Comparing the effects of lidocaine cream and mefenamic acid on post episiotomy pain
Introduction: A large number of womensuffered from the post episiotomy pain and the common method for pain relief is oral non-steroidal anti-inflammatory drugs. With considering the adverse effects of these drugs, the aim of present study was to compare the effects of lidocaine cream and mefenamic acid capsule on post episiotomy pain.
Methods: This clinical trial was carried out in 2011 on 60 women with singleton pregnancy at 38-42 weeks of gestation who underwent episiotomy. Cases were randomly divided into both groups and received lidocaine cream and mefenamic acid capsule. Data were collected by questionnaire and Visual Analogue Scale. Pain intensity was compared with the first complaint of mother and 6, 12 and 24 hours after the delivery in both groups. Data were analyzed by SPSS software (version 16), t-test and paired t-test. P value less than 0.05 was considered significant.
Results: With the first complaint of women, the mean of pain intensity was 4.92±1.9 in lidocaine group and 4.90±1.5 in mefenamic acid group that was not significant (p=0.20). Mean intensity of post episiotomy pain in two groups receiving lidocaine cream and mefenamic acid capsule were not significant at hours 6 (3.26±1.3 vs. 3.10±1.6, p=0.05), 12 (2.26±1.7 vs. 2.86±1.4, p=0.36) and 24 (1.46±1.2 vs. 1.49±1.2, p=1) after delivery.
Conclusion: The effects of lidocaine cream and mefenamic acid capsule in relief of post episiotomy pain were similar. With considering the side effects of mefenamic acid, lidocaine cream is an appropriate alternative drug in relief of post episiotomy pain
Photooxidative Removal of p-Nitrophenol by UV/H2O2 Process in a Spinning Disk Photoreactor: Influence of Operating Parameters
In this paper, spinning disk photoreactor (SDP) has been used for the removal of a refractory pollutant, namely p-nitrophenol (PNP), in UV/H2O2 process. The effect of various parameters such as the plate type in the SDP, concentration of oxidant (H2O2), fluid volume, initial concentration of PNP, distance of the lamps from the spinning disk, distance of the lamps from each other, pH, and rotation speed of the spinning disk in the removal efficiency has been investigated. The results indicated that the use of scrobiculate disc instead of flat disc significantly increased the removal percentage of PNP from 46 to 100 % for the irradiation time of 20 min; it also increased with increasing H2O2 concentration, but the increase in fluid volume and the initial concentration of PNP reduced the removal percentage of PNP in the SDP. The increase in the distance of UV lamps from each other and from disc surface in the SDP reduced the removal percentage of PNP. However, the increase in pH to 5.5 increased removal efficiency while increasing pH above 5.5 reduced PNP removal efficiency. The disk rotation speed from 0 to 90 rpm increased the removal percentage from 49 to 70 % for the irradiation time of 5 min, but increasing the rotation speed to more than 90 rpm reduced the removal efficiency.

Effects of a Novel Dental Gel on Plaque and Gingivitis: A Comparative Study.
ObjectivesThe goal of this prospective, randomized, controlled, double-blinded study was to evaluate the effects of a novel dental gel on plaque and gingival health. The dental gel was designed to (1) break up and prevent re-accumulation of microbial biofilm, and (2) inhibit metal mediated inflammation.Materials and methodsTwenty-five subjects with moderate gingival inflammation (Löe and Silness Gingival Index ≥2) and pocket depths <4 were randomly assigned to brush twice daily for 21 days with the test or the control dental gel. On Days 0, 7, 14 and 21, plaque levels (Quigley-Hein, Turesky Modification Plaque Index), gingival inflammation (Löe and Silness Gingival Index) and gingival bleeding (modified Sulcus Bleeding Index) were determined by one blinded, investigator using a pressure sensitive probe.ResultsAfter 3 weeks, all 3 clinical indices were significantly improved in both groups (P<0.05) and significantly lower in the test group (P<0.05).ConclusionThe novel dental gel formulation was provided effective plaque control and reduced gingival inflammation.Clinical relevanceA novel dentifrice formulation may be an effective tool for plaque removal and maintaining gingival health
The effect of Cornus mas in preventing recurrent urinary tract infections in women: A randomized controlled trial
Background and aims: Urinary tract infections (UTIs) are one of the most common and the second leading infections, after respiratory tract infections, in women. Currently, various chemical drugs are used to prevent the UTIs. Chemical drugs may cause antibiotic resistance and cause resistant strains likely grow in the long-term treatment with antibiotics. The aim of this study was to investigate the effect of Cornus mas in preventing recurrent UTIs in women aged 15-45 years referring to Ayatollah Kashani Hospital Clinic in Shahrekord. Methods: This experimental study (Parallel Design and Triple-blind) was conducted on 42 women aged 15-45 years referring to Ayatollah Kashani Hospital Clinic of Shahrekord and diagnosed with chronic cystitis. The exclusion criteria were neurogenic bladder, genitourinary system anatomical abnormalities (hydronephrosis, ureterocele stone, etc), and bacterial resistance. The women were randomly assigned to 2 groups. At baseline, the women were examined for any functional and anatomical disorders and if it was necessary, they underwent ultrasound. After the current UTIs were treated and the women clinically recovered, one group was administered with Cornus mas tablet 500 mg and another group administered with placebo for 6 months. All the women were followed up for 6 months. Every 2 months, the patients were clinically examined and their urine cultures were investigated for the clinical signs of cystitis. As the symptoms of the UTIs occur, the patients were recommended to refer for repeated urine culture. All patients (42 women) completed the study. Results: In our study, no significant difference was observed between the groups in terms of recurrent UTI recurrence, although there were differences (P>0.005). Positive urine culture in Cornus mas group was 19 and in placebo was 33.4. In terms of dysuria in 6 months and the second time, there was a significant difference between placebo and Cornus mas (P=0.004) Dysuria in Cornus mas group was 14.2 and in placebo was 56.2. Conclusion: Cornus mas can decrease dysuria and frequent urination in patients with recurrent UTIs, so it can be used in the treatment of these patients
