    Long-term follow-up of Philadelphia chromosome-positive (Ph+) chronic myeloid leukaemia (CML) in children and adolescents managed at a single institution over a 20-year period

    Chronic myeloid leukaemia (CML) is rare in childhood. In our Institution we managed 30 consecutive Ph+CML patients aged <18 years, according to our adults’ guidelines. Patients with HLA-identical related donor (RD) underwent stem cell transplant (SCT). Since 1989, patients without RD were systematically treated with -interferon (IFN) (median dosage: 6 MU/day). Of 18/19 evaluable patients, 17 (94.5%) achieved haematologic response (HR), 11/17 (65%) cytogenetic response (CyR), complete (CCyR) in 4 (23.5%). Three patients remain in CCyR, 2 achieved BCR-ABL transcript disappearance. Of 13 patients without CCyR, 5 underwent SCT, 4 switched to STI571, 4 progressed. All patients receiving STI571 in chronic phase (CP) obtained sustained CCyR and 3 a persistent molecular response. 8-year survival among IFN-treated patients, censored or not for subsequent therapies, is 62% and 63%. Overall, 13/30 patients underwent SCT: 5 HLA-identical-RD, 5 matched unrelated donor, 2 mismatched-RD, 1 unrelated mismatched umbilical cord blood. Eight allotransplanted patients (6/6 in 1st CP) are in cytogenetic and molecular remission with 8-year survival of 61% from SCT and 69% from diagnosis. In our 20-year experience, the use of IFN in children without matched RD led to prolonged cytogenetic and molecular responses and long-term survival, without impairing the outcome of subsequent SCT

    Toxina botulínica A para o trismo no tétano cefálico

    Cephalic tetanus is a localized form of tetanus. As in generalized forms , trismus is a prominent feature of the disease, leading to considerable difficulty in feeding, swallowing of the saliva and mouth hygiene. These difficulties often precede respiratory problems and aspiration bronchopneumonia is a frequent life-threatening complication. Muscle relaxants other than curare drugs may show a limited benefit for relieving trismus. Tetanospasmin, the tetanic neurotoxin, and botulinum toxin share many similarities, having a closely related chemical structure, an origin from related microorganisms (Clostridium tetani and Clostridium botulinum, respectively), and presumably, the same mechanisms of action in the neuron. The difference between the two lies in their peculiar neurospecificity, acting in different neurons. Injection of minute doses of botulinum toxin in the muscles involved in focal dystonias or other localized spastic disorders have proved to be very effective in these conditions. We describe the use of botulinum toxin A in the successful treatment of trismus in a patient suffering from cephalic tetanus. We believe that this form of treatment may be of value in lowering the risk of pulmonary complications in tetanic patients.O tétano cefálico é uma forma localizada de tétano. Da mesma maneira que nas formas generalizadas, o trismo é componente clínico proeminente, que leva a grande dificuldade na alimentação, na deglutição da saliva e para a higiene da boca. Essas dificuldades freqüentemente precedem os problemas respiratórios e as pneumonias aspirativas, que se constituem em complicações que ameaçam a vida dos pacientes. Drogas relaxantes musculares de outro tipo que as drogas curarizantes trazem pouco benefício para o trismo. A neurotoxina tetânica (tetanospasmin) e a toxina botulínica compartilham muitas semelhanças, como estrutura química símile, origem de microorganismos relacionados (Clostridium tetani e Clostridium botulinum, respectivamente) e, presumivelmente, os mesmos mecanismos de ação no neurônio. A diferença entre as duas reside na sua peculiar neuroespecificidade, atuando em diferentes neurônios. Injeção de doses reduzidas da toxina botulínica em músculos envolvidos em distonias focais ou outras desordens com espasticidade muscular localizada tem se mostrado eficaz em abolir as contrações. Descrevemos o uso da toxina botulínica A com sucesso no tratamento do trismo num paciente sofrendo de tétano cefálico.     Numerical investigations about the sound transmission loss of a fuselage panel section with embedded periodic foams

    The scope of this paper is to investigate the sound transmission loss of a typical fuselage panel section, as well as to propose solutions based on the inclusion of a periodic pattern inside its foam core, which aim at passively improving the acoustic performance in a mid-high range of frequencies. In detail, a new fuselage panel configuration is numerically studied, starting from the state of the art regarding the acoustic packages based on porous meta-materials. The main novelties of the present work are represented by the application of a meta-core solution inside an acoustic package of aeronautical interest, as well as a systematic investigation of the effects deriving from its geometrical parameters. In order to reach this goal, a numerical model of a fuselage panel section is studied, and the effect of several periodic patterns are simulated; more specifically, twelve configurations are taken into account, each with different radius of the inclusions and number of unit cells along the thickness. For each of these layouts, the mass increase of the so-called meta-core, compared to that of its classical homogeneous counterpart, is estimated, together with the associated mid-band frequency and amplitude of the sound transmission loss peak, which is caused by the additional acoustic modes excited by the periodic nature of the meta-core itself. Results are presented in terms of tables and graphs, which may constitute a good basis in order to perform preliminary design considerations that could be interesting for further generalizations

    Effects of periodic potentials on the critical velocity of superfluid Fermi gases in the BCS-BEC crossover

    We study the effects of an external periodic potential on the critical velocity of a superfluid Fermi gas in the crossover between the Bardeen-Cooper-Schrieffer (BCS) phase and Bose-Einstein condensation (BEC). We numerically solve the Bogoliubov-de Gennes equations to model a three-dimensional (3D) gas of ultracold atoms in the superfluid phase flowing through a 1D optical lattice. We find that when the recoil energy is comparable to the Fermi energy, the presence of the periodic potential reduces the effect of pair-breaking excitations. This behavior is a consequence of the peculiar band structure of the quasiparticle energy spectrum in the lattice. When the lattice height is much larger than the Fermi energy, the periodic potential makes pairs of atoms to be strongly bound even in the BCS regime and pair-breaking excitations are further suppressed. We have also found that when the recoil energy is comparable to or larger than the Fermi energy, the critical velocity due to long-wavelength phonon excitations shows a non-monotonic behavior along the BCS-BEC crossover.Comment: 9 pages, 7 figures, added an appendix on the dynamical instabilit

    A Specialized Long-Term Distribution System Expansion Planning Method With the Integration of Distributed Energy Resources

    The electrical distribution system (EDS) has undergone major changes in the last decade due to the increasing integration of distributed generation (DG), particularly renewable energy DG. Since renewable energy resources have uncertain generation, energy storage systems (ESSs) in the EDS can reduce the impact of those uncertainties. Besides, electric vehicles (EVs) have been increasing in recent years leveraged by environmental concerns, bringing new challenges to the operation and planning of the EDS. In this context, new approaches for the distribution system expansion planning (DSEP) problem should consider the distributed energy resources (DG units, ESSs, and EVs) and address environmental impacts. This paper proposes a mixed-integer linear programming model for the DSEP problem considering DG units, ESSs, and EV charging stations, thus incorporating the environmental impact and uncertainties associated with demand (conventional and EVs) and renewable generation. In contrast to other approaches, the proposed model includes the simultaneous optimization of investments in substations, circuits, and distributed energy resources, including environmental aspects (CO 2 emissions). The optimization method was developed in the modeling language AMPL and solved via CPLEX.
