The Brazilian coast: exploration, occupation and preservation - A comparative study between littoral regions of the States of São Paulo and Rio Grande do Norte
This paper is a comparative study of the spaces systems evolution of two zones in the Brazilian coast: Ubatuba (in the São Paulo State, south-eastern Brazil) and Tibau do Sul (in the Rio Grande do Norte State, north-eastern Brazil). These systems changed by successive deteriorations associated to the society evolution. The study researched the changes of the space organization and the social and environmental conflicts characterized for two patterns of territory occupation: the tourism areas (for recreational and tourist uses) and the environmental protected areas (national parks and permanent protection areas).Este artigo propõe um estudo comparativo da evolução dos sistemas espaciais de duas áreas do litoral brasileiro: Ubatuba (Estado de São Paulo, sudeste do Brasil) e Tibau do Sul (Estado do Rio Grande do Norte, nordeste do Brasil). Estes sistemas foram moldados ao longo do tempo por sucessivas alterações em responda à evolução da sociedade. Interessou-nos investigar a evolução da organização espacial dessas localidades sob a ótica dos conflitos sócio-ambientais derivados de dois tipos de ocupação territorial, a turística (pelas residências secundárias) e a ligada à preservação/conservação da natureza (pelas unidades de conservação e áreas de preservação)
Prevalence of non-obstructive dysphagia in patients with heartburn and regurgitation
OBJECTIVE: Heartburn and regurgitation are the most common gastroesophageal reflux symptoms, and dysphagia could be a possible symptom. This investigation aimed to evaluate the prevalence of non-obstructive dysphagia in patients with heartburn and regurgitation. METHODS: A total of 147 patients (age, 20-70 years; women, 72%) complaining of heartburn and regurgitation, without esophageal stricture, previous esophageal surgery, or other diseases, were evaluated. Twenty-seven patients had esophagitis. The Eating Assessment Tool (EAT-10) was employed to screen for dysphagia; EAT-10 is composed of 10 items, and the patients rate each item from 0 to 4 (0, no problems; 4, most severe symptom). Results of the 147 patients were compared with those of 417 healthy volunteers (women, 62%; control group) aged 20-68 years. RESULTS: In the control group, only two (0.5%) had an EAT-10 score X5, which was chosen as the threshold to define dysphagia. EAT-10 scores X5 were found in 71 (48.3%) patients and in 55% of the patients with esophagitis and 47% of the patients without esophagitis. This finding indicates a relatively higher prevalence of perceived dysphagia in patients with heartburn and regurgitation and in patients with esophagitis. We also found a positive correlation between EAT-10 scores and the severity of gastroesophageal reflux symptoms based on the Velanovich scale. CONCLUSION: In patients with heartburn and regurgitation symptoms, the prevalence of dysphagia was at least 48%, and has a positive correlation with the overall symptoms of gastroesophageal reflux
Hydroxychloroquine decreases Th17-related cytokines in systemic lupus erythematosus and rheumatoid arthritis patients
OBJECTIVES: Hydroxychloroquine is an antimalarial agent that has been used in systemic lupus erythematosus and rheumatoid arthritis treatment for many years. Recently, novel mechanisms of action have been proposed, thereby broadening the therapeutic perspective of this medication. The purpose of this study was to evaluate the immunomodulatory activity of hydroxychloroquine in T helper 17 (Th17) cytokines in healthy individuals and patients. METHODS: Eighteen female patients with systemic lupus erythematosus (mean age 39.0±12.9 years) and 13 female patients with rheumatoid arthritis (mean age 51.5±7.7 years) were recruited from Universidade Federal de Pernambuco-Brazil. The patients were included after fulfilling four classification criteria for systemic lupus erythematosus or rheumatoid arthritis from the American College of Rheumatology. After being stimulated with phorbol 12-myristate 13-acetate and ionomycin in the absence or presence of different concentrations of hydroxychloroquine, the interleukin 6, 17 and 22 levels were quantified with an enzyme-linked immunosorbent assay in culture supernatants of peripheral blood mononuclear cells from healthy individuals and patients. RESULTS: We demonstrated that in peripheral blood mononuclear cells from healthy volunteers and in systemic lupus erythematosus and rheumatoid arthritis patients, there was a significant reduction in the IL-6, IL-17 and IL-22 supernatant levels after adding hydroxychloroquine. CONCLUSIONS Our in vitro results demonstrated that hydroxychloroquine inhibits IL-6, IL-17 and IL-22 production and contributes to a better understanding of the mechanism of action of this medication
Clonal chromosomal mosaicism and loss of chromosome Y in elderly men increase vulnerability for SARS-CoV-2
The pandemic caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2, COVID-19) had an estimated overall case fatality ratio of 1.38% (pre-vaccination), being 53% higher in males and increasing exponentially with age. Among 9578 individuals diagnosed with COVID-19 in the SCOURGE study, we found 133 cases (1.42%) with detectable clonal mosaicism for chromosome alterations (mCA) and 226 males (5.08%) with acquired loss of chromosome Y (LOY). Individuals with clonal mosaic events (mCA and/or LOY) showed a 54% increase in the risk of COVID-19 lethality. LOY is associated with transcriptomic biomarkers of immune dysfunction, pro-coagulation activity and cardiovascular risk. Interferon-induced genes involved in the initial immune response to SARS-CoV-2 are also down-regulated in LOY. Thus, mCA and LOY underlie at least part of the sex-biased severity and mortality of COVID-19 in aging patients. Given its potential therapeutic and prognostic relevance, evaluation of clonal mosaicism should be implemented as biomarker of COVID-19 severity in elderly people. Níveis séricos de 25-hidroxivitamina D3 e sua associação com parâmetros clínicos e laboratoriais em pacientes com lúpus eritematoso sistêmico
