    How service‐users with intellectual disabilities understand challenging behaviour and approaches to managing it

    Background This study explored understandings that service‐users with intellectual disabilities and challenging behaviour held around their behaviour, what shaped these understandings, and the relationship between how behaviours are managed and well‐being. Methods Eight participants (three female, five male) partook in individual semi‐structured qualitative interviews. Interviews were transcribed and analysed using interpretative phenomenological analysis. Results Three master themes emerged from this analysis: (a) challenging behaviour can be explained via an internal or external frame of reference, with each framework having different implications for how participants attempted to manage behaviour. (b) Positive relationships provide a long‐term buffer to challenging behaviour, with positive relationships with family, staff and peers operating through different mechanisms to achieve this. (c) A greater ability to exert power and control in day‐to‐day life was perceived to reduce challenging behaviour in the long term. Conclusions Implications for practice are discussed

    Predictors and moderators of the response of adults with intellectual disabilities and depression to behavioural activation and guided self-help therapies

    Background No previous studies have reported predictors and moderators of outcome of psychological therapies for depression experienced by adults with intellectual disabilities (IDs). We investigated baseline variables as outcome predictors and moderators based on a randomised controlled trial where behavioural activation was compared with guided self-help. Methods This study was an exploratory secondary data analysis of data collected during a randomised clinical trial. Participants (n = 161) were randomised to behavioural activation or guided self-help and followed up for 12 months. Pre-treatment variables were included if they have previously been shown to be associated with an increased risk of having depression in adults with IDs or have been reported as a potential predictor or moderator of outcome of treatment for depression with psychological therapies. The primary outcome measure, the Glasgow Depression Scale for Adults with Learning Disabilities (GDS-LD), was used as the dependant variable in mixed effects regression analyses testing for predictors and moderators of outcome, with baseline GDS-LD, treatment group, study centre and antidepressant use as fixed effects, and therapist as a random effect. Results Higher baseline anxiety (mean difference in outcome associated with a 1 point increase in anxiety 0.164, 95% confidence interval [CI] 0.031, 0.297; P = 0.016), lower performance intelligence quotient (IQ) (mean difference in outcome associated with a 1 point increase in IQ 0.145, 95% CI 0.009, 0.280; P = 0.037) and hearing impairment (mean difference 3.449, 95% CI 0.466, 6.432; P = 0.024) were predictors of poorer outcomes, whilst greater severity of depressive symptoms at baseline (mean difference in outcome associated with 1 point increase in depression −0.160, 95% CI −0.806, −0.414; P < 0.001), higher expectation of change (mean difference in outcome associated with a 1 point increase in expectation of change −1.013, 95% CI −1.711, −0.314; p 0.005) and greater percentage of therapy sessions attended (mean difference in outcome with 1 point increase in percentage of sessions attended −0.058, 95% CI −0.099, −0.016; P = 0.007) were predictors of more positive outcomes for treatment after adjusting for randomised group allocation. The final model included severity of depressive and anxiety symptoms, lower WASI performance IQ subscale, hearing impairment, higher expectation of change and percentage of therapy sessions attended and explained 35.3% of the variance in the total GDS-LD score at 12 months (R2 = 0.353, F4, 128 = 17.24, P < 0.001). There is no evidence that baseline variables had a moderating effect on outcome for treatment with behavioural activation or guided self-help. Conclusions Our results suggest that baseline variables may be useful predictors of outcomes of psychological therapies for adults with IDs. Further research is required to examine the value of these potential predictors. However, our findings suggest that therapists consider how baseline variables may enable them to tailor their therapeutic approach when using psychological therapies to treat depression experienced by adults with IDs

    Factor structure of the Hospital Anxiety and Depression Scale in Japanese psychiatric outpatient and student populations

    <p>Abstract</p> <p>Background</p> <p>The Hospital Anxiety and Depression Scale (HADS) is a common screening instrument excluding somatic symptoms of depression and anxiety, but previous studies have reported inconsistencies of its factor structure. The construct validity of the Japanese version of the HADS has yet to be reported. To examine the factor structure of the HADS in a Japanese population is needed.</p> <p>Methods</p> <p>Exploratory and confirmatory factor analyses were conducted in the combined data of 408 psychiatric outpatients and 1069 undergraduate students. The data pool was randomly split in half for a cross validation. An exploratory factor analysis was performed on one half of the data, and the fitness of the plausible model was examined in the other half of the data using a confirmatory factor analysis. Simultaneous multi-group analyses between the subgroups (outpatients vs. students, and men vs. women) were subsequently conducted.</p> <p>Results</p> <p>A two-factor model where items 6 and 7 had dual loadings was supported. These factors were interpreted as reflecting anxiety and depression. Item 10 showed low contributions to both of the factors. Simultaneous multi-group analyses indicated a factor pattern stability across the subgroups.</p> <p>Conclusion</p> <p>The Japanese version of HADS indicated good factorial validity in our samples. However, ambiguous wording of item 7 should be clarified in future revisions.</p

    Cognitive Behavior Therapy for Anxious Adolescents: Developmental Influences on Treatment Design and Delivery

