    Factors associated with global and leisure-time physical activity in the elderly: a health survey in São Paulo (ISA-SP), Brazil

    The purpose of this study was to measure the prevalence of global and leisure-time physical activity and associated factors in the elderly. This was a population-based cross-sectional study covering a multiple-stage sample of 1,950 subjects 60 years or older living in areas of São Paulo State, Brazil. Prevalence of global physical activity (assessed through the short version of the International Physical Activity Questionnaire - IPAQ) was 73.9%, and prevalence of leisure-time physical activity was 28.4%. The results highlight the differences between factors associated with global and leisure-time physical activities. The social groups most prone to overall sedentary lifestyle and especially to lack of leisure-time physical activity should be the main targets of health policies aimed at promoting healthier lifestyles.O propósito deste estudo foi medir a prevalência e os fatores associados à prática de atividade física global e de lazer em idosos (60 anos e mais). Estudo transversal de base populacional, com amostragem em múltiplos estágios que envolveu 1.950 idosos, residentes em áreas do Estado de São Paulo. A prevalência de atividade física global (avaliada pela versão curta do Questionário Internacional de Atividade Física - QIAF) foi de 73,9% e no lazer de 28,4%. Os resultados apontam para diferenças entre os fatores associados à atividade física global e no lazer.     Factors associated with dietary quality among adults in Greater Metropolitan São Paulo, Brazil, 2002

    Objetivou-se avaliar os fatores demográficos, sócio-econômicos e de estilo de vida associados à qualidade da dieta de adultos residentes na Região Metropolitana de São Paulo, Brasil. Estudo transversal, por meio de inquérito domiciliar, de base populacional, foi realizado no Distrito do Butantã e nos municípios de Itapecerica da Serra, Embu e Taboão da Serra. Utilizaram-se dados de um questionário e um recordatório de 24 horas de 1.840 adultos de 20 anos ou mais, de ambos os sexos, incluídos em um inquérito de saúde (ISA-SP). A qualidade da dieta foi avaliada através do índice de qualidade da dieta (IQD) adaptado para a realidade local. Utilizou-se análise de regressão linear para avaliar a associação entre o IQD e as demais variáveis. A maioria da população (75%) apresentou dieta que necessita de melhora. Observaram-se médias baixas para os componentes: frutas, verduras e legumes, leite e derivados. Número de bens de consumo duráveis, escolaridade do chefe da família e ter 60 anos ou mais se associaram ao IQD em homens. Para as mulheres, a faixa etária se associou ao IQD.     The objective of this study was to assess the prevalence of reported hypertension among the elderly in Campinas, São Paulo, Brazil, identifying related factors, the use of healthcare services, and knowledge and practices related to treatment options. This was a cross-sectional population-based study, with stratified clustered multiple-stage sampling. Data analysis for the 426 individuals (60 years of age and older) considered the sampling design and outlining effect. Hypertension prevalence was 51.8% (46.4% in men and 55.9% in women) and was higher among the elderly with less education (55.9%), immigrants from other States (60.2%), and the overweight or obese (57.2%). The results indicate that healthcare services are ensuring access to medical care (71.6% have regular physician appointments) and medication (86.7% take their routine medication), without distinction as to socioeconomic status. However, social inequalities persist in knowledge and use of other measures to control blood pressure, like appropriate diet and exercise, which are also underused by more privileged socioeconomic strata.Este estudo teve como objetivo avaliar a prevalência da hipertensão arterial referida em idosos de Campinas, São Paulo, Brasil, identificando os fatores associados, o uso de serviços de saúde e o conhecimento e as práticas quanto às opções do tratamento. Trata-se de estudo transversal, de base populacional, com amostra de conglomerados, estratificada e em múltiplos estágios. A análise dos dados referentes aos 426 indivíduos (sessenta anos e mais) levou em conta o desenho amostral e o efeito do delineamento. A prevalência de hipertensão foi de 51,8% (46,4% nos homens e 55,9% nas mulheres) e mostrou-se mais elevada em idosos: com menor escolaridade (55,9%), migrantes de outros estados (60,2%) e com sobrepeso ou obesidade (57,2%). Os resultados indicam que os serviços de saúde estão garantindo o acesso ao atendimento médico (71,6% visitam o médico regularmente) e aos medicamentos (86,7% tomam medicamento de rotina), sem distinção de nível sócio-econômico. Persistem, no entanto, desigualdades sociais quanto ao conhecimento e utilização de outras práticas de controle da pressão arterial, como dieta adequada e atividade física, que são insuficientemente utilizadas também pelos segmentos socialmente mais favorecidos.28529