INTRODUÇÃO: O papel imunorregulatório da vitamina D tem sido alvo de um crescente número de estudos em pacientes com lúpus eritematoso sistêmico (LES). Objetivos: Determinar os níveis séricos de 25-hidroxivitamina D3 [25(OH)D] em pacientes com LES e verificar a associação da insuficiência/deficiência de 25(OH)D com parâmetros clínicos e laboratoriais. MÉTODOS:Estudo de corte transversal, prospectivo, realizado no ambulatório de LES do Serviço de Reumatologia do Hospital das Clínicas da Universidade Federal de Pernambuco. Foram incluídos 78 pacientes portadores de LES e 64 voluntários (grupo de comparação) pareados por gênero e idade. RESULTADOS: Constatou-se insuficiência/deficiência de 25(OH)D em 45 (57,7%) pacientes com LES e em 25 (39%) indivíduos do grupo de comparação. Os níveis séricos médios de 25(OH)D foram 29,3 ng/mL (6,1-55,2 ng/mL) nos pacientes com LES e 33,12 ng/mL (15,9-63,8 ng/mL) no grupo de comparação; essa diferença é considerada estatisticamente significante (P = 0,041). Não houve diferença estatisticamente significante entre as médias de idade dos dois grupos. Não houve associação estatisticamente significante entre insuficiência/deficiência de 25(OH)D e tempo de diagnóstico, atividade de doença (SLEDAI > 6), fadiga, uso de corticosteroides e de antimaláricos e anti-DNA. CONCLUSÕES:Foi constatada alta prevalência de insuficiência/deficiência de 25(OH)D nos pacientes com LES (57,7%), com diferença estatisticamente significante em relação ao grupo de comparação. Não evidenciamos associação de insuficiência/deficiência de vitamina D com as variáveis clínicas e laboratoriais estudadas. Os autores enfatizam a importância da determinação dos níveis séricos de 25(OH)D em todos os pacientes com LES, independente de onde residam e do tempo de diagnóstico da doenç
Galectin-9 gene (LGALS9) polymorphisms are associated with rheumatoid arthritis in Brazilian patients.
IntroductionRheumatoid arthritis (RA) is a chronic autoimmune disease characterized by synovial inflammation and hyperplasia, as well as cartilage and bone destruction. Several proteins are associated with the pathogenesis of the disease. Galectin-9 belongs to the family of lectins that are involved in various biological processes and have anti-inflammatory activity.ObjectiveTo investigate associations between the SNPs of the GAL-9 gene (LGALS9) and serum levels in rheumatoid arthritis patients. We extracted DNA from 356 subjects, 156 RA patients and 200 healthy controls from northeastern Brazil. Three polymorphisms (rs4795835, rs3763959, and rs4239242) in the LGALS9 gene were selected and genotyped using TaqMan SNP genotyping assay. Serum concentrations of galectin-9 were analyzed by ELISA.ResultsThe rs4239242 TT genotype showed a positive association with RA (p = 0.0032, odds ratio = 0.28), and heterozygous TC were prevalent in the control group compared to RA patients (p = 0.0001, odds ratio = 7.99). Galectin-9 serum levels were significantly increased in RA patients compared to the control group (p<0.0001). Patients in remission had high levels of galectin compared to the moderate activity group (p<0.0001). Regarding the Clinical Disease Activity Index (CDAI), patients in remission or low activity presented high levels of galectin when compared to patients in severity (p<0.0001). Patients performing moderate activity had a significant value compared to patients who were in high disease severity (p = 0.0064). Interestingly, the AG genotype (rs3763959) has been associated with a higher presence of bone erosion in RA patients (p = 0.0436). The SNP rs4239242 TT genotype showed a positive association with RA in comparison to the control group. Interleukin-18 in Brazilian Rheumatoid Arthritis Patients: Can Leflunomide Reduce It?
Objectives. Rheumatoid arthritis affects about 1% of the world’s population. This is a chronic autoimmune disease. It is predominant in females with progressive joint damage. Immune cells are involved, especially Th1/Th17 lymphocytes and their inflammatory cytokines. These proteins have different functions in the immune system, such as IL-16 is a chemotactic factor; IL-18 can activate NFκB transcription producing inflammatory proteins; IL-31 can activate the JAK/STAT pathway which leads to the production of inflammatory factors in chronic diseases; IL-33 promotes IL-16 secretion which causes lymphocyte recruitment, and IL-32 and IL-34 appear to increase TNF secretion by macrophages activation in AR. The aim of this study was to evaluate serum levels of IL-16, IL-18, IL-31, IL-32, IL-33, and IL-34 and compare them with the severity and treatment of RA patients if there are any correlations. Methods. A total of 140 RA patients and 40 healthy donors were recruited from the Department of Rheumatology at Hospital das Clínicas from the Federal University of Pernambuco. 60 AR patients were naïve for any treatment. Serum cytokine levels were determined using an ELISA kit. Results. Serum IL-16 (p = 0.0491), IL-18 (p < 0.0001), IL-31 (p = 0.0004), and IL-32 (p = 0.0040) levels were significantly increased in RA patients compared with healthy donors. It was observed that patients using leflunomide had the lowest IL-18 levels, close to controls levels (p = 0.0064). Conclusion. IL-16, IL-18, IL-31, and IL-32 are increased in the serum of RA patients. IL-18 is at lower levels in those AR who are taking leflunomide as treatment
