with the conduct and consequences of PE. Values were captured via post-it notes and by nominated note-takers. Data were thematically analysed: emergent themes were coded and agreement checked. Association between emergent themes, values and the Public Involvement Impact Assessment Framework were explored. ResultsEighty participants, including 12 patient partners, participated in the 90-min event. Three core values were defined: (1) building relationships; (2) improving research quality and impact; and (3) developing best practice. Participants valued the importance of building genuine, collaborative and deliberative relationships—underpinned by honesty, respect, co-learning and equity—and the impact of effective PE on research quality and relevance. Conclusions An explicit statement of values seeks to align all stakeholders on the purpose, practice and credibility of PE activities. An innovative, flexible and transparent research environment was valued as essential to developing a trustworthy evidence-base with which to underpin future guidance for good PE practice.Peer reviewe

    Design of the POINT study: Pharmacotherapy Optimisation through Integration of a Non-dispensing pharmacist in a primary care Team (POINT)

    __Background:__ In the Netherlands, 5.6 % of acute hospital admissions are medication-related. Almost half of these admissions are potentially preventable. Reviewing medication in patients at risk in primary care might prevent these hospital admissions. At present, implementation of medication reviews in primary care is suboptimal: pharmacists lack access to patient information, pharmacists are short of clinical knowledge and skills, and working processes of pharmacists (focus on dispensing) and general practitioners (focus on clinical practice) match poorly. Integration of the pharmacist in the primary health care team might improve pharmaceutical care outcomes. The aim of this study is to evaluate the effect of integration of a non-dispensing pharmacist in general practice on the safety of pharmacotherapy in the Netherlands. __Methods:__ The POINT study is a non-randomised controlled intervention study with pre-post comparison in an integrated primary care setting. We compare three different models of pharmaceutical care provision in primary care: 1) a non-dispensing pharmacist as an integral member of a primary care team, 2) a pharmacist in a community pharmacy with a predefined training in performing medication reviews and 3) a pharmacist in a community pharmacy (care as usual). In all models, GPs remain accountable for individual medication prescription. In the first model, ten non-dispensing clinical pharmacists are posted in ten primary care practices (including 5 – 10 000 patients each) for a period of 15 months. These non-dispensing pharmacists perform patient consultations, including medication reviews, and share responsibility for the pharmaceutical care provided in the practice. The two other groups consist of ten primary care practices with collaborating pharmacists. The main outcome measurement is the number of medication-related hospital admissions during follow-up. Secondary outcome measurements are potential medication errors, drug burden index and costs. Parallel to this study, a qualitative study is conducted to evaluate the feasibility of introducing a NDP in general practice. __Discussion:__ As the POINT study is a large-scale intervention study, it should provide evidence as to whether integration of a non-dispensing clinical pharmacist in primary care will result in safer pharmacotherapy. The qualitative study also generates knowledge on the optimal implementation of this model in primary care. Results are expected in 2016

    Clinical pharmacists in Dutch general practice:an integrated care model to provide optimal pharmaceutical care

    Background: Medication-related harm is a major problem in healthcare. New models of integrated care are required to guarantee safe and efficient use of medication. Aim: To prevent medication-related harm by integrating a clinical pharmacist in the general practice team. This best practice paper provides an overview of 1. the development of this function and the integration process and 2. its impact, measured with quantitative and qualitative analyses. Setting: Ten general practices in the Netherlands. Development and implementation of the (pragmatic) experiment: We designed a 15-month workplace-based post-graduate learning program to train pharmacists to become clinical pharmacists integrated in general practice teams. In close collaboration with general practitioners, clinical pharmacists conduct clinical medication reviews (CMRs), hold patient consultations for medication-related problems, carry out quality improvement projects and educate the practice staff. As part of the Pharmacotherapy Optimisation through Integration of a Non-dispensing pharmacist in a primary care Team (POINT) intervention study, ten pharmacists worked full-time in general practices for 15 months and concurrently participated in the training program. Evaluation of this integrated care model included both quantitative and qualitative analyses of the training program, professional identity formation and effectiveness on medication safety. Evaluation: The integrated care model improved medication safety: less medication-related hospitalisations occurred compared to usual care (rate ratio 0.68 (95% CI: 0.57–0.82)). Essential hereto were the workplace-based training program and full integration in the GP practices: this supported the development of a new professional identity as clinical pharmacist. This new caregiver proved to align well with the general practitioner. Conclusion: A clinical pharmacist in general practice proves a feasible integrated care model to improve the quality of drug therapy

    Elevated formation of pyridinoline cross-links by profibrotic cytokines is associated with enhanced lysyl hydroxylase 2b levels

    AbstractThe hallmark of fibrosis is the excessive accumulation of collagen. The deposited collagen contains increased pyridinoline cross-link levels due to an overhydroxylation of lysine residues within the collagen telopeptides. Lysyl hydroxylase 2b (LH2b) is the only lysyl hydroxylase consistently up-regulated in several forms of fibrosis, suggesting that an enhanced LH2b level is responsible for the overhydroxylation of collagen telopeptides. The present paper reports the effect of profibrotic cytokines on the expression of collagen, lysyl hydroxylases and lysyl oxidase in normal human skin fibroblasts, as well as the effect on pyridinoline formation in the deposited matrix. All three isoforms of TGF-ÎČ induce a substantial increase in LH2b mRNA levels, also when expressed relatively to the mRNA levels of collagen type I α2 (COL1A2). The TGF-ÎČ isoforms also clearly influence the collagen cross-linking pathway, since higher levels of pyridinoline cross-links were measured. Similar stimulatory effects on LH2b/COL1A2 mRNA expression and pyridinoline formation were observed for IL-4, activin A, and TNF-α. An exception was BMP-2, which has no effect on LH2b/COL1A2 mRNA levels nor on pyridinoline formation. Our data show for the first time that two processes, i.e., up-regulation of LH2b mRNA levels and increased formation of pyridinoline cross-links, previously recognized to be inherent to fibrotic processes, are induced by various profibrotic cytokines
