This research was conducted from April to June 2022 . The research method used in the form of interviews and direct observation. Determination of respondents using the census method. The data obtained were then analyzed descriptively and quantitatively based on household income analysis (TRPN) and household expenditure analysis (Ct) as well as indicators of prosperous families from the Central Statistics Agency (BPS). The results showed that the fishing community in Bubaa Village, Paguyaman Pantai District, Boalemo Regency, Gorontalo Province was included in the high welfare category with a score of 2.64

Long-Term Outcomes of Everolimus Therapy in De Novo Liver Transplantation: A Systematic Review and Meta-Analysis of Randomized Controlled Trials

BACKGROUND: Risk of nephrotoxicity in liver transplant patients on calcineurin inhibitors (CnIs) is a concern. Several controlled trials reported benefit of everolimus (EVR) in minimizing this risk when combined with a reduced CnI dose. BACKGROUND: To systematically review the efficacy and safety of EVR, alone or with reduced CnI dose, as compared to CnI alone post-liver transplantation. METHODS: We searched MEDLINE, Scopus, and the Cochrane Library for randomized controlled trials comparing EVR- and CnI-based regimens post-liver transplantation. Assessment of studies and data extraction were undertaken independently. RESULTS: Eight studies were selected, describing 769 patients. Cockcroft-Gault GFR was higher at one (P = .05), 3, and 5 years (P = .030) in patients on EVR compared to those receiving CnI therapy. The composite endpoint of efficacy failure was similar between the 2 arms after 1, 3, and 5 years of study. More patients discontinued EVR due to adverse effects in 1 year; however, no difference was noted after 3 or 5 years. A higher rates of proteinuria, peripheral edema, and incisional hernia occurred in patients on EVR. CONCLUSIONS: The analysis confirms noninferiority of EVR and reduced CnI combination. Combination regimen resulted in better renal function compared to standard CnI therapy

Multi-Service Group Key Establishment for Secure Wireless Mobile Multicast Networks

yesRecently there is high demand in distributing multimedia services over the internet to ubiquitous and computational intelligent mobile subscribers by the service providers (SPs). In this instance, provision of those services must be restricted to authorized subscribers via integration of authentication and group key management (GKM). GKM with diverse group services subscribed dynamically by moving subscribers in wireless networks has been omitted in conventional approaches. However it is expected that significant key management overhead will arise in them due to multi-services co-existing in the same network. In this paper, we propose a scalable decentralized multi-service GKM scheme considering host mobility in wireless environment. In the scheme, authentication of mobile subscribers and key management phases are delegated from the trusted domain key distributor (DKD) to the subgroup controllers known as area key distributors (AKD). The trusted intermediate AKDs can then establish and distribute the service group keys to valid subscribers in a distributed manner using identity-based encryption without involving the domain key distributor (DKD). This alleviates unnecessary delays and possible bottlenecks at the DKD. We show by simulation that the proposed scheme has some unique scalability properties over known schemes in terms of optimized rekeying communication and storage overheads. Les traumatismes vertebro-medullaires par chute de la hauteur d’un arbre a propos de 73 cas au Mali.

Introduction Les chutes du haut d’un arbre sont des accidents graves et fréquents au MALI.Objectif Analysez les facteurs épidémiologiques, étiologiques et circonstanciels des chutes de la hauteur des arbres Matériels et méthodes Il s’agit d’une étude prospective continue d’octobre 2007 à septembre 2009 à l’hôpital Gabriel Touré de Bamako (Mali). Elle a concerné tous les cas de chute du haut d’un arbre pendant cette période.Résultats Au cours de cette étude, nous avons recensé 73 patients dont les âges étaient compris entre 5 et 65 ans. Les couches socioprofessionnelles les plus atteintes ont été les cultivateurs et les bergers aux conditions socioéconomiques défavorables. Pendant les mois de décembre à mai 79,45% (58 patients) des patients ont été enregistrés. Cette période correspondait à la traite des fruits et à la saison sèche avec le manque de pâturages pour les animaux. L’intervention chirurgicale a concerné 32 patients. La mortalité a été de 12,32% (9 patients) tous traumatisés cervicaux.Conclusions Les accidents par chutes d’arbres sont en rapport avec les conditions socio-économiques et climatiques au Mali.Mots clés : Arbre, Accident, Chute, Mali, Rachis, Traumatism

Under development at the Caltech Submillimeter Observatory is a dual polarization, continuous comparison (correlation) receiver. The instrument has two beams on the sky; a reference and a signal beam. Using only cooled reflecting optics, two polarizing grids, and a quadrature hybrid coupler, the sky beams are coupled to four tunerless SIS mixers (both polarizations). The 4-8 GHz mixer IF outputs are, after amplification, correlated against each other. In principle, this technique results in flat baselines with very low RMS noise and is especially well suited for high redshift Galaxy work. At the same time an upgrade is planned to the existing facility heterodyne instrumentation. Dual frequency mode receivers are under development for the 230/460 GHz and 345/660 GHz atmospheric windows. The higher frequency receivers are implemented in a balanced configuration, which reduces both the LO power requirement and noise. Each mixer has 4 GHz of IF bandwidth and can be controled remotely. Not only do these changes greatly enhance the spectroscopic capabilities of the CSO, they also enable the observatory to be integrated into the Harvard-Smithsonian Submillimeter Array (SMA) as an additional baseline
