Recurring retrieval needs in diverse and dynamic dataspaces: Issues and reference framework
Processing information requests over heterogeneous data spaces is very challenging because aimed at guaranteeing user satisfaction with respect to conflicting requirements on result quality and response time. In [3], it has been argued that, in dynamic contexts pre-venting substantial user involvement in interpreting requests, information on similar requests recurring overtime can be exploited during query processing. In this paper, referring to a graph-based modeling of dataspaces and requests, we propose a preliminary approach in this direction centered on the enabling concept of Profiled Graph Query Pattern (PGQP) as an aggregation of information on past evaluations of a set of previously executed queries. The information maintained in PGQP is not query results, as in materialized queries, but can include different kinds of data and metadata
A single tree model to consistently simulate cooling, shading, and pollution uptake of urban trees
Extremely high temperatures, which negatively affect the human health and plant performances, are becoming more frequent in cities. Urban green infrastructure, particularly trees, can mitigate this issue through cooling due to transpiration, and shading. Temperature regulation by trees depends on feedbacks among the climate, water supply, and plant physiology. However, in contrast to forest or general ecosystem models, most current urban tree models still lack basic processes, such as the consideration of soil water limitation, or have not been evaluated sufficiently. In this study, we present a new model that couples the soil water balance with energy calculations to assess the physiological responses and microclimate effects of a common urban street-tree species (Tilia cordata Mill.) on temperature regulation. We contrast two urban sites in Munich, Germany, with different degree of surface sealing at which microclimate and transpiration had been measured. Simulations indicate that differences in wind speed and soil water supply can be made responsible for the differences in transpiration. Nevertheless, the calculation of the overall energy balance showed that the shading effect, which depends on the leaf area index and canopy cover, contributes the most to the temperature reduction at midday. Finally, we demonstrate that the consideration of soil water availability for stomatal conductance has realistic impacts on the calculation of gaseous pollutant uptake (e.g., ozone). In conclusion, the presented model has demonstrated its ability to quantify two major ecosystem services (temperature mitigation and air pollution removal) consistently in dependence on meteorological and site conditions
Extracorporeal life support in mitral papillary muscle rupture: Outcome of multicenter study
Background: Post-acute myocardial infarction papillary muscle rupture (post-AMI PMR) may present variable clinical scenarios and degree of emergency due to result of cardiogenic shock. Veno-arterial extracorporeal life support (V-A ECLS) has been proposed to improve extremely poor pre- or postoperative conditions. Information in this respect is scarce.Methods: From the CAUTION (meChanical complicAtion of acUte myocardial infarcTion: an InternatiOnal multiceNter cohort study) database (16 different Centers, data from 2001 to 2018), we extracted adult patients who were surgically treated for post-AMI PMR and underwent pre- or/and postoperative V-A ECLS support. The end-points of this study were in-hospital survival and ECLS complications.Results: From a total of 214 post-AMI PMR patients submitted to surgery, V-A ECLS was instituted in 23 (11%) patients. The median age was 61.7 years (range 46-81 years). Preoperatively, ECLS was commenced in 10 patients (43.5%), whereas intra/postoperative in the remaining 13. The most common V-A ECLS indication was post-cardiotomy shock, followed by preoperative cardiogenic shock and cardiac arrest. The median duration of V-A ECLS was 4 days. V-A ECLS complications occurred in more than half of the patients. Overall, in-hospital mortality was 39.2% (9/23), compared to 22% (42/219) for the non-ECLS group.Conclusions: In post-AMI PMR patients, V-A ECLS was used in almost 10% of the patients either to promote bridge to surgery or as postoperative support. Further investigations are required to better evaluate a potential for increased use and its effects of V-A ECLS in such a context based on the still high perioperative mortality
Guidelines on the diagnosis, treatment and management of visceral and renal arteries aneurysms: a joint assessment by the Italian Societies of Vascular and Endovascular Surgery (SICVE) and Medical and Interventional Radiology (SIRM)
: The objective of these Guidelines is to provide recommendations for the classification, indication, treatment and management of patients suffering from aneurysmal pathology of the visceral and renal arteries. The methodology applied was the GRADE-SIGN version, and followed the instructions of the AGREE quality of reporting checklist. Clinical questions, structured according to the PICO (Population, Intervention, Comparator, Outcome) model, were formulated, and systematic literature reviews were carried out according to them. Selected articles were evaluated through specific methodological checklists. Considered Judgments were compiled for each clinical question in which the characteristics of the body of available evidence were evaluated in order to establish recommendations. Overall, 79 clinical practice recommendations were proposed. Indications for treatment and therapeutic options were discussed for each arterial district, as well as follow-up and medical management, in both candidate patients for conservative therapy and patients who underwent treatment. The recommendations provided by these guidelines simplify and improve decision-making processes and diagnostic-therapeutic pathways of patients with visceral and renal arteries aneurysms. Their widespread use is recommended
Adaptation of Users to Future Climate Conditions in Naturally Ventilated Historic Buildings: Effects on Indoor Comfort
User behaviour can significantly affect indoor thermal comfort conditions, as well as energy consumption, especially in existing buildings with high thermal masses where natural cross ventilation is the main strategy to reduce cooling loads. The aims of this paper were: (i) to compare how behavioural changes evaluated by means of rule-based and stochastic models lead to changes in indoor thermal comfort levels, and (ii) to define the patterns of indoor thermal comfort in historic residential buildings in future scenarios. To this end, a historic building located in Molfetta (Southern Italy) was analysed using a dynamic energy simulation engine in five weather scenarios (Typical Meteorological Year, current extreme weather file 2018, predicted weather files for 2020, 2050, and 2080 generated by morphing method), and stochastic and rule-based models for window openings were adopted and compared. The results showed that the stochastic model was more accurate than the rule-based one, resulting in a reduction of discomfort conditions during the summer period between 30% and 50% in all climate scenarios. However, although the differences between predicted discomfort levels using rule-based and stochastic models tended to increase, discomfort levels still appeared to be not acceptable in the 2050 and 2080 scenarios due to the rising temperature driven by climate change
Transient hair loss during treatment with dimethyl-fumarate for multiple sclerosis
Introduction Dimethyl-fumarate is a recently approved drug for relapsing-remitting Multiple Sclerosis in Italy. Clinical case A 55-year-old woman started therapy with dimethyl-fumarate on June 2014; it was well-tolerated aside from moderate flushing. Starting September 2014 she noticed a progressive hair loss, that neither the dermatological examination nor clinical and medical history nor blood investigations could explain. The hair loss slowed down after two months and was followed by a hair growth back. Discussion Transient hair loss is not a reported side effect of dimethyl-fumarate therapy but by excluding any known cause we attributed it to the beginning of the new therapy