    The objective of this study was to identify the prevalence of sedentary leisure time (no type of leisure-time exercise once a week or more) among the elderly in the city of Campinas, São Paulo State, Brazil, according to demographic and socioeconomic factors, other health-related behaviors, and the presence of morbidity. This was a population-based cross-sectional study with multiple-stage sampling. Data analysis considered the sample design. Prevalence of sedentary leisure time was 70.9%, and the prevalence ratio was significantly higher than 1.0 for the elderly with lower socioeconomic status (1.31: 1.11-1.55), smokers (1.39: 1.23-1.57), those with common mental disorders (1.20: 1.04-1.39), and females (1.16: 1.00-1.35). Prevalence of walking was 23.5%, followed by fitness or bodybuilding programs (3.8%), and swimming or water aerobics (3.6%). The results show the need to develop global action for health-related behaviors in order for this approach to succeed. Attention should focus on the elderly with common mental disorders and those with lower socioeconomic status to ensure equity in health promotion practices.231329-3

    Depression with melancholic features is associated with higher long-term risk for dementia

    BACKGROUND: Depression has been reported to increase the risk of subsequently developing dementia, but the nature of this relation remains to be elucidated. Depression can be a prodrome/manifestation of dementia or an early risk factor, and the effect may differ according to depression subtypes. Our aim was to study the association between early-onset depression and different depression subtypes, and the later occurrence of dementia. METHODS: We conducted a cohort study including 322 subjects with depression, recruited between 1977 and 1984. A comparison cohort (non-exposed) was recruited retrospectively, to include 322 subjects admitted at the same hospital for routine surgery (appendicectomy or cholecystectomy), at the same period as the depressed cohort. Subjects were contacted again between 2009 and 2014, to assess their dementia status. We computed the risk for dementia in subjects with early onset depression and quantified the association between different depression subtypes (namely melancholic, anxious, and psychotic) and dementia. RESULTS: The odds of dementia were increased by 2.90 times (95% C.I. 1.61-5.21; p<0.0001) for the depressed cohort when compared to the surgical cohort. When the analysis was restricted to patients younger than 45 years old at baseline, the odds for dementia in the depressed cohort were also significantly higher when compared to the surgical cohort (8.53; 95% C.I. 2.40-30.16). In the multivariate Cox analysis, subjects having depression with melancholic features had an increased risk for developing dementia compared to those without melancholic features (HR=3.64; 95% C.I. 1.78-11.26; p=0.025). LIMITATIONS: About 59% of the participants with depression and 53% of those non-exposed were lost during follow up. The inclusion of biological biomarkers would strengthen the results. The sample included a low number of bipolar patients. CONCLUSIONS: These results support depression as an early risk factor for dementia. Depression with melancholic features was found as an important risk factor for dementia, playing a main role in the relation between these disorders

    Domains of physical activity and education in Sao Paulo, Brazil: a serial cross-sectional study in 2003 and 2008

    O objetivo foi estimar a prevalência de atividade física em diferentes domínios e a sua associação com escolaridade. Trata-se de estudo seriado de base populacional de duas edições do Inquérito de Saúde de São Paulo, Brasil. Participaram 1.667 adultos, em 2003, e 2.086, em 2008. Amostragem probabilística foi realizada por conglomerados em dois estágios. O International Physical Activity Questionnaire (IPAQ) longo permitiu avaliar diversos domínios de atividade física. Foi utilizada regressão de Poisson. Os homens foram mais ativos no lazer e no trabalho; as mulheres, no lar. Encontrou-se aumento de ativos no deslocamento e na atividade total em homens e, também, no trabalho, entre as mulheres. A escolaridade foi associada de forma direta com lazer (2003 e 2008) e de forma inversa com o trabalho (2003), para os homens, e casa, para as mulheres; pessoas com menor nível de escolaridade estão se tornando menos ativas. As estratégias de intervenção devem considerar os níveis de escolaridade.     Factors associated with smoking in the elderly: a health survey in Sao Paulo (ISA-SP)

    This article reports on smoking prevalence and associated factors in the elderly, based on a population-based cross-sectional study with multistage sampling including 1,954 individuals 60 years or older living in four areas of Sao Paulo State, Brazil. Overall smoking prevalence was 12.2%, and higher rates were associated with male gender, age 60-69 years, not belonging to an Evangelical church, lower income, low body weight, lack of leisure-time physical activity, depression/anxiety, and hypertension. There was a high prevalence of smokers among individuals with a history of stroke, cancer, and chronic obstructive pulmonary disease. The results point to the need for effective interventions in healthcare services to promote smoking cessation among the elderly, since many are unable to stop on their own, even when they have tobacco-related illnesses. Special attention should be paid to individuals that depend on the National Health System, since smoking prevalence is higher in underprivileged socioeconomic groups