Sol–Gel Synthesis of Iron-Doped Sepiolite as a Novel Humidity-Sensing Material
Nowadays, humidity sensors are attracting a great deal of attention, and there are many studies focusing on enhancing their performances. Nevertheless, their fabrication through facile methods at reasonable cost is a significant factor. In this article, a new magnesium silicate nanopowder was successfully synthesized using a simple and low-cost sol–gel method. Subsequently, modified sepiolite was achieved by the substitution of iron ions in the synthesized nanopowders. The specimens were then characterized by X-ray diffraction, field emission–scanning electron microscopy, X-ray photoelectron spectroscopy, thermogravimetric–differential thermal analysis, infrared spectroscopy, and nitrogen adsorption. Furthermore, humidity sensors were manufactured by screen printing the prepared powders on alumina substrates with interdigitated Pt electrodes. The results showed that the fabricated sensors with modified sepiolite exhibited interesting characteristics for humidity detection
A case of antiphospholipid syndrome following gastric signet ring cell adenocarcinoma
Objective: Rare disease Background: Antiphospholipid syndrome (APS) is a rare autoimmune disease characterized by arterial, venous, and small-vessel thrombosis, pregnancy-related morbidity and the presence of antiphospholipid antibodies such as an-ticardiolipin antibody, and/or anti-beta2-glycoprotein I. In the recent years, APS was observed in patients with solid tumors and the renal cancer, lung carcinoma and breast tumors were the most common tumors linked with APS. Case Report: A 53-year-old female presented with pain and pitting edema of left lower extremity that had begun 6 months prior to hospitalization. Deep vein thrombosis (DVT) in the popliteal vein diagnosed by Doppler ultrasonogra-phy and the patient was treated with heparin followed by warfarin. Following subdural hematoma, anticoagu-lant therapy was stopped, and the patient underwent craniotomy. One month later, the patient returned with pain and DVT diagnosed in its right leg. Laboratory tests showed high levels of lupus anticoagulant, IgM and IgG anticardiolipin antibodies. Following a high alkaline phosphatase, diffuse bone marrow involvement was found by whole body bone scan. Looking to find primary tumor, a large infilterable lesion in gastric was seen by endoscopic images, and biopsy histopathology showed a signet ring cell adenocarcinoma. The patient re-fused chemotherapy and died 6 months after diagnosis. Conclusions: APS is associated with gastric signet ring cell adenocarcinoma. © Am J Case Rep, 2020
Alert Correlation through a Multi Components Architecture
Alert correlation is a process that analyzes the raw alerts produced by one or more intrusion detection systems, reduces nonrelevant ones, groups together alerts based on similarity and causality relationships between them and finally makes aconcise and meaningful view of occurring or attempted intrusions. Unfortunately, most correlation approaches use just a few components that aim only specific correlation issues and so cause reduction in correlation rate. This paper uses a general correlation model that has already been presented in [9] and is consisted of a comprehensive set of components. Then some changes are applied in the component that is related to multi-step attack scenario to detect them better and so to improve semantic level of alerts. Fabrication and characterization of the modified ev31-based metal matrix nanocomposites
Metal matrix nanocomposites (MMNCs) with high specific strength have been of interest for numerous researchers. In the current study, Mg matrix nanocomposites reinforced with AlN nanoparticles were produced using the mechanical stirring-assisted casting method. Microstructure, hardness, physical, thermal and electrical properties of the produced composites were characterized in this work. According to the microstructural evaluations, the ceramic nanoparticles were uniformly dispersed within the matrix by applying a mechanical stirring. At higher AlN contents, however, some agglomerates were observed as a consequence of a particle-pushing mechanism during the solidification. Microhardness results showed a slight improvement in the mechanical strength of the nanocomposites following the addition of AlN nanoparticles. Interestingly, nanocomposite samples were featured with higher electrical and thermal conductivities, which can be attributed to the structural effect of nanoparticles within the matrix. Moreover, thermal expansion analysis of the nanocomposites indicated that the presence of nanoparticles lowered the Coefficient of Thermal Expansion (CTE) in the case of nanocomposites. All in all, this combination of properties, including high mechanical strength, thermal and electrical conductivity, together with low CTE, make these new nanocomposites very promising materials for electro packaging applications
Micromechanics of fatigue in woven and stitched composites
The goal is to determine how microstructural factors, especially the architecture of microstructural factors, control fatigue damage in 3D reinforced polymer composites. Test materials were fabricated from various preforms, including stitched quasi-isotropic laminates, and through-the-thickness angle interlock, layer-to-layer angle interlock, and through-the-thickness stitching effect weaves. Preforms were impregnated with a tough resin by a special vacuum infiltration method. Most tests are being performed in uniaxial compression/compression loading. In all cases to date, failure has occurred not by delamination, but by shear failure, which occurs suddenly rather than by gradual macroscopic crack growth.