    Anxiety disorders in adolescence are common and disruptive, pointing to a need for effective treatments for this age group. Cognitive behavior therapy (CBT) is one of the most popular interventions for adolescent anxiety, and there is empirical support for its application. However, a significant proportion of adolescent clients continue to report anxiety symptoms post-treatment. This paper underscores the need to attend to the unique developmental characteristics of the adolescent period when designing and delivering treatment, in an effort to enhance treatment effectiveness. Informed by the literature from developmental psychology, developmental psychopathology, and clinical child and adolescent psychology, we review the ‘why’ and the ‘how’ of developmentally appropriate CBT for anxious adolescents. ‘Why’ it is important to consider developmental factors in designing and delivering CBT for anxious adolescents is addressed by examining the age-related findings of treatment outcome studies and exploring the influence of developmental factors, including cognitive capacities, on engagement in CBT. ‘How’ clinicians can developmentally tailor CBT for anxious adolescents in six key domains of treatment design and delivery is illustrated with suggestions drawn from both clinically and research-oriented literature. Finally, recommendations are made for research into developmentally appropriate CBT for anxious adolescents

    Development of a supported self-management intervention for adults with type 2 diabetes and a learning disability

    Background: Although supported self-management is a well-recognised part of chronic disease management, it has not been routinely used as part of healthcare for adults with a learning disability. We developed an intervention for adults with a mild or moderate learning disability and type 2 diabetes, building on the principles of supported self-management with reasonable adjustments made for the target population. Methods: In five steps, we: 1. Clarified the principles of supported self-management as reported in the published literature 2. Identified the barriers to effective self-management of type 2 diabetes in adults with a learning disability 3. Reviewed existing materials that aim to support self-management of diabetes for people with a learning disability 4. Synthesised the outputs from the first three phases and identified elements of supported self-management that were (a) most relevant to the needs of our target population and (b) most likely to be acceptable and useful to them 5. Implemented and field tested the intervention Results: The final intervention had four standardised components: (1) establishing the participant’s daily routines and lifestyle, (2) identifying supporters and their roles, (3) using this information to inform setting realistic goals and providing materials to the patient and supporter to help them be achieved and (4) monitoring progress against goals. Of 41 people randomised in a feasibility RCT, thirty five (85%) completed the intervention sessions, with over three quarters of all participants (78%) attending at least three sessions. Twenty-three out of 40 (58%) participants were deemed to be very engaged with the sessions and 12/40 (30%) with the materials; 30 (73%) participants had another person present with them during at least one of their sessions; 15/41 (37%) were reported to have a very engaged main supporter, and 18/41 (44%) had a different person who was not their main supporter but who was engaged in the intervention implementation. Conclusions: The intervention was feasible to deliver and, as judged by participation and engagement, acceptable to participants and those who supported them.     Prevalence et facteurs associes a l’anemie de l’adolescente dans le district sanitaire de Bouafle en Cote d’ivoire : Prevalence and factors associated with adolescent anemia in the health district of Bouafle in Cote d'Ivoire

    Introduction : Les stratégies qui visent à réduire la prévalence de l’anémie impliquent plus les femmes adultes que les adolescentes. Cette étude vise à disposer de données nécessaires à la mise en place d’une stratégie adaptée pour la prévention de cette affection chez l’adolescente.Matériels et Méthode : Il s’est agi d’une étude transversale à visée descriptive et analytique issue d’un échantillonnage par sondage en grappes. Ce sont au total 300 ménages qui ont été enquêtés à raison d’une adolescente par ménage. Le seuil retenu pour définir l’anémie était un taux d’hémoglobine inférieur à 110g/L. le test de Khi deux a été utilisé pour les analyses uni variées et la régression logistique pour les analyses multi variées. Les données ont été traitées à l’aide du logiciel Stata.Résultats : Les adolescentes enquêtées avaient un âge moyen de 14,01 ± 2,6 ans et vivaient majoritairement en milieu rural. Quinze pourcent (15%) avaient une goutte épaisse positive, 20% avaient une ferritinémie basse et 93,7% avaient une hypovitaminose B12. La prévalence de l’anémie était de 67,7 %. Les facteurs explicatifs de cette anémie étaient le jeune âge et la ferritinémie basse. La richesse du ménage était un facteur protecteur contre l’anémie.Conclusion : La prévalence de l’anémie chez l’adolescente est élevée dans le district sanitaire de Bouaflé. Les stratégies de lutte contre cette affection devraient tenir compte de l’âge, de la ferritinémie et du niveau socio économique des ménages de ce district. This study aims to obtain the data necessary for the implementation of an appropriate strategy for the prevention of this disease in adolescents.Materials and Method: This was a cross-sectional descriptive and analytical study based on sample cluster sampling. A total of 300 households were surveyed at the rate of one adolescent per household. The threshold used to define anemia was a hemoglobin level below 110g / L. Chi-square test was used for uni-variate analyzes and logistic regression for multivariate analyzes. Data was processed using Stata software.Results: The adolescent girls surveyed had an average age of 14.01 ± 2.6 years old and lived mainly in rural areas. Fifteen percent (15%) had a positive malaria test, 20% had low serum ferritin, and 93.7% had B12 hypovitaminosis. The prevalence of anemia was 67.7%. The explanatory factors for this anemia were young age and low ferritinemia. Household wealth was a protective factor against anemia.Conclusion: The prevalence of anemia in adolescents is high in the health district of Bouaflé. Strategies to control this condition should take into account the age, ferritinemia and socioeconomic level of households in this district
